Management of traumatic degloving injury of the dorsal aspect of the midfoot and ankle: A case report.

Traumatic degloving injuries of the foot and ankle are severe, relatively rare lesions often resulting from high-energy mechanisms such as ejections in road traffic accidents. These injuries present significant challenges in both immediate management and long-term functional and aesthetic reconstruction. The involvement of severe soft tissue damage, neurovascular compromise, and potential joint involvement increases the risk of complications that can negatively impact clinical outcomes and patient quality of life. This case report details the management of a 32-year-old male who sustained exte

A Portable Insole System for Actively Controlled Offloading of Plantar Pressure for Diabetic Foot Care.

Plantar pressure monitoring is decisive in injury prevention, especially in at-risk populations such as people with diabetic foot. In this context, innovative solutions such as pneumatic insoles can be essential in plantar pressure management. This study describes the development of a variable pressure system that promotes the monitoring, stabilization, and offloading of plantar pressure through a pneumatic insole. This research was also intended to evaluate its ability to redistribute plantar pressure, reduce peak pressure in both static and dynamic conditions, and validate its pressure measu

The Epidemiology of Snakebite in Bhutan: A Retrospective Study.

Bhutan is a tropical country where snakebite is supposedly common, but official data are scanty and unmethodical. Deaths from snakebites were reported from areas where snakebites are common. Four-year (2018-2021) data of snakebite from 45 Bhutanese hospitals were collected and analysed to describe the burden and map by districts. A total of 371 snakebites were recorded from 45 hospitals during the 4 years. Most cases were seen in the southern and central parts of the country. There was a definite rise in the number of cases in the warmer months, starting from March and peaking between June and

Socioeconomic and racial-ethnic disparities in flame retardant exposure and executive function skills in preschool children.

BACKGROUND: Polybrominated diphenyl ethers (PBDEs) and organophosphate esters (OPEs) are commonly used as flame retardants. Limited research exists on socioeconomic and racial/ethnic disparities in exposure to these compounds and their impact on executive functioning (EF) in early childhood. The present study examined independent and joint effects of income and race/ethnicity on flame retardant exposure in early childhood and investigated associations between flame retardant exposure and children's EF. METHODS: This cross-sectional study used data from 349 preschool children recruited in Oreg

Semi-Immediate Autotransplantation of a Maxillary Third Molar With Unfavorable Root Anatomy: A Case Report.

Tooth autotransplantation is a procedure in which a donor tooth is transplanted within the same patient's jaw to replace a missing tooth. Donor tooth root morphology and periodontal ligament integrity are key factors influencing success. We report a semi-immediate autotransplantation of a maxillary third molar with a 90° divergent root into a site previously affected by a periapical abscess. After the extraction of the compromised tooth (#15) and removal of the interradicular septum, the site was left to heal to allow soft tissue closure and infection resolution. During this period, CBCT imagi

Digital Infrared Thermographic Imaging for Limb Salvage in Patients at Risk of Amputation: Prospective Observational Study.

BACKGROUND: Scores and prediction models such as the mangled extremity severity score (MESS) for trauma and the Wound, Ischemia, and foot Infection (WIfI) classification for diabetic foot ulcers help in the decision-making process for amputation. However, these tools can be subjective as they depend on the experience of the medical staff applying them. OBJECTIVE: This study aimed to assess the impact of temperature measurement using infrared thermal imaging in extremity salvage in patients at risk of limb amputation. METHODS: We included 29 patients who sought a second opinion after an amput

Changes in Gait Metrics and Motor Strategies Post-Neurocognitive Rehabilitation in Subacute Stroke: A Retrospective Mixed-Methods Study.

Background It is known that neurocognitive rehabilitation (NCR) improves post-stroke functional outcomes. However, systematic evaluations of changes in stride regularity and step regularity remain limited. Furthermore, a few studies have combined quantitative gait metrics with qualitative analyses of self-reported motor strategies, particularly in individuals with subacute stroke. Objective To examine immediate changes in key gait metrics - specifically vertical stride regularity and walking speed - as well as self-reported motor strategies following a single-session NCR intervention targeting

Simultaneous resurfacing of two fingers using a free medial plantar artery perforator flap: A case report.

This case report describes the reconstruction of two adjacent fingertip pulp defects using a single free medial plantar artery perforator flap. A 30-year-old man sustained traumatic amputations of the left index and middle finger pulps. A flap harvested from the medial plantar region was used to resurface both defects and was successfully anastomosed to the digital vessels of the index finger. Despite the need for a donor-site skin graft and secondary flap division, the procedure yielded excellent contour, mobility, and sensory recovery. The medial plantar artery perforator flap offers a relia

Gait abnormalities and ability to alter gait in children with idiopathic toe walking with a mild/moderate calf contracture.

BACKGROUND: Idiopathic toe walking is a common pediatric condition with those who continue to walk on their toes beyond childhood described as having persistent idiopathic toe walking (ITWp). Children with ITWp and mild/moderate contractures might be able to alter their gait pattern to a more typical gait. The purpose of this study was to compare self-selected toe walking to a "best-walk" pattern for children with ITWp and mild/moderate gastrocnemius/soleus contractures. METHODS: This prospective multi-site study recruited children with ITWp, aged 6-18 years, and physical exam dorsiflexion wi

Adjunctive Hyperbaric Oxygen Treatment for Grade-Four Frostbite: A Case Study and Short Discussion From Tasmania, Australia.

This case study describes adjunctive hyperbaric oxygen treatment (HBOT) of grade 3-4 frostbite, along with vasodilators and rheological agents. The patient in this study presented with established frostbite 3 days after an 11-day walk in cold, snowy, wet conditions in Tasmania, Australia. Hyperbaric oxygen treatment in this case seemed to reduce the penumbra of ischemia in both feet and thus possibly reduced the level of joint amputation in the toes. If available, HBOT is an adjuvant option for the treatment of frostbite, along with the more established treatments. More studies, however, are r

Complex genetic architecture underlies human hand and foot evolution.

The transition to bipedal locomotion is a key event in human evolution, involving substantial changes to the skeleton, including the bones of the hands and feet (autopods). Hominins evolved a more muscular and opposable thumb while the other fingers are relatively shorter, enhancing manipulative capacity. The feet evolved robust first toes and short lateral toes to meet the challenges of bipedal walking and running. While adaptations in the hand and foot have often been considered separately, the fore- and hind limbs of primates are morphologically integrated, serially homologous structures, r
