Maternal and fetal outcomes of spinal tuberculosis in pregnancy: A systematic review and descriptive pooled analysis.

BACKGROUND: Spinal tuberculosis (TB) in pregnancy is rare and poses diagnostic, neurological, and obstetric challenges. Data are limited to isolated case reports and small series, precluding evidence-based guidance on management and outcomes. This review aims to systematically review reported cases of spinal TB in pregnancy, summarize maternal and fetal outcomes, and perform a descriptive pooled synthesis of reported live births and maternal neurological recovery. MATERIALS AND METHODS: A systematic search of PubMed, Embase, and Scopus, (inception-October 2025) identified case reports and ser

When Infection Masquerades as Metastasis: Tuberculous Spondylodiscitis with Fibrous Dysplasia and Capillary Hemangioma in a Patient with Newly Diagnosed Hepatic Mass.

Tuberculous spondylodiscitis can closely mimic spinal metastasis, particularly in patients with coexisting malignancy risk factors and atypical imaging findings. We report a diagnostically challenging case of a 70-year-old male with chronic hepatitis B infection who presented with progressive midback pain and was found to have a hypervascular hepatic mass with elevated serum alpha-fetoprotein levels, raising strong suspicion for hepatocellular carcinoma with vertebral metastasis. Magnetic resonance imaging demonstrated destructive D10-D11 spondylodiscitis with posterior element involvement and

Atypical bone lesion of the fifth metatarsal bone in an HIV-positive patient: A challenging radiologic-pathologic correlation of tuberculous osteomyelitis.

Musculoskeletal tuberculosis (TB) represents a small fraction of extrapulmonary TB, accounting for only 1%-3% of cases. Involvement of the foot bones is exceedingly rare, particularly in adults, and often mimics neoplastic or pyogenic processes. Early diagnosis is challenging, especially in immunocompromised individuals, where radiologic findings can be atypical. We describe a 66-year-old HIV-positive male (CD4 count 17 cells/μL) who presented with progressive bilateral foot swelling. Radiography revealed a permeative osteolytic lesion with cortical thinning of the left fifth metatarsal bone.

Pott's disease in a person with progressive massive fibrosis.

Silicosis is the most common occupational lung disease and association with Mycobacterium tuberculosis infection is common. Although extrapulmonary tuberculosis cases are rare, skeletal tuberculosis is responsible for 10-35% of these cases. Pott's disease is the most common form of skeletal tuberculosis, but it is not commonly encountered today. A 49-year-old male patient presented to our clinic with complaints of back pain, leg pain, and weight loss for 9 months. His occupational history revealed that he worked as a dental technician and a sandblasting for 30 years. In 2002, he was diagnosed

Development and evaluation of a nomogram model for predicting prolonged hospitalization after spinal tuberculosis focus decompression, fusion, and internal fixation surgery.

BACKGROUND: Spinal tuberculosis remains a significant clinical challenge in high-prevalence regions. Despite advances in medical treatment, surgical interventions, such as debridement, decompression, and fusion, are often required, but their complexity increases perioperative morbidity and prolongs hospitalization. Identifying preoperative predictors of extended hospital stay may improve patient management and resource allocation. METHODS: In this retrospective study, 256 patients with confirmed spinal tuberculosis who were treated between January 2021 and December 2024 were included. Prolong

Unveiling the microbiological experience in exploring spinal TB cases from a tertiary care center.

PURPOSE: Spinal tuberculosis, a severe form of extrapulmonary tuberculosis, poses significant diagnostic and therapeutic challenges, often leading to neurological complications and deformity. Microbiological confirmation is crucial for diagnosis and effective treatment. We aim to evaluate the microbiological profile and diagnostic yield of various methods in spinal TB cases at a tertiary care centre and to correlate with clinical features and treatment outcomes. METHODS: A prospective study was conducted on samples from patients with suspected spinal TB (n = 58) from July 2023 to September 20

Two needles, one haystack: Lessons from spinal tuberculosis mimicking lumbar epidural complication in a post-partum patient.

While both considered rare in contemporary Australian anaesthetic practice, the symptoms of epidural complication can mimic those of spinal tuberculosis, as we outline here in this unusual case. A 33-year-old woman presented to hospital with new neurological deficits days after receiving a lumbar epidural for labour analgesia. A complication of epidural was initially suspected; however, spinal tuberculosis was found be the cause. This case demonstrates many of the challenges and unique requirements involved in providing neuraxial anaesthesia to the obstetric cohort, namely: consenting linguist

Utility of nucleic acid amplification tests in the diagnosis of osteoarticular tuberculosis.

