Skeletal muscle is frequently damaged by ischemia-reperfusion when exposed to direct injury or in the surgical practice. The purpose of the present experiments was to examine how the different types of skeletal muscles (fast & slow) react functionally to one and two hours of ischemia followed by two weeks of reperfusion. The fast-twitch (m. extensor digitorum longus/EDL) and the slow-twitch (m. soleus/SOL) muscle were prepared. They were stimulated, in vivo, either directly or indirectly at different reperfusion times following tourniquet ischemia, and the contraction force (CF) was recorded.

We present a case of primary mesenchymal chondrosarcoma of the proximal phalanx of the first toe. The bones of the foot represent an infrequent primary site for this neoplasm. A neurovascular island flap has been developed to reconstruct volar-oblique fingertip amputations. Recent studies have shown a partial similarity between posttraumatic stress disorder (PTSD) and alexithymia. In this study, the authors examined the relationship between PTSD and alexithymia in two samples of 26 patients with burn injury and 27 patients with digit amputation during rehabilitation. The prevalence rates of DSM-III-R PTSD and alexithymia assessed by the Toronto Alexithymia Scale (TAS) were significantly higher for injury patients than for healthy volunteers. During the last 4 years, three infants have presented with finger-tip injuries secondary to entrapment in woollen/synthetic mittens. The accident happened at home in one case but the other two occurred in different neonatal units. Spontaneous amputation of the terminal phalanx of the index finger occurred in two patients but in the other there was complete healing. This problem may be avoided by restricting the use of mittens, by changing their design, and by a greater awareness of this hazard.

Two soldiers are reported who sustained ring avulsion injuries while performing duties as part of Operation Provide Promise in Croatia (former Yugoslavia). These traction injuries inflicted on a ringed finger can present difficult diagnostic and treatment challenges in the field environment. They also can result in long-term disfigurement and disability. Due to the type of work performed and the equipment used by soldiers in the field environment, they are at particular risk for ring avulsion injuries. However, with appropriate education and precautions, these risks can be minimized.
