Adenoid Cystic Carcinoma of Bartholin's Gland: A Case Report with Emphasis on Surgical Management.

BACKGROUND Adenoid cystic carcinomas of Bartholin's gland are rare among gynecological malignancies, accounting for 0.1% to 7% of vulvar carcinomas and 0.001% of all female genital tract malignancies. There are no specific guidelines regarding treatment recommendations; therefore, they are commonly treated like vulvar cancer. CASE REPORT We present the case of a 42-year-old premenopausal woman with an adenoid cystic carcinoma of Bartholin's gland diagnosed upon biopsy of a palpable, predominantly vaginally located mass causing foreign-body sensation, vaginal pain, and extreme dyspareunia. The

Imaging of the Vagina: Spectrum of Disease with Emphasis on MRI Appearance.

The vagina is a median fibromuscular structure of the female reproductive system that extends from the vulva inferiorly to the uterine cervix superiorly. As most vaginal lesions are detected at gynecologic examination, imaging performed for nongynecologic indications can frequently cause concomitant vaginal pathologic conditions to be overlooked. The vagina is often underevaluated at routinely performed pelvic transvaginal US because of a narrow scan area and probe positioning. MRI has progressively become the imaging method of choice for vaginal pathologic conditions, as it provides excellent

Evaluation of vulvovaginitis in the adolescent patient.

Vulvovaginitis, referring to inflammation of the vulva and vagina, is a commonly reported concern among adolescents and young women presenting for gynecologic care. Symptoms of vulvovaginitis may include vaginal discharge, odor, itching, pain, dysuria, skin irritation, burning, and dyspareunia. Vulvovaginitis may result from infectious or non-infectious causes. Bacterial vaginosis, vulvovaginal candidiasis, and trichomoniasis represent the three most common infectious causes of vulvovaginitis in adolescents and young adults. Additionally, non-infectious causes such as the presence of a foreign

Human papillomavirus-related diseases of the female lower genital tract: oncogenic aspects and molecular interaction.

The causal role of human papillomavirus (HPV) in all cancers of the uterine cervix has been firmly established biologically and epidemiologically. Most cancers of both the vulva and the vagina are also induced by HPV. Papillomaviruses are perfectly adapted to their natural host tissue, the differentiating epithelial cell of skin or mucosae, and exploit the cellular machinery for their own purposes. The infectious cycle is initiated once the infectious particles reach the basal layer of the epithelium, where they bind to and enter the cells. The critical molecules in the process of virus replic

Genital injuries in adults.

The examination of the rape victim should focus on the therapeutic, forensic and psychological needs of the individual patient. One aspect will be an examination for ano-genital injuries. From a medical perspective, they tend to be minor and require little in the way of treatment. They must be considered when assessing the risk of blood-borne viruses and the need for prophylaxis. From a forensic perspective, an understanding of genital injury rates, type of injury, site and healing may assist the clinician to interpret the findings in the context of the allegations that have been made. There a

Infections of the lower female genital tract during childhood and adolescence.

PURPOSE: To review the pathogenesis, clinical presentation, diagnosis and treatment of lower female genital tract infections at a young age. METHODS: Review study. CONCLUSIONS: Lower female genital tract infections at a young age may involve the vulva, the vagina and, less frequently, the fallopian tubes. Good knowledge of the physiology and anatomy of the respective areas plays an important role in the diagnosis and treatment of vulvovaginitis. Candida albicans is the most frequent cause of infection, while Gardnerella vaginalis, Chlamydia trachomatis, Mycoplasma, and Trichomonas vaginalis

[Modern aspects in the diagnosis and treatment of gynecologic diseases in childhood and adolescence].

Gynecological problems in juvenile and adolescent age deserve a special attention. Disorders appearing at this age are not identical with those developing in adults. Diagnostics and therapy of gynecological diseases in juvenile and adolescent age are far more complicated than the same procedures with adult women, while instruments and examination protocols have to be adjusted to the age of patients. To maintain the purpose of preserving the generative function and sexuality of an adult woman early detection and treatment of gynecological diseases of juvenile age are essential. The paper focuse

Genital findings in prepubertal girls selected for nonabuse: a descriptive study.

This project was designed to collect normative data on the genital anatomy from a sample of nonabused prepubertal girls. A total of 114 girls between the ages of 10 months and 10 years were examined and photographed with a colposcope. After screening for the onset of puberty and the possibility of undetected abuse, 93 subjects remained. Examination techniques included a supine labial separation approach, a supine labial traction method, and a prone knee-chest position. Common genital finding included erythema of the vestibule (56%), periurethral bands (50.6%), labial adhesions (38.9%), lymphoi

[Indications and the process of vaginoscopy in children].

The optimal recognition of gynecological diseases in childhood and adolescence requires gynecological examination with empathy and subtility. Just like the elder woman the young girl can demand that we take care of her gynecological problems in order to avert upcomming dangers. Concordant to adult's gynecological diseases we find the same problems in children and adolescents. That's why the examination cannot be different in newborns, girls and adolescents. The complete gynecological examination requires vaginoscopy or examination by means of speculum. It's necessary beyond the inspection of t

[Rare foreign body fistulas of the female bladder].

Fistulae between kidney and vagina caused by foreign bodies are very rare nowadays. Two casuistics illustrate the curious anamnesis of such patients: in an 18-year-old girl a foreign body inserted during masturbation led to a perforation, in the case of a 66-year-old woman a pessary which had been forgotten since 35 years penetrated into the urinary bladder.

Vaginal disorders.

Chronic vaginitis is the most common vaginal disorder. Dogs with vaginitis show no signs of systemic illness but often lick at the vulva and have purulent or hemorrhagic vaginal discharges. Vaginitis is most commonly secondary to a noninfectious inciting factor such as congenital vaginal anomalies, clitoral hypertrophy, foreign bodies, trauma to the vaginal mucosa, or vaginal tumors. Inspection of the caudal vagina and vestibule both visually and digitally will often reveal the source of vaginal irritation. Vaginal cytology is used to establish the stage of the estrous cycle as well as disting

Pediatric vulvovaginitis.

Childhood gynecologic problems are different from adult problems in physiology, disease, psychology, evaluation and management. The most common is vulvovaginitis. The child is susceptible to vulvovaginitis because of a relatively exposed vulva, a thin vaginal wall and poor hygiene. Additional problems are heat and moisture, clothing, coverings and possible sexual abuse. Most vulvovaginitis is primary, nonspecific vulvitis with secondary vaginitis. It is set off by poor hygiene and responds to keeping the vulva clean, cool and dry. Persistent or recurrent vulvovaginitis may be due to a variety
