Comparison of a novel methadone rotation method with other commonly used methods.

OBJECTIVES: To compare a novel method of methadone rotation used in a specialist palliative care inpatient unit (SPCU) in Cork, Ireland, with rapid titration methods using Perth and Brisbane Protocols as well as the Edmonton method of methadone rotation. METHODS: A retrospective chart review was performed in March-June 2022. All patients who completed rotation to methadone during 2018-2019 in the SPCU were included. 2018-2019 was selected to study a population not affected by the coronavirus pandemic. Oral morphine equivalent (OME) was calculated using the opioid conversion chart. From the OM

Impact of guidance issued during COVID-19 to expand take-home doses of opioid agonist treatment (OAT) in Ireland: protocol for a population-based analysis of prescribing practices and patient outcomes 2018 to 2023.

BACKGROUND: It is increasingly suggested that clinical guidelines and practices be updated to permanently expand relaxation around access to opioid agonist treatment (OAT) take-home doses after COVID-19. Despite a risk of OAT drug diversion, flexibility in take-home doses is valued by patients and associated with improved quality of life and retention. However, few studies have examined the effects of changes to take-home dose policies on prescribing practices and patient outcomes, with mixed results. AIMS: This protocol relates to three inter-related studies. The first study will examine the

A study of micro-dosing-transferring from methadone to buprenorphine in NHS Dumfries & Galloway.

OBJECTIVES: To find a novel way to successfully transition patients from methadone to buprenorphine and in turn reduce the percentage of methadone prescribed within National Health Service Dumfries & Galloway (NHS D&G). METHODS: Three micro-dosing schedules were developed. Outcomes measured: patient numbers, success rates, prior methadone dose, Opiate Substitution Therapy following micro-dosing. A patient feedback questionnaire was developed. KEY FINDINGS: 177 patients micro-dosed between 06/09/21 and 28/05/24. Around 132 patients successfully transitioned from methadone to buprenorphine thr

Intraoperative Intravenous Methadone and Postoperative Opioid Requirements in Adult Patients With Burns.

Postoperative pain management is a significant challenge in patients undergoing burn excision. Pharmacologic pain management strategies include both opioid and non-opioid medications. Given the national overuse of opioids and the associated negative effects, it is prudent and essential to find ways to manage pain with fewer or no opioids. We hypothesize that intraoperative administration of intravenous methadone reduces total morphine milligram equivalents per weight used in the 36 h following surgery. This is a retrospective, single-center cohort study of adult burn patients who underwent a f

The efficacy and safety profile of methadone for intractable cancer pain in advanced lung cancer patients: a single-center retrospective analysis of 37 Japanese patients.

BACKGROUND: For patients suffering from intractable cancer pain, which cannot be sufficiently relieved even with strong opioid analgesics, methadone is recommended in Japan. However, the real-world data on the efficacy and safety of methadone for intractable pain in patients with lung cancer remain scarce in clinical setting. The aim of this clinical study was to investigate the efficacy and safety of methadone for intractable pain in advanced lung cancer patients. METHODS: All the cases of advanced lung cancer patients who were administered methadone for intractable pain at the Shizuoka Canc

Increasing ketamine administration in children's hospitals for youth with sickle cell disease.

Ketamine is recommended as an opioid-sparing adjunct for sickle cell disease (SCD)-related pain management. Little is known regarding its use in hospitalized youth with SCD. We aimed to describe trends in ketamine administration and examine associations between ketamine administration and outcomes in hospitalized youth with SCD. We conducted a cross-sectional, multicenter study examining hospital admissions for youth with SCD from 44 children's hospitals in the United States from 2016 to 2023. Youth aged ≥6 months with SCD were identified using International Classification of Diseases tenth re

Opioids regulate the functional state of immune cells and reduce inflammatory cardiac injury: Role of opioid receptors, MRGPRX2, and TLR4.

Neutrophils, macrophages, CD3 +, CD4 +, and CD8 + T lymphocytes express µ-, δ-, and κ-opioid receptors (ORs) with varying affinities for opioids. Mast cells express the atypical OR Mas-related G-protein-coupled receptor X2 (MRGPRX2), which has a low affinity for morphine. Neutrophils and macrophages can synthesize and release endogenous opioid peptides. Activation of ORs enhances the synthesis of proinflammatory cytokines and the production of reactive oxygen species (ROS) in unstimulated leukocytes. Conversely, OR activation reduces proinflammatory cytokine synthesis in stimulated neutrophils

Population Pharmacokinetics to Support Intravenous and Enteral Methadone Dosing in Children.

Methadone is used in hospitalized children to treat pain and iatrogenic opiate withdrawal. Optimal pediatric dosing for both enteral and intravenous methadone is unknown. We conducted two prospective, multi-center, open-label studies to characterize the pharmacokinetics of methadone in the pediatric population. These studies were conducted at a total of 23 US children's hospitals. Ninety-nine children with a median (range) age of 2.3 (0-19.0) years and weight of 13.0 (0.72-159) kg were prescribed methadone per standard of care for treatment of pain or iatrogenic opiate withdrawal. Ninety-nine

Animal Models of Prenatal Opioid Exposure: Insights into Impaired Neurodevelopment.

With opioids being increasingly prescribed and illicit opioids being misused, substance use disorder has become a growing public health concern. The impacts of the opioid epidemic have been devastating, especially for pregnant people, infants, and children. Pre- and perinatal opioid exposure is complex. Opioids affect multiple body systems and have detrimental effects on the placenta, brain, and immune system. Pharmacological properties make each class of opioid unique, thereby compounding effects on development based on the type, receptors engaged, or combination of drugs used. Accordingly, a

Service provider perceptions of incorporating hepatitis C testing technologies into standard practice: Considerations for widespread implementation and scale-up.

BACKGROUND: Simplification of hepatitis C virus (HCV) testing technologies has facilitated scale-up of testing and treatment of at-risk populations in various service settings. Service provider perspectives on the incorporation of simplified diagnostics into standard of care would inform optimised implementation of these technologies. The aim of this study was to qualitatively explore provider perceptions of simplified HCV testing modalities and how they envisage their implementation into standard practice in Australia. METHODS: Between October 2022 and July 2023, semi-structured interviews o

Harm reduction engagement and ongoing opioid use among adults receiving methadone or buprenorphine in Philadelphia.

BACKGROUND: Opioid Use Disorder (OUD), a chronic condition with significant health and social consequences, has been intensified by synthetic opioids such as fentanyl. Although medications for OUD (MOUD) reduce mortality and improve treatment retention, many individuals continue to engage in non-prescribed opioid use while in treatment. Harm reduction services (HRSs), including syringe exchange sites, supervised injection facilities, or sites that distribute fentanyl test strips or naloxone, seek to mitigate the risks associated with substance use. This study explores engagement with harm redu

The Experiences and Attitudes of Rural Black People who Use Drugs.

INTRODUCTION: In recent years, higher rates of overdose deaths have been observed among Black individuals in the United States compared to individuals who are white or other races, as part of the potent fourth wave of the opioid epidemic. However, much of the research addressing this surge has focused on Black individuals in urban areas, despite evidence that usage and overdose rates are accelerating among rural Black People Who Use Drugs (PWUD), a gap we aim to fill here via a qualitative assessment. METHODS: Between September 2023 and June 2024, we conducted semi-structured interviews with
