A Biomechanical Analysis of Oblique Metacarpal Metadiaphyseal Fracture Fixation in a Cadaver Model.

PURPOSE: Although metacarpal fractures are typically managed nonoperatively, when surgical management is indicated, metacarpal fractures are commonly treated with crossed Kirschner wires (K-wires), which may limit early range of motion. Intramedullary implants are increasing in use with the potential advantage of early range of motion; however, stability in oblique metacarpal neck fractures remains a theoretical concern. The purpose of this study was to determine the biomechanical stability of noncompressive intramedullary fixation for oblique metacarpal neck fractures compared with crossed K-

Non-traditional Surgical Treatment of a Rolando Fracture.

Rolando fractures are comminuted, multifragmented, complete intra-articular fractures at the base of the thumb metacarpal. A review of the literature and our clinical practice shows that it can be treated in many ways. Patients with a non-displaced fracture require immobilization of the thumb. Nonoperative treatment is rare. Displaced fractures should be treated surgically. The primary goal of Rolando fracture treatment is to achieve anatomical reduction, ensuring that the fractured bones realign in their original anatomical position. As described in the article, the procedure of implanting an

Biomechanical evaluation of spiral fracture in human metacarpal using chicken humerus model.

INTRODUCTION: Animal bones have been used as a model to mimic human bones for investigation of fracture patterns and fixation techniques. This study aims to investigate the effects of torsional forces of varying speeds on the spiral fracture in human metacarpal using a chicken humerus model. MATERIALS AND METHODS: Thirty fresh-frozen chicken humerus bones were dissected and equally divided into three groups. The bones were mounted onto a customized jig and subjected to different torsional loading rate at 22.5°/s, 30°/s and 45°/s, respectively. The fracture pattern, angle and length were analy

HarDNet-based deep learning model for osteoporosis screening and bone mineral density inference from hand radiographs.

PURPOSE: Osteoporosis, affecting over 200 million individuals, often remains unrecognized and untreated, increasing the risk of fractures in older adults. Osteoporosis is typically diagnosed with bone mineral density (BMD) measured by dual-energy X-ray absorptiometry (DXA). This study aims to develop DeepDXA-Hand, a deep learning model using the efficient CNN-based deep learning architecture, for opportunistic osteoporosis screening from hand radiographs. METHODS: DeepDXA-Hand utilizes a CNN-based, HarDNet, approach to predict BMD non-invasively. A total of 10,351 hand radiographs and DXA pai

Carpal Boss: A Case Series of a Radiological Enigma in Dorsal Wrist Pathology.

Carpal boss, a relatively rare and often underdiagnosed condition, is characterized by a bony protuberance at the dorsal aspect of the carpometacarpal (CMC) joint. It is commonly misdiagnosed due to its nonspecific clinical presentation, which can mimic other conditions, such as ganglion cysts or fractures. This case series aims to document and analyze the clinical presentations and radiological findings of three patients diagnosed with carpal boss, highlighting the importance of advanced imaging techniques in accurate diagnosis and management. A case series was conducted at Tenet Diagnostics,

Roles of collagen cross-links and osteon collagen/lamellar morphotypes in equine third metacarpals in tension and compression tests.

Many bones experience bending, placing one side in net compression and the other in net tension. Because bone mechanical properties are relatively reduced in tension compared with compression, adaptations are needed to reduce fracture risk. Several toughening mechanisms exist in bone, yet little is known of the influences of secondary osteon collagen/lamellar 'morphotypes' and potential interplay with intermolecular collagen cross-links (CCLs) in prevalent/predominant tension- and compression-loaded regions. Paired third metacarpals (MC3s) from 10 adult horses were prepared for mechanical test

Biomechanical analysis of plate versus K-wire fixation for metacarpal shaft fractures with wedge-shaped bone defects.

BACKGROUND: Metacarpal shaft fracture is a common type of hand fracture. Numerous studies have explored fixing transverse fractures in the midshaft of the metacarpal bone. However, this section of the metacarpal bone is often susceptible to high-energy injury, resulting in comminuted fracture or bone loss. In such cases, wedge-shaped bone defects can develop in the metacarpal shaft, increasing the difficulty of performing fracture fixation. Notably, the research on this type of fracture fixation is limited. This study compared the abilities of four fixation methods to fix metacarpal shaft frac

Recurrent Pediatric Thumb Carpometacarpal Joint Dislocation Due to Generalized Joint Laxity Successfully Treated With Ligament Reconstruction: A Case Report.

Acute traumatic dislocation without fractures of the thumb carpometacarpal (CMC) joint is extremely rare in children. Treatment options, such as closed reduction with casting or pinning and open reduction with primary ligament repair, remain controversial. Here, we report the first case of an 11-year-old boy with recurrent left thumb CMC joint dislocation due to idiopathic generalized hyperjoint laxity, even after primary open reduction with capsular ligament repair of the thumb CMC joint, eventually treated with Eaton-Littler's ligament reconstruction. Intraoperatively, a drill hole was made

Prediction models incorporating second metacarpal cortical index for osteoporosis in rheumatoid arthritis: Externally validated machine learning models developed using data from the KURAMA cohort.

OBJECTIVE: Osteoporosis and osteopenia are significant concerns in rheumatoid arthritis (RA), predisposing patients to fragility fractures. While dual-energy X-ray absorptiometry (DXA) is the gold standard for bone mineral density (BMD) assessment, simpler screening tools are needed. This study aims to assess the correlation between the second metacarpal cortical index (2MCI) and BMD in RA patients, and to evaluate machine learning (ML) models utilizing 2MCI and clinical parameters for predicting osteoporosis/osteopenia presence and BMD. METHODS: Data from the KURAMA cohort (n = 302) and an e
