[Translated article] Reamputation prevalence after minor feet amputations in patients with diabetic foot: A cross sectional study.

INTRODUCTION: Reported prevalence for reamputation in diabetic foot is diverse, risk factors are not clear for minor amputations. This study aims to determine the prevalence for reamputation in diabetic foot from minor amputations and to evaluate associated factors for such outcome. METHODS: Cross sectional study developed in 2 hospitals. Patients hospitalized for diabetic foot ulcer requiring a minor amputation were included. A descriptive analysis of all variables is presented, as well as prevalence ratios (PR) and a multivariate logistic regression. RESULTS: The prevalence was of 48% for

Ingrown toenail is a common condition with many different treatment options. This review finds evidence that mild cases can be treated safely with non-surgical methods like change of ill-fitted shoes and nail brace. With failure in non-surgical treatment or in more severe cases surgical methods can be performed with note to the risk of complications - most frequently infection and the risk of recurrence at 3-13%. This review does not find evidence to use oral antibiotics in the treatment in general.

Acute Focal Dystonia as a Presentation of Uncontrolled Hyperglycemia.

Hyperglycemia-induced involuntary movements (HIIM) include tremors, hemichorea-hemiballismus (HCHB), and more rarely, dystonia. Presentations may vary, but hyperintensity involving the basal ganglia area on the T1 sequence of MRI brain remains a commonality.  We report the occurrence of focal dystonia with uncontrolled hyperglycemia but no focal abnormalities on MRI. On admission, the patient's blood glucose was 861, and she claimed to have never missed insulin dosage. A physical exam revealed no cranial nerve abnormalities and weakness in the right upper extremity with no sensory involvement.

Financial Toxicity Among Patients With Traumatic Finger Amputation: A Retrospective Study.

BACKGROUND: Patients face increasing financial toxicity (FT), defined as emotional distress due to the cost of medical treatment. However, little is known regarding FT in the context of upper extremity trauma. METHODS: We surveyed patients who sustained traumatic finger amputation (October 21, 2011-January 1, 2021). We collected patient-reported financial distress using the Comprehensive Score for Financial Toxicity (COST-11), where a lower score indicates worse FT. We also collected data of patients' perceptions regarding the costs of their treatment. We used linear regression to assess pati

Unilateral two-headed extensor digitorum longus muscle: atypical configuration and clinical implications.

This report describes a new configuration of the extensor digitorum longus (EDL) with two heads and two main tendons divided into four slips. During routine cadaver dissection, unilateral (right) EDL muscle belly and tendon variations were identified. The morphometric measurements of the EDL were conducted using Image J software. Two bellies and two tendons of the EDL muscle were observed in the right leg of an 87-year-old male cadaver. The second belly of the EDL muscle originates 2 cm below the first belly. The tendons originate at the ends of the muscle bellies. First and second tendons spl

What is the Optimal Treatment Protocol for Traumatic Popliteal Artery Injury? A Comparative Study between Two Institutions.

INTRODUCTION: While rare, popliteal artery injury is a potentially devastating complication of knee trauma. Delayed diagnosis or inadequate management of popliteal artery injury can result in limb ischemia, amputation, and long-term disability. This study aimed to compare the treatment outcomes of popliteal artery injury between different medical centers and to reconsider the optimal protocol at our institution. METHODS: Medical records of patients sustaining traumatic popliteal artery injury from 2011 to 2022 at two institutions (one hospital: Group A; the other hospital: Group B) were retro

Digital Replantation In Hours Versus Out of Hours: A Retrospective Review of Outcomes.

INTRODUCTION: The British Society for Surgery of the Hand's (BSSH) Getting It Right First Time (GIRFT) guidelines recommend that surgery for traumatic amputations of the digits is ideally performed during normal working hours even if this results in a delay of up to 24 hours. OBJECTIVE: To compare current practice against the BSSH GIRFT guideline regarding the timing of digital replantation and to compare the success rate of replantation performed within working hours (same or next day) and outside of working hours. MATERIALS AND METHODS: A single-centre retrospective analysis of two years o

Total neoadjuvant treatment using short-course radiotherapy and four CAPOX cycles in locally advanced rectal cancer with high-risk criteria for recurrence: a Swedish nationwide cohort study (LARCT-US).

