Polydopamine-coated electroconductive nanofibrous scaffold with sustained NGF release for peripheral nerve regeneration.

Peripheral nerve injuries (PNIs) remain a major clinical challenge due to permanent functional impairment and limited effective therapeutic options. To address this, we developed a novel tissue-engineered nerve scaffold that integrates multiple regenerative elements to mimic a favorable microenvironment for nerve regeneration. The scaffold consists of electrospun aligned polycaprolactone (PCL) nanofibers for structural guidance of axonal growth, reduced graphene oxide (rGO) to provide electrical conductivity for enhanced neural signaling, and polydopamine (PDA) surface modification to improve

Abnormal lateral malleolus morphology and its association with physical function in Japanese adolescent competitive athletes; Cross-sectional study.

BACKGROUND: Avulsion fractures of the lateral malleolus, detected as abnormal lateral malleolus morphology (ALM), occur concomitantly with lateral ankle sprain (LAS) in children and adolescents. We hypothesized that structural changes in the ankle joint due to ALM may contribute to deterioration in dynamic balance function. RESEARCH QUESTION: What are the differences in physical function between competitive adolescent athletes with and without ALM, and what factors influence dynamic postural stability on landing, a critical physical function associated with LAS? METHODS: This is the cross-se

A synergistic multimodality treatment approach to address the key drivers of wound chronicity.

BACKGROUND: Chronic wounds remain a major clinical and economic burden, affecting millions worldwide. Despite advances in wound care, many wounds fail to heal due to persistent tissue hypoxia, unresolved inflammation, lymphatic dysfunction, edema, and ischemia-reperfusion injury. These interrelated mechanisms are further compounded by comorbidities such as obesity, diabetes, and vascular disease, highlighting the need for therapeutic approaches that address multiple barriers to repair simultaneously. METHODS: We review the pathophysiological drivers of wound chronicity-including the inflammat

Nonoperative Management of Multiple Displaced Metatarsal Fractures in a Diabetic Neuropathic Patient.

The management of fractures in diabetic patients poses unique challenges, particularly due to complications such as neuropathy, poor bone quality, and impaired healing. This case study explores the nonoperative management of multiple displaced metatarsal fractures in a 61-year-old male with type 2 diabetes and neuropathy. The patient presented with swelling and minimal pain in the right foot without a clear history of acute injury. Despite radiographic evidence of displaced fractures, the patient opted for conservative treatment due to high surgical risks, including comorbidities and concerns

Social vulnerability index enhances FRAX prediction of hip fractures in fall patients.

The Fracture Risk Assessment Tool (FRAX), widely used for predicting the 10-year likelihood of hip fractures, does not incorporate factors like prior falls and sociodemographic characteristics, notably the Social Vulnerability Index (SVI). Recognizing these limitations, we aim to evaluate the predictive accuracy of FRAX by integrating fall frequency, fall energy, and SVI into the model for assessing the risk of fall-induced hip fractures. A retrospective case-control study was conducted, and patients aged ≥ 40 years with a documented diagnosis of a fall-induced hip fracture were age-matched wi

Effects of carbon-fiber plate design on foot stress injury risk: a finite element analysis.

INTRODUCTION: Carbon-fiber plate (CFP) running shoes may alter ankle and foot biomechanical loading patterns, thereby potentially increasing the risk of foot stress injuries. However, there still a lack of systematic quantitative analysis of how multiple key design parameters (determining CFP performance) influence foot biomechanical responses. This study aimed to clarify how composite CFP influence the mechanical response of the foot and ankle, with the goal of reducing overuse injuries and providing biomechanical guidance for the structural design of CFP running shoes. METHODS: A three-dime

Preoperative Ankle Swelling and the Effect On Postoperative Wound Complications Following Ankle Fracture Surgery.

