BACKGROUND: Periprosthetic femoral fractures account for up to 16% of all revision total hip arthroplasties with one- and two-year mortality rates of 23.4 and 29.2%, respectively. Given the substantial clinical and financial burden, accurate preoperative risk assessment is essential. This study evaluated the predictive performance of the Five-Item Modified Frailty Index (mFI-5) and Risk Analysis Index (RAI) in patients undergoing surgery for PFs, aiming to improve preoperative assessment, inform surgical decision-making, and facilitate postoperative planning.
METHODS: A total of 824 patients

To systematically review Machine Learning (ML) fracture risk prediction models developed solely using administrative data, evaluating their development, performance, risk of bias, and concerns regarding applicability. A systematic search was conducted in PubMed, Embase, IEEE, and Web of Science following PRISMA guidelines (up to November 2025). We included studies developing or validating ML models for osteoporotic fracture risk in adults using administrative data without clinical measurements. Risk of bias and concerns for applicability were assessed using the PROBAST tool. Seven studies were

Introduction Falls continue to be among the leading causes of nonfatal injuries in the United States. Falls are common among the older population, and the Appalachian region is considered one with an aging population. Pet ownership is common and has even increased since 2020. Few studies exist on the topic of falls caused by dogs. Methods Data were retrospectively pulled from the trauma registry at a rural Appalachian Level 1 trauma center between January 1, 2017, and June 15, 2022. Patients ≥18 who presented to the facility due to falling or tripping caused by dogs were included. Injuries due

Iliac crest avulsion injuries are rare and frequently misdiagnosed in adolescents. We report a case of a 16-year-old boy who presented with iliac apophysitis following a noncontact injury during a football game. The diagnosis was confirmed with radiography. The patient was treated conservatively with activity modification, limited weight bearing, and physical therapy, resulting in full recovery. This case highlights the importance of considering rare apophyseal injuries and obtaining appropriate imaging in adolescent athletes.

CASES: We describe 2 cases of ballistic hip arthrotomies with retained intra-articular fragments lodged deep to the quadrilateral plate, where arthroscopic retrieval alone may be challenging and often requires an open approach. Both patients were treated using a strategy combining hip arthroscopy for joint debridement and fragment mobilization followed by a Pfannenstiel intrapelvic approach for definitive removal.
CONCLUSION: This combined arthroscopic and intrapelvic surgical technique offers an alternative method for addressing difficult-to-access intra-articular ballistic fragments while m

Although MRI and dual-energy CT (DECT) are considered highly sensitive tools for detecting bone marrow edema and subtle fractures, occult femoral neck fractures may still be missed. Here, we report a rare case in which an occult femoral neck fracture was not detected on MRI or DECT during the initial evaluation. A 64-year-old woman with myelodysplastic syndrome (MDS) and residual left hemiparesis presented with persistent left hip pain after a fall. Initial radiography, CT, MRI, and DECT revealed no evidence of fractures or bone marrow edema. Despite conservative management, the patient's symp

INTRODUCTION: Hip fractures are among the most common orthopaedic injuries and are a vital element of orthopaedic surgery residency training. Multiple studies have examined resident involvement on surgical time, length of stay (LOS), cost, and outcomes. This study compared the time from orthopaedic hip fracture consultation to operative fixation before and after implementation of a new orthopaedic residency at a rural community hospital.
METHODS: A retrospective review of electronic medical records was performed for patients who underwent surgical fixation of a hip fracture from July 2019 to

BACKGROUND: Surgical fixation of acetabular fractures is technically demanding, and operative time is often treated as a surrogate for surgical complexity and a presumed driver of complications. Whether prolonged operative time reflects identifiable preoperative factors, and whether it independently worsens outcomes, remains unclear. We aimed to identify factors associated with prolonged operative time in acetabular fracture fixation and to determine whether longer operative time is associated with worse postoperative outcomes.
METHODS: We retrospectively reviewed patients undergoing surgical

Traditional spica casting is the standard treatment for pediatric femoral fractures in children under five years of age, but it carries well-documented limitations, including the need for general anesthesia, increased caregiver burden, and high rates of skin complications. Prefabricated functional bracing, such as the Dynamic Femur Fracture (DF2) brace (OrthoPediatrics, Warsaw, IN), has emerged as an alternative that eliminates anesthesia exposure and improves patient mobility. However, clinical data regarding the practical limitations, complication profiles, and patient selection boundaries d

The iliopsoas tenotomy aims at correcting the impaired hip extension that compromises patients' ability during gait, creating discomfort during postural transfers and prolonged sitting. The standard, open approach adopted for neurological disorders requires general anesthesia and sutures, with considerable odds of adverse events, such as nerve injuries, pain, and infections. To date, percutaneous release interventions have been done only on cadavers or adult patients with orthopaedic conditions. In this article, we introduce the ultrasound-guided minimally invasive approach in neuro-orthopaedi

PURPOSE: Acetabular fractures with cartilage impaction are associated with a high risk of post-traumatic osteoarthritis, frequently requiring total hip arthroplasty (THA). Conventional open reduction and internal fixation (ORIF) often fail to restore joint surface congruity. This case series evaluates the radiological and clinical outcomes of a novel minimally invasive technique using balloon acetabuloplasty to reduce dome impaction in traumatic acetabular fractures.
METHODS: This retrospective case series included patients with acetabular fractures involving cartilage impaction. Balloon acet

BACKGROUND: In laboratory-based injury risk assessments, there is always a small and not entirely avoidable risk of injury to participants. This case report describes an anterior cruciate ligament (ACL) re-rupture of a female participant during the approach run of a stimulus-response change-of-direction (COD) task.
METHODS: Marker-based kinematic data from the injury trial were compared with 14 successful trials of the same participant. These trials included unanticipated CODs in response to an opponent's kicking action with varying cognitive demands, including one trial under identical high-

PURPOSE: Periprosthetic femoral fractures (PFF) are the second cause of revision surgery after total hip arthroplasty and occur predominantly in elderly, fragile patients. Despite high associated morbidity and mortality, data specifically focusing on early postoperative complications remain scarce.
METHODS: A single-center retrospective study was conducted on 192 patients who underwent surgery for PFF between January 2010 and April 2023. Patients were treated by open reduction and internal fixation (ORIF, n = 75) or revision surgery (n = 117). All complications occurring within the first post

BACKGROUND: Spinal cord injury (SCI) significantly affects the quality of life of affected individuals and their caregivers. Assisted transfers after SCI are physically demanding and one of the leading causes of musculoskeletal injuries among caregivers. While mechanical lift systems can reduce injury risk, their adoption is limited by high costs, large size, and portability. Functional neuromuscular stimulation (FNS) presents a promising alternative for facilitating transfers by activating the paralyzed muscles below the level of injury. However, determining the correct and optimal stimulatio
