Trigger finger and carpal tunnel syndrome: a meta-analysis of co-occurrence and risk.

Carpal tunnel syndrome (CTS) and trigger finger (TF) are two of the most common conditions of the hand. Co-occurrence is mentioned by a number of publications, but there are few high-level evidence studies that summarise the results of large databases to quantify the chances of concurrency in detail making it difficult to gauge clinical significance. We aimed to quantify the impact of CTS, TF and surgery for these conditions on the prevalence of the other condition. Systematic review based on PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, with registrat

Efficacy of dry needling with the fascial winding technique in reducing the percentage of surgery in carpal tunnel syndrome: A randomized clinical trial.

INTRODUCTION: Numerous techniques for the treatment of carpal tunnel syndrome (CTS) have been described. The present study aims to determine the proportion of wrists with CTS in which the application of DN avoids surgery. METHODS: An assessor-blind randomized clinical trial clinical trial was performed. Eighty-six wrists with diagnosis of mild or moderate CTS and indication for surgery were recruited from a hospital traumatology service. The intervention group was treated with DN, with a frequency of one session/week over six weeks, while the control group did not follow any specific treatmen

The effectiveness of single-dose blinded platelet-rich plasma (PRP) vs. corticosteroid injection in mild-to-moderate carpal tunnel syndrome: A prospective, randomized, double blind study.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and patients suffer from nocturnal paresthesia, numbness, and weakness. The many different methods used in the treatment of CTS include physical therapy modalities, oral medication, exercises, or injections. However, no single method has been shown to be superior to others. The aim of this study was to evaluate the effectiveness in the relief of CTS symptoms of single-dose platelet-rich plasma injection (PRP) vs. corticosteorid (CS) injection. MATERIAL AND METHODS: The study included 78 wrists of 59 female patie

Carpal tunnel release surgery restores force control but not force coordination in CTS patients.

Efficient manipulation of objects is crucial for daily interactions with the world. Compression neuropathies like carpal tunnel syndrome (CTS) can have detrimental effects on hand function, which can be improved by surgical intervention. Twelve patients with CTS grasped and lifted an instrumented handle while maintaining static equilibrium under different torque conditions. We compared fingertip forces and grip force variability across trials before and after surgery. Additionally, we evaluated the coordination of fingertip forces using a metric defined as synergy index to determine if there w

New-Onset Dupuytren After Surgical Treatment of Carpal Tunnel and Trigger Finger.

BACKGROUND: Dupuytren disease is a progressive fibrotic condition of the hand that often leads to contractures. Although its etiology remains multifactorial, recent studies suggest potential associations with surgical interventions for carpal tunnel syndrome (CTS) and trigger finger (TF). Understanding the incidence and risk factors for Dupuytren disease development following these procedures may improve postoperative management and early detection. METHODS: A retrospective study was conducted on 426 patients who underwent surgical treatment for CTS or TF. The incidence of Dupuytren disease d

Common Occupational Upper Extremity Musculoskeletal Disorders.

Family physicians play a valuable role in the diagnosis, management, and treatment of musculoskeletal disorders that result from occupational exposures, including overexertion, repetitive motion, and vibration. Carpal tunnel syndrome is a common peripheral nerve entrapment syndrome often identified in workers whose jobs require repetitive hand motion. Treatment of carpal tunnel syndrome includes splinting, rehabilitation exercises, and oral or injected corticosteroids; surgical release is considered in patients for whom conservative treatment has failed or who have severe symptoms. Lateral epi

Diagnosis of carpal tunnel syndrome with ultrasound: should we go more distal?

OBJECTIVES: To assess the value of adding the ultradistal level to other more thoroughly studied levels of the carpal tunnel when measuring the cross-sectional area (CSA) of the median nerve (MN) by ultrasound (US) in diagnosing patients with primary carpal tunnel syndrome (CTS). METHODS: Patients clinically diagnosed with primary CTS and healthy controls were included. The MN-CSA was measured by US at three wrist levels: proximal, distal and ultradistal. The best cut-off to differentiate cases and controls was determined for the CSA and for the difference between levels of the same wrist. Th

Predictors of Patient Migration for Staged Bilateral Carpal Tunnel Release Procedures.

PURPOSE: Staged bilateral carpal tunnel release (CTR) is a common surgical treatment for patients with bilateral carpal tunnel syndrome refractory to nonsurgical measures. Factors influencing whether a patient undergoes surgery on both hands by the same or different surgeons are not well characterized. This study aimed to identify patient and clinical factors associated with patient migration during staged bilateral CTR within 90 days of the index procedure. METHODS: Adult patients who underwent CTR between 2015 and 2022 were identified using the PearlDiver Mariner Database. Differences in pa

Pyogenic Flexor Tenosynovitis of the Finger Following Endoscopic Carpal Tunnel Release: A Case Report.

CASE: A 60-year-old woman presented to the emergency department with hand pain and edema 1 week following endoscopic carpal tunnel release (CTR). She was diagnosed with pyogenic flexor tenosynovitis, which was successfully treated with surgical drainage and intravenous antibiotics, resulting in no long-term side effects. CONCLUSION: This case highlights the first reported instance of pyogenic flexor tenosynovitis as a complication after endoscopic CTR. Although this relatively safe procedure carries a low infection risk, prophylactic antibiotics should be considered for high-risk patients. In

Ultrasonographic Assessment of Median Nerve and Carpal Tunnel Variations: Implications for the Interventional Management of Carpal Tunnel Syndrome.

OBJECTIVES: Carpal tunnel syndrome (CTS) is the most common peripheral entrapment neuropathy. Anatomic variations of the median nerve (MN) and carpal tunnel (CT) may predispose patients to CTS or increase procedural complexity and risk during carpal tunnel release (CTR). This study aimed to systematically evaluate the prevalence of 25 MN and CT anatomic variations and measurements using ultrasound (US) in patients referred for electrodiagnostic testing (EDX). METHODS: In this cross-sectional observational study, patients underwent diagnostic US exams of the MN (mid-brachium to distal CT), foc

Dynamic Analysis of Ultrasound Images of the Median Nerve Using Particle Image Velocimetry.

Carpal tunnel syndrome (CTS) diagnosis is typically confirmed through electrophysiologic studies and ultrasound (US) imaging. Recent studies have suggested that median nerve gliding is reduced in patients with CTS. This study employed particle image velocimetry (PIV) to analyze median nerve dynamics. Ultrasound movies were captured during finger flexion and extension for 20 healthy volunteers (control group) and 20 CTS patients (CTS group). These movies were then analyzed using the PIV method. Within a 10 x 10 mm² region of interest (ROI) that included the median nerve and flexor tendon, the v

Incidence of Posttraumatic Carpal Tunnel Syndrome After Perilunate Injuries.

BACKGROUND: Perilunate injuries are a spectrum of high-energy carpus injuries with significant long-term sequelae and generally poor outcomes. Posttraumatic carpal tunnel syndrome (CTS) is an established sequela of these injuries, although its incidence varies widely in literature. This study aims to evaluate the risk of posttraumatic CTS after these injuries and contrast between nonoperative and operative perilunate injuries. METHODS: The TriNetX database was queried for all patients treated for either closed or open management of a perilunate injury between January 1, 2010, and December 31,