BACKGROUND: Swelling following an ankle fracture is commonly believed to preclude surgical fixation; swelling is thought to be associated with increased wound complications. Delaying surgery until swelling subsides is thought to secure better outcomes, although no guidelines exist to direct surgeons when an appropriate time to intervention is or whether a visual inspection of the swelling is correlated to quantitative measurement. This study aimed to identify whether preoperative ankle swelling influences postoperative wound complications following ankle fracture surgery. METHODS: This prospe

Systematic Review and Meta-Analysis Comparing Open FHL Tendon Transfer, Hamstring Transfer, and Turndown Flaps to Treat Chronic Achilles Tendon Ruptures.

BACKGROUND: Chronic Achilles tendon ruptures (CATRs) present significant challenges because of delayed diagnosis and more complicated salvage treatments that can result in unfavorable outcomes and increased rates of adverse events. Various reconstructive surgical options have been described, each with its own unique advantages and disadvantages. Comprehensive comparison of these techniques, however, is lacking. This meta-analysis aimed to compare clinical outcomes following 3 of the most commonly used salvage techniques including flexor hallucis longus (FHL) transfer, hamstring transfer, and g

A Southeast Asian Experience in Biodegradable Implants: Tips and Tricks for Radiological Findings.

Nondegradable implants, such as titanium or steel screws, are commonly used in orthopedic surgery as they provide strength and stability. However, metallic implants have been associated with multiple issues, such as additional surgery to remove hardware, interference with imaging modalities, and stress shielding. Biodegradable magnesium-based implants are an innovative alternative due to good biocompatibility and osteoconductivity, similar to Young's elastic modulus to bone, and they exhibit less metallic distortion in imaging modalities. The MAGNEZIX compression screw became the first magnesi

Fourteen-year outcome of unilateral leg replantation after bilateral lower leg and unilateral upper extremity amputation following traumatic injury.

In cases of traumatic major extremity amputations, particularly in the lower extremity, every stage-ranging from decision-making to implementation and outcomes-remains a topic of debate. Managing major amputation cases with high mortality rates is extremely challenging both during the injury and treatment processes. We present a rare and severe case of a 30-year-old patient who sustained injuries from a concrete machine, resulting in bilateral lower extremity amputations and a unilateral proximal arm amputation. We successfully performed a replantation at the proximal ankle level on one side.

Technical Tip: The Use of Arthroscopic Portals to Safeguard Medial Neurovascular Structures During Suture Button Fixation.

IntroductionAnkle fractures with concurrent syndesmotic instability are commonly encountered orthopaedic injuries. Suture button syndesmotic fixation has emerged as a favorable alternative to traditional screw fixation due to its comparable biomechanical stability and reduced hardware removal rates. However, concerns persist regarding potential injury to medial neurovascular structures during tibial cortical button deployment and a medial incision is often recommended for verification.TechniqueThis paper describes a safe and reproducible technique for suture button fixation that offers an alte

Anatomical Variations in the Formation of the Sural Nerve: A Pilot Study in a Sample of Lithuanian Cadavers.

Background and Objectives: The sural nerve (SN) is a pure sensory nerve that supplies the lateral aspect of the ankle and foot. Its anatomical variability has been extensively documented, with multiple classifications describing its different formation patterns. The SN is commonly used for nerve grafting and is a critical structure in lower-limb surgeries. Due to its superficial course, it is vulnerable to iatrogenic injuries, particularly in procedures involving the Achilles tendon. The presence of anatomical variations in SN formation and trajectory has significant implications for surgical

Persuasive of lateral supra malleolar flap for distal leg and foot reconstruction. A cross-sectional study.

INTRODUCTION: Distal leg and foot have unique anatomical characteristics, and multiple etiologies may cause defects in various patterns that mandate reconstruction. Local flaps, such as perforator flaps, can give good outcomes with less complication. The lateral supra malleolar flap can effectively cover the distal leg and foot defect. In this article, we highlight the utility of this flap and its outcome. MATERIAL AND METHODS: This study is a descriptive, retrospective, multi-center, hospital-based performed to assess the efficacy of lateral supra malleolar flap in the reconstruction of dist
