Penicillin Allergy and Dental Implant Outcomes: A Review.

Dental implants are widely used to replace missing teeth, providing a long-term solution for patients seeking functional and esthetic restoration. Concerns about implant failure and post-surgical complications, however, can arise, particularly in patients with known penicillin allergies. Because penicillin is often the first-line antibiotic used for infection prophylaxis in dental procedures, alternative antibiotics such as clindamycin are commonly prescribed for these patients. Penicillin allergy is associated with a higher risk of dental implant failure than in non-allergic patients, particul

Intracameral Cefuroxime Use in Cataract Surgery After Penicillin Allergy Reclassification.

IMPORTANCE: Among patients who have a penicillin allergy label (a history of any reaction to penicillin in their medical records), many cataract surgeries are performed yearly. Less than 1% of these patients have a true allergy to penicillin. The evidence supports limited concern regarding cephalosporin cross-reactivity; however, the guidance for surgeons regarding antibiotic selection in these patients remains unclear. OBJECTIVE: To examine rates of perioperative antibiotic selection after penicillin allergy label reclassification vs before reclassification. DESIGN, SETTING, AND PARTICIPANT

Prediction of penicillin concentration using time series forecasting and machine learning techniques.

BACKGROUND: Time-series forecasting is crucial in pharmaceutical fermentation, where early detection of deviations safeguards product quality. As the industry evolves toward Pharma 4.0, integrating machine learning into process monitoring and control presents an opportunity to enhance efficiency, consistency, and regulatory compliance. However, currently there is a notable lack of reliable machine learning based predictive tools for complex pharmaceutical fermentations. OBJECTIVE: To develop a machine learning model based on Gated Recurrent Units (GRUs) for predicting penicillin concentration

Pharmacokinetic drug-drug interactions with flucloxacillin and other isoxazolyl penicillins: A systematic literature review and practical guide.

Recent years have seen a notable increase in the number of publications concerning pharmacokinetic drug-drug interactions involving the isoxazolyl penicillins cloxacillin, dicloxacillin, flucloxacillin and oxacillin. Given that the findings predominantly rely on clinical observations, the interaction mechanisms and their clinical relevance remain insufficiently elucidated, thereby posing challenges for clinicians in managing these interactions. The aim of this literature review was to provide an overview of all reported pharmacokinetic drug-drug interactions involving isoxazolyl penicillins an

Creating a Feasible, Safe and Sustainable Penicillin Oral Challenge Program in a Rural Health Service.

PROBLEM: Programs that identify people with a low-risk allergy who can be safely delabeled by a direct oral challenge have emerged but are mostly reported in tertiary centres. There is limited information in the rural setting where resources are more constrained. SETTING: A medium-sized rural health service in Victoria (Australia). KEY MEASURES FOR IMPROVEMENT: The outcome measure was the number of patients with a low-risk penicillin allergy who completed a penicillin oral challenge. STRATEGIES FOR CHANGE: The penicillin oral challenge program developed considered four critical workflows: c

Characterization of the novel class A β-Lactamase gene blaKOB-1 in an Enterobacter isolate.

BACKGROUND: Class A serine β-lactamases are important enzymes that contribute to bacterial resistance to β-lactam antibiotics, especially in gram-negative bacilli (e.g., Enterobacteriaceae). These enzymes are highly diverse with many molecular variants, and some enzymes (such as SHV and TEM) can be used to extend the inactivation spectrum to the latest generation of cephalosporins by mutation. Consequently, the mechanisms of resistance to antimicrobial agents in bacteria and the clinical implications of antimicrobial resistance need to be more comprehensively studied. METHODS: Animal fecal sa

Antibiotic use and gut microbiome composition links from individual-level prescription data of 14,979 individuals.

Disruptions in gut microbiome are implicated in cardiometabolic disorders and other health outcomes. Antibiotics are known gut microbiome disruptors, but their long-term consequences remain underexplored. Here we combined individual-level data from the Swedish Prescribed Drug Register with fecal metagenomes of 14,979 adults to examine the association between oral antibiotic use over 8 years and gut microbiome. In multivariable confounder-adjusted regression models, antibiotic use <1 year before fecal sampling was associated with the greatest reduction in species diversity, but significant asso

Outcomes in hospitalised patients with penicillin allergy: a systematic review and meta-analysis protocol.

