Rare Novel Genetic Variants of the OFD1 Gene Associated With a Familial Form and a Sporadic Case of Long Bone Atypical Fractures.

We describe a family with 11 members affected by oral, facial and digital findings, as well as adult-onset multiple long bone insufficiency fractures mimicking atypical femur fractures (AFF) on radiographs. The analysis of the pedigree of this family suggested an X-linked dominant inheritance. We performed whole-exome sequencing in this family. A rare novel variant, p.Ala642Asp (NM_003611, c.1925C>A, hg19) of the OFD1 gene was the only candidate on the X chromosome segregating with the phenotype within the family. The OFD1 gene targeted sequencing was carried out on 49 patients with AFF and 10

Comparison of the effects of different osteotomy methods on bone healing; an experimental study.

BACKGROUND: Bone healing after osteotomy remains a major issue in orthopedics. This study aimed to evaluate the radiological, histopathological, and biomechanical aspects of bone healing using three different osteotomy techniques. METHODS: Fifty four Wistar Albino male rats were used. The animals were randomly divided into 3 groups as gigli (n:18), saw (n:18) and drill+osteotome (n:18). Transverse osteotomy was applied to the left femur with the gigli wire, microsaw and drill+osteotome. All femurs were evaluated radiologically, biomechanically and histopathologically at the end of the 15th, 30

Lateral decubitus positioning was associated with improved efficiency and functional outcome compared with traction table positioning for cephalomedullary nailing of subtrochanteric femur fractures: a retrospective comparative study.

BACKGROUND: Subtrochanteric femur fractures are challenging to treat due to complex anatomy and high mechanical stresses. Cephalomedullary nailing is the standard treatment, often performed with the patient supine on a traction table, but this can prolong setup and cause complications (pressure or nerve injuries). Lateral decubitus positioning without a traction table may facilitate reduction and reduce traction-related risks, yet comparative data in subtrochanteric fractures are limited. This study compared outcomes of lateral decubitus versus traction table positioning for subtrochanteric fr

Bone microarchitecture and microfractures in disorganized bone in a young female with pycnodysostosis and atypical femur fractures: A case for novel fracture prediction technology using ALIGNOGRAM1.0 analysis.

Pycnodysostosis (PYCD) is a rare genetic bone disease characterized by impaired osteoclastic bone resorption. The inability to resorb bone leads to sclerosis, with increased bone mineral density (BMD) but paradoxically increased bone fragility. We report a case of a young female with PYCD with multiple fragility fractures including bilateral atypical femur fractures (AFF). Areal and volumetric bone density were supranormal, measured by dual-energy x-ray absorptiometry (DXA) and high-resolution peripheral quantitative computed tomography (HR-pQCT), respectively. However, bone disorganization wa

Efficacy of Tranexamic Acid in a Large Population with Proximal Femur Fractures-A Matched Pair Study of 7394 Patients from the Registry for Geriatric Trauma (ATR-DGU).

Background: Perioperative blood loss represents a major challenge in elderly patients undergoing hip fracture surgery. Although tranexamic acid (TXA) has been shown to reduce bleeding complications, evidence from large registry-based cohorts remains limited. This study assessed the efficacy and safety of perioperative TXA administration in geriatric proximal femoral fracture surgery and evaluated in a subgroup analysis whether repeated TXA administration provides additional benefit compared with a single intraoperative dose. Methods: Data were derived from the Registry for Geriatric Trauma of

Intraoperative Traction for Cephalomedullary Nailing in Amputation Patients.

INTRODUCTION: Femoral fracture fixation in lower-limb amputees is challenging due to limited limb purchase, altered biomechanics, and sparse guidance. We describe an intraoperative skeletal-traction technique to facilitate cephalomedullary nailing (CMN). CASE REPORT: Two amputee patients with extracapsular proximal femur fractures underwent CMN. Under biplanar fluoroscopy, a 5-mm distal femoral traction pin was placed through the residual limb and secured to the traction boot to restore length, correct alignment, and permit controlled reduction. RESULTS: Patient 1 (65-year-old female; prior

Factors Affecting Transfusion Rate for Patients After Hip Fracture.

BACKGROUND: Hip fractures are a common indication for red blood cell transfusion among elderly patients. Blood transfusion may be clinically necessary, yet it carries risks and should be anticipated according to patient and surgical characteristics. OBJECTIVE: This retrospective study aimed to identify clinical and perioperative factors associated with the need for blood transfusion among patients hospitalized with hip fracture. METHODS: The study included 150 patients with hip fracture treated at the Orthopedic Department of Bnei Zion Medical Center between April 2014 and June 2015. The tra

Clinical Implications of ChatGPT-assisted Multimodal Pre-operative Assessment in Elderly Pertrochanteric Fracture Patients: An Exploratory Study.

INTRODUCTION: Given the high mortality associated with femoral trochanteric fractures, reliable pre-operative estimation of post-operative death risk is essential. Commonly used scoring systems demonstrate only moderate predictive ability, largely because they rely on limited and sometimes outdated clinical parameters. ChatGPT (Generative Pre-Training Transformer) may be capable of synthesizing diverse pre-operative clinical information within a single multimodal analytical framework. Therefore, this study aimed to (i) Explore whether ChatGPT-based multimodal integration of pre-operative data

Preoperative Geriatric Nutritional Risk Index (GNRI) and Comorbidity Burden as Mortality Risk Markers After Proximal Femoral Nailing in Older Patients with Pertrochanteric Hip Fractures.

