Continuous versus interrupted epineural repair in forearm nerve injuries.

BACKGROUND: Peripheral nerve repair traditionally employs interrupted suture techniques, but continuous suturing may offer advantages in operative efficiency without compromising outcomes. We compared functional recovery and surgical parameters between continuous and interrupted epineural repair techniques in forearm nerve injuries. METHODS: We retrospectively analyzed 78 patients with acute forearm nerve injuries (median, radial, ulnar) treated between 2017 and 2020. Patients underwent primary epineural repair using either continuous (n = 40, 51.3%) or interrupted (n = 38, 48.7%) suture tech

Presence of Martin-Gruber Anastomosis among a Cohort of Mexican Origin; a Corpse Dissection Study.

BACKGROUND: At the forearm level, Martin-Gruber anastomosis is described as the most common anatomic variation. Early recognition of this anastomosis in patients with peripheral nerve lesions and preserved function of the hand musculature lowers the rate of misdiagnosis and prevents unnecessary treatment approaches. We aimed to determine the prevalence and characteristics of Martin-Gruber anastomosis in a Mexican cohort and discuss its impact on clinical diagnostics and approach. METHODS: We conducted a cross-sectional dissection study on 40 inert anatomical models at the National Forensic Sc

Surgical Outcomes of Distal Biceps Brachii Tendon Rupture by Use of a New Instrument, PushLock® Knotless Suture Anchor.

Background: Ruptures of the distal biceps brachii tendon (DBT) have only a few published reports in Asia. In this study, we report on the diagnosis and surgical outcomes of six acute traumatic cases surgically treated using a new internal fixation instrument, PushLock® Knotless Suture Anchor. Methods: In the past 3 years, we treated six patients with DBT rupture. All were males, aged 39-67 years at the time of surgery. Three patients had complete rupture of both the long and short head and three patients had only rupture of the long head. The ruptured DBT tendon was tightly fixed to the anatom

Minimizing risk to the dorsal sensory branch of the ulnar nerve in wrist arthroscopy: an anatomical study.

Wrist arthroscopic procedures using ulnar wrist portals, such as the 6-ulnar and direct foveal portals, have a risk of injuring the dorsal sensory branch of the ulnar nerve. This study investigated the anatomy of the dorsal sensory branch of the ulnar nerve and its positional changes during supination, neutral rotation and pronation at specific landmarks to find an optimal forearm position to minimize the risk of iatrogenic nerve injury during arthroscopy. The concept of 'nerve density' was introduced to quantify the likelihood of encountering the dorsal sensory branch of the ulnar nerve withi

Targeted Muscle Reinnervation Using the Anterior Interosseous Nerve for Symptomatic Wrist Level Neuromas.

BACKGROUND: Iatrogenic and traumatic sensory nerve injury at the level of the wrist can lead to debilitating neuroma. Targeted muscle reinnervation (TMR) is an effective treatment for the management of symptomatic neuromas. We investigate the use of the anterior interosseous nerve (AIN) as a recipient nerve for the treatment of iatrogenic neuromas. This case series describes 4 patients treated for neuromas of the lateral antebrachial cutaneous nerve (LABC), palmar cutaneous branch of median nerve (PCB), and radial sensory nerve (RSN). METHODS: Four cases involved a symptomatic neuroma of the

Wrist Extension Reconstruction Using Distal Anterior Interosseous to Extensor Carpi Radialis Brevis Nerve Transfer in Brachial Plexus Injuries.

PURPOSE: Reconstruction of wrist extension in C5-C8 root brachial plexus injuries (BPIs) is challenging, because of limited nerve and tendon donors. The purpose of this study was to report outcomes relating to the pronator quadratus branch of the anterior interosseous nerve (AIN) to extensor carpi radialis brevis (ECRB) nerve transfer from three BP surgeons. METHODS: Ten patients with C5-C8 BPI underwent AIN to ECRB nerve transfer. The mean age of the patients was 30.5 ± 15.9 years. All patients were men. The mean time to nerve surgery following initial injury was 4.5 ± 1.2 months. In all pat

Topographic anatomical landmarks for targeted nerve infiltration in distal radius fracture surgery.

The study focuses on the increasing incidence of distal radius fractures, particularly in individuals under 15 and over 50, and the critical need for effective perioperative pain management strategies. Distal radius fractures often require surgical intervention and pain control, essential to enhancing recovery and minimizing complications like chronic regional pain syndrome (CRPS). Standard analgesic options include hematoma blocks, Intravenous regional anesthesia (IVRA), regional nerve blocks, sedation, and general anesthesia. This research proposes a nerve block approach targeting the anteri

Comparative evaluation of posterior interosseous nerve strain in lateral approach to radial head fixation: a cadaveric study.

BACKGROUND: The posterior interosseous nerve (PIN) is at significant risk during proximal radius exposure. This study investigates the risk of PIN traction injury by comparing PIN strain between the Kocher and Kaplan approaches during simulated plate fixation of radial head fractures using upper limb cadaveric models. METHODS: Fourteen fresh-frozen upper extremity specimens were dissected with Kaplan (n = 7) and Kocher (n = 7) approaches, and the PIN was exposed at the anterior elbow with a separate incision. A Microminiature Displacement sensor (M-DVRT-9; LORD MicroStrain, Williston, VT) was

Open elbow arthrolysis with hinged external fixation for postburn elbow stiffness: a case series with an averaged 4-year follow-up.

