The burden of wrist joint dislocations: A nationwide 20-year analysis and review of incidence and injury mechanism.

BACKGROUND: Wrist dislocations are rare injuries that present substantial diagnostic challenges, yet large-scale epidemiological data on injury mechanisms and trends are limited. This study characterizes wrist dislocation epidemiology using nationally representative emergency department data from 2004 to 2023. METHODS: A retrospective analysis was performed using the National Electronic Injury Surveillance System (NEISS) database to estimate national wrist dislocation trends. Cases were identified by diagnosis and body part codes. Demographics, mechanisms, and annual incidence were examined.

Update on peripheral nerve injuries in Germany 2019-2023.

INTRODUCTION: Fractures and soft tissue damage are the main causes for traumatic peripheral nerve injuries. Discontinuity of peripheral nerve after injury results in loss of motor function or sensation or both combined. This is often associated with debilitating consequences for the affected person. Current data on the epidemiology of peripheral nerve injuries in Germany are scarce. MATERIAL AND METHODS: In a non-interventional retrospective population-based cohort study (registry-based), publicly available, anonymized patient data (2019-2023) were analyzed with respect to traumatic periphera

Patient-Related Factors Associated With Scaphoid Proximal Pole Avascular Necrosis Healing.

BACKGROUND: Avascular necrosis (AVN) of the proximal pole is a well-known complication of scaphoid fractures. Avascular necrosis is poorly understood, including the transition from ischemia to necrosis, optimal treatment, and why some AVN heals but others do not. The primary purpose of this study is to evaluate patient-related factors that are associated with healing outcomes in individuals with proximal pole AVN following scaphoid fractures. METHODS: This is a retrospective review of all patients diagnosed with scaphoid proximal pole AVN secondary to a fracture from 2018 to 2024 in a single

Second Metacarpal Index as a Predictor of Secondary Displacement in Conservatively Treated Distal Radius Fractures.

Background and Objectives: Distal radius fractures (DRFs) represent the most common fracture in the elderly population and are typically the first fractures to occur in the sequence of fragility fractures. Although fracture instability is an important criteria for guiding treatment, there is no universal consensus on how to define an unstable DRF. Given the demonstrated influence of bone quality on fracture stability, it has been hypothesized that second metacarpal index (2MCI) may also serve as a predictor of instability in DRFs. This study aimed to evaluate the predictive value of 2MCI for f

Do Patients Older Than 40 Years with Work Claims for Unilateral Wrist Symptoms Have Symmetric, Bilateral MRI Signal Changes That Do Not Correspond with Symptoms?

PURPOSE: New symptoms from established idiopathic or senescent pathophysiology are often misinterpreted as an injury (damage to tissues by an external force). Misinterpretation of age-related imaging abnormalities as damage from an external force introduces potential for overdiagnosis, overtreatment, overprotection, and misplacement of a condition under work insurance. Evidence from the shoulder and knee suggests that awareness of the bilateral nature of many idiopathic and senescent pathophysiologies can limit erroneous diagnosis of traumatic pathophysiology. METHODS: Sixty-four scrutinized

Double Dislocation of Proximal and Distal Interphalangeal Joints With Volar Plate Avulsion Fracture in the Fifth Digit of an Elderly Female: Successful Conservative Management.

A 77-year-old, right-hand-dominant woman presented after a low-energy fall with dorsal displacement involving the proximal and distal interphalangeal joints of the right small finger, along with a volar plate avulsion fracture at the base of the middle phalanx. Closed reduction was performed without anesthesia, followed by static aluminum splinting and occupational therapy. At her two-month follow-up, she reported minimal pain, improved function, and maintained joint alignment. This case highlights successful conservative management of a rare double interphalangeal dislocation with associated

Traumatic and Overuse Wrist Injuries: Osseous and Tendon Pathologies.

Traumatic and overuse injuries of bones and tendons of the wrist represent a significant proportion of the musculoskeletal disorders encountered in both athletic and occupational settings. Conventional radiography remains the first-line imaging modality for osseous trauma, but it can miss up to a third of scaphoid and hamate fractures. Computed tomography is superior for characterizing occult or nondisplaced fractures. Magnetic resonance imaging provides important information on occult fractures, overuse injuries of osseous structures, and tendinous abnormalities.A comprehensive understanding

Imaging for Scapholunate Ligament Injuries: Techniques, Challenges, and Innovations.

Scapholunate (SL) ligament injuries are a common cause of wrist pain and instability but can be diagnostically challenging. Standard radiographs, ultrasound, computed tomography, and magnetic resonance imaging have been used for many years to assist in the diagnosis of these injuries, and all of them have benefits and limitations. Advanced dynamic modalities, such as real-time magnetic resonance imaging and four-dimensional computed tomography, can distinguish functional from static injuries but are not widely available. The emergence of artificial intelligence applications is promising to imp

The Effect of Scapholunate Interval and Angle on Radiocarpal Joint Loading.

