BACKGROUND: Obturator-type hip dislocation is a rare condition in pediatric patients, with the simultaneous occurrence of an ipsilateral subtrochanteric femoral fracture being exceptionally uncommon. Although various treatment methods have been described for hip dislocation or subtrochanteric femoral fractures, managing these injuries remains challenging due to the potential risk of femoral head avascular necrosis and concerns about fixation stability.
CASE PRESENTATION: An 8-year-old boy sustained a traumatic obturator hip dislocation and an ipsilateral subtrochanteric fracture following a f

BACKGROUND: Conservative treatment is considered the gold standard for managing greater trochanteric pain syndrome (GTPS), with a reported success rate of 90%. However, when conservative measures fail, surgical intervention may be necessary. Various surgical techniques have been described, but consistent results remain elusive. This study aims to present our outcomes using trochanteric reduction osteotomy (TRO) as a rescue technique for refractory GTPS cases without gluteal tears.
METHODS: We retrospectively evaluated all cases of GTPS refractory to conservative treatment that were managed wi

Background/Objectives: The preoperative length of hospital stay (PLHS) is the only major modifiable factor in hip fracture surgery. Identifying the optimal timing for the procedure is crucial for reducing the risk of death. We aimed to explore the association between PLHS and all-cause mortality within six years among adult trauma care patients, as well as to identify independent predictors of mortality. Methods: This retrospective study included all patients ≥ 18 years with primary hip fractures who were admitted to our level I trauma center from 1 January 2015, to 31 December 2018, and who u

Rotational malalignment is relatively common after intramedullary nailing of femoral fractures and may lead to significant clinical symptoms. Comminution, inherent differences in femoral version, and the absence of reliable clinical markers make achieving anatomical rotational reduction challenging, even for experienced surgeons. Various methods have been described to surgically correct rotational deformities, depending on the time elapsed since the original surgery and whether the fracture has already united. Both open and closed techniques have been used, along with different types of osteos

BACKGROUND: Subtrochanteric femur fractures present complex biomechanical and biological challenges with considerable variability in management approaches. Despite a structured approach to operative fixation using the intramedullary nail being accepted as the gold standard for most subtrochanteric fractures, a number of high-impact clinical dilemmas lack clarity and consensus on management approaches due to limited high-level clinical and published evidence.
METHODOLOGY: We identified 11 key controversies through a comprehensive literature review of the PubMed, Scopus, and Cochrane databases

We present a case of an 82-year-old woman with an atypical femoral fracture associated with prolonged bisphosphonate therapy. After a decade of ibandronate use for osteoporosis, she sustained a subtrochanteric femoral fracture following minimal trauma. Radiographic findings were consistent with atypical femoral fracture characteristics, and surgical management with intramedullary nailing was performed. Laboratory evaluation revealed no evidence of secondary bone disease. This case highlights the importance of monitoring patients on long-term bisphosphonates and considering drug holidays to min

BACKGROUND: Intra or extra medullary fixation for subtrochanteric fractures (STF) is still controversial. And the strain on fracture site and implant have not been reported. The aim of our study is to evaluate the differences in strain at the medial and lateral cortex of the subtrochanteric region with PFNA, 'upside-down' LISS and PFLP fixing different types of subtrochanteric femoral fractures under static loads.
HYPOTHESIS: Our hypothesis was that the PFNA was superior than LISS and PFLP for fixation of STF. However, PFNA also had some weakness.
METHODS: Thirty composite femurs with PFNA,

PURPOSE: Femoral neck fractures (FNFs) have become increasingly common among young adults as a result of high-energy trauma. Despite the variety of implants available for the surgical treatment of FNFs, compelling evidence of clinically uniform outcome is lacking, and high complication rates are still reported. In this scenario, it seems reasonable to investigate novel implants and surgical, especially in Pauwels type III (PIII) FNFs. The objective of this study was to describe the fatigue test results of a construct using the self-compression screw model (X-PIN + P) in PIII FNFs within a vali

PURPOSE: The aim of this study was to compare the biomechanical results of long proximal femoral bionic nail (PFBN) and three conventional intramedullary nails in the treatment of subtrochanteric fractures (STFs).
METHODS: Using finite element analysis, we compared the therapeutic efficacy of four long intramedullary nails: the PFBN, reconstruction nail (RCN), InterTAN nail (ITN), and proximal femoral nail antirotation (PFNA) for the treatment of Seinsheimer type IIIA and type V STFs. The biomechanical stability of the implants was evaluated by calculating of von Mises stress (VMS), contact p

