Outcomes of simultaneous microsurgical replantation of complete amputations at the distal third of the right forearm and left wrist: a case report and 22-month follow-up.

Microsurgical replantation of simultaneous bilateral upper extremity amputations is a complex procedure requiring a highly coordinated multidisciplinary team. Functional outcomes depend on anatomical level of injury, duration of ischemia, extent of crushing, degree of contamination, and quality of postoperative rehabilitation. We report a 45-year-old male industrial worker who sustained complete simultaneous amputations through the distal third of the right forearm and at the left wrist following an industrial paper-cutting machine accident. Total ischemia time was 8 h. Two microsurgical teams

Classification and Outcomes of 214 Periprosthetic Patellar Fractures in Primary Total Knee Arthroplasty.

BACKGROUND: Long-term data following periprosthetic patellar fractures around primary total knee arthroplasty (TKA) are lacking. The purpose of this study was to describe the classification, management, and outcomes of periprosthetic patellar fractures in, to our knowledge, the largest series to date. METHODS: We identified 214 (213 cemented and one uncemented) periprosthetic patella fractures (205 patients) around a primary TKA sustained between 1989 and 2020. There were eight intraoperative fractures and 206 postoperative fractures. Postoperative fractures were classified according to the O

Postoperative infection following volar locking plate fixation of closed distal radius fractures: a retrospective cohort study of infection rates and antibiotic prophylaxis practice.

BACKGROUND: Distal radius fractures are among the most common surgically treated upper-extremity fractures, and volar locking plate fixation is the predominant operative technique. Although perioperative antibiotic prophylaxis is widely used in orthopaedic surgery, procedure-specific evidence for closed distal radius fractures remains limited. Contemporary data regarding postoperative infection rates and antibiotic prophylaxis practice following volar locking plate fixation are scarce. The aim of this study was to describe postoperative infection rates following volar locking plate fixation of

Baseball has an elbow injury problem: biomechanics may have the solution.

Elbow injuries-particularly to the ulnar collateral ligament (UCL)-have been a problem in sports medicine for at least half a century. As a pitcher rotates his trunk to face the target and externally rotates his throwing arm, varus torque is produced at the elbow to decelerate arm cocking and initiate arm acceleration. With today's pitchers being larger, throwing harder, and playing more often during their development years, repetition of large varus torque leads to more UCL injuries. Pitch count limits for games, seasons, and per year may provide some prevention of overuse injuries. Varying e

Reconstruction of a Rare Split-Level Bilaminar Patellar Tendon Rupture With Concomitant Multiligament Knee Injury Using Peroneus Longus Autograft: A Case Report.

INTRODUCTION: Patellar tendon ruptures (PTRs) are critical causes of knee extensor mechanism failure. Although typically isolated, a split-level bilaminar rupture pattern, in which different layers of the PTR at different anatomical levels, is extremely rare in adults, particularly when associated with complex multiligamentous injuries (occurring in only 0.02%-0.2% of orthopedic cases). CASE PRESENTATION: A 60-year-old female presented with acute knee pain and loss of active extension following a fall. Clinical examination revealed patella alta and a palpable infrapatellar gap. Imaging confir

A rare traumatic triad: Open bicondylar non-conjoint Hoffa fracture with partial tibial tubercle and medial epicondyle avulsions - A case report and biomechanical analysis.

UNLABELLED: Coronal shear fractures of the distal femoral condyles are rare intra-articular injuries, and the simultaneous occurrence of a non-conjoint bicondylar Hoffa fracture with a partial anterior tibial tubercle avulsion and a medial epicondyle avulsion constitutes an exceptionally complex traumatic triad. We report the case of a 35-year-old male motorcyclist who sustained this "floating" articular block injury (AO/OTA 33-B3.3) following a high-velocity accident. Diagnostic assessment utilizing Computed Tomography with 3D reconstruction revealed a vertical Letenneur Type II fracture of t

Open reduction and internal fixation of Type I Monteggia Equivalent fractures: A case series with excellent functional outcomes.

INTRODUCTION AND IMPORTANCE: Monteggia equivalent fractures, a spectrum of proximal ulnar and radial injuries extending beyond classic Monteggia patterns, are rare and diagnostically challenging, particularly in adults. Delayed presentation further complicates management and increases the risk of long-term morbidity. This report details a case series of two adult patients with Type I Monteggia Equivalent fractures, one with a delayed presentation, successfully treated with open reduction and internal fixation (ORIF) of the radial fracture. We aim to elucidate effective diagnostic and treatment

The perforator-based axial adipofascial turnover flap combined with juncturae tendinum grafting for complex dorsal phalangeal defects.

INTRODUCTION: Complex dorsal finger soft tissue defects remain a challenging reconstructive problem, particularly when associated with exposure of bone and extensor apparataus. Advances in the anatomical understanding of the dorsal cutaneous vascular system of the digits have allowed adipofascial flaps in this region to evolve from random-pattern to axial-pattern designs, improving vascular reliability. However, a structured and comprehensive approach for managing these combined defects has not been clearly defined in the literature. This study aims to present a holistic loco-regional strategy

Radioulnar Synostosis After Isolated Undisplaced Distal Ulna Fracture: A Rare Case Report and Review of the Literature.

CASE: A 50-year-old man developed radioulnar synostosis and presented with restricted forearm rotation, 15 months after sustaining an undisplaced fracture of the ulna. Radiographs and computed tomography revealed a mature bony bridge between the forearm bones. Surgical excision of the heterotopic bone was performed, and bone wax was applied to mitigate hemorrhage and prevent recurrence. Postoperatively, the patient achieved near-complete pronation and supination and no recurrence was found at the end of 2 years of follow-up. CONCLUSION: Radioulnar synostosis is an infrequent complication asso

Bone Loss After Implant Removal.

This case illustrates the complex challenges of managing forearm trauma complicated by bone loss after implant removal. A long-standing radial shaft plate, integrated into cortical bone with overgrowth, had to be removed to treat a new peri-prosthetic fracture. Removal caused massive cortical loss, leaving poor bone stock for fixation. Dual plating was performed to bridge the defect and stabilize the distal radius fracture, but mechanical failure prompted a staged revision. A deliberately delayed second operation allowed time for biological preparation. Definitive reconstruction included corre

Paediatric Patella Tendon Rupture: A Systematic Review.

The patella tendon is an important part of the extensor mechanism, and ruptures can lead to significant impairment. The condition is rare in children and adolescents, and as a result, the paediatric literature is limited. Injury patterns may differ from those seen in adults. This systematic review therefore aims to evaluate the presentation, diagnosis, management and outcomes of patella tendon ruptures in children and adolescents. A literature search was performed using PubMed, Medline and Embase to identify studies based on the Preferred Reporting Items for Systematic Reviews and Meta-analyse

MRI severity of common extensor origin damage associated with prior glucocorticoid injection in chronic lateral epicondylitis: a retrospective multicenter analysis.

BACKGROUND: Lateral epicondylitis (LE) is a degenerative tendinopathy of the common extensor origin (CEO). Although local glucocorticoid injections are widely used for symptom relief, concerns exist regarding their potential adverse effects on tendon structure. This study aimed to investigate the association between prior glucocorticoid injection and magnetic resonance imaging (MRI)-based structural CEO damage in patients with chronic LE. METHODS: In this retrospective, multicenter cross-sectional study, 147 patients with clinically diagnosed LE (symptom duration ≥ 6 months) who underwent elb
