Efficacy of Levetiracetam in Patients With Pediatric Epilepsy: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Levetiracetam (LEV) is widely used in pediatric epilepsies because of its favorable pharmacokinetics, ease of administration, and perceived tolerability. However, its comparative efficacy relative to established antiseizure medications (ASMs) in children remains uncertain. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) to evaluate LEV efficacy in pediatric epilepsies and compare outcomes vs placebo and active comparators. METHODS: We systematically searched PubMed/MEDLINE and Embase (2000-6 August 2025) for RCTs enrolling p

Prolonged and fluctuating consciousness after mixed overdose: navigating the complexities of polypharmacy, hepatic impairment and flumazenil use.

Mixed drug overdose presents a complex and potentially unpredictable clinical course, particularly in the presence of comorbidities that may alter drug metabolism, half-life and pharmacodynamic effects. We report a man in his 50s presenting with reduced consciousness, Glasgow Coma Scale (GCS) 11, requiring oxygen therapy after ingesting 100 mg diazepam, 1200 mg pregabalin and suspected co-ingestion of zopiclone and gabapentin. Flumazenil produced rapid but transient improvement. However, fluctuating consciousness persisted, with further deterioration in GCS on day 8 that again responded to flu

Methodological considerations on repeated measures and sub-LOQ values in passive cannabis hair contamination.

Giorgetti et al. report a carefully designed in vivo cross-over experiment showing that brief passive exposures to cannabis or "light cannabis" smoke inside a vehicle can deposit detectable Δ9-tetrahydrocannabinol (Δ9-THC) or cannabidiol (CBD) into hair, even after a validated dichloromethane washing step. Their findings are forensically relevant because washed-hair concentrations sometimes approached or exceeded Society of Hair Testing cut-offs used to infer active use. The work advances beyond in vitro models by using realistic exposure durations. However, two statistical practices warrant f

Cannabidiol-thiosemicarbazone exhibits dual tyrosinase inhibition and antioxidant activity in human skin-derived cells.

The search for modulators of melanogenesis with improved biological compatibility remains an active area of investigation, as existing tyrosinase (TYR) inhibitors are often limited by low potency, instability, or cytotoxicity. Here, we investigated CBD-TSC1, a cannabidiol-based thiosemicarbazone derivative, as a TYR-targeting scaffold. Structural characterization confirmed a single, stable E-isomer, and pKa profiling together with kinetic analyses indicated reversible mixed-type inhibition of human TYR, involving interactions with both free enzyme and enzyme-substrate complexes. CBD-TSC1 exhib

Microbiota-gut-brain axis and treatment resistance in epilepsy: a multicentre prospective study protocol (CARE).

INTRODUCTION: Approximately one-third of people with epilepsy (PWE) experience resistance to treatment, including pharmacological therapies, epilepsy surgery, vagus nerve stimulation (VNS) and dietary interventions such as the ketogenic diet (KD). Emerging evidence suggests that the gut microbiota may influence seizure susceptibility and treatment response through the microbiota-gut-brain axis, potentially contributing to treatment resistance. The MiCrobiota-gut-brain Axis in Resistant Epilepsy project investigates how gut microbial features and associated host epigenetic signatures affect cli

Neurocognitive Outcomes of In Utero Exposure to Antiseizure Medication: An Australian Cohort Study.

BACKGROUND AND OBJECTIVES: Continuation of antiseizure medication (ASM) during pregnancy is essential for seizure control in most women with epilepsy, yet few studies have characterized the neurocognitive phenotypes associated with prenatal ASM exposure, with limited data on newer agents. The aim of this study was to determine whether in utero exposure to specific types of ASM monotherapies is associated with poorer neurocognitive outcomes compared with ASM-unexposed children. METHODS: This semiprospective cohort study included children aged 3-18 years born to women enrolled in the Raoul Wall

Seizure relapse in new onset epilepsy: It is not always drug resistance.

OBJECTIVE: Seizure recurrence in new onset epilepsy (NOE) can result from various factors. Although drug ineffectiveness is frequently investigated, other causes-such as nonadherence, inadequate treatment, nonepileptic events (e.g., functional/dissociative), or acute symptomatic seizures-also impact patient outcomes. This retrospective, longitudinal single-center study evaluated relapse causes over a 5-year period in a cohort of NOE patients. METHODS: We reviewed the NOE registry of Geneva University Hospital, including adult patients diagnosed between 2005 and 2019. Of 740 consecutive patien

Assessment of gabapentin efficacy in patients with KCNQ2-developmental epileptic encephalopathy.

KCNQ2‑related developmental epileptic encephalopathy (KCNQ2‑DEE) is characterized by intractable epilepsy, cognitive impairment, and behavioral problems, highlighting the urgent need for precision therapies. This study aimed to promote the clinical application of gabapentin, a known Kv7 channel opener, in KCNQ2‑DEE by evaluating its efficacy in four patients carrying different KCNQ2 variants. Three patients with S6‑segment variants (p.A294V, p.F305I, p.F305del) showed clinical improvement in both seizure control and cognition following add‑on gabapentin. In contrast, the patient with the S4 va

Cannabidiol Limits Early Aβ-Induced Glial Activation and Preserves Synaptic Integrity in Primary Mouse Hippocampal Neuron-Glia Cultures.

