Characteristics and predictors of clinical outcome in patients with pleural effusions caused by heart, liver and renal failure: results from the ERS International Multicentre Pleural Research Collaborative (IMPACT) registry.

INTRODUCTION: Pleural effusions caused by organ dysfunction are the commonest pleural disease and account for a huge healthcare burden. Previous work has demonstrated poor survival rates, but there is still uncertainty about determinants of prognosis. This study describes the characteristics and risk factors for poor outcomes in patients with pleural effusion secondary to organ failure in an international cohort. METHODS: The European Respiratory Society International Multicentre Pleural Research Collaborative (IMPACT) registry includes an international retrospective study of patients with ef

A Rare Case of Hemophagocytic Lymphohistiocytosis Triggered by Intravesical Bacillus Calmette-Guérin (BCG) for Bladder Cancer.

Intravesical bacillus Calmette-Guérin (BCG) therapy is widely used for non-muscle-invasive bladder cancer but can rarely lead to severe systemic complications. Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome characterized by fever, cytopenias, and organ dysfunction. We describe a 63-year-old man who developed HLH after re-induction BCG therapy. He presented with fever, hepatosplenomegaly, pancytopenia, and liver and kidney dysfunction. Laboratory tests revealed hyperferritinemia and elevated soluble interleukin-2 receptor levels. Liver biopsy showed no

Isolated anal tuberculosis presenting as an anal fistula in an immunocompetent child.

Extra-pulmonary tuberculosis accounts for less than 15% of all tuberculosis cases, while intestinal tuberculosis accounts for less than 1% of extra-pulmonary forms of the disease. Abdominal organ involvement is more common, but extension to the ano-perineal region is extremely rare. We report a case of a 13-year-old child with an anal fistula without any other signs suggestive of tuberculosis. Diagnosis was confirmed by histopathological examination of the excised fistula and a positive GeneXpert test on fistula material. The initial work-up ruled out Crohn's disease and other localizations se

Screening and monitoring across the lifespan of an ageing cystic fibrosis population.

Cystic fibrosis (CF) is a genetic disorder with involvement of many organ systems. For some of the affected organs, the dysfunction is a direct consequence of cystic fibrosis transmembrane conductance regulator (CFTR) deficiency, while in others the pathophysiology is complex and may be influenced by treatments and social factors. Recent decades have seen improved wellbeing and survival of people with CF, even more so with the introduction of CFTR modulator treatments. For many manifestations of CF, the focus has shifted from treatment of complications to screening and monitoring for early det

Extended depth-of-field fluorescence microscope for stable imaging during random positioning machine running.

BACKGROUND: Takayasu arteritis (TA) is a chronic granulomatous large-vessel vasculitis that primarily affects the aorta and its major branches. It is rare in children and adolescents, often leading to diagnostic delays due to its nonspecific initial symptoms such as fever, fatigue, and arthralgia. CASE PRESENTATION: We report a 15-year-old male who presented with a several-month history of low-grade fever, easy fatigability, and progressive bilateral multiple joint pain. Notably, he had a history of BCG vaccination and no known tuberculosis exposure. Three weeks prior to diagnosis, he develop

Global, regional, and national burden of Chagas disease, 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Chagas disease is a neglected tropical disease caused by the protozoan Trypanosoma cruzi, primarily transmitted by infected bugs, but also through contaminated food, transfusions, congenital transmission, and organ transplantation. Chagas disease has acute and chronic phases; the chronic phase can occur decades after infection, leading to complications such as heart failure, arrhythmias, and megaviscera. Accurate mortality and morbidity estimates are hindered by under-reporting and misclassification. Comprehensive and updated estimates are needed to improve global assessments of Ch

Tuberculosis disease characteristics associated with mortality, severe morbidity and unsuccessful treatment in people living with HIV treated for tuberculosis - a secondary analysis of the ANRS 12300 Reflate TB2 trial.

BACKGROUND: Tuberculosis is a severe disease, not only due to its lethality but also to a significant morbidity occurring in people living with HIV (PLWH). If factors associated to mortality, severe morbidity and unsuccessful treatment related to the host are well identified in PLWH, there is scarce knowledge on factors related to the disease itself such as bacillary load, extent of lung involvement and disease dissemination to other organs. We sought to assess whether tuberculosis-related factors were associated with key patient outcomes in PLWH using data from an international clinical trial

Infectious facade of autoimmunity: when SLE masquerades as tuberculosis.

