Differential associations between treatment delay and survival across head and neck cancer subsites: a national hospital-based framework for risk-stratified prioritization.

PURPOSE: Delays in time to treatment initiation (TTI) have been associated with inferior outcomes in head and neck cancer, yet prior studies have typically evaluated tumors collectively, assuming uniform time sensitivity across diseases. This study quantified subsite- and stage-specific associations between TTI and overall survival (OS) and derived groups of tumors with similar TTI-mortality associations. METHODS: We conducted a retrospective cohort study using the National Cancer Database (2010-2017), including adults with head and neck malignancies treated with curative intent. Multivariabl

Immune checkpoint inhibitor-related pancreatitis with fulminant type 1 diabetes mellitus: a case report.

BACKGROUND: Immune checkpoint inhibitors (ICIs) are widely used in tumor therapy, and the immune-related adverse events (irAEs) they induce can involve multiple organs. As an organ with both endocrine and exocrine functions, pancreatic involvement may manifest as acute pancreatitis, new-onset diabetes mellitus, or even a combination of both. Among these, ICI-induced acute pancreatitis concurrent with fulminant type 1 diabetes mellitus (FT1DM) is extremely rare but highly critical and easily missed in clinical practice. CASE PRESENTATION: A 59-year-old male patient with squamous cell carcinoma

Radiation-Associated Internal Lymphoedema and Swallowing Impairment in Head and Neck Cancer: Insights From Imaging and Manometry.

INTRODUCTION: Lymphoedema is a significant complication following head and neck cancer (HNC) treatment. We examined the use of computed tomography (CT) to detect internal lymphoedema (IL) based on serial assessments of volumes of swallowing structures before, during, and after radiation treatment. Volume changes were correlated with videomanometry and Functional Lumen Imaging Probe (EndoFLIP) findings to determine whether these assessments might provide insights into the impact of lymphoedema on swallowing dysfunction. METHOD: Data were collected retrospectively from 26 HNC patients who compl

Robotic-Assisted Extended Sistrunk Approach (RESA) for Salvage Surgery After Head and Neck Irradiation.

BACKGROUND: The robotic-assisted extended "Sistrunk" approach (RESA) is a minimally invasive technique providing access to hypopharyngeal and laryngeal structures via a submental incision and vallecular pharyngotomy, bypassing the base of tongue. METHODS: We report 10 RESA procedures in previously irradiated head and neck cancer patients, including six organ-preservation surgeries and four total laryngectomies. RESULTS: RESA enabled completion of the planned surgery, achieving R0 margins in all cases. Postoperative bleeding occurred in four patients (three Clavien-Dindo IIIb; one II). One ce

Imaging findings of basaloid squamous cell carcinoma: a case report.

Basaloid squamous cell carcinoma (BSCC) is a rare, high-grade variant of SCC. In the oral cavity, BSCC predominantly originates from the floor of the mouth and base of the tongue and is characterized by aggressive behavior, with frequent nodal and distant metastases. To date, no case has comprehensively reported BSCC findings across multiple imaging modalities in a single patient. A 78-year-old male presented with persistent erythema, swelling, and pain in the anterior mandible. Clinical examination revealed exposed bone with an exophytic soft tissue mass and cutaneous fistula. The lesion was

Artificial Intelligence in Oral Cancer Diagnosis: Overcoming Challenges for Enhanced Outcomes.

Oral-related cancer accounts for the sixth leading cause of cancer-related deaths and one death every hour in the United States. Several factors may contribute to the formation of oral tumors, including tobacco use, alcohol consumption, unhealthy diets low in fruits and vegetables, age, and general lifestyle. Smoking and alcohol consumption, in particular, have been found to contribute 80% and 61% to oral cancer in men and women, respectively. It is also well-known that oral cancer is more prevalent in underprivileged groups, where access to healthcare and health education, particularly educat

Transoral robotic surgery using CO2 laser in oropharyngeal squamous cell carcinoma: a clinical case series.

PURPOSE: Transoral robotic surgery (TORS) is an established surgical approach for oropharyngeal squamous cell carcinoma (OPSCC). Carbon dioxide (CO₂) laser offers high cutting precision and minimal collateral thermal injury, yet its use in TORS remains underreported. The purpose of this study is therefore to describe the first UK case series detailing the use of CO₂ laser via hollow waveguide in TORS. METHODS: Eight patients (six male, two female; mean age 60 years) presenting with either left tonsillar (seven cases) or right tongue base (one case) OPSCC underwent primary TORS resection with

[Prehabilitation training program for patients with head and neck tumors-pathophysiology, target structures, and evidence-based exercises].