INTRODUCTION: Carpal instability related to scapholunate ligament injury can manifest as an increased scapholunate (SL) interval and/or angle. Radiocarpal arthrosis is expected with chronic carpal instability due to an imbalance of normal joint loading. This study seeks to quantify the effect of an increased SL interval and angle on joint loading properties. METHODS: Radioscaphoid (RS) and radiolunate (RL) contact area and pressure were collected in 13 cadaveric specimens tested under the following conditions (#): normal SL interval and SL angle (1), increased interval only (2), increased ang

Temporal Changes in Finger and Upper Limb Usage During Hospitalization After Distal Radius Fracture Surgery: A Case Report.

Recovery after distal radius fracture surgery is commonly evaluated using physical function measures; however, actual upper limb use in daily life may not be fully captured by these assessments. In this case report, a postoperative distal radius fracture patient was monitored during hospitalization using a ring-shaped wearable device to quantitatively assess finger and upper limb usage, together with patient-reported outcomes and physical function measures. Finger and upper limb usage ratios increased during the mid-postoperative period, accompanied by reductions in swelling and improvements i

Outcome analysis of conservative treatment of a distal radius fracture with OPTIVOhand orthosis versus plaster cast: a randomized controlled trial.

BACKGROUND: Distal radius fractures (DRF) are often immobilized using a conventional plaster cast although orthoses offer a time- and resource-saving alternative. This prospective randomized study compared the conservative DRF treatment using an orthosis (OPTIVOhand®) with plaster cast immobilization. Besides the maintenance of reduction result (primary endpoint), functional and subjective outcomes (secondary endpoints) were examined. METHODS/ DESIGN: 53 patients with isolated DRF were randomized to the orthosis group (OG) or the control group (CG). The follow-up examinations included radiolo

More Frequent Electrical Stimulation Impairs Nerve Regeneration After Median Nerve Repair in a Rat Model.

BACKGROUND: Peripheral nerve regeneration via electrical stimulation on nerve repair has been shown to potentiate axonal growth and functional recovery. However, it remains unknown whether increasing the frequency of electrical stimulation improves, diminishes, or has no effect on the quality of nerve regeneration after repair. Additionally, few animal studies have investigated the potential beneficial effects of serial stimulations on accelerating and improving end-organ functionality over time. We therefore sought to evaluate the additive effect of weekly versus biweekly serial electrical st

PROMIS computerized adaptive testing is effective and efficient for outcome assessment in patients with wrist or hand fractures.

INTRODUCTION: Various patient-reported outcome measures (PROMs) are available to assess functional outcomes in patients treated for wrist-hand fractures. Among the most frequently applied instruments are the Patient-Rated Wrist/Hand Evaluation (PRWHE), Quick Disabilities of the Arm, Shoulder and Hand and Short Musculoskeletal Function Assessment. Computerized adaptive tests (CATs) utilizing the PROM-Information System (PROMIS) offers a standardized alternative. This study compared the psychometric properties of PROMIS CATs with these three legacy instruments and assessed independent factors as

Enhancing hand motor recovery in neonatal total brachial plexus palsy with a novel surgical approach.

Neonatal total brachial plexus palsy (NBPP) is one of the most challenging birth injuries, especially if it is associated with hand palsy and sensory loss. Current techniques offer limited success for both extrinsic and intrinsic hand muscles recovery, especially for the latter. We present an innovative combined surgical method designed to reinnervate the hand in complete palsies: (1) the ulnar nerve of the affected limb, which is insensate and nonfunctional, is harvested as an autologous graft and used to connect the contralateral C7 root to the medial (motor) trunk contribution to the median

Cartilage damage in patients with scapholunate lesions: arthroscopic prevalence, location and associated clinical factors.

INTRODUCTION: Scapholunate ligament (SLL) injuries can lead to instability and osteoarthritis through stages of scapholunate dissociation and scapholunate advanced collapse. Although cartilage damage is traditionally seen as a late finding, recent evidence suggests that it may occur earlier. This study aimed to assess the prevalence and distribution of cartilage damage in patients with isolated SLL lesions confirmed by arthroscopy and to explore associated sociodemographic and clinical factors. METHODS: A retrospective cohort study was conducted in patients with arthroscopically confirmed SLL

A Comparative Study Between Denervation and Extensor Release for Management of Resistant Tennis Elbow.

BACKGROUND: Extensor tendon release for tennis elbow is fraught with complications as persistence of pain because of adhesions together with weakness of wrist extension. Denervation of the epicondylar region through division of branches of posterior cutaneous nerve of the forearm (PCNF) was proposed to alleviate pain without drawbacks of tendon release. METHODS: The first group included 19 patients treated through dividing PCNF. The second group included 23 patients treated through tendon release. Inclusion criteria included symptoms for 6 months or longer. Clinical diagnosis was based on pos
