Second Metacarpal Index as a Predictor of Secondary Displacement in Conservatively Treated Distal Radius Fractures.

Background and Objectives: Distal radius fractures (DRFs) represent the most common fracture in the elderly population and are typically the first fractures to occur in the sequence of fragility fractures. Although fracture instability is an important criteria for guiding treatment, there is no universal consensus on how to define an unstable DRF. Given the demonstrated influence of bone quality on fracture stability, it has been hypothesized that second metacarpal index (2MCI) may also serve as a predictor of instability in DRFs. This study aimed to evaluate the predictive value of 2MCI for f

Simulated wrist fractures displace in well molded casts and splints under fall simulation forces: A cadaveric study.

BACKGROUND: Distal radius fractures are commonly treated with cast/splint immobilization, but variations exist in activity restrictions. METHODS: Fractures of the distal radius/ulna were created in cadaveric forearms. A force was applied through the long axis of the forearm simulating a ground-level fall. Each sample underwent testing in a short-arm well-molded fiberglass cast and a commercial fracture splint. Initial and final position lateral radiographs at 0 N and 400 N of force and dynamic fluoroscopy were taken for calculation of the primary outcome, change in volar tilt. Secondary outco

Cost-effectiveness of cast treatment vs. surgery in elderly patients with substantially displaced intra-articular distal radius fractures: A trial-based economic evaluation.

BACKGROUND: The number of surgical procedures for distal radius fractures in the elderly has increased, even though most studies show little or no benefit over cast treatment. While medical costs of surgical treatment are higher than those of cast treatment, surgery may enable faster recovery and help patients maintain independence, potentially reducing their use of other healthcare resources and informal care. METHODS: We evaluated whether cast treatment is cost-effective compared to surgery for patients aged 65 years or older with substantially displaced intra-articular distal radius fractu

A study of factors influencing distal radius fractures and predictive modeling in emergency department patients.

The aim of this study was to explore the independent risk factors for distal radius fractures in emergency department patients and to establish a corresponding prediction model based on these factors. We retrospectively collected 500 patients with distal radius fractures attending the emergency department of our hospital between January 2020 and December 2024 as an observation group, and selected 500 non-fracture patients attending the emergency department during the same period as a control group. Through univariate analysis and multifactorial logistic regression analysis, we identified indep

MFSD-YOLO: A multi-scale feature detection network for pediatric wrist abnormalities in radiographic images.

Timely and accurate diagnosis of wrist abnormalities, especially distal radius and ulna fractures, is critical in children and adolescents, yet complicated by skeletal immaturity, overlapping anatomy, and low-contrast fracture lines. To address these challenges, we propose MFSD-YOLO, a multi-scale detection model for pediatric wrist abnormality analysis. The model integrates a Cross-Stage Partial Progressive Multi-Scale Feature Aggregation (CSP_PMSFA) module inspired by GhostNet that applies lightweight multi-scale convolutions on partial channels with partial convolution and residual connecti

Inter-rater reliability of a classification systems for distal radius fractures using radiology text and x-rays: what really matters?

PURPOSE: This study used different metrics to assess the reliability of radiology text and images in Distal Radial Fractures (DRF) classifications using classifiers with varying levels of experience. METHODS: A random sample of 534 patients (16 + years) admitted to two major trauma centres for > 24 h for DRF management with 1,269 radiology images and radiology text reports were reviewed. Eight classifiers, with varying levels of experience, were randomly assigned patients, with overlap, to classify four different DRF classifications, nine radiological features and one treatment type: (two int

Addressing Bilateral Post-traumatic Distal Radius Growth Arrest with Positive Ulnar Variance with Acute Ulnar Shortening Osteotomy and Distal Radioulnar Joint Fixation in a 13-Year-Old Child: A Case Report.

INTRODUCTION: Pediatric forearm fractures are very common. The distal radius growth physis contributes the majority of the radial length and nearly half of the entire upper extremity. Although growth arrest associated with physeal fractures is rare but the sequelae can be very detrimental and challenging. Partial or complete distal radius physis arrest can lead to radial shortening, alteration of radial tilt or inclination, distal radioulnar joint (DRUJ) incongruity, ulnocarpal abutment syndrome, and injury to the triangular fibrocartilage complex. Post-traumatic distal radius physis arrest is

Outcome analysis of conservative treatment of a distal radius fracture with OPTIVOhand orthosis versus plaster cast: a randomized controlled trial.

BACKGROUND: Distal radius fractures (DRF) are often immobilized using a conventional plaster cast although orthoses offer a time- and resource-saving alternative. This prospective randomized study compared the conservative DRF treatment using an orthosis (OPTIVOhand®) with plaster cast immobilization. Besides the maintenance of reduction result (primary endpoint), functional and subjective outcomes (secondary endpoints) were examined. METHODS/ DESIGN: 53 patients with isolated DRF were randomized to the orthosis group (OG) or the control group (CG). The follow-up examinations included radiolo

EXTROS (EXtensor Repair & Transfer Outcome Score): Validation of a unified, percentage-based metric for extensor tendon repairs and transfers.

