Anatomy of the palmar branch of the ulnar nerve: application to ulnar and median nerve decompressive surgery.

OBJECT: The palmar cutaneous branch of the ulnar nerve (PCUN) has received little attention in the literature, and to the authors' knowledge, has received no attention in the neurosurgical literature. The present study was performed to help the surgeon minimize postoperative complications of nerve decompression at the wrist. METHODS: Forty cadaveric upper limbs underwent dissection of the ulnar nerve in the forearm, at the wrist, and in the palm. The PCUN was investigated and when identified, measurements were made and relationships documented between this cutaneous branch and the ulnar arter

Hypothenar hammer syndrome: Long-term follow-up of selective thrombolysis by 3.0-T MR angiography.

OBJECTIVE: To assess the long-term outcome of selective thrombolysis in patients with hypothenar hammer syndrome by 3.0-T magnetic resonance (MR) angiography. MATERIALS AND METHODS: Seven patients (6 men, 1 woman; mean age, 58.6 +/- 14.4 years) were investigated. Long-term follow-up examinations (9-11 years post-interventional) were based on 3.0-T MR angiography. Pre- versus post-thrombolysis images and post-thrombolysis versus long-term follow-up images were compared with respect to arterial diameter. Additionally, changes in patients' symptoms were assessed. RESULTS: The long-term follow-u

Anatomical landmarks to the superficial and deep palmar arches.

BACKGROUND: Knowledge of the relationship of the palmar arches to anatomical landmarks would decrease iatrogenic injuries, facilitate treatment of vascular occlusive disease, and ease interpretation of abnormal arteriograms. The purpose of this study was to identify the location of the palmar arches in relation to surface and bony landmarks. METHODS: The palmar arches in 48 cadavers were identified through dissection. The most distal points of the palmar arches were measured in relation to Kaplan's cardinal line, the distal wrist crease, and the carpometacarpal joint of the ring finger. The d

Digital ischemia after carpal tunnel injection: a case report.

A 51-year-old woman described paresthesias in her right thumb, index, and middle fingers consistent with carpal tunnel syndrome. Using a sterile technique, a 25-G, 1 1/4-in needle was introduced ulnar to the palmaris longus tendon, and a mixture of 1.5 mL of 40 mg/mL of triamcinolone acetonide and 1.5 mL of 1% lidocaine was injected. After the injection, the patient's hand exhibited signs of ischemia including coolness and discoloration. Rewarming of the hand with paraffin was performed immediately, and normative color returned. At follow-up visits, the patient described burning in the hand, a

[Management of hot press injury complicating with blood vessel injury in the upper extremity].

OBJECTIVE: To explore the therapeutic strategy of hot press injury complicating with blood vessel injury in the upper extremity. METHODS: Decompression procedure was carried out in 8 patients with hot press injury complicating with blood vessel injury in the upper extremities, but the effect was not of satisfactory because there were injuries to brachial, radial and ulnar arteries and also injury to the superficial or deep palmar arch. The blood vessels were repaired with direct anastomosis, anastomosis with flexion of the limb, or transplantation with great saphenous vein, and the wounds wer

Safety of carpal tunnel release with a short incision. A cadaver study.

The standard long incision technique for carpal tunnel release causes inevitable damage to skin sensation, the inter-thenar plexus and especially the distal branches of the palmar cutaneous branch of the median nerve (PCM), and may cause long-term disabling pain and scar tenderness. There are many variations in the distal branches of the median nerve at the wrist. Anatomic studies of this region also have important clinical implications to prevent injury to important anatomic structures. The purpose of this study was to evaluate the short-incision carpal tunnel release in cadavers. Several imp

Microsurgical second dorsal metacarpal artery cutaneous and tenocutaneous flap for distal finger reconstruction: anatomic study and clinical application.

In this paper, we report on the anatomical study of 34 cadaveric forearms with red latex injection and the clinical application of this study to 11 cases of microsurgical second dorsal metacarpal artery (SDMA) flaps. There were 8 cutaneous cases and 3 tenocutaneous cases using SDMA flaps for distal finger reconstruction. The SDMA was classified into 2 types and 4 subtypes according to its anatomical origin and course. Type I (76.5%) originated from the dorsal branch of the radial artery at the snuffbox. Type II (23.5%) originated from the perforating branch of the deep palmar arch at the bases

The neglected rupture of deep transverse metacarpal ligament.

The deep transverse metacarpal ligament (DTML) extends in radio-ulnar direction between the palmar plates of the metacarpophalangeal (MCP) joints of the second through fifth finger rays. On the radial aspect of the index and the ulnar aspect of the small fingers, the ligament merges with the collateral ligament of the MCP joints. The ligament has palmar grooves for the flexor tendons at the MCP joints and act as a support for the metacarpal arch. Closed DTML injury is an unusual case, but the physical findings of DTML injury are a decrease in grip strength, deviation of the finger in flexion a

Practical anatomy of the carpal tunnel.

