A Case of Severe Necrotizing Soft-tissue Infection With Reconstruction of the Mid-lower Leg Using a Distally Based Pedicled Gracilis Muscle Flap.

Here, we report the management of a severe necrotizing soft-tissue infection involving the entire lower limb of a man in his 70s. The infection originated from a lower leg hematoma that developed during anticoagulant therapy for deep vein thrombosis. Necrosis extended from the deep thigh to the lower leg, involving areas surrounding critical structures such as the femoral artery, sciatic nerve, and tibia. Limb salvage was achieved through multiple debridement procedures and skin grafts performed in collaboration with orthopedic and cardiovascular surgery teams. The exposed cortical bone on the

Dual-purpose Laparoscopy and Bipedicled Anterolateral Thigh Flap for Abdominal Wall Reconstruction After High-voltage Electrical Burn.

High-voltage electrical burns involving the abdominal wall pose complex management challenges, including delayed bowel perforation, progressive tissue necrosis, and durable reconstruction in compromised tissue. To our knowledge, no standardized approach has been described that combines visceral surveillance with abdominal wall reconstruction in this setting. We report a case of a 43-year-old man who sustained a 22,000-volt electrical injury with 30% total body surface area burns and a full-thickness lower abdominal wall defect (16 × 13 cm) exposing the intact peritoneum. Partial injury of the triradiate cartilage following low-energy trauma.

BACKGROUND: Acute traumatic injuries of the triradiate cartilage are very rare in children and usually result from high-energy trauma associated with other pelvic injuries. Only two sports-related cases of complete injuries of the triradiate cartilage have been reported to date. CASE PRESENTATION: We herein describe the first case of a partial injury of the triradiate cartilage in a 14-year-old athlete. The patient presented with sudden-onset hip pain and moderate functional impairment. The fracture was confined to the ilioischial synchondrosis, while the remaining triradiate cartilage and th

Flap Reconstruction for Deep Hand Burns: A Case Series.

Introduction Deep hand burns are severe injuries that frequently result in exposure of tendons and bone because of the thin subcutaneous tissue of the hand. In such cases, skin grafting alone is often insufficient, and flap reconstruction is required. Heat-press injuries are particularly challenging because combined thermal and crush mechanisms make tissue viability difficult to evaluate. The optimal timing of debridement and reconstruction also remains controversial. Methods We retrospectively reviewed the medical records of six patients (six hands) with acute deep hand burns distal to the wr

High-Voltage Electrical Injury Causing a Full-Thickness Laryngotracheal Wall Defect: Reconstruction With an Anterolateral Thigh Free Flap and an Inverted Split-Thickness Skin Graft.

Full-thickness loss of the laryngotracheal wall is a rare and severe consequence of high-voltage electrical injury. It is anatomically and clinically distinct from the epithelialised tracheocutaneous fistula that follows tracheostomy decannulation, and it poses a far greater reconstructive problem because of progressive tissue necrosis, wound contamination, and exposure of the airway and major cervical vessels. We report the case of a 30-year-old man with approximately 30% total body surface area deep electrical burns involving the neck, abdomen, and all four limbs, who presented with an expos

Surgical Approaches to Perineal Reconstruction: A Narrative Review.

The reconstruction of the perineum remains one of the most demanding fields in reconstructive surgery. The region combines a contaminated operative field, a high prevalence of prior radiotherapy, complex three-dimensional dead space, and functional structures (the anus, urethra, and vagina) whose preservation or restoration directly determines the quality of life. The principal indications for perineal reconstruction are oncologic resection, most commonly abdominoperineal resection and pelvic exenteration for anorectal and gynecologic malignancy, radical vulvectomy, and the soft tissue destruc

Dislocation analysis on bio-inspired open hip joint with active-and-passive stabilizers.

This study presents a simulator framework to replicate joint dislocation that can be used to evaluate the contributions of various joint stabilizers. Compared to conventional axial joint configurations, biologically inspired open joints allow for a large range of motion with fewer rotational components and no singularities. Despite these advantages, open joints have few practical applications due to joint dislocation. To facilitate the development of bio-inspired open joints, our study evaluates the performance of biomimetic joint stabilizers involving (1) the muscle actuators as the active st

Intermittent pneumatic thigh compression for the treatment of lower limb wounds (IPCOTT): a randomised control trial.

OBJECTIVE: Hard-to-heal (chronic) venous leg ulcers (VLUs) are challenging to heal with standard of care (SoC) alone. This study evaluated whether adding a thigh-administered intermittent pneumatic compression (IPC) device (WoundExpress, Huntleigh Healthcare Ltd., UK) to SoC improved outcomes in patients with hard-to-heal venous or mixed leg ulcers (venous and arterial), was suitable for home use and was safe. METHOD: A multicentre, pragmatic, open-label, randomised controlled trial (RCT) was conducted across the UK, US, Germany and France. Adults with hard-to-heal lower limb VLUs and mixed u

Donor site morbidity and patient satisfaction after arthroscopic superior capsule reconstruction using fascia lata autograft for irreparable rotator cuff tears.

BACKGROUND: Donor site morbidity-previously a major concern-remains a considerable issue in arthroscopic superior capsule reconstruction using a fascia lata autograft due to the limited scientific evidence. We aimed to evaluate donor site morbidity and patient satisfaction after arthroscopic superior capsule reconstruction using a fascia lata autograft for irreparable rotator cuff tears. We hypothesized that the procedure would result in certain donor site morbidities, would not have a significant impact on daily activities, and that improvements in shoulder function could compensate for donor

Posterior Hip Dislocation with Ipsilateral Subtrochanteric Femur Fracture: A Rare Case Report.