OBJECTIVE: Patients with severe burns may develop heterotopic ossification (HO), articular cartilage loss, and soft tissue contracture in the elbow, significantly limiting elbow function and diminishing quality of life. Open elbow arthrolysis (OEA) combined with hinged external fixation (HEF) has demonstrated efficacy in managing severe elbow stiffness. However, no recent studies have specifically investigated its application in postburn elbow stiffness. This study evaluates the clinical outcomes of OEA with HEF in patients with postburn elbow stiffness, with an average follow-up period of fou

Utilising an internal joint stabilizer to ensure post-release elbow stability for patients with severe post-traumatic elbow stiffness: A retrospective cohort study.

OBJECTIVE: The treatment of severe post-traumatic elbow stiffness is extremely complex. Complete open release of the elbow joint and reconstruction of stiffness-related injuries are considered crucial; however, these procedures may lead to elbow instability, particularly chronic instability due to underlying conditions. This retrospective study aimed to assess the outcomes of using an internal joint stabilizer (IJS) to ensure post-release stability in these complex cases. HYPOTHESIS: IJS would demonstrate reliability as a treatment technique for severe elbow stiffness associated with intraope

Preserved finger flexion following high median nerve transection: a rare case report and review of literature.

BACKGROUND: High-level complete transection of the median nerve will impair the flexion function and sensation of the thumb and index finger, and will also result in weakened wrist flexion strength. In rare cases, atypical clinical manifestations may arise, potentially due to ulnar-to-median nerve anomalies, such as Marinacci communication (MC), or the function of muscles directly innervated by ulnar nerve branches. CASE PRESENTATION: A 52-year-old male sustained a chainsaw injury to his left elbow and forearm, resulting in complete transection of the median nerve. Despite this, he retained p

A Nonhuman Primate Model to Evaluate Treatments for Long-Gap Ulnar Nerve Injury.

Among all upper extremity nerves, the ulnar nerve is both the most commonly injured and notoriously difficult to regenerate. Despite this, ulnar nerve injuries remain understudied. Nonhuman primates (NHPs) offer an ideal model for the human upper extremity, but existing NHP nerve trauma literature is biased towards median and radial injury models. To address this, a nonhuman primate ulnar nerve injury model was developed and regeneration assessed following sural nerve autografting using electrophysiological and histological techniques. Unilateral 4 cm ulnar nerve injuries were created at the m

Effects of Forearm Rotational Movements on Median Nerve and Carpal Tunnel Morphology: A Magnetic Resonance Imaging Study.

PURPOSE: This study used magnetic resonance imaging (MRI) to investigate the effects of forearm pronation and supination on the morphology of the median nerve and carpal tunnel. MATERIALS AND METHODS: The MRI images of 177 patients were analyzed. Median nerve cross-sectional area (CSA) and carpal tunnel dimensions (height and width) were measured at four levels in pronation and supination. Statistical analyses evaluated positional differences. RESULTS: Median nerve CSA was significantly smaller, carpal tunnel height decreased, and width increased in supination compared with pronation. Pronat

Investigation of variations in the innervation of the thenar muscles by the median nerve.

INTRODUCTION: This study aims to investigate the anatomical variations in the median nerve and its branches, specifically focusing on their distribution to the muscles and the points at which they enter the muscles. In addition, the branching level of the ramus muscularis of the median nerve supplying the thenar muscles was examined to explore potential correlations with carpal tunnel syndrome. Understanding these variations is crucial for minimizing iatrogenic nerve damage during surgical procedures in the carpal tunnel region. METHODS: Dissections were conducted on 15 forearm and wrist spec

New Ultrasonographic Parameter for Diagnosing Carpal Tunnel Syndrome: The Median Nerve-to-Ulnar Artery Cross-Sectional Area Ratio.

Background and Objectives: Carpal Tunnel Syndrome (CTS) is the most common entrapment neuropathy. Ultrasonography is a widely used diagnostic method for CTS, and the median-to-ulnar nerve cross-sectional area ratio (MUR) is a well-known parameter. However, the ulnar nerve may be affected by various conditions of the median nerve; therefore, we considered finding further parameters. The aim of the study is to identify the correlation between the median nerve-to-ulnar artery cross-sectional area ratio (MUAR) and existing ultrasonographic parameters used as diagnostic indicators of CTS. The feasibility of the lateral antebrachial cutaneous nerve transfer for median sensory restoration: a cadaveric study.

PURPOSE: Median nerve injuries can result in substantial functional deficits with both motor and sensory components. Sensory loss results in impaired hand function. The most important areas are the ulnar and radial aspects of the thumb and index finger, respectively. This study aimed to evaluate the feasibility of nerve transfer from the lateral antebrachial cutaneous nerve (LACN) to the median nerve for restoring hand sensation. METHODS: This study used 16 cadaveric upper limbs. After dissecting the LACN and its branches, the length and size were recorded. The median nerve was identified via

Clinical Outcome of Endoscopically Assisted Mini-open Carpal Tunnel Release.

Background: Carpal tunnel syndrome (CTS) can be treated surgically. Although the minimally invasive open surgical method is widely used, it is not possible to directly visualise the entire length of the carpal tunnel, especially the proximal end, which is on the side away from the skin incision. In this study, we performed a mini-open carpal tunnel release with endoscopic assistance to release the entire length of the carpal tunnel under direct vision and investigated the treatment outcomes. Methods: The surgical method included an incision of ≤2 cm in the palm, cutting of the transverse carpa

Case Report: The unique case of flexible intramedullary nailing of pediatric radius complicated with temporary radial nerve's motor branch damage.

This study reported a case of radius flexible intramedullary nailing complicated by temporary paralysis of the posterior interosseous nerve due to compression of the ESIN on the nerve in an 8-year-old boy. The nerve damage resulted from an essential misconception at the surgery. Despite bad decisions made during qualifications and the procedure undertaken, restoring the nerve function, and gaining satisfactory functional fracture recovery was possible. Although it is generally acknowledged to perform retrograde flexible intramedullary nailing from the level of the distal radial metaphysis, the
