[Surgical treatment of obstructive sleep apnea hypopnea syndrome after velopharyngeal ring ligation procedure].

Purpose  A degenerative meniscus lesion is a slowly developing process typically involving a horizontal cleavage in a middle-aged or older person. When the knee is symptomatic, arthroscopic partial meniscectomy has been practised for a long time with many case series reporting improved patient outcomes. Since 2002, several randomised clinical trials demonstrated no additional benefit of arthroscopic partial meniscectomy compared to non-operative treatment, sham surgery or sham arthroscopic partial meniscectomy. These results introduced controversy in the medical community and made clinical dec

Correlation between seminal carnitine and functional spermatozoal characteristics in men with semen dysfunction of various origins.

von Willebrand disease (vWD) is a single-locus disorder resulting from a deficiency of von Willebrand factor (vWF): a multimeric multifunctional protein involved in platelet adhesion and platelet-to-platelet cohesion in high shear stress vessels, and in protecting from proteolysis and directing circulating factor VIII (FVIII) to the site of injury. vWD is the most frequent bleeding disorder, with an estimated prevalence in the general population of 1%. Almost all these cases are represented by a partial quantitative deficiency of von Willebrand factor (vWF) (type 1 vWD). Type 1 is transmitted

Large lateral meniscal ganglion cyst extending into the intercondylar fossa of the knee.

We report the case of a 31-year-old, otherwise healthy man with a large intra-articular meniscal ganglion cyst (27.7 x 13.5 mm) originating from the dorsal horn of the lateral meniscus. Clinically, the patient presented with knee pain in a squatting position. Magnetic resonance imaging revealed a large intra-articular cyst in the posterior compartment. At arthroscopic surgery, the ganglion cyst was found in the intercondylar space posteriorly to the posterior cruciate ligament. After removal of the ganglion cyst, a horizontal tear in the dorsal horn of the lateral meniscus was revealed and tre

Meniscal allograft transplantation--part I: background, results, graft selection and preservation, and surgical considerations.

Removal of the meniscus leads to progressive degenerative arthritis of the knee on a long-term basis. Therefore, meniscal allograft transplantation has been proposed as an alternative to meniscectomy. Although several experimental and clinical studies have documented that meniscal allografts show capsular ingrowth in meniscectomized knees, it remains to be established whether meniscal allograft transplantation can prevent degenerative changes after meniscectomy. Part 1 of this Current Concepts review will discuss the function, anatomy, and composition of the meniscus, followed by the history o

[Arthroscopic meniscus surgery].

Surgery on the meniscus of the knee joint is one of the most frequently performed operations. The arthroscopic technique currently represents the general treatment standard and except in a few cases has completely superseded the open approach. Arthroscopy facilitates precise diagnosis and classification of all forms of meniscus lesions. In many cases even today partial resection on irreparably damaged meniscus tissue cannot be avoided. The goal is to preserve as much healthy meniscus tissue as possible. Types of tears amenable to reconstruction always require arthroscopic refixation. Various t

Mechanical stimulation toward tissue engineering of the knee meniscus.

Current clinical practices do not adequately regenerate the meniscus of the knee secondary to a tear. Complete or partial meniscus removal leads to degenerative changes within the joint. Tissue engineering of the meniscus promises a potent solution. Before embarking on tissue engineering of the meniscus, it is crucial to have a thorough comprehension of the biomechanical role that this tissue fulfills and how the structure of meniscus is uniquely suited to that purpose. To better understand this, we have examined the meniscus, as well as associated tissues, within the body. For the first time,

Subchondral giant-cell tumor of the proximal tibia: arthroscopic treatment for accelerated articular cartilage and meniscal degeneration in two patients.

Giant-cell tumor most commonly occurs in the distal femur and proximal tibia and characteristically involves the subchondral bone. Incomplete resection leads to recurrence rates of up to 50%. Intralesional curettage, adjuvant treatments, and polymethyl methacralate (PMMA) reconstruction is the current mainstay of treatment and has produced recurrence rates of less than 10%. Achieving adequate curettage while preserving the articular cartilage of the tibial plateau poses a significant challenge, especially when the tumor involves the subchondral bone. We report on 2 cases, both with symptomatic

MR appearance of SONK-like subchondral abnormalities in the adult knee: SONK redefined.

OBJECTIVE: To investigate the MR characteristics of SONK-like (spontaneous osteonecrosis of the knee) subchondral abnormalities in the adult atraumatic knee and to recategorize these patients into two subgroups: a subacute to chronic process associated with osteoarthritis and an acute process associated with insufficiency fractures. DESIGN: We retrospectively examined the knee MRIs of 39 patients with non-specific interpretations of osteochondral abnormalities. PATIENTS: There were a total of 52 subchondral lesions without any known traumatic event and no prior surgery. All lesions evaluated

Torn discoid lateral meniscus treated using partial central meniscectomy and suture of the peripheral tear.