INTRODUCTION: When patients report a penicillin allergy (self-reported penicillin allergy, SRPA), clinicians alter their antibiotic management of the patient. However, fewer than 5% of SRPA have life-threatening reactions, and receiving narrower spectrum antibiotics may result in under treatment. Reports from community studies suggest that patients with SRPA are more likely to come to harm, but as SRPA implies exposure to healthcare, it is unclear whether additional harm is as a result of co-morbidities and the need to receive healthcare, rather than a harm in itself. This raises the question

Outpatient antibiotic prescribing among Utah Medicaid recipients: A 4-year population-based analysis.

BACKGROUND: Inappropriate antibiotic use can accelerate the evolution and spread of antimicrobial resistance and increase Clostridioides difficile infection and adverse drug events. Utah typically reports lower overall outpatient antibiotic prescribing than the average across the US, but Medicaid-specific trends are less well described. METHODS: We conducted a retrospective analysis of Utah Medicaid outpatient pharmacy claims (January 2019-July 2023). Prescription rates were calculated per 1,000 enrollees and compared with all Utah and US rates. Demographics, provider types, age distribution,

Three-Year Effectiveness of a Multidisciplinary Approach to Penicillin Allergy De-labelling in Pregnant Patients.

OBJECTIVE: Allergy testing and de-labelling for pregnant patients reporting penicillin allergies is standard care to promote first-line antimicrobial use in the peripartum period. We aimed to evaluate the rate of penicillin allergy label reappearance ("sticky label phenomenon") in pregnant patients who underwent multidisciplinary allergy de-labelling during the antenatal period. STUDY DESIGN: We completed a prospective cohort study of pregnant patients who successfully completed an antenatal oral amoxicillin challenge from July 2019 to December 2020. We conducted a chart review after 3 years

First Report of Klebsiella pneumoniae Clinical Isolates Carrying pAmpc Gene ACT-5 with Reduced Sensitivity to Cefepime from Brazil.

Antimicrobial resistance remains a major threat to global public health, with increasing reports of the spread of β-lactamase genes posing significant risks. The number of Enterobacteriaceae harboring plasmid-mediated AmpC enzymes has been rising worldwide, with prevalence rates varying across regions. These β-lactamases are of particular epidemiological concern because they confer resistance to several β-lactam antibiotics, including penicillins, second and third-generation cephalosporins, and monobactams, while isolates typically remain susceptible to cefepime and carbapenems. This study rep

Breaking barriers: A communication bundle for penicillin allergy label removal.

Background: Many barriers exist in disseminating the results of successful penicillin allergy delabels and preventing relabels. Methods: This study used electronic faxing to directly communicate the completion of successful direct oral challenges in children hospitalized or in the emergency department to outpatient pharmacies and out-of-network primary care providers. Results: This communication led to a decreased rate of the penicillin allergy label presence in pharmacy records, from 32% (36/112) to 12% (16/136) (p < 0.001). In conjunction with more conventional discharge communication mechan

Genomic characterisation of extensively drug-resistant Acinetobacter baumannii isolates from a tertiary hospital in Ghana.

Acinetobacter baumannii (A. baumannii) is an emerging "superbug" whose infections have become extremely difficult to treat due to its diverse antimicrobial resistance mechanisms and resistance to last-resort antibiotics including carbapenems. Despite this, data on genetic determinants and genomic context of carbapenemase genes in A. baumannii are scarce in Ghana. This study investigated the genetic determinants of carbapenem resistance in clinical isolates of A. baumannii (Ab) and explored the genetic contexts of carbapenemase-encoding genes in extensively drug-resistant A. baumannii (XDR-Ab).

Pseudomonas aeruginosa in Chinese mitten crab Eriocheir sinensis: Identification, pathogenic potential and control with Bdellovibrio powder.