OBJECTIVE: About 7%-34% of the femur fracture contributes to subtrochanteric fracture, and only very little research is available about these fractures when compared to common hip fractures. Hence, the aim of this study was to develop a clinically relevant and reproducible open fracture model in rabbits at the subtrochanteric region to understand the fracture healing mechanism at this site and to explore treatment effects of biophysical intervention in future studies.
METHODS: An open osteotomy was created in 32 adult New Zealand white rabbits at the subtrochanteric region, followed by custom

BACKGROUND: Subtrochanteric fractures, which constitute 5-10% of proximal femoral fractures, are particularly challenging due to their high incidence of nonunion and failure of internal fixation. The incidence of deep venous thrombosis (DVT) in the lower extremities is notably higher among patients with orthopedic trauma, leading to significantly increased healthcare costs and prolonged hospitalization. The number of studies focusing on the incidence and risk factors among the patients with subtrochanteric fractures was quite limited. Therefore, there is a lack of effective methods of identifi

OBJECTIVES: The Femoral Neck System (FNS) is a fixed-angle side plate device approved for use in fixation of femoral neck fractures. The FNS perforates the lateral cortex of the subtrochanteric femur, which may increase the risk of postoperative subtrochanteric fractures compared with the inverted triangle cannulated screw (CS) construct. The purpose of this biomechanical study was to compare forces required to create subtrochanteric fractures in FNS and CS constructs in a synthetic bone biomechanical model.
METHODS: Osteopenic sawbones were divided into five groups: Noninstrumented femurs, C

The Femoral Neck System (FNS) offers high resistance against angular instability and femoral head rotation by simultaneously remaining minimally invasive. This study reports the complication profile of FNS in real-time to assess the risk-benefit of this device. The Manufacturer and User Facility Device Experience (MAUDE) nationwide database was used to identify complications related to the FNS between January 1, 2018 and January 1, 2022. Data collected included the date the reports were received by the FDA, the type of complication, event description, and the source of report. Of the 182 compl

INTRODUCTION: Femoral shaft fractures are a common entity in trauma surgery. The gold standard is closed reduction and intramedullary nailing. However, in some fracture patterns, the surgeon may choose to open the fracture for reduction. The purpose of this retrospective study was to analyze the outcome of femoral shaft fractures treated with open reduction, cerclage wiring and intramedullary nailing.
METHODS: We included adult patients with femoral shaft fractures treated with open reduction, cerclage wiring and intramedullary nailing at a level 1 trauma center in Switzerland during an 11-ye

INTRODUCTION: Bisphosphonate-associated atypical femur fractures have been reported to have high rates of delayed union and nonunion, as well as high rates of contralateral fracture. There have been limited studies looking at specific factors associated with delayed union or nonunion of atypical fractures. Many studies are limited by small numbers of patients. To date, no studies have examined the effect of reduction technique (closed versus open versus percutaneous) and reduction quality on healing.
METHODS: This was a multicentered retrospective review of patients greater than 50 years of a

INTRODUCTION: The acromioclavicular (AC) ganglion cyst first described by a shoulder arthrogram by Craig in 1984, still remains an uncommon presentation of a shoulder pathology. Classified as a type one cyst by Hiller in the presence of a rotator cuff pathology, the management of these lesions is governed by ill-defined guidelines. Literature shows the variability of surgical management ranges from simple cyst excision and lateral end clavicular excision to reverse shoulder arthroplasty. Type two cysts are usually benign ganglion cysts, filled with mucinous putty material, and arise from the A

BACKGROUND: Subtrochanteric fracture is a recognized complication following osteosynthesis for femoral neck fractures, typically occurring within 1-2 months postoperatively. At the time of onset, incomplete healing of the femoral neck fracture often results in a complex fracture configuration that poses challenges for management.
CASE PRESENTATION: A 60-year-old woman with schizophrenia sustained a Garden Type I femoral neck fracture after a fall and underwent osteosynthesis with three sliding hip screws. Three months later, she developed marked hip pain without any new trauma. Radiographs re