Alzheimer's disease (AD) initiates with subtle neuroimmune alterations that precede overt synaptic loss and neuronal death, yet the early sequence linking Aβ exposure to glial activation remains incompletely understood. To capture early neuroimmune dynamics with greater physiological relevance, we employed primary mixed neuron-glia cultures derived from the hippocampi of postnatal day 1 (P1) mice. Unlike conventional coculture systems, these hippocampal mixed cultures preserve intrinsic neuron-astrocyte-microglia communication and recapitulate key features of the in vivo hippocampal microenvir

Mechanisms of weight gain associated with valproic acid: a scoping review.

INTRODUCTION: Valproic acid (VPA), a widely prescribed mood stabilizer and antiepileptic drug, is frequently associated with metabolic side effects, particularly weight gain. Despite its clinical relevance, the range and diversity of mechanisms underlying VPA-associated weight gain remain incompletely mapped. OBJECTIVES: To map the breadth of evidence on mechanisms of VPA-associated weight gain, characterize the types of studies and populations investigated, and identify knowledge gaps to inform future research. METHODS: This PRISMA-ScR-guided scoping review searched PubMed, Cochrane Library

Study protocol for a prospective, cluster randomised control, hybrid type 1 effectiveness-implementation trial to evaluate the Reduction Of Seizures through Education and Support: the ROSES study.

INTRODUCTION: Epilepsy affects 1% of the US population, imposing a significant burden through seizures, distress and medication side effects. Many adverse outcomes can be reduced with patient education and an acute seizure action plan. Electronic health record clinical templates can support this care but are often seen as burdensome. This study will develop a clinical template embedding an acute seizure action plan into routine neurology visits and examine its effect on care quality and healthcare use, as well as barriers and facilitators to adoption. METHODS AND ANALYSIS: This study is a sin

Monitoring Antiseizure Medication Change Using Ultra Long-Term Electroencephalogram: A Multicenter Study.

OBJECTIVE: Medication dose adjustments are common in treatment-resistant epilepsy, but lack robust evidence, and patient-reported seizure diaries have known limitations. We assessed whether ultra long-term subcutaneous electroencephalogram (sqEEG) improves detection of seizure frequency changes after antiseizure medication adjustments. METHODS: A retrospective, multicenter cohort study was carried out across 9 centers in adults with treatment-resistant epilepsy. SqEEG seizure frequency was compared with seizure diaries before and after medication dose increases and reductions, over 7-, 14-, a

Effects of Sativex® Versus Placebo on Glucagon-like Peptide-1, Total Ghrelin, and Subjective Appetite in Older Adults with Poor Appetite: A Protocolized Secondary Analysis of a Double-Blind, Randomized, Placebo-Controlled Crossover Trial.

BACKGROUND: Poor appetite, a major contributor to malnutrition, is highly prevalent among older adults and associated with adverse clinical outcomes. Nevertheless, there are currently no widely accepted or consistently effective pharmacological treatments targeting appetite that are routinely recommended alongside nutritional interventions. Medical cannabis has been proposed as a potential appetite stimulant in older adults with poor appetite through modulation of appetite-regulating hormones such as glucagon-like peptide-1 (GLP-1) and total ghrelin, but the evidence remains limited. Altered default mode network homogeneity in chronic insomnia disorder: modulation by modified suanzaoren decoction and estazolam, and machine learning classification.

OBJECTIVE: Chronic Insomnia Disorder (CID) is characterized by maladaptive self-referential thinking linked to default mode network (DMN) dysfunction. Network Homogeneity (NH), a measure of local functional integration within networks, may capture this dysregulation but remains underexplored in CID and its response to treatment, particularly herbal medicine. METHODS: This study employed resting-state functional magnetic resonance imaging (fMRI) to investigate DMN NH in 82 patients with CID (52 treated with Modified Suanzaoren Decoction [MSZRD], 30 with Estazolam) and 65 matched healthy contro

Posterior reversible encephalopathy syndrome in the setting of severe systemic lupus erythematosus activity with a rapidly fatal outcome: a case report.

BACKGROUND: Posterior reversible encephalopathy syndrome (PRES) is typically considered a self-limiting neurological disorder with a favorable prognosis when promptly diagnosed and treated. However, we report a rare fulminant case of PRES in a patient with systemic lupus erythematosus (SLE), challenging the notion of its uniformly benign course. CASE PRESENTATION: A 50-year-old Asian woman with a recent diagnosis of active SLE and dialysis-dependent renal failure presented with an 8-hour history of headache, dizziness, visual impairment, and seizures. Brain MRI revealed bilateral parieto-occip