Pseudomonas aeruginosa is a significant opportunistic pathogen affecting both humans and aquaculture animals. However, there has been no documented evidence linking P. aeruginosa to hepatopancreatic necrosis disease in Chinese mitten crabs Eriocheir sinensis. In the present study, P. aeruginosa isolate HX-1 was identified as a causative agent of hepatopancreatic necrosis disease in E. sinensis, and its pathogenic characteristics in terms of pathogenicity-related genes, antimicrobial resistance profiles, and virulence was comprehensively analyzed. Additionally, the in vitro and in vivo efficaci

Deficiencies in the Assessment of Penicillin Allergy in Hospitalized Children in Hawai'i, 2021-2023.

Although claims of penicillin allergy are common, after evaluation they are seldom confirmed and can lead to unnecessary interventions. National recommendations are to thoroughly evaluate potential allergies and, if appropriate, to delabel. This retrospective study reviewed records of children hospitalized at Kapi'olani Medical Center for Women and Children in Honolulu, Hawai'i, between January 1, 2021 and June 30, 2023 to assess the frequency of penicillin allergy claims, the appropriateness of their evaluation, and initiation of interventions. Of 3484 hospitalized children, 97 (2.8%) reporte

Penicillin Allergy Delabeling Can and Should Be Performed in Pregnant Patients: A Work Group Report of the AAAAI Women's Health in Allergy and Immunology and Adverse Reaction to Drugs, Biologics, and Vaccines Committees.

Penicillin allergy is frequently reported yet rarely confirmed in pregnant women, similar to the general population. Historically, there has been hesitation to perform penicillin allergy testing in pregnant patients due to perceived risk to the patient and fetus in the event of an allergic reaction. Given the multiple clinical indications for penicillin in pregnancy and delivery, there are also risks of using non-first-line antibiotics; thus clarity is needed regarding the safety of undergoing penicillin allergy testing during pregnancy. Magnitude and Predictors of Antimicrobial Use-Related Drug Problems in Geriatric Inpatients: Evaluation of Pharmacist Interventions.

INTRODUCTION: Infections remain a major cause of illness and death in older adults. Physiological changes, polypharmacy, and chronic conditions complicate antimicrobial therapy and increase the risk of drug problems. Pharmacist-led interventions improve prescribing, but evidence from the United Arab Emirates is limited. This study evaluated the magnitude and determinants of antimicrobial use-related drug problems and assessed clinical pharmacist interventions in hospitalized older adults with infections. METHODS: A retrospective observational study was conducted from February to July 2022 at

Increase of post-SARS-CoV-2 rashes mimicking penicillin allergy in children with Group A beta-hemolytic Streptococcus infection: An emerging and challenging issue.

BACKGROUND: Group A beta-hemolytic Streptococcus (GABHS) infections have surged globally in the post-SARS-CoV-2 era, coinciding with an increase in the number of children referred for suspected delayed hypersensitivity to penicillin administered during acute infection. GABHS-related rashes may resemble non-immediate drug reactions, complicating diagnosis, and contributing to inappropriate penicillin allergy diagnosis. METHODS: Medical charts from patients followed at Allergy Unit of Meyer Children's Hospital IRCCS, Florence, Italy and Pediatric Department of "Vittore Buzzi" Children's Hospita

Comparison of Oral Cephalosporin and Penicillin Step-Down Therapy for the Treatment of Gram-Negative Blood Stream Infections: A Retrospective Observational Study.

BACKGROUND: Oral step-down therapy for the treatment of gram-negative blood stream infections (GNBSIs) has been shown to be noninferior to full courses of intravenous (IV) therapy. Studies comparing oral fluoroquinolones, sulfamethoxazole-trimethoprim (SMX-TMP), and beta-lactams have reported similar treatment outcomes, but beta-lactam groups have been small, and efficacy has been questioned given lower oral bioavailability. OBJECTIVE: This study aims to evaluate the safety and efficacy of oral cephalosporins compared with penicillins for step-down therapy for the treatment of GNBSI. METHODS