Systemic lupus erythematosus (SLE) is a chronic autoimmune disorder characterised by widespread inflammation affecting multiple organ systems. Its varied clinical manifestations often complicate diagnosis, as it can closely mimic other conditions, particularly infectious diseases such as tuberculosis. This case report describes a woman in her early 20s initially suspected of having disseminated tuberculosis and treated accordingly due to overlapping symptoms, including fever, night sweats, lymphadenopathy and bilateral exudative pleural effusion. Subsequent evaluation, however, confirmed a dia

Nutritional Deficiencies and Management in Tuberculosis: Pharmacotherapeutic and Clinical Implications.

Tuberculosis is an infectious condition caused by Mycobacterium tuberculosis, primarily targeting the pulmonary system, with the potential to disseminate to various other organs via the haematogenous pathway, ranking among the top ten causes of global mortality. Tuberculosis remains a serious public health problem worldwide. This narrative review aims to emphasise the clinical importance of the inter-relationships between nutrition, pharmacotherapy, and the most common drug-nutrient interactions in the context of tuberculosis and multi-drug-resistant tuberculosis management. Nowadays, pharmaco

Potential biomarkers for tuberculous meningitis diagnosis using metabolomics and proteomics: a systematic review.

Tuberculosis (TB) is the leading cause of death from a single infectious agent, with approximately 1.2 million deaths reported in 2023. While TB primarily affects the lungs, it can also spread to other organs, where it is classified as extrapulmonary TB. Tuberculous meningitis (TBM) is the most severe form of extrapulmonary TB, affecting 1–5% of TB cases. Delayed diagnosis contributes to its high mortality and severe neurological complications, with approximately 10% of affected individuals dying or suffering permanent neurological damage. When combined with adjunctive therapy, early detection

Advances in tuberculosis co-infections: Mechanisms, diagnosis, and therapeutics.

Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis, which is widespread and harmful worldwide. It can attack multiple organs throughout the body, with pulmonary tuberculosis being the most common. The Global Tuberculosis Report 2024 shows an increase in the incidence of TB from the previous year, and its transmission is closely linked to the response of the host immune system. In clinical settings, co-infections with other pathogens are becoming more common. 10.8 million new TB cases were diagnosed globally in 2023, of which 662,000 were co-infected with HIV, and t

Epidemiological and clinical analysis, and outcomes of tuberculosis co-infection among people living with HIV in Türkiye (2014-2024) ClinSurv HIV cohort: A large case series.

BACKGROUND: Tuberculosis (TB) is one of the most common opportunistic infections in people living with HIV (PLHIV). Mycobacterium tuberculosis may cause more TB in all stages of HIV infection than in the general population, with the incidence of TB and the spread of pulmonary TB to other organs increasing as the CD4 count decreases. OBJECTIVE: In this HIV cohort study, we aimed to evaluate the clinical features, diagnosis, and prognosis of TB among PLHIV in Türkiye. MATERIALS AND METHODS: We conducted a retrospective cohort study to analyze clinical outcomes and identify determinants of mort

Isolated splenic tuberculosis mimicking a solid splenic neoplasm in an immunocompetent patient: A surgical case report.

INTRODUCTION AND IMPORTANCE: Tuberculosis is the leading cause of death by an infectious agent globally, especially among immunocompromised patients. Extrapulmonary tuberculosis (EPTB) can occur as a sequela of pulmonary tuberculosis through lympho-hematogenous or miliary spread, or can occur in isolation. EPTB rarely occurs in isolation in a single organ, especially in immunocompetent patients. Limited data exist on the incidence of isolated splenic tuberculosis in immunocompetent patients. CASE PRESENTATION: This report highlights a case of a 45-year-old immunocompetent female with a one-mo

Urogenital tuberculosis in a patient with persistent sterile pyuria.

Urogenital tuberculosis (UGTB) can be a challenging diagnosis, and a delay in treatment may lead to irreversible organ damage. We report the case of a Guinean man in his 40s, residing in Portugal, who presented with lower urinary tract symptoms, urethral discharge and a right scrotal mass. Laboratory results revealed acute kidney injury, hypercalcaemia and persistent sterile pyuria, and ultrasound findings were suggestive of orchiepididymitis. Interferon gamma release assay was positive, as were the urine cultures for Mycobacterium tuberculosis, confirming the diagnosis of UGTB. The patient wa