Background: Mortality after geriatric hip-fracture surgery remains substantial. This study evaluated whether preoperative GNRI, comorbidity burden, and CBC-derived inflammatory indices were associated with one-year and long-term mortality after PFN for pertrochanteric/intertrochanteric fractures. Methods: In this single-centre retrospective cohort study of prognostic risk markers, 248 PFN records were screened. After excluding six incomplete records and 25 patients aged <65 years, 217 older fracture/surgical episodes formed the time-to-event cohort; 194 were evaluable for binary one-year morta

Association of preoperative modified frailty Index-5 with 30-day mortality and complications in elderly hip fracture surgery: A prospective observational study.

BACKGROUND: Frailty is an established predictor of adverse outcomes during surgical procedures in the elderly. Despite this, the prognostic utility of the mFI-5 for early mortality and postoperative complications remains poorly characterised in this population, particularly among those who underwent surgical procedures for hip fracture. METHODS: This prospective observational study was performed in a single tertiary care hospital in South India from 2023 to 2024. Patients aged 60 years and above who underwent surgical management for fracture of neck of femur or intertrochanteric fracture were

Analysis of Postoperative Knee Dysfunction Following a Distal Femur Fracture Managed by Different Surgical Methods.

Background Distal femur fractures, in particular, are complicated injuries that have a major effect on knee biomechanics and functionality. As all fractures primarily affect older females from low-energy falls and younger males from high-energy injuries, patients treated with open reduction-internal fixation (ORIF) and intramedullary nailing (IMN) had their postoperative knee dysfunction and functional results assessed in this study. Objective This study aimed to analyze postoperative knee dysfunction and functional outcomes following distal femur fracture managed by ORIF and IMN and to evalua

Rapid transfer to specialist orthopaedic ward reduces mortality in hip fracture patients, but several factors reduce the ability to achieve the < 4-hour target.

PURPOSE: Hip fractures are one of the most common fracture types in adults. Rapid admission to orthopaedic wards from ED (< 4 h) and optimal timing for surgery within 36 h, is linked with good patient outcomes. This study aims to identify factors that influence achievement of timely admission of elderly hip fracture patients to specialist wards at a UK tertiary centre. Additionally, it seeks to examine the impact of timely admission on 30-day mortality. METHODS: This retrospective, single-centre study of a cohort of 6,170 patients aged over 60 who were admitted to our hospital’s ED with hip fr

Effect of Marker Ball Positioning on Templating Accuracy in Hip Arthroplasty: A Closed-Loop Audit.

Introduction Positioning of external calibration devices (ECDs) in anteroposterior (AP) pelvic radiographs influences magnification correction and therefore the accuracy of digital templating in hip arthroplasty. Orthopaedic departments rely on radiology teams to accurately position ECDs on pre-operative radiographs, but due to multiple factors, placement is not always satisfactory, which may incorrectly guide selection of the hip prosthesis size. This closed-loop audit assessed the accuracy of templating femoral head size in patients with intra-capsular neck of femur (NOF) fractures before an

Protocol of a multi-centre randomized controlled trial to compare pericapsular nerve group block, fascia-iliaca compartment block and femoral nerve block for pain management in patients with a hip fracture in the emergency department (CPFF-ED).

BACKGROUND: Patients presenting in the emergency department (ED) with a hip fracture are often in significant pain. Opioids are frequently prescribed as part of standardized pain protocols, but are associated with many adverse events, especially in the elderly. Regional anaesthesia with a femoral nerve block (FNB), Fascia Iliaca compartment block (FICB) or Pericapsular nerve block (PENG) may provide a reasonable alternative to these patients. We designed a prospective multicentre study to compare the efficacy and safety of these blocks in the ED setting, which has been registered in the Centra

Consequences of Periprosthetic Fracture After Hemiarthroplasty of the Hip.

Introduction Low-energy femoral neck fracture (FNF) in the elderly is a serious injury. For patients with multiple existing comorbidities and frailty, the risk of complications, poor functional outcomes, and premature death may be higher. The treatment aims for immediate weight-bearing, preferably by hemiarthroplasty (HA). Previous research has focused on prosthetic joint infection and dislocation after HA. In this study, we aimed to describe the possible surgical consequences of periprosthetic fracture after HA. Methods HA patients with a periprosthetic fracture occurring between 2009 and 201

Statins Do Not Affect Bone Anabolism in Older Women Treated With Teriparatide.

CONTEXT: Teriparatide [rhPTH (1-34)] is an established anabolic agent for the treatment of severe osteoporosis. Statins, 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors, have demonstrated antiresorptive properties, but their impact on the anabolic efficacy of rhPTH remains unclear. OBJECTIVE: To evaluate whether concurrent statin therapy influences changes in bone mineral density (BMD) and bone turnover markers in women receiving rhPTH. METHODS: This retrospective cohort study included 104 postmenopausal women with severe osteoporosis who completed 24 months of daily rhPTH (20 mcg

Improvements in bone quality by parathyroid hormone treatment are enhanced in the Nmp4 knockout mouse model.

Osteoporosis is linked to increased bone fragility. Unlike anti-resorptive therapies, the analogue of parathyroid hormone, PTH 1-34, is an FDA-approved therapeutic for osteoporosis that enhances bone formation. However, as PTH treatment potency declines over time, it is necessary to investigate the mechanisms involved in this attenuation to reinforce its long-term efficacy. This need has led to investigations into the transcription factor nuclear matrix protein 4 (Nmp4), in which PTH treatment of mice globally lacking Nmp4 (Nmp4-/-) enhanced bone formation. Yet, the changes in the compositiona
