Associations of Serum Testosterone and SHBG With Incident Fractures in Middle-aged to Older Men.

CONTEXT: As men age, circulating testosterone (T) decreases, circulating SHBG increases, and the risk of fracture increases. It is unclear if circulating T, independently of comorbidities, is associated with fracture risk in men. OBJECTIVES: To determine associations for T and SHBG with incident fractures in men. METHODS: We utilized the large (n = 205 973 participants, 11 088 any fracture cases, 1680 hip fracture cases, 1366 forearm fracture cases) and well-characterized UK Biobank cohort. Associations were modeled using Cox regressions, adjusting for multiple comorbidities/covariates, impu

Management of an Adult Open Diaphyseal Both-Bone Forearm Fracture With Severe Bone Loss: A Case Report, Literature Review, and Surgical Techniques.

Open forearm fractures involving both the radius and ulna are challenging orthopedic injuries, typically resulting from high-energy impacts. These injuries require complex surgical interventions, such as open reduction and internal fixation (ORIF) or intramedullary nailing, and often necessitate multiple approaches due to the proximity of neurovascular structures. Severe cases can lead to complications such as infection, nonunion, and functional impairment of the limb. Here, we present the case of a 42-year-old male who sustained multiple injuries in a motor vehicle accident, including an open

Global and regional prevalence of osteoporosis in kidney transplant recipients: a systematic review and meta-analysis.

Kidney transplant recipients (KTRs) are at a heightened risk of low bone density and fractures. This study sought to determine the overall prevalence of osteoporosis in KTRs. Published observational studies and those with a baseline report on osteoporosis were evaluated in databases such as PubMed, Scopus, Science Direct, Web of Science, and EMBASE, in addition to Google Scholar from January 1, 2000, to January 1, 2024. The methodological quality of the included studies was rigorously assessed using the JBI Critical Appraisal checklist. Heterogeneity across primary study results was evaluated

[Shaft fractures of the upper extremities and concomitant injuries : Epidemiology, classification, diagnostics and clinical relevance].

Shaft fractures of the upper extremities represent a significant challenge in trauma surgery due to their functional importance. The epidemiological analysis reveals characteristic distribution patterns: humeral shaft fractures (1.2-5% of all fractures) show a bimodal age distribution, with peaks at 20-30 years and above 60 years. Forearm shaft fractures (13.8/100,000 inhabitants [1]) occur particularly frequently in children and decrease with advancing age. The classification primarily follows the AO/OTA system, which was comprehensively revised in 2018, supplemented by specific classificatio

Nonoperative management of terrible triad injuries: clinical outcomes of 39 cases.

BACKGROUND: Terrible triad injury (TTI) poses therapeutic challenges in joint stability and functional recovery. While surgery has traditionally been preferred, recent interest in nonoperative management for select cases has emerged, with studies suggesting positive outcomes for carefully chosen patients. METHODS: This retrospective study, conducted at a tertiary medical center, included patients treated conservatively for TTI between 2014 and 2022. Patients younger than 18 years with open or pathological fractures or multiple injuries were excluded. Conservative treatment was chosen for elbo

Isolated Carpometacarpal Dislocation Without Associated Fracture: A Case Report of High-Energy Wrist Trauma.

Carpometacarpal (CMC) joint dislocations without fractures are rare and often missed due to subtle clinical and radiographic signs. We report a case of a 28-year-old male patient involved in a motorbike rollover who sustained open comminuted radius and ulna fractures, an ulnar styloid fracture, and dorsal dislocation of multiple CMC joints without metacarpal fractures. He presented with absent distal pulses and finger numbness, which resolved after emergency reduction. CT angiography ruled out arterial injury. He underwent internal fixation of forearm fractures and Kirschner wire stabilization

Plate Osteosynthesis for Segmental Forearm Fractures: A Comparison of Outcomes in Floating Elbow and Isolated Forearm Fractures.

BACKGROUND: Segmental forearm fractures are challenging to treat due to their instability. A floating elbow, characterized by ipsilateral fractures of both the humerus and forearm, is a rare but severe injury often caused by high-energy trauma and associated with significant soft tissue damage. The objective of this study is to compare the surgical outcomes of plate osteosynthesis in segmental forearm fractures, with and without the concomitant presence of a floating elbow. METHODS: A total of 35 adult patients who underwent open reduction and internal fixation for segmental forearm fractures

Management of a Case of Adult Mid-Shaft Femur Fracture in Osteogenesis Imperfecta by Long Proximal Femoral Nail: A Case report.

INTRODUCTION: Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder caused by defective type I collagen, which predisposes patients to recurrent fractures, skeletal deformities, and long-term disability. Intramedullary nailing is widely regarded as the gold standard for surgical stabilization of long bone fractures in OI due to its load-sharing capacity, deformity correction, and ability to prevent future fractures. CASE REPORT: We describe the case of a 34-year-old female with type IA OI who presented with a spontaneous mid-shaft femur fracture while weight-bearing. She

Intersecting Pathologies: COL1A1-Related Syndrome in the Setting of Childhood-Onset Hypopituitarism: Case Report and Literature Review.

Background: Type I collagen is the most abundant protein of the extracellular matrix. Pathogenic variants in COL1A1 or COL1A2 are classically associated with osteogenesis imperfecta (OI) and Ehlers-Danlos syndrome (EDS). An emerging clinical entity-COL1-related overlap disorder-encompasses individuals exhibiting phenotypic features of both conditions. Methods: We report a 55-year-old male presenting with disproportionate short stature, grayish-blue sclerae, multiple fractures, long bone deformities, joint hypermobility, and atrophic surgical scarring. The patient also had long-standing, untrea

The Novel Use of a Custom Three-Dimensional-Printed Polycaprolactone-Tricalcium Phosphate Synthetic Bone Scaffold (Osteopore®) in the Management of a Critical-Sized Bone Defect for a Patient with an Open Intra-Articular Calcaneal Fracture - A Case Report.

INTRODUCTION: Intra-articular calcaneal fractures are generally treated with open reduction and internal fixation in order to restore calcaneal anatomy as well as subtalar and calcaneocuboid joint congruency. Bone loss is common due to the impaction of cancellous bone beneath the posterior facet as a result of axial loading. Critical-sized bone defects are commonly addressed with the Masquelet "induced membrane technique" incorporating autogenous or allogenous bone graft. Synthetic bone scaffolds are readily available and customizable through three-dimensional (3D) printing. This case report d

Tweener Fractures in Children and Adolescents.

Tweener fractures are defined as long bone or metadiaphyseal fractures in a pediatric population for which multiple treatment options may be used. It is important to focus specifically on patients nearing skeletal maturity who present with length-unstable femoral shaft fractures, both-bone forearm fractures, distal radius metadiaphyseal fractures, and adolescent tibial shaft fractures. Although there is no gold standard of treatment for any of these fracture patterns, it is important to discuss the risks and benefits, proper application, and important technical aspects of each treatment method

Unusual body weight loss due to primary hyperparathyroidism: A case study with literature review.

Brown tumors (osteitis fibrosa cystica) are rare pathognomonic signs that occur in patients with primary hyperparathyroidism (PHPT). Brown tumors can exist in multiple bones and can easily be misdiagnosed as a metastatic tumor or multiple myeloma. It is also localized in the forearm, humerus, and leg. The symptoms of hypercalcemia, pathologic fracture, and bodyweight loss may increase the diagnostic difficulty of brown tumors because multiple myeloma and bone metastasis also show the same symptoms. We studied a 68-year-old woman who had experienced unusual bodyweight loss in the past 6 months