BACKGROUND: Total neoadjuvant treatment (TNT) for locally advanced rectal cancer (LARC) increases pathologic complete response (pCR) rate and reduces the risk of systemic recurrences over chemoradiotherapy (CRT) in randomised trials, e.g., the RAPIDO trial. A modified RAPIDO schedule was prospectively explored in Sweden to evaluate TNT in routine health care before the RAPIDO results were published. METHODS: Between July 2016 and June 2020, 273 patients with high-risk LARC (clinical tumour stage cT4, clinical nodal stage cN2, extramural vascular invasion, involved mesorectal fascia or enlarge

Invasive Fungal Diseases of Combat Wounds: Burden, Epidemiology, and Mycology.

During the last two decades, wound invasive fungal diseases (WIFDs) have reemerged as important causes of mortality and morbidity in military personnel and civilian casualties in war areas. Historically, mycotic infections acquired in combat operations during Vietnam War and were associated with burn wounds. Modern combat related WIFDs are almost exclusively associated with severe traumatic events which encompass blast exposure as the primary mechanism of injury and subsequent extremity amputation and extensive blood loss. Such infections often lead to deep tissue necrosis, long hospitalizatio

The efficacy of arterial only microsurgical nasal replantation: a systematic review.

BACKGROUND: Microsurgical nasal replantation is a rare yet important procedure in order to regain the respiratory, olfactory, and aesthetic features of the nose. However, due to the traumatic nature of most nasal amputations, appropriate veins for anastomosis are difficult to find- complicating replantation. This is the first systematic review that compares peri operative and post-operative outcomes in arterial only versus arterial and venous anastomosis for nasal replantation. METHODS: A comprehensive search of Medline, SCOPUS, and Embase databases up to 30th November 2023 was undertaken. In

Transverse Distraction of Great Toe to Enlarge the Donor Site before Finger Reconstruction.

Partial great toe transfer is widely used in finger reconstruction. Although satisfactory results have been reported at the recipient's hand, the donor foot still presents with many problems due to the large amount of tissues harvested. In this study, the Ilizarov technique was utilized to enlarge the great toe in order to minimize the amount of tissue sacrificed of the donor foot. In this retrospective study, 23 patients (30 toes) underwent transverse distraction of the great toe for finger reconstruction from September 2020 to December 2022. The width of the contralateral normal finger was s

Results of innervated digital artery perforator flap and direct-flow homodigital flap application in fingertip soft tissue amputations.

OBJECTIVE: The purpose of this study is to compare the results of the innervated digital artery perforator (IDAP) flap and the direct-flow homodigital flap as reconstruction methods for fingertip soft tissue amputations. This issue is important in hand surgery, and we aim to identify the method that provides the best functional and cosmetic outcomes. METHODS: Between 2020 and 2022, 32 patients with fingertip amputations were reconstructed by the same surgeon using two different methods. The patients were retrospectively divided into two groups: those who underwent IDAP (n = 14) and those who

Transfemoral Osseointegration for Amputees with Well-Managed Diabetes Mellitus.

BACKGROUND: The most common reason for lower-extremity amputations remains the management of complications of diabetes mellitus (DM) and/or peripheral vascular disease. Traditional socket prostheses remain the rehabilitation standard, although transcutaneous osseointegration for amputees (TOFA) is proving a viable alternative. Limited studies of TOFA for vascular amputees have been published, but no study has focused on TOFA for patients with DM, neglecting this important patient population. The primary aim of the present study exploring this potential care option was to report the frequencies

Pneumatic Relaxation Oscillator with Passive Components for Continuous Toes Movement.

Continuous passive motion of the toes improves blood circulation and may help ankle injuries heal faster. Soft devices are more appropriate than rigid devices to accommodate a variety of foot shapes. Most conventional pneumatic soft robots are actuated cyclically by solenoid valves, which require large amounts of power for valves and controllers. In this paper, we introduce a pneumatic oscillator comprised only of passive elements. The device is implemented with a relaxation oscillator based on RC filters from pneumatic components and connected to an inflatable soft actuator. We measured the in

Case Report: Reduction in post-amputation phantom limb pain intensity accompanying the onset of phantom limb telescoping.

INTRODUCTION: Individuals with limb loss frequently report post-amputation phenomena, including nonpainful phantom sensations, phantom limb pain (PLP), and residual limb pain (RLP). Although post-amputation pain is common, not all patients benefit from widely accepted treatments. A greater understanding of phantom limb "telescoping", the experience of one's phantom hand or foot gradually approaching the residual limb, may assist in developing more effective interventions for reducing post-amputation pain. This case report explores the relationships between PLP, RLP, telescoping, and psychosoci