INTRODUCTION: Posterior dislocation of the hip associated with an ipsilateral subtrochanteric femur fracture is a rare and technically challenging injury pattern, most commonly seen after high-energy trauma such as road traffic accidents. The associated proximal femoral fracture disrupts the normal femoral lever arm, making closed reduction difficult or impossible and often necessitating open reduction. Delay in diagnosis or treatment may result in complications such as avascular necrosis of the femoral head, post-traumatic arthritis, sciatic nerve injury, and poor functional outcomes. CASE R

Limb Salvage Using Negative Pressure Wound Therapy and Split-Thickness Skin Grafting for Polymicrobial Ecthyma Gangrenosum in a Patient With Acute Lymphoblastic Leukemia: A Case Report.

Ecthyma gangrenosum (EG) is a rare and potentially life-threatening cutaneous infection predominantly affecting profoundly immunocompromised patients. Although classically associated with Pseudomonas (P.) aeruginosa, polymicrobial infections may occur and can result in extensive tissue necrosis and limb-threatening defects. Reconstruction in severely immunocompromised patients is particularly challenging when exposed tendons are present. We present the case of a 32-year-old woman with relapsed B-cell acute lymphoblastic leukemia undergoing re-induction chemotherapy who developed profound neutr

Satisfaction regarding shared decision-making for operative and non-operative hip fracture treatment in frail older adults from a proxy-perspective: a mixed methods study.

OBJECTIVES: Shared decision-making (SDM) is essential in hip fracture treatment, as both operative and non-operative management are viable options in a select group of frail older adults with a very limited life expectancy. The aim of this study is to evaluate patient and proxy satisfaction with SDM and their final decision and to gain insight into their experiences. DESIGN: A multicentre study with a concurrent mixed methods design. SETTING: The study was conducted in 14 hospitals across the Netherlands. PARTICIPANTS: Proxies from a select group of frail older adults, with a limited life e

Concordance of Dislocation Direction and Surgical Approach Is an Approach-Dependent Risk for Recurrent Instability Following Total Hip Arthroplasty.

BACKGROUND: Hip instability after total hip arthroplasty (THA), while uncommon, is a leading cause for revision. The purpose of the study was to quantify the risk of recurrent hip dislocation in relation to concordance of dislocation directionality and surgical approach. METHODS: Included patients underwent anterior-based or posterior-based elective primary THA and sustained at least one dislocation treated at a single institution. Surgical approach, timing, direction, and management of dislocations, including closed reduction and reoperation, were obtained. Concordance was defined as disloca

Periacetabular Osteotomy and Combined Open Femoral Head-Neck Junction Osteochondroplasty: A Concise Follow-up of a Previous Report at a Mean 10-Year Follow-up.

BACKGROUND: Hip dysplasia is a well-established cause of hip dysfunction and secondary osteoarthritis in young adults. Periacetabular osteotomy (PAO) is a common surgical treatment for symptomatic acetabular dysplasia. Recent awareness of concomitant impingement in dysplastic hips has led to inclusion of osteochondroplasty (OCP) for femoral head-neck abnormalities in select cases to improve survivorship and clinical outcomes. PURPOSE: To compare outcomes, reoperation rates, radiography, survivorship, and complications at a mean of 10 years after PAO with and without femoral head-neck OCP. ST

Case Report: Chronic Morel-Lavallée lesion: dynamic MRI progression, histopathological findings and surgical treatment.

BACKGROUND: The Morel-Lavallée lesion (MLL) is a closed soft tissue avulsion injury resulting from shear force trauma. Chronic Morel-Lavallée lesions present significant management challenges due to the formation of fibrous caps, persistent fluid accumulation, and a tendency for recurrence. Nevertheless, the pathogenesis and imaging characteristics of these lesions remain poorly understood. METHODS: This study presents a case of a chronic Morel-Lavallée lesion located in the right hip, with a disease progression lasting over 10 years. The MRI characteristics of the lesion were compared across

Diffuse Bilateral Multifocal Bone Stress Injuries in a 19-Year-Old Male Military Trainee: A Case Report.

Bone stress injuries (BSIs) are common among high-performance athletes and military trainees. BSIs typically present with point tenderness, loading pain, and nighttime discomfort, and most involve only one or two bones, with clinical symptoms roughly proportional to the degree of osseous injury. We present the case of a 19-year-old male engaged in high-intensity Air Force Special Warfare training who initially presented with vague low back, thigh, and foot pain and was ultimately diagnosed with diffuse multifocal grade 3 and 4a BSIs involving numerous bones of the bilateral lower extremities.

Multicentre randomised controlled trial protocol in China comparing postless hip arthroscopy with conventional perineal post-assisted hip arthroscopy to reduce perineal complications in patients undergoing hip arthroscopy for femoroacetabular impingement: the POSTLESS-HIP trial.

INTRODUCTION: Perineal complications-including nerve injury (eg, pudendal neurapraxia), soft tissue injury and sexual or urinary dysfunction-are well-documented adverse effects of conventional hip arthroscopy using a perineal post. Postless hip arthroscopy avoids direct perineal pressure, but high-quality randomised evidence is lacking. METHODS AND ANALYSIS: This multicentre, prospective, parallel-group, randomised controlled trial will enrol 270 patients aged 15-60 years with femoroacetabular impingement syndrome requiring hip arthroscopy. Participants will be randomised 1:1 via a centralise
