Surgical Techniques for Managing Complex Distal Radius Fractures: A Technical Report.

Fractures of the distal radius are common injuries and may result from high-energy or low-energy trauma. Treatment options include non-surgical and surgical management, and although high-level evidence does not support surgical management for all of these fractures, there is a clear increasing trend in plate fixation for fractures of the distal radius. Where there is an indication to manage these surgically, open reduction and internal fixation of distal radius fractures is a core surgical skill that most trauma surgeons possess. However, some of these fractures may present in complex or atypi

Carbon Fiber-Reinforced Polyetheretherketone (PEEK) vs. Titanium Plates for Upper Limb Fractures: A Systematic Review.

Carbon fiber-reinforced polyetheretherketone (CFR-PEEK) plates have emerged as alternatives to titanium locking plates for fixation of upper-limb fractures. Advantages may include an elastic modulus closer to bone, fewer imaging artefacts, and reduced stress shielding. This systematic review synthesizes comparative clinical evidence. We analyzed data from five comparative studies (n = 210 patients; CFR-PEEK = 108, titanium = 102) provided in the dataset. Searches of PubMed and the Cochrane Library identified relevant trials. Data were extracted for demographics, interventions, functional outco

Does Pronator Quadratus Repair Affect the Functional Outcome Following Volar Plate Fixation for Distal End Radius Fracture?

Background and objective Distal radius fractures represent the most common upper extremity injury requiring surgical intervention. Volar plate fixation necessitates division of the pronator quadratus muscle, yet the necessity of systematic muscle repair remains controversial. While some studies suggest enhanced functional outcomes with repair, others demonstrate equivalent recovery patterns regardless of muscle reconstruction. This study aimed to analyze functional outcomes following volar plate fixation for distal radius fractures, comparing patients with and without pronator quadratus repair

Exercise Improves Upper-Limb Bone Health in Women: A Systematic Review and Meta-Analysis of Exercise Type and Intensity.

PURPOSE OF REVIEW: Distal radius fractures are common fragility fractures in women, but exercise-related evidence has focused mainly on the hip and spine. This review and meta-analysis examined whether exercise improves upper-limb bone outcomes and whether exercise modality and intensity influence the response. RECENT FINDINGS: Twenty-six controlled trials involving 1,880 women were included. The pooled estimate favored exercise with a standardized mean difference (SMD) of 1.05; with a 95% confidence interval (CI) from 0.68 to 1.43; p < 0.001), representing a large positive average effect. Anteromedial Tibia Plateau Fracture without Significant Ligamentous Injury Treated Successfully with Open Reduction and Internal Fixation: A Case Report.

INTRODUCTION: Treatment of distal radius fractures (DRFs) has evolved throughout time. The current management practices typically entail the use of locking compression plates. The inclusion of locking plates with variable angle options made it easier for surgeons to handle comminuted fracture fragments. This study examines the functional and radiological results of DRFs treated using a variable angle locking compression plate. MATERIALS AND METHODS: Between July 2022 and May 2024, a cohort of 25 patients with DRFs presented to the Department of Orthopaedics and was preoperatively examined bef

Functional and Radiological Outcomes Following Volar Locking Plate Fixation for Distal Radius Fractures: A Prospective Analytical Study.

INTRODUCTION: Most frequent injuries of the Upper limb are a distal radius fracture (DRF). If they aren't handled well, they might cause problems for a long time. There are several techniques to treat this issue, but volar locking plate fixation has become increasingly prevalent since it may restore anatomical alignment, provide stable fixation, and allow for early movement. This study aimed to evaluate the functional and radiological results of distal end radius fractures treated with volar locking plate fixation. MATERIALS AND METHODS: In this prospective analytical investigation, 60 patien

Epidemiologic Patterns of Musculoskeletal Disorders by Body Region Across 1.2 Million Orthopedic Visits: A 15-Year Experience From a Middle-Income Country.

BACKGROUND: Musculoskeletal problems occupy an important place among outpatient clinical applications. The International Statistical Classification of Diseases and Related Health Problems, Tenth Revision (ICD-10), has facilitated the understanding of disease diagnoses in a common language worldwide and the processing of data. However, studies examining all orthopedic diagnoses in a large population are limited. In this study, we analyzed over 1.2 million ICD-10-coded orthopedic visits over 15 years in a secondary-level hospital located in a low-income, working-class district of Türkiye, provid

The Novel Use of a Custom Three-Dimensional-Printed Polycaprolactone-Tricalcium Phosphate Synthetic Bone Scaffold (Osteopore®) in the Management of a Critical-Sized Bone Defect for a Patient with an Open Intra-Articular Calcaneal Fracture - A Case Report.