Osteoarticular TB (OATB) is a manifestation of extrapulmonary TB (EPTB), wherein spinal TB constitutes ∼50% of OATB individuals. Diagnosis of OATB is quite difficult owing to deep inaccessible lesions within tissues, scanty bacterial load and imitation of disease with other osteoarticular conditions. OATB diagnosis mostly relies on clinical features and imaging in endemic countries, however, it is difficult to distinguish between OATB and inflammatory arthritis, i.e. rheumatoid arthritis, gout, ankylosing spondylitis, etc. using these modalities. Other laboratory tests include bacteriological

Risk factors for complications after complete debridement in patients with thoracolumbar spinal tuberculosis: A retrospective study.

Many studies have been published on the characteristics of spinal tuberculosis, but not yet on the risk factors for complications after complete debridement. This study aimed to investigate the risk factors of postoperative complications in patients with thoracolumbar spinal tuberculosis (TB) after complete debridement. The clinical data of patients with thoracolumbar spinal TB after complete debridement admitted to General Hospital of Ningxia Medical University from January 2013 to December 2021 were included in this retrospective study. Patients were included if they had complete clinical da

Identification of an autophagy-related novel signature for spinal tuberculosis: a multi-cohort machine learning study and Mendelian randomization analysis.

BACKGROUND: Spinal tuberculosis (STB) is an infectious disease caused by Mtb with unclear diagnosis and molecular mechanisms. Autophagy is reported to be associated with the pathology of spinal tuberculosis. The present study intends to elucidate the role of autophagy-related miRNAs and genes in STB. METHODS: Core miRNAs were identified through WGCNA and differential analysis. A total of 113 machine learning algorithms were used to develop a diagnostic model. Target genes were predicted and overlapped by TargetScan, miRDB, and miRTarBase. The two-sample Mendelian randomization analysis was ut

Tuberculous abscess of the left gluteal region and iliac fossa following transvaginal ultrasound-guided oocyte retrieval: a case report.

Osteoarticular and soft-tissue tuberculosis is relatively uncommon in clinical practice and is often characterized by insidious onset and non-specific symptoms. Its manifestations may mimic those of degenerative disorders, frequently leading to delayed or incorrect diagnosis. These challenges are further amplified when the infection occurs at rare, procedure-related sites, which are easily overlooked. We report the case of a 31-year-old woman who developed persistent pain in the left gluteal and lower abdominal regions after ultrasound-guided transvaginal ovarian needle drilling (UTND) perform

Rare Presentation of Calcaneal Tuberculosis Treated with Debridement and Antibiotic-impregnated Cement.

Calcaneal tuberculosis (TB) is an uncommon manifestation of osteoarticular TB, accounting for <0.1% of skeletal cases. Its nonspecific features such as heel pain and swelling without systemic signs frequently mimic chronic osteomyelitis or bone tumors, often resulting in delayed diagnosis and risk of long-term disability. We report a case of a 20-year-old male presenting with progressive heel pain and swelling. Diagnostic evaluation included plain radiographs, which demonstrated a lytic lesion in the posterior-inferior calcaneus, and magnetic resonance imaging, which revealed marrow involvemen

Uncommon Sacroiliac and Presacral Tuberculosis Mimicking Chronic Low Back Pain in a Young Female.

Tuberculosis (TB) of the sacroiliac (SI) joint is an uncommon manifestation of osteoarticular TB, typically presenting with vague, long-standing back pain that resembles mechanical or inflammatory disorders. Because of its deep anatomical location, paucibacillary load, and lack of awareness, diagnosis is often delayed. We describe an 18-year-old female from a TB-endemic area with a 3-year history of persistent low back pain radiating to the left gluteal region. Despite repeated symptomatic treatment, symptoms persisted. Examination showed SI joint tenderness and positive FABER and Gaenslen's t

Spinal Tuberculosis Diagnosed by Metagenomics Capture (MetaCAP) in a Patient Undergoing Maintenance Hemodialysis: A Case Report.

BACKGROUND Spinal tuberculosis is difficult to diagnose in patients undergoing maintenance hemodialysis (MHD) because of immunosuppression, atypical clinical manifestations, and the limited sensitivity of conventional microbiological assays. Rapid and accurate pathogen identification is essential to distinguish spinal tuberculosis from other causes of vertebral destruction, including metastatic malignancy and bacterial spondylitis. This report aims to illustrate the diagnostic value of capture-based targeted sequencing for detecting Mycobacterium tuberculosis in extrapulmonary infection when r

Evaluation of the efficacy of retroperitoneoscopic debridement for lumbar tuberculosis: a retrospective study and preliminary results.

BACKGROUND: Lumbar tuberculosis (LTB) is a significant global health concern, often requiring surgical intervention when medical treatment is insufficient. Retroperitoneoscopic debridement offers a minimally invasive approach to manage LTB, potentially reducing complications and recovery time compared to traditional open surgery. However, its efficacy and safety remain understudied. This retrospective cohort study aims to evaluate the clinical outcomes, complication rates, and long-term effectiveness of retroperitoneoscopic debridement in patients with LTB. METHODS: This retrospective cohort