We present the cases of 5 patients with a torn discoid lateral meniscus treated using partial central meniscectomy in conjunction with the suture repair of the tear. The patients were 4 boys and 1 girl with a mean age of 15.4 years (range, 11 to 17). Preoperatively, all patients complained of knee pain during daily or sports activities. At the final follow-up evaluation, more than 2 years later, 4 patients' conditions were graded as excellent and 1 patient's condition was graded as fair according to Ikeuchi's grading scale. The average Lysholm score improved from 83.4 points (range, 70-90) to

Cyclic joint loading can affect the initial stability of meniscal fixation implants.

OBJECTIVE: To identify the effect of cyclic joint loads on the initial fixation strength of different meniscal repair techniques. DESIGN: Three different meniscal refixation implants and one suturing technique were tested for initial stability in porcine knee joints after cyclic joint loading. BACKGROUND: Existing studies tested the initial pull out strength of meniscal fixation implants. It remains unknown how such implants perform under cyclic joint loading. METHODS: The Meniscal Fastener, the Meniscal Arrow, the ClearFix Screw, and a horizontal suture (PDS 1) were implanted in 64 porcine

Trabecular bone strain changes resulting from partial and complete meniscectomy.

Previous studies have documented how partial and complete meniscectomy affect articular contact pressure, but changes in load transfer through the complete osteochondral structure of the proximal tibia after partial and complete meniscectomy are not well known. The current study measured trabecular bone strain changes in the medial tibial plateau resulting from partial and complete medial meniscectomy. Midcoronal sections were prepared from knees from cadavers. High quality digital images, made from contact radiographs of loaded samples, were compared with digital images of unloaded samples us

Gapping phenomenon of longitudinal meniscal tears.

OBJECTIVE: To investigate the gapping behaviour of longitudinal meniscal tears. DESIGN: The gap size of longitudinal meniscal tears was measured in porcine knee joints under various joint loading conditions. BACKGROUND: Many meniscal fixation implants have low pull out forces. However, it is unknown if these forces are less than the forces the implants must resist in vivo. It is also unknown if gapping occurs in longitudinally torn menisci and what joint loads induce gapping. METHODS: Longitudinal tears were set in the medial menisci of eight porcine knee joints. To observe the tears an opa

Effects of selected growth factors on porcine meniscus in chemically defined medium.

Some evidence shows that selected growth factors can increase proteoglycan synthesis and that fibrin clot aids in the repair of meniscal tears. A significant (P<.05) dose-dependent stimulation of proteoglycan synthesis was found with platelet-derived growth factor AB (PDGF-[AB]), transforming growth factor-beta1 and bone morphogenetic protein-7 (osteogenic protein-I). In separate experiments, a significant increase in cellularity was found in the deep and superficial aspects of the explants treated with PDGF-(AB) compared with those treated with basal medium alone. The current study provides d

Repeat tears of repaired menisci after arthroscopic confirmation of healing.

We undertook 114 arthroscopic meniscal repairs in 111 patients and subsequently carried out second-look arthroscopy to confirm meniscal healing at a mean of 13 months after repair. Stable healing at the repaired site was seen in 90. Of these, however, 13 had another arthroscopy later for a further tear. The mean period between the repair and the observation of a repeat tear was 48 months. Of the 13 patients, 11 had returned to high activity levels (International Knee Documentation Committee level I or II) after the repair. An attempt should be made to preserve meniscal function by repairing te

Lack of posteromedial tibiofemoral congruence at full flexion as a causative factor in isolated medial meniscal tears.

The aim of the current study was to investigate posteromedial tibiofemoral congruence at full flexion of the knee in control knee joints and those affected by an isolated medial meniscal tear, to examine whether lack of such congruence was a causative factor in isolated medial meniscal tears. In this study, 1677 knee joints in 875 subjects were evaluated. The joints were classified as a control group (1345 joints), an isolated medial meniscal tear group (224 joints), and a contralateral isolated medial meniscal tear group (108 joints). Posteromedial tibiofemoral congruence was examined on a la

A comparative study to assess the clinical use of Fluorescein Meniscus Time (FMT) with Tear Break up Time (TBUT) and Schirmer's tests (ST) in the diagnosis of dry eyes.

INTRODUCTION: The clinical diagnosis of dry-eye is confirmed by a suitable test of tear production and the technique commonly used today to diagnose dry eye is the Schirmer's test (ST). Although the ST is easy to perform it gives variable results, poor reproducibility and low sensitivity for detecting dry eyes. Another test, the tear break up time (TBUT) is used to assess the stability of tears which if abnormal may also cause symptomatic dry-eye. We present the results of both these tests and a new test, which shows greater sensitivity than the ST in detecting aqueous tear deficiency. The flu