Oropharyngeal dysphagia secondary to head and neck cancer is common and significantly impacts quality of life, morbidity, mortality, and 5‑year survival rates. Functional impairments can result from tumor mass, location, surgical interventions, and (adjuvant) radio- or chemotherapy. Early identification of deficits and assessment of individual patient resources by a multidisciplinary team enable the development of tailored, targeted prehabilitative exercise programs aimed at preventing functional decline and fostering compensatory mechanisms. Central pathophysiological factors of dysphagia inc

How I do it: transpharyngeal resection of base of tongue cancer with hyoid suspension.

OBJECTIVE: Transoral robotic surgery is increasingly used for selected base-of-tongue tumours, but is often impractical in patients with severe trismus and remains cost prohibitive. Conventional transmandibular approaches are associated with significant morbidity. We describe a transpharyngeal technique incorporating hyoid suspension to address these limitations. METHODS: A retrospective series of patients undergoing transpharyngeal resection for base-of-tongue carcinoma is presented. The technique includes neck dissection with vascular control, lateral-inferior pharyngotomy, tumour excision

Disease control with disitamab vedotin in HER2-low polymorphous adenocarcinoma of the tongue base after chemotherapy failure: A case report.

Polymorphous adenocarcinoma (PAC) is a distinct subtype of salivary gland carcinoma (SGC), and its occurrence at the tongue base is uncommon. Effective systemic treatment options for patients with advanced disease remain limited. Although HER2-low expression has been identified in a subset of SGCs, evidence supporting the use of HER2-targeted antibody-drug conjugates (ADCs) in HER2-low PAC is currently lacking. CASE PRESENTATION: A 72-year-old female with HER2-low (immunohistochemistry 1+) PAC of the tongue base presented with cervical nodal metastases (cT3N2cM0, Stage IVA). The tumor progress

Complete locoregional response to radiotherapy and pembrolizumab in an elderly and frail patient with oropharyngeal squamous cell carcinoma: A case report.

RATIONALE: Immune checkpoint inhibitors (ICIs) have revolutionized the management of head and neck squamous cell carcinoma, particularly in the recurrent/metastatic setting. Although the combination of ICIs and radiotherapy (RT) in locally advanced disease has not yet demonstrated clear survival benefits, the strong biological rationale for their synergistic action continues to support investigation, especially in frail or elderly patients unfit for standard chemoradiotherapy. PATIENT CONCERNS: An 83-year-old male presented with odynophagia, dysphagia, significant weight loss, and right cervi

Histone lactylation-driven feedback loop modulates pyrimidine metabolism to promote oral carcinogenesis.

Metabolic reprogramming and epigenetic alterations promote oral squamous cell carcinoma (OSCC). Lactate-dependent histone modification is a novel histone mark that connects the epigenetic process of lactylation to glycolytic metabolites. However, the role of histone lactylation in oral carcinogenesis remains poorly understood. In this study, the levels of histone lactylation in oral leukoplakia (OLK) and OSCC tissues were determined by immunohistochemistry. The involvement of histone lactylation in OSCC initiation was assessed by the inhibition of lactylation using glycolysis inhibitors or sil

Diagnostic utility of EWSR1 in hyalinizing clear cell carcinoma: a systematic review and meta-analysis.

Hyalinizing clear cell carcinoma (HCCC) is a rare, low-grade salivary gland tumor that poses diagnostic challenges due to its morphological and immunohistochemical overlap with other clear cell neoplasms of the head and neck. EWSR1 gene rearrangements, particularly EWSR1-ATF1 fusion, have been identified as distinctive molecular markers; however, their diagnostic significance remains unclear. This systematic review and meta-analysis, conducted in accordance with PRISMA guidelines and registered in PROSPERO (CRD42022382551), evaluated the diagnostic role of EWSR1 rearrangements in HCCC. The prevalence, nature and trajectory of oropharyngeal dysphagia within the first year following curative open resection for esophageal cancer.

In the absence of a mechanical stricture, oropharyngeal dysphagia following esophageal cancer surgery, and swallowing physiology more broadly, is poorly understood. This study investigated oropharyngeal dysphagia within the first year following curative open esophageal resection. A prospective cohort study was conducted (January 2022 to January 2024) at the National Esophageal Cancer Centre in Ireland. Participants were recruited between 6 to 12 months post-esophagectomy. A standardized videofluoroscopy was completed. Outcome measures included the Dynamic Imaging Grade of Swallowing Toxicity (
