We devised a new flap using the palmar cutaneous branch of the superficial radial artery proximally and anastomosed with a cutaneous perforator of the superficial palmar arch distally. We named our flap "the volar glabrous palmar flap." The flap was used both as a free flap and as a reverse-flow island flap. Thirty-six patients with volar hand defects (24 digits, 7 palms, and 5 first web space) were reconstructed with this flap. The flap was used as a proximally based free flap in 15 patients and was used as a reverse-flow island flap based on distal communication in the remaining 21 patients.

INTRODUCTION: False aneurysm of the arteries of the hand is extremely rare.
CASE PRESENTATION: We report a case of false aneurysm of the superficial palmar arch in a 4-year-old boy following injury with a piece of glass. Stereoscopic magnetic resonance angiography depicted the lesion effectively. The aneurysm was resected, and the vascular reconstruction was performed with end-to-end anastomosis using microsurgical techniques.
CONCLUSIONS: Follow-up care is necessary after a hand injury in order to properly diagnose any lesion that may be present and treat it successfully.

The median artery usually regresses after the eighth week of intrauterine life, but in some cases it persists into adulthood. The persistent median artery (PMA) passes through the carpal tunnel of the wrist, accompanying the median nerve. During anatomical dissection in our department, we found two unilateral cases of PMA originating from the ulnar artery. In both cases the PMA passed through the carpal tunnel, reached the palm, and anastomosed with the ulnar artery, forming a medio-ulnar type of superficial palmar arch. In addition, in both cases we observed a high division of the median nerv

OBJECTIVE: To assess the long-term outcome of selective thrombolysis in patients with hypothenar hammer syndrome by 3.0-T magnetic resonance (MR) angiography.
MATERIALS AND METHODS: Seven patients (6 men, 1 woman; mean age, 58.6 +/- 14.4 years) were investigated. Long-term follow-up examinations (9-11 years post-interventional) were based on 3.0-T MR angiography. Pre- versus post-thrombolysis images and post-thrombolysis versus long-term follow-up images were compared with respect to arterial diameter. Additionally, changes in patients' symptoms were assessed.
RESULTS: The long-term follow-u

This cadaveric study evaluates the margin of safety and technical efficacy of mini open carpal tunnel release performed using Knifelight (Stryker Instruments) through a transverse 1 cm wrist incision. A single investigator released 32 wrists in 17 cadavers. The wrists were then explored to assess the completeness of release and damage to vital structures including the superficial palmar arch, palmar cutaneous branch and recurrent branch of the median nerve. All the releases were complete and no injury to the median nerve and other structures were observed. The mean distance of the recurrent mo

We present a 48-year-old male cadaver in which the right superficial palmar arterial arch had no contribution from the radial artery and terminated by giving rise to a common trunk for the princeps pollicis and radialis indicis arteries. The dorsal radial artery of the thumb was absent and no arterial supply was provided by the radial artery. Accordingly, the entire arterial supply to the right thumb was provided by the superficial palmar arterial arch, which was composed solely of the ulnar artery, which received no collateral supply from the radial artery system. Considering that the arteria

PURPOSE: Vascularized bone grafting has been proposed as a treatment for scaphoid nonunions with avascular necrosis of the proximal pole. The purpose of this investigation is to report the results of vascularized bone graft and internal fixation for established scaphoid nonunions with proximal pole avascular necrosis as measured by validated outcome instruments.
METHODS: From 1996 to 2004, 30 consecutive patients with established scaphoid nonunion, proximal pole avascular necrosis, and no prior surgery were treated with open reduction and internal fixation in addition to a vascularized bone g

Significant soft tissue defects of the fingertip with exposure of tendon or bone represent a challenging problem when homodigital or heterodigital artery island flaps are not available. In an attempt to resolve this problem, 15 patients with fingertip injuries involving palmar skin defects or amputation were treated with a vascularized flap transfer from the midpalmar area. A 2.5 x 1.5 to 4.5 x 2 cm flap from the radial or distal aspect of the midpalm, pedicled on the terminal branch of the superficial palmar arch or common/proper palmar digital artery and vein, was transferred in a retrograde