The carpal tunnel is most narrow at the level of the hook of the hamate. The median nerve is the most superficial structure. It has specific relationships to surrounding structures within the carpal tunnel to the ulnar bursa, flexor tendons, and endoscopic devices placed inside the canal. The importance of the ring finger axis is stressed. Knowledge of topographical landmarks that mark the borders of the carpal tunnel, the hook of the hamate, superficial arch, and thenar branch of the median nerve ensure appropriate incision placement for endoscopic as well as open carpal tunnel release surger

Use of the distally-based radial forearm flap supplied by the dorsal carpal arch, or palmar carpal arch, or both, in mutilating injuries. Two case report.

The distally-based radial forearm flap is safe, easy, and versatile as a regional, one-stage procedure to reconstruct soft tissue defects of the hand. However, there is a general perception that the deep and superficial palmar arches have to be intact to raise a distally-based radial forearm flap. We successfully used two flaps supplied by the dorsal carpal arch, or the palmar carpal arch, or both, despite the fact that the deep and the superficial palmar arches were damaged in mutilating injuries.

Ulnar conduction block at the wrist.

Two cases of ulnar nerve lesions at the wrist are reported. The lesions had an acute onset and exclusively impaired the ulnar motor deep branch. The coexistence of carpal tunnel syndrome in each case allowed an early diagnosis but was somewhat misleading. In both cases, the use of classic motor and sensory conduction studies did not provide clear abnormalities that would have precisely determined the site of the nerve lesion. In both cases, only palmar stimulation of the ulnar motor deep branch showed an important conduction block. This electrodiagnostic finding showed definitively the site of

Deep palmar arch patterns in Brazilian individuals.

The deep palmar arch is very important in the blood supply to the hand. Consequently, the radial artery and the deep palmar arch were studied in 60 hands from 30 cadavers of adult Brazilian individuals, of both sexes. The cadavers belong to the Universidade Federal de São Paulo, Brazil. The hand arteries were injected with red stained latex neoprene: the deep palmar arch was observed in 59 preparations (98.3%). The arch conformation was classified in two groups, according to the course of the radial artery through the interosseous spaces from the dorsal to the deep palmar region. In group I th

Revisitation of the vascular anatomy of the lumbrical and interosseous muscles.

Functional outcome after transmetacarpal replantations and revascularizations is discouragingly poor and often associated with a high incidence of intrinsic-related complications. In order to explore the hypothesis that intrinsic muscle ischemia may play a significant role, we revisited the vascular anatomy of the lumbrical and interosseous muscles. Six fresh-frozen cadaver hands were injected with latex-barium sulfate, and dissections were performed focusing on the contributions of the deep and superficial palmar arches and their branches to the intrinsic muscle vasculature. We found that the

Ulnar nerve lesion in the palm: report of an unusual case.

We report the case of a patient who had weakness of the fourth dorsal interosseous (DI) and third palmar interosseous (PI) muscles resulting in the abducted position of the little finger at rest. Electromyographic abnormalities were limited to only these muscles with partial denervation findings. The results of routine nerve conduction studies and imaging techniques were normal. Clinical and electrophysiological findings suggested that the lesion had occurred distal to the point where the very small motor branches to these muscles exit the deep palmar branch of the ulnar nerve.

[Anatomic observation of the dorsal metacarpal arteries].

Anastomosis between the first to the fourth dorsal metacarpal arteries was observed in 43 hands of adult corpses. It was found that 86% of the first dorsal metacarpal artery arises from the radial artery, 14% is replaced by the main artery of thumb or superficial branches of the radial arteries; eighty-six percent of the second dorsal metacarpal artery originates from the dorsal carpal artery. Sixty-seven percent of the third and the fourth dorsal metacarpal arteries is formed by anastomosis of the dorsal carpal artery and its deep palmar arch. The first to the fourth dorsal metacarpal arterie

Flexor carpi radialis tendinitis. Part I: Operative anatomy.

The anatomical characteristics of the flexor carpi radialis tendon, the tendon sheath, the tunnel boundaries, and the adjacent structures were examined in twenty-five adult cadaveric specimens. Transverse and sagittal sections of the wrist and forearm, tenograms, and histological sections of the tendon were made from two additional specimens each. The musculotendinous portion of the flexor carpi radialis tendon begins an average of fifteen centimeters (range, twelve to seventeen centimeters) proximal to the radiocarpal joint; the muscular fibers end an average of eight centimeters (range, six

The vascular blood supply of the second metacarpal bone: anatomic basis for a new vascularized bone graft in hand surgery. An anatomical study in cadavers.

After traumatic loss of the index finger the second metacarpal bone is often used as a free graft to reconstruct bony defects of the thumb. Since clinical experience has shown numerous advantages of using a blood supplied bone graft, an anatomical study was performed in 104 cadaveric hands to investigate the vascularization supply of this bone. After dye injections into the arterial system the vascular situation of the second metacarpal was studied and a classification was made. Six variations in arterial vascularization were found. The nutrient vessels to the bone originate from the radial ar
