Prevalence of carpal tunnel syndrome and its associated factors among patients with musculoskeletal complaints: a single-center experience from Eastern Ethiopia.

BACKGROUND: Carpal tunnel syndrome (CTS) is a well-known chronic focal mononeuropathy caused by the mechanical distortion of the nerve at the carpal tunnel. It affects approximately 5% of the population worldwide and 12.1% in Africa. However, it has severe pain that causes discomfort and disturbs daily activities, the magnitude and its risk factors are poorly understood in Ethiopia. OBJECTIVES: This study aimed to assess the prevalence and associated factors of carpal tunnel syndrome among patients with musculoskeletal complaints in Dire Dawa, Eastern Ethiopia. METHODOLOGY: An institution-ba

Ultrasound-guided release of the fibro-osseous tunnels around the wrist and hand: a technical review.

Fibro-osseous tunnels allow the unhindered passage of the neurovascular bundle and sliding tendons across the mobile joint. Release of the fibro-osseous tunnels is performed for entrapment neuropathy and stenosing tenosynovitis which occur as a result of increase in the volume/pressure within these tunnels. The common surgical principle in all these pathologies is releasing the pressure by transecting the fascial covering. Primarily these pathologies are treated with open surgery but are potentially amenable to curative percutaneous ultrasound (US)-guided release procedures. US is the preferab

Hybrid-J shape needle in ultrasound-guided looped thread carpal tunnel release: a cadaveric study on safety and efficacy.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most prevalent peripheral nerve entrapment disorder, often requiring surgical intervention. While Ultrasound-guided Looped Thread Carpal Tunnel Release (LTCTR) offers a minimally invasive alternative to carpal tunnel release, its implementation faces limitations due to insufficient specialized instruments. This cadaveric study assesses the Hybrid-J Shape Needle (HJSN), a blunt-tip, J-shaped device designed to improve safety in LTCTR procedures. METHODS: Twenty fresh-frozen cadaveric specimens (40 wrists) were included. Ten junior surgeons were r

Carpal tunnel dimensions following osteopathic manipulation utilizing dorsal carpal arch muscle energy: a pilot study.

CONTEXT: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy. When mild to moderate in severity, nonoperative treatments including osteopathic manipulative treatment (OMT) have been found to be effective. Studies have been carried out to quantify the mechanism of such treatments with cadaver studies, magnetic resonance imaging (MRI), and ultrasound. OBJECTIVES: This pilot project investigated the mechanism of a previously undescribed technique of nonoperative carpal tunnel treatment, dorsal carpal arch muscle energy (DCA-ME), which focuses on the dorsal arch (trapezium, trap

Effect of anthropometric measurements of the hand on the development of carpal tunnel syndrome in female patients.

This study aimed to evaluate whether anthropometric measurements and body mass index (BMI) are associated with the occurrence of carpal tunnel syndrome (CTS) in female patients. This cross-sectional, observational, case-control study included 100 women with symptoms of CTS and electrophysiological evidence of median nerve entrapment at the level of the carpal tunnel. 100 volunteers without symptoms of CTS and with normal electrophysiological results were included in the study as a control group. The external dimensions of the extended hand were measured in both groups. Hand ratio (HR), wrist r

Revealing the causal relationship between cathepsin and inflammatory disease of wrist through Mendelian randomization.

Although observational studies have suggested associations between cathepsins and inflammatory wrist diseases, critical controversies persist regarding the causality, directionality, and disease-specific mechanisms of these relationships. Conventional clinical studies are unable to determine whether cathepsin-disease associations reflect true causality or spurious links driven by confounding factors or reverse causation. Moreover, systematic causal analyses focusing on protease-mediated mechanisms in wrist-specific inflammatory pathologies remain notably absent. To address these gaps, this stu

Effects of electroacupuncture on carpal tunnel syndrome: a clinical, electrophysiological and ultrasonographical pilot study.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common compressive neuropathy and is associated with significant strain on both patients and healthcare systems. Acupuncture is one of the conservative treatments used for this syndrome, although more evidence is needed regarding the efficacy of this intervention. The aim of this pilot study was to evaluate the effects of electroacupuncture (EA) in patients with CTS through clinical, electrophysiological and ultrasonographical assessments. METHODS: Seventeen wrists, belonging to a total of 12 patients who had been diagnosed with CTS, were i

Secondary Carpal Tunnel Release After Surgical Fixation of the Distal Radius: A Case-Control Study.

BACKGROUND: Carpal tunnel syndrome (CTS) may occur after surgical fixation of a distal radius fracture (DRF) and can result in muscle atrophy and permanent nerve damage if untreated. Despite increased surgical treatments for DRFs, the frequency and risk factors for secondary carpal tunnel release (CTR) after DRF fixation remain unclear. This study aims to identify the frequency of secondary CTR after DRF fixation and factors associated with secondary release. METHODS: Adult patients who underwent either a secondary CTR within 1 year after surgical fixation of the DRF (secondary release case g

Elastography findings of thenar muscles and the median nerve in patients with carpal tunnel syndrome.

AIM: Electrodiagnostic testing (EDT) is a preferred invasive diagnostic approach because it provides the most accurate results for patients with carpal tunnel syndrome (CTS). However, researchers are now trying to develop noninvasive techniques for diagnosis. The aim of this study was to detect alterations associated with muscular atrophy and hardening in the thenar muscles of individuals with CTS using shear wave elastography (SWE). MATERIALS AND METHODS: The study was conducted on 113 hands and wrists of 77 individuals who were diagnosed with CTS using EDT between February 2024 and June 202

Guyon's canal syndrome caused by mass effect after chronic dog bite hand injury.

Epidermal inclusion cysts are defined as dermal or subcutaneous epithelial cysts which are filled with keratin and lined with true epidermis. Although exceedingly rare, they can lead to pathological conditions, such as nerve compression syndrome. To the best of our knowledge, this is the first documented case of a 59-year-old woman with ulnar nerve compression at the wrist due to dog bite hand injury. An epidermal inclusion cyst was located adjacent to the ulnar nerve, resulting in significant compression. Notably, no direct connection was observed between the cyst and ulnar nerve. Surgical re

Modified UNet-enhanced ultrasonic superb microvascular imaging feature extraction and grading of carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) is recognized as the most frequently encountered median nerve (MN) entrapment neuropathy, with a disproportionate burden in middle-aged and elderly individuals and in occupational groups with repetitive wrist use. Anatomically, CTS is characterized by compression of the median nerve within the confined space between the transverse carpal ligament and flexor tendons, and microcirculatory impairment is regarded as one of its key pathological bases. Although electrodiagnostic assessments are considered as diagnostic gold standard, their utility is limited by suboptima

Investigation of variations in the innervation of the thenar muscles by the median nerve.

INTRODUCTION: This study aims to investigate the anatomical variations in the median nerve and its branches, specifically focusing on their distribution to the muscles and the points at which they enter the muscles. In addition, the branching level of the ramus muscularis of the median nerve supplying the thenar muscles was examined to explore potential correlations with carpal tunnel syndrome. Understanding these variations is crucial for minimizing iatrogenic nerve damage during surgical procedures in the carpal tunnel region. METHODS: Dissections were conducted on 15 forearm and wrist spec