INTRODUCTION: Intra-articular calcaneal fractures are generally treated with open reduction and internal fixation in order to restore calcaneal anatomy as well as subtalar and calcaneocuboid joint congruency. Bone loss is common due to the impaction of cancellous bone beneath the posterior facet as a result of axial loading. Critical-sized bone defects are commonly addressed with the Masquelet "induced membrane technique" incorporating autogenous or allogenous bone graft. Synthetic bone scaffolds are readily available and customizable through three-dimensional (3D) printing. This case report d

Central band interosseus membrane reconstruction for longitudinal instability injuries of the forearm.

AIMS: Acute and chronic injuries of the interosseus membrane can result in longitudinal instability of the forearm. Reconstruction of the central band of the interosseus membrane can help to restore biomechanical stability. Different methods have been used to reconstruct the central band, including tendon grafts, bone-ligament-bone grafts, and synthetic grafts. This Idea, Development, Exploration, Assessment, and Long-term (IDEAL) phase 1 study aims to review the clinical results of reconstruction using a synthetic braided cross-linked graft secured at either end with an Endobutton to restore

Outcomes Following Surgical Fixation of Upper Extremity Fractures in Patients with Chronic Kidney Disease.

PURPOSE: Moderate to severe (stage III-IV) chronic kidney disease (CKD) and end stage renal disease (ESRD) have been shown to be independent risk factors for sustaining a fragility fracture. High rates of complications and mortality are associated with fracture fixation in patients with CKD, but existing literature is limited. It is unknown how CKD stage III-IV or ESRD affects outcomes in upper-extremity fractures. We hypothesize that patients with CKD stage III-IV or ESRD will have high complication rates after surgical fixation of upper extremity fractures. METHODS: We identified all patien

Bone Lengthening Radius using Limb Reconstruction System - A Successful Treatment for Radius Shortening: A Case Report.

Fractures of the distal radius are the most common type of forearm fractures seen in children. The most serious outcome of physeal injuries is growth arrest, which can result in deformity and even significant differences in limb length. Therefore, we'd like to share our experience with treating a patient whose left radius stopped growing after she had a physeal injury in an accident. Case presentation: we encountered a 10-year-old girl, who was involved in a road traffic accident. She sustained closed fracture distal end left radius (Salter Harris 2). She sought medical assistance late, so ost

Single-Stage Press-Fit Osseointegration of the Radius and Ulna for Rehabilitation After Trans-Forearm Amputation.

BACKGROUND: Upper limb (UL) amputation is disabling. ULs are necessary for many domains of life1, and few effective motor and sensory replacements are accessible2. Approximately 41,000 people in the United States have UL amputation proximal to the fingers3, two-thirds of (all) traumatic amputations are UL4, and 80% of UL amputations are performed for trauma-related etiologies5. Socket prosthesis (SP) abandonment remains high because of the lack of sensation, limited prosthesis control, perceived weight, and difficulty comfortably wearing the SP6. Using Next-Generation Sequencing to Understand Infection Prevention in Surgical Treatment of Upper Extremity Fractures-A Prospective Cohort Study.

INTRODUCTION: Postoperative fracture site infection can lead to notable patient morbidity, increase cost of care, and further contribute to healthcare disparities globally. Dogma suggests surgical blades as a vehicle for introducing bacteria into the surgical site; however, there is a paucity of literature to support this claim. This study uses advanced DNA sequencing to detect bacterial DNA on surgical blades used in upper extremity fracture surgeries. METHODS: This was a prospective study, conducted at a high-volume level 1 trauma center. All acute, closed upper extremity fractures requirin

Ipsilateral fractures of the ulnar styloid with distal radius fractures; to fix or not? A systematic review and meta-analysis.

BACKGROUND: The most common upper limb fracture encountered in clinical practice is distal radius fracture (DRF). They frequently occur due to falls onto an outstretched hand or other traumatic incidents, resulting in a break in the radius near the wrist joint. DRFs often present a spectrum of injuries and are a common reason for emergency department visits, affecting approximately one out of every six patients seeking medical attention in this setting. METHODS: This systematic review was performed according to the guidelines of PRISMA, using the electronic database search of PubMed, Embase,

Fixation of 3-Part Intra Articular Distal Radius Fracture with Combined Volar Plate and Dorsal Spanning Plate: A Case Report.

INTRODUCTION: The aim of this study is to illustrate a case in which the use of a dorsal spanning plate followed by volar plating may be indicated in the setting of a complex intra-articular distal radius fracture. Combined dorsal spanning plate and volar plating is a rare procedure reserved typically for comminuted fractures of the distal radius. This case highlights the first reported incidence in which a dorsal spanning plate of a distal radius fracture resulted in the volar displacement of the fragments, requiring subsequent volar plating. In the end, the outcome was favorable, as the foll