BACKGROUND: The third common digital nerve (TCDN) has been described as the most commonly injured digital nerve during carpal tunnel release (CTR). Anatomic variations of the origin and course of the TCDN from the median nerve may place this structure at risk. Anatomic landmarks may be useful to predict the location of the TCDN to minimize the risk for injury to this structure during CTR.
METHODS: Twenty cadaveric hands were used to determine the origin and course of the TCDN. The origin of the TCDN from the median nerve was identified in relation to the transverse carpal ligament (TCL), card

BACKGROUND: Knowledge of the relationship of the palmar arches to anatomical landmarks would decrease iatrogenic injuries, facilitate treatment of vascular occlusive disease, and ease interpretation of abnormal arteriograms. The purpose of this study was to identify the location of the palmar arches in relation to surface and bony landmarks.
METHODS: The palmar arches in 48 cadavers were identified through dissection. The most distal points of the palmar arches were measured in relation to Kaplan's cardinal line, the distal wrist crease, and the carpometacarpal joint of the ring finger. The d

A 51-year-old woman described paresthesias in her right thumb, index, and middle fingers consistent with carpal tunnel syndrome. Using a sterile technique, a 25-G, 1 1/4-in needle was introduced ulnar to the palmaris longus tendon, and a mixture of 1.5 mL of 40 mg/mL of triamcinolone acetonide and 1.5 mL of 1% lidocaine was injected. After the injection, the patient's hand exhibited signs of ischemia including coolness and discoloration. Rewarming of the hand with paraffin was performed immediately, and normative color returned. At follow-up visits, the patient described burning in the hand, a

The glabrous skin of the palm provides the best colour and texture match for reconstructing volar hand defects, but only a limited number of local axial pattern flaps of the palm have been described in the literature. This report describes a new reverse thenar perforator flap harvested from the midpalm and the thenar region. A total of 12 patients with volar hand defects of the index finger, palm, or first web space were reconstructed with this flap between August 2005 and August 2007. The causes of the defects were trauma (one patient), burn contracture release of the first web and index fing

OBJECTIVE: To explore the therapeutic strategy of hot press injury complicating with blood vessel injury in the upper extremity.
METHODS: Decompression procedure was carried out in 8 patients with hot press injury complicating with blood vessel injury in the upper extremities, but the effect was not of satisfactory because there were injuries to brachial, radial and ulnar arteries and also injury to the superficial or deep palmar arch. The blood vessels were repaired with direct anastomosis, anastomosis with flexion of the limb, or transplantation with great saphenous vein, and the wounds wer

INTRODUCTION: Avulsion and crush injuries constitute a particularly difficult problem due to extensive damage of vessels and nerves. In cases where a crush is the dominating injury factor causing complex fractures of forearm and carpal bones, shortening of the extremity is necessary for primary vessel and nerve reconstruction. Surgical experience in vessels dissection and optimal sequence of reconstruction procedures using vessels and nerve grafts are of paramount importance.
MATERIAL AND METHODS: In the years 1986-2006 the author carried out 18 replantations and 4 revascularizations at vario

The reconstruction of the flexor tendon in zones I and II of a ring finger in a single stage, using a free vascularized tendon graft, is reported in a single clinical case. The flexor digitorum superficialis (FDS) tendon of the same finger was freely transferred based on a vascular branch off the ulnar vessels. The ulnar artery was reconstructed with an end-to-end anastomosis, and the free flap was connected to the superficial palmar arch. The functional result was satisfactory.

We present our experience of two cases in which we carried out pollicisation of the index metacarpal based on the first dorsal metacarpal artery and venae comitans. Both cases were complex severe hand injuries where the initial injury was the result of a firework exploding while held in the hand. In both cases the radial side of the superficial palmar arch was destroyed by the injury. In both cases there was virtually complete loss of the thumb ray and amputation of the index through the base of the proximal phalanx. A useful opposition post has been created from vascularised index metacarpal
