Modified UNet-enhanced ultrasonic superb microvascular imaging feature extraction and grading of carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) is recognized as the most frequently encountered median nerve (MN) entrapment neuropathy, with a disproportionate burden in middle-aged and elderly individuals and in occupational groups with repetitive wrist use. Anatomically, CTS is characterized by compression of the median nerve within the confined space between the transverse carpal ligament and flexor tendons, and microcirculatory impairment is regarded as one of its key pathological bases. Although electrodiagnostic assessments are considered as diagnostic gold standard, their utility is limited by suboptima

Investigation of variations in the innervation of the thenar muscles by the median nerve.

INTRODUCTION: This study aims to investigate the anatomical variations in the median nerve and its branches, specifically focusing on their distribution to the muscles and the points at which they enter the muscles. In addition, the branching level of the ramus muscularis of the median nerve supplying the thenar muscles was examined to explore potential correlations with carpal tunnel syndrome. Understanding these variations is crucial for minimizing iatrogenic nerve damage during surgical procedures in the carpal tunnel region. METHODS: Dissections were conducted on 15 forearm and wrist spec

Unraveling the causality between carpal tunnel syndrome and trigger finger through genetic instrumental variables.

OBJECTIVE: Carpal tunnel syndrome (CTS) and trigger finger (TF) are the two most common non-traumatic connective tissue disorders of the hands. These conditions often manifest concurrently in certain patients; however, the mechanisms underlying this phenotypic association remain inadequately understood. In this study, we aimed to systematically investigate the causal relationship between CTS and TF using a large sample size and advanced methods. METHOD: In this study, comprehensive summary-level data on CTS and TF from genome-wide association studies (GWAS) were collected. The causal relation

First Clinical Evaluation of a Modular Lumboperitoneal Shunt System With a Gravitational Unit for Communicating Hydrocephalus: A Retrospective Case Series.

BACKGROUND AND OBJECTIVES: In Europe and North America, communicating hydrocephalus (HC) is commonly treated with ventriculoperitoneal shunts, whereas lumboperitoneal shunts (LPS) are preferred in Japan and other Asian countries. However, no LPS valve with an integrated gravitational unit had been available until recently, leaving overdrainage as a persistent complication. The objective of this study was to report initial clinical experiences with a novel modular LPS valve system that incorporates a gravitational unit and to assess its safety, feasibility, and early clinical outcomes across va

Efficacy of Low-Dose Versus High-Dose Corticosteroid Injections for Soft Tissue Pathology of the Hand.

PURPOSE: This retrospective study evaluated the efficacy of high- versus low-triamcinolone injection doses to guide dosing for injections used in hand soft tissue pathologies. We hypothesized that there would be difference in efficacy between high-dose injections (40 mg) and low-dose injections (5-10 mg) used for trigger finger, carpal tunnel syndrome, and de Quervain tenosynovitis. The study compared rate of disease resolution, need for secondary injection, and conversion to surgery between low-dose and high-dose injections. METHODS: A retrospective chart review of patients from 2020 to 2023

Perioperative Opioid Prescriptions in Patients on Oral Anticoagulants Undergoing Elective Soft Tissue Hand Surgery.

PURPOSE: Oral anticoagulation use is increasing in prevalence in the hand surgery population and poses a challenge for perioperative pain management, given the relative contraindication of nonsteroidal anti-inflammatory drug use. Anticoagulated patients may be prescribed opioids after hand surgery, leading to opioid use-related risks. In this study, we tested the hypothesis that preoperative oral anticoagulant use was associated with the incidence of perioperative opioid prescriptions in opioid-naive patients undergoing elective soft tissue hand surgery. METHODS: We identified opioid-naive ad

[Correlation of the Boston Scale with findings in neuroconductive studies in patients with carpal tunnel syndrome].

INTRODUCTION: carpal tunnel syndrome (CTS) is the most common peripheral nerve entrapment disorder. Electrodiagnostic studies are useful for confirming the diagnosis, as they reveal focal demyelinating involvement predominantly affecting sensation. However, their correlation with symptoms is not always adequate, and there are cases of discrepancy between the study results and the patient's clinical presentation. The objective was to analyze the correlation between the findings in median nerve conduction studies and the severity of symptoms on the Boston scale. MATERIAL AND METHODS: an observa

Modified Camitz versus flexor digitorum superficialis of the fourth finger opponensplasty in severe carpal tunnel syndrome: A systematic review.

