Chiropractic manipulation in carpal tunnel syndrome.

OBJECTIVE: To determine if chiropractic manipulation could relieve carpal tunnel syndrome (CTS). CLINICAL FEATURES: A 42-yr-old female suffered from pain, tingling and numbness in the right wrist. Paresthesia along the C6 dermatome, a positive Phalen's test and Tinel's sign was present. EMG testing confirmed the clinical diagnosis of CTS. INTERVENTION AND OUTCOME: Chiropractic manipulations were rendered 3 times per week for 4 wk, to the subject's cervical spine, right elbow and wrist using a low amplitude, short lever, low force, high velocity thrust. Significant increase in grip strength a

Upper extremity peripheral nerve entrapments among wheelchair athletes: prevalence, location, and risk factors.

Wheelchair athletes commonly experience hand pain and numbness. This investigation studied the prevalence, location, and risk factors of upper extremity peripheral nerve entrapment among wheelchair athletes. Clinical and electrodiagnostic assessments were performed on both upper extremities of 28 wheelchair athletes and 30 able-bodied controls. Included in the assessment were short-segment stimulation techniques of the median nerve across the carpal tunnel and the ulnar nerve across the elbow. By clinical criteria, the prevalence of nerve entrapment among the wheelchair athletes was 23%, where

Deficits in the function of small and large afferent fibers in confirmed cases of carpal tunnel syndrome.

Nerve conduction studies and measurements of thermal thresholds (hot and cold) were performed in 25 healthy control subjects and 24 patients with carpal tunnel syndrome: first, to compare the extent of abnormality demonstrable in the function of large and small afferent axons; second, to correlate these abnormalities with clinical severity; and third, to validate a modified form of palmar stimulation as an additional neurophysiological test in suspected carpal tunnel syndrome. The routine nerve conduction studies compared the amplitudes and conduction velocities from digit II and digit V to wr

Overuse syndromes among endoscopists.

A survey of 400 gastroenterological endoscopists was carried out to determine what kinds of overuse syndromes they suffer from as a result of doing endoscopic procedures. The response rate was 72%. Thumb pain, hand pain, elbow pain, low back pain, and possibly shoulder pain all appear to be caused by endoscopy to at least some extent. Physicians who perform the most procedures tend to have the highest risk of developing such problems. Age, sex, practice setting, and hobbies did not seem to have an impact on the overuse conditions identified. These conditions are most likely due to the equipmen

Revision ulnar neuroplasty.

Revision ulnar neuroplasty should be performed in cases of recurrence or persistence of symptoms or signs of ulnar entrapment neuropathy at the elbow following cubital tunnel release, with or without previous epicondylectomy or anterior transposition. Most cases of recurrence or persistence of symptoms reported in the literature could be traced at reoperation to failure of the initial treating surgeon to decompress the nerve adequately at all potential sites of compression. Included in this assessment were those cases of unwarranted postoperative nerve tension in elbow flexion, the result of m

The use of patterned neuromuscular stimulation to improve hand function following surgery for ulnar neuropathy.

A 60-year-old man with wasting and weakness of the right hand following ulnar nerve entrapment at the elbow was referred for electrotherapy. An ulnar nerve transposition had been performed 2 years previously. This had produced some improvement in nerve conduction without significantly improving hand muscle function. The right first dorsal interosseous muscle (FDI) was stimulated for 4 hours per day over a 6-week period with a stimulus pattern replicating the discharge of a single motor unit from a healthy, fatigued FDI (patterned neuromuscular stimulation or PNMS). The response was assessed us

Ulnar collateral ligament in the growing elbow: MR imaging of normal development and throwing injuries.

PURPOSE: To study the magnetic resonance (MR) imaging appearance of the ulnar collateral ligament (UCL) and its insertion into the medial epicondyle. MATERIALS AND METHODS: Sixteen normal and 20 symptomatic elbows were examined with a 1.5-T unit. Normal elbows were imaged with axial T2*-weighted three-dimensional Fourier transform sequences. Symptomatic elbows were imaged with coronal T1-, T2-, and T2*-weighted and/or short-inversion-time inversion-recovery sequences. RESULTS: In normal immature elbows, the ulnar periosteum was seen as an extension of the UCL, and its enthesis had signal int

Provocative testing for cubital tunnel syndrome.

This prospective study evaluated the clinical usefulness of provocative testing in 32 subjects with electrodiagnostically proven cubital tunnel syndrome and 33 control subjects. Four provocative tests were included: Tinel's sign, elbow flexion, pressure provocation, and combined elbow flexion and pressure provocation. The mean age of the control group was 41 years and 46 years for the group with cubital tunnel syndrome. In the control group, provocative tests were rarely positive. In 44 extremities with cubital tunnel syndrome, 31 had a Tinel's sign, 33 had a positive elbow flexion test, 39 ha

[Study of the proximal conduction of the arms using SEP and MEP].