BACKGROUND: Outcome reporting for extensor tendon surgery is fragmented. Legacy systems-Miller for repairs and Bincaz for transfers-use different success definitions, limiting comparison. Building on the composite framework proposed by Jerome et al. (wrist arc, forearm rotation, MCP extension/flexion, first-web opening, grip/pinch; all normalized to the contralateral side), we adopted this construct as the EXTensor Repair & Transfer Outcome Score (EXTROS) and validated its measurement properties across repairs and transfers. METHODS: EXTROS summarizes domain percentages into an overall grade:

Correlation between fracture pattern and chronological age and skeletal maturity in distal radial fractures in children.

BACKGROUND: Distal radius fractures are common injuries in children, accounting for 20-30 % of all childhood fractures. Although fracture location is thought to reflect skeletal maturity in children, the precise relationship between skeletal maturity and fracture location remains unclear. This retrospective observational study of 83 pediatric patients with distal radius fractures aimed to investigate the relationships between chronological age, skeletal maturity, fracture location, and fracture type. METHODS: Patients were divided into two groups: those with physeal fractures (38 patients) an

Incidence proportion of complex regional pain syndrome (CRPS) after distal radius fracture: a population-based register study.

BACKGROUND AND PURPOSE: One of the most severe complications of distal radius fractures (DRF) is the development of complex regional pain syndrome (CRPS). The incidence proportion (IP) of CRPS following DRF varies widely in the literature. Our aim is to report the incidence proportion of CRPS in DRF patients, subgrouping on age, sex, and treatment choice, and secondarily to assess development over time. METHODS: Data was extracted from the Danish National Patient Register on patients > 18 years diagnosed with a DRF (S525) in the period 1998-2017. RESULTS: There were 247,128 DRF in 203,533 pa

Exploring the Feasibility of Ultrasound-Guided Supraclavicular Nerve Blocks for Distal Forearm Fracture Management in Pediatric Patients.

BACKGROUND: Forearm fractures are a common presentation to the emergency department (ED), frequently requiring reduction and immobilization. In the pediatric population, procedural sedation (PS) is the gold standard to allow for appropriate analgesia, patient compliance, and ultimately successful reduction. When clinical or patient-specific factors limit PS, a possible alternative for analgesia to facilitate reduction of forearm fractures in the ED is ultrasound-guided nerve blocks. This is especially significant for the pediatric population, given that nonsurgical treatment is frequently the

Have Commonly Utilised Patient-Reported Outcome Measures Been Validated for the Japanese Language and Culture Amongst Patients with Distal Radius Fractures? A Systematic Review.

Background: Patient-reported outcome measures (PROMs) are questionnaires used to evaluate a patient's functional status, examine outcomes and inform clinical decisions. PROMs have been primarily developed with English-speaking populations and should be validated in the patient population in whom they are being applied. As present, the validity of commonly utilised distal radius fracture (DRF) PROMs in patients who speak Japanese is unknown. The purpose was to evaluate the validity and psychometric properties of common DRF PROMs in the Japanese population. Methods: A systematic review was condu

Clinical practice for closed reduction of distal radius fractures in Denmark.

INTRODUCTION: Distal radius fractures (DRFs) are common, with over 14,000 annual cases in Denmark. Most cases are treated non-surgically and may require closed reduction prior to casting. National guidelines for anaesthesia and reduction technique for closed reduction of DRFs are lacking, impacting clinical practices and potentially optimal care. This survey aimed to describe Danish clinical practice for initial treatment and closed reduction of DRFs. METHODS: Between January and April 2025, the survey was distributed online to the 23 physicians responsible for all 44 emergency departments an

Rare pediatric Galeazzi lesion with progressive wrist deformity.

Galeazzi injuries are rare in early childhood and may be missed at initial presentation. Undiagnosed instability of the distal radioulnar joint can lead to progressive deformity and functional impairment. We report a rare case of Galeazzi injury in early childhood that led to progressive wrist malalignment during growth. Over the years this resulted in a severe restriction of forearm rotation. Using computed tomography-based three-dimensional analysis, a multiplanar corrective osteotomy was planned with reference to the contralateral healthy side. The procedure reduced the initial supination d

Should we remove flexible intramedullary nails after diaphyseal forearm pediatric fracture? A cohort study of 288 patients.

BACKGROUND AND PURPOSE: Routine removal of elastic stable intramedullary nails (ESIN) after pediatric diaphyseal forearm fractures remains common practice despite limited supporting evidence. We aimed to evaluate refracture risk following ESIN retention compared with implant removal and assessed complications and healthcare resource use. METHODS: In this retrospective cohort study, patients treated with ESIN for diaphyseal forearm fractures at a single center were included. Following a policy change, routine implant removal was discontinued in favor of retaining implants unless symptomatic. P

Atraumatic Distal Radius and Ulna Fracture Following An Electrocution in a 14 Years Old Child: A Case Report.

Fractures following a household electrical injury are uncommon, and it is even rarer to pick up fractures in patients who devoid a history of falls. The authors are presenting a case of a 14-year-old boy who had an electric shock at home and had continuous pain in his left wrist before presenting to us. He has sustained both distal radius and ulna uni-cortical break which were treated non-operatively. Household low-voltage electrical injuries resulting in fractures due to tetany are very uncommon. Having a uni-cortical break at both the distal radius and ulna with ventromedial displacement mak