BACKGROUND: Several opponensplasty techniques have been employed to address thenar muscle atrophy associated with severe carpal tunnel syndrome (CTS). Although modified Camitz opponensplasty (MCO) using the palmaris longus (PL) tendon is widely used by hand surgeons, alternative tendon transfer donors have been proposed, such as opponensplasty using the flexor digitorum superficialis of the ring finger (OF4). However, it is unclear which surgical technique provides superior outcomes. Hence, this systematic review aims to examine the difference in functional outcomes between MCO and OF4 for res

The safety and cost of repeated corticosteroid injections for carpal tunnel syndrome.

We studied the safety and cost of repeated local steroid injections for carpal tunnel syndrome. Medical records of patients treated by injection between 2016-2017 were retrospectively reviewed for 5 years. Two-hundred and twenty-one patients were included. The number of injections, the reason for surgery and any complications were recorded. A regression analysis was done for predictors of carpal tunnel release. The costs of the treatments were calculated. The mean number of injections was 2.3 (SD2.3; range 1-26). Two patients reported severe pain after the first injection. Thirty per cent of w

New Ultrasonographic Parameter for Diagnosing Carpal Tunnel Syndrome: The Median Nerve-to-Ulnar Artery Cross-Sectional Area Ratio.

Background and Objectives: Carpal Tunnel Syndrome (CTS) is the most common entrapment neuropathy. Ultrasonography is a widely used diagnostic method for CTS, and the median-to-ulnar nerve cross-sectional area ratio (MUR) is a well-known parameter. However, the ulnar nerve may be affected by various conditions of the median nerve; therefore, we considered finding further parameters. The aim of the study is to identify the correlation between the median nerve-to-ulnar artery cross-sectional area ratio (MUAR) and existing ultrasonographic parameters used as diagnostic indicators of CTS. Materials

[Diagnosis of Amyloidosis in Hand and Plastic Surgery Patients: Optimal Tissue Selection and Examination Methods for Early Detection].

Amyloidoses are a heterogeneous group of diseases characterised by the fibrillar deposition of peptides and proteins in a β-sheet configuration. In Germany, AL and ATTR amyloidosis are among the most common systemic forms. ATTR amyloidosis is characterised by the involvement of ligamentous and tenosynovial tissue prior to cardiac involvement, which, in turn, can be be life-limiting. This highlights the growing clinical relevance of diagnosing ATTR amyloid in resection specimens from hand and plastic surgery procedures. Diseases considered relevant in this context include carpal tunnel syndrome

Outcomes of Opponensplasty with Transfer of the Palmaris Longus Tendon to the Rerouted Extensor Pollicis Brevis Tendon for Severe Carpal Tunnel Syndrome.

Background: Camitz opponensplasty is a widely used reconstruction procedure for severe carpal tunnel syndrome. However, it has several drawbacks, including poor pronation during thumb opposition and a tendency for flexion at the thumb metacarpophalangeal (MP) joint. We developed a novel procedure to overcome these issues while preserving its advantages. Methods: Our procedure involves mini open carpal tunnel release (KnifeLight) and the transfer of the palmaris longus (PL) tendon to the rerouted extensor pollicis brevis (EPB) tendon using the distal part of the PL tendon as a pulley. We assess

Impact of carpal tunnel syndrome on multidigit synergies and anticipatory synergy adjustments.

This study investigates the impact of Carpal Tunnel Syndrome (CTS) on multidigit synergies and anticipatory synergy adjustments (ASA) during isometric force production tasks, using the Uncontrolled Manifold (UCM) framework. CTS, characterized by sensory feedback and motor impairment due to compression of the median nerve at the hand's carpal tunnel, was hypothesized to: 1) reduce the synergy index (normalized difference between within-manifold variance and task-relevant variance) due to increased task-relevant variability (VORT, variance affecting task performance) while preserving within-mani

Prevalence of amyloid deposits in synovium and flexor retinaculum, risk factors and asymptomatic cardiac involvement in patients with carpal tunnel syndrome.

INTRODUCTION: Amyloid deposition in the tenosynovium and transverse carpal ligament, and particularly cardiac involvement, are rare in patients with carpal tunnel syndrome; however, the prevalence may be underreported. This prospective study assessed the prevalence of amyloid deposits in the synovium and transverse carpal ligament and whether dual-site sampling improved detection. Risk factors for a positive test and asymptomatic cardiac involvement were also investigated. METHODS: In a prospective cohort of 100 consecutive patients undergoing carpal tunnel release, intraoperative biopsies we
