Enhanced Reticulospinal Output in Subacute Spinal Cord Injury Patients with Spasticity.

In the chronic phase of spinal cord injury (SCI), damage to the reticulospinal and corticospinal tracts has been associated with the development of spasticity. Because spasticity can emerge within just a few weeks postinjury, this suggests that reorganization of these motor pathways may begin during the subacute phase. To test this hypothesis, we assessed the StartReact response-an involuntary release of a planned movement via a startling stimulus that engages the reticulospinal tract-by measuring reaction times from electromyographic activity, motor evoked potentials (MEPs) by stimulating the

No Differences Exist in Leg-Length-Normalized Y-Balance Test Scores Between Injured and Uninjured Baseball Athletes.

CONTEXT: Deficits in balance can negatively impact throwing biomechanics in overhead throwing athletes. Previous work has demonstrated discrepancies in balance, measured by the Y-Balance Test Lower Quarter (YBT-LQ), in baseball athletes. However, there is limited data regarding anterior reach (ANT) YBT-LQ scores in baseball athletes with shoulder and elbow injuries and between athletes undergoing surgical versus conservative treatment. Therefore, the purpose of this study was to determine whether differences exist in ANT YBT-LQ scores between baseball athletes with elbow and shoulder injuries

Acute Medial Collateral Ligament With Postero-Medial Complex Repair in Multiligament Knee Injuries Using a Novel "Double Shoelace Repair" Technique: 10-Year Clinical and Functional Outcomes.

BACKGROUND: Femoral-sided medial collateral ligament (MCL) avulsion and posteromedial corner (PMC) injury frequently occur together in the setting of multiligament knee injuries (MLKIs). Untreated PMC injuries are prone to develop residual valgus and rotatory instability. Early anatomic repair optimises outcomes. This study evaluates the long-term results of a novel "double shoelace repair" technique for acute MCL and PMC injuries performed with concurrent cruciate reconstructions. METHODS: A retrospective analysis was conducted on patients with MLKI who underwent acute (less than three weeks

Does Insertional Reattachment for Acute Sleeve Avulsion Fracture of the Achilles Tendon Provide Sustained Function and Sports Participation?

BACKGROUND: Insertional reattachment of the Achilles tendon is the recommended surgical treatment for acute sleeve avulsion fractures at the tendon insertion in patients fit for surgery and who are comfortable taking the risks associated with the procedure. However, there is a paucity of evidence regarding the longitudinal clinical outcomes and sustained sports participation at minimum 2- and 5-year follow-up after this procedure. QUESTIONS/PURPOSES: (1) What are the 2- and 5-year patient-reported outcomes after insertional reattachment for acute sleeve avulsion fractures of the Achilles tend

Constancy of the anterior lateral malleolar artery anastomosis in retrograde lateral supramalleolar flaps: Anatomical basis and clinical relevance.

BACKGROUND: The lateral supramalleolar (LSM) flap is traditionally based on peroneal perforators, but its retrograde vascularization via the anterior lateral malleolar artery (ALMA) remains insufficiently defined. Clarifying this pathway may expand reconstructive options for dorsal forefoot defects. METHODS: Anatomical dissections were performed on 31 fresh above-knee amputated limbs, focusing on the retrograde anastomosis between the ALMA and the descending branch of the peroneal artery. Distances, diameters, and perfusion were documented by direct measurements and contrast radiography. Surf

The Use of Suture Tape through a Single Tunnel to Fix Posterior Cruciate Ligament Avulsion Fracture of Tibia, Via Arthroscopy: A Case Series of 16 Patients.

Posterior cruciate ligament (PCL) avulsion fractures from the tibial insertion are rare but clinically notable injuries that can compromise knee stability if not treated effectively. Arthroscopic approaches have increasingly replaced open surgery for such injuries due to their less invasive nature and lower risk of postoperative issues. This retrospective case series evaluates the clinical and radiological outcomes of 16 patients treated with a single suture tape pullout approach for PCL tibial avulsion fractures. The average age of the patients was 29.6 years, and all were treated within 3 we

Effects of early acute in-bed leg cycling initiated from the intensive care unit following severe traumatic spinal cord injury: final results from the PROMPT-SCI trial.

BACKGROUND: Preclinical studies suggest that initiating activity-based therapy (ABT) within days of an acute spinal cord injury (SCI) best enhances recovery, but may also increase the risk of inducing spinal cord damage. This study assesses the effects of early ABT initiated within 48 h of spinal surgery (or 3 days of the SCI), while patients are in the intensive care unit (ICU) for hemodynamic management. METHODS: This single-arm proof-of-concept trial included 45 adults with a severe traumatic SCI receiving daily 30-minute sessions of in-bed leg cycling for 14 consecutive days, in comparison

A hypothalamus centred pathogenesis of heat stroke deaths - A postmortem-based human study.

Heat stroke deaths are increasing during the summer in tropical countries but the pathogenesis mechanisms are not well established. Postmortem-based human studies in heat stroke cases are few in the literature, which makes post mortem forensic determination of the cause of death extremely difficult. We consider four cases (three male and one female, age range: 40-60 years) who were brought to the mortuary for forensic determination of the cause of death. Autopsy followed standard protocol and gross observations were made. Visceral tissue samples taken for histological staining and microscopic

Knee and trunk biomechanics during a single leg drop vertical jump. The effects of horizontal distance, sex, and limb dominance.

BACKGROUND: The drop vertical jump (DVJ) is used to identify biomechanical risk factors associated with non-contact anterior cruciate ligament (ACL) injuries, which often occur during eccentric loading. However, the influence of the horizontal jump distance, sex, and limb laterality on lower extremity and trunk biomechanics remains unclear. METHODS: This cross-sectional study recruited 132 adolescents (83 males, 49 females, 15.0 ± 1.6 y) from a school strength and conditioning program. Athletes jumped from a 0.30 m box at two distances (0.15 m and 0.45 m). Kinematic and kinetic data were coll

Distal femoral replacement carries higher infection and revision risk than ORIF for distal femoral periprosthetic fractures in elderly patients.

INTRODUCTION: Distal femoral periprosthetic fractures following TKA are increasingly common in elderly patients. Surgical management most commonly involves either open reduction and internal fixation (ORIF) or distal femoral replacement (DFR); however, comparative data regarding short-term and long-term complications remain limited. As such, this study compared complications between ORIF and DFR in elderly patients with distal femoral periprosthetic fractures following TKA. METHODS: A retrospective cohort study was performed using the TriNetX Research Network. Patients aged ≥65 years with dis