The proximal conduction of the upper limb has been studied with the somatosensory evoked potentials (SEP) and motor evoked potentials (MEP) in 40 controls which provided normal data. The SEP allowed the sensory conduction study of the Erb/C7 segment with N9/N13 delay determination after the median nerve stimulation at the wrist, and after the ulnar nerve stimulation at the wrist (20 cases) or above the elbow (20 cases). The N9/N13 delay was 3.24 ms +/- 0.35 for the median nerve, 3.30 ms +/- 0.52 for the ulnar nerve after stimulation at the wrist and 3.30 ms +/- 0.40 after stimulation at the el

[Nocturnal orthosis: a new treatment of compression of the ulnar nerve at the elbow. 30 cases].

Compression of the ulnar nerve at the elbow was confirmed by electromyography in 30 patients including 29 cases of idiopathic aetiology and 1 case of late onset ulnar palsy. Six cases occurred after general anaesthesia and 3 had undergone an unsuccessful operation. All patients were managed solely with an orthesis worn at night. The orthesis held the elbow in 60 degrees flexion. Clinical improvement was noted in all cases (including the 3 cases of unsuccessful surgery) evaluated by the patient and the physician at 88% on the average. Improvement occurred rapidly and was evaluated at 50% on day

Clinical biomechanics of the elbow in tennis: implications for evaluation and diagnosis.

Elbow injuries constitute a sizeable percentage of tennis injuries. Biomechanical analysis of the forces, loads, and motions on the elbow in tennis, and the constraint systems operating the control the forces, can lead to an understanding of the pathophysiology of these injuries. A biomechanically based evaluation framework can be used to document all of the clinical symptoms, anatomic alterations, and biomechanical alterations that are associated with the pathological problem.

Type 4 median nerve entrapment after elbow dislocation.

A rare case of median nerve entrapment after posterior elbow dislocation is reported. Nerve entrapment was both in the healed medial epicondyle fracture and within the elbow joint and we recommend placing this type of entrapment separately in an extended classification of median nerve entrapment following elbow dislocation.

Cubital tunnel syndrome in a patient in long-term haemodialysis.

The onset mechanism of cubital tunnel syndrome and carpal tunnel syndrome may be similar in haemodialysis patients. Carpal tunnel syndrome is well recognized as a consequence of dialysis-associated amyloidosis. This case report documents the development of cubital tunnel syndrome in a patient on haemodialysis treatment for 10 years. Proliferating granulation tissue at the elbow had entrapped and displaced the ulnar nerve. This was corrected surgically, and the patient experienced immediate relief of the numbness and the "tingling", but the muscular atrophy had not improved after 8 months.

Intraneural topography of the ulnar nerve in the cubital tunnel facilitates anterior transposition.

The surgical management of cubital tunnel syndrome includes anterior transposition of the ulnar nerve. The success of all transposition procedures is dependent on placement of the nerve anterior to the medial epicondyle without tension. Fifteen cadaveric upper extremities underwent anterior transposition followed by anterior transposition with separation of the most proximal motor branches from the main ulnar nerve for a distance of 1, 2, and 3 cm. Proximal dissection of these motor branches achieved an average gain in distance from the epicondyle of 71%, with an average distance from the epic

Intraneural ulnar nerve pressure changes related to operative techniques for cubital tunnel decompression.

To evaluate the effect of critical anatomic structures on the ulnar nerve after cubital tunnel decompression, we determined the intraneural ulnar nerve pressure in 50 fresh cadavers after the following surgical procedures: simple decompression, medial epicondylectomy, subcutaneous transposition, and submuscular transposition by the Learmonth and by the musculofascial lengthening technique. Intraneural pressure was measured in 0 degrees, 30 degrees, 60 degrees, and 90 degrees elbow flexion at locations that were proximal, within, and distal to the cubital tunnel. Statistical analysis compared t

Carpal tunnel syndrome in 100 patients: sensitivity, specificity of multi-neurophysiological procedures and estimation of axonal loss of motor, sensory and sympathetic median nerve fibers.

This prospective study meets all six criteria recently recommended by a quality assurance committee of the AAEM and defines criteria of abnormality, sensitivity and specificity of 19 sensorimotor and sympathetic parameters in 100 patients who were suspected on clinical grounds of having carpal tunnel syndrome (CTS), and in 70 control subjects. Nine parameters reached a specificity of 97%, permitting the electrodiagnosis of CTS in 87% of the patients studied. The results in this study confirm that median sensory nerve conduction studies are more frequently abnormal than are studies of motor ner

[Simple decompression of the ulnar nerve in cubital tunnel syndrome with and without morphologic changes. Report of experiences based on 523 cases].

The transposition of the ulnar nerve has proved to be an effective therapy in ulnar neuritis at the elbow but has also many risks and technical problems. In contrast the simple decompression or release of the nerve within the cubital tunnel which was first described by Osborne and Feindel and Stratford is an operation which is free of complications and postoperative morbidity and can be performed in local anesthesia. According to the results of a former study of 139 cases and this follow-up study of further 523 cases the simple decompression proved to be an efficacious and almost entirely subs
