Tenosynovitis caused by Mycobacterium malmoense.

BACKGROUND: Mycobacterium malmoense is a slow-growing nontuberculous mycobacterium (NTM) increasingly recognized as a cause of extrapulmonary infections. Tenosynovitis due to M. malmoense is rare and diagnostically challenging because of its indolent course and nonspecific clinical presentation. CASE PRESENTATION: We report a 65-year-old immunocompetent woman presenting with progressive wrist pain, swelling, and impaired hand function without prior trauma. Symptoms developed after exposure to fresh fish and shellfish. Surgical exploration revealed extensive tenosynovitis with median nerve com

Characterization of Novel Mycobacteriophages of Subcluster A3: Evolutionary Plasticity of the Lysogeny Module and Lytic Potential.

The increasing prevalence of drug-resistant Mycobacteriumtuberculosis (Mtb) and nontuberculous mycobacteria (NTM) underscores the need for novel strategies. Mycobacteriophages are attracting growing interest for therapeutic and diagnostic applications. Here, we describe four new subcluster A3 phages (Cat, Goose1D, Goose1X, and Shep) and investigate their genomic diversity and lytic activity. The phages share >98% nucleotide identity with known A3 members but carry distinct deletions within the lysogeny module: Cat and Goose1X have different repressor-loss variants, and Goose1D lacks the integr

Activity of a three-phage combination against Mycobacterium tuberculosis in disease-relevant conditions.

BACKGROUND: Phage therapy offers promise to combat antimicrobial resistance, including drug-resistant tuberculosis. Understanding phage activity against Mycobacterium tuberculosis (Mtb) in granuloma-like microenvironments that induce non-replicating states, characterized by hypoxia and acidity, is essential. OBJECTIVES: To evaluate a phage combination against Mtb under hypoxic, acidic (pH 5.5), and stationary-phase conditions in vitro. METHODS: Mtb H37Rv cultures were grown to exponential phase for time-kill studies (106 colony forming units (CFU)/mL) under normal, hypoxic, and acidic condit

Ten simple rules to build genomics capacity for routine laboratory practice in the Global South.

Developing sustainable genomics capacity in the Global South requires more than technology transfer; it depends on human-centered approaches based on the workflows and existing expertise of laboratory personnel, institutional collaboration, and long-term integration into health systems. Drawing on lessons learned from regional capacity-building initiatives, this article proposes ten simple rules for implementing and sustaining genomics capacity within public health and clinical laboratory settings. These rules emphasize local context, strategic planning, continuous learning, and equitable coll

Infectious, Environmental, and Chronic Disease Burden among 1,844 Newly Arrived Humanitarian Migrants in Oklahoma, 2019-2025.

Global displacement has brought increasingly diverse migrant populations into health systems with limited experience managing large-scale humanitarian arrivals, yet contemporary data describing infectious, environmental, and noncommunicable disease burden across heterogeneous newcomer groups remain scarce. A cross-sectional analysis of 1,844 consecutively screened newly arrived individuals who completed the U.S. domestic medical examination at the University of Oklahoma Center for Travel and Immigrant Health between October 2019 and May 2025 was conducted. Screening was performed in accordance

Small proteins CrgA and Rv0207c modulate the essential mycolic acid transporter MmpL3.

MmpL protein complexes are major contributors to lipid/metabolite and drug transport in Mycobacterium tuberculosis and related mycobacteria and are important targets in current drug discovery efforts. At the core of these complexes are proton-motive force driven transporters belonging to the Resistance-Nodulation-cell Division superfamily of proteins that enable export of various substrates across complex bacterial cell envelopes. This study focuses on MmpL3, which transports trehalose monomycolate in M. tuberculosis and related bacterial species and plays an essential role in the assembly of

Beyond Tuberculosis: Late Diagnosis of Cystic Fibrosis in an Adolescent with Bronchiectasis and Recurrent Pseudomonas aeruginosa Isolation: A Case Report.

Background and Clinical Significance: Cystic fibrosis may remain undiagnosed until adolescence, when recurrent respiratory infections and bronchiectasis are attributed to a previous severe infection. Case Presentation: We report the case of a male adolescent in whom approximately five years elapsed between microbiologically confirmed pulmonary tuberculosis in July 2021 and recognition of cystic fibrosis in August 2026. Archived records documented two sputum specimens that were acid-fast bacilli smear-positive (1+) and culture-positive (1+) for Mycobacterium tuberculosis complex. During adolesc

[Cutaneous coinfection with tuberculosis and leishmaniasis].

We describe the case of a 16-year-old female patient from Bolivia, with a family history of pulmonary tuberculosis, who presented with a two-month history of ulcerated lesions on the dorsum of her left hand. A skin biopsy revealed amastigotes consistent with Leishmania spp., and antiparasitic treatment was initiated. Subsequently, Mycobacterium tuberculosis was also isolated, and antituberculosis treatment was started, resulting in a favorable outcome.

A Colorimetric Approach to Identifying Single-Nucleotide Mutations Using Gold-Based Magnetic Nanoparticles.

Detection of single-nucleotide mutations (SNPs) in the genome of infectious agents is one of the ways to identify their drug-resistant strains. Antibiotic-resistant strains of Mycobacterium tuberculosis (MTB) impede advances in the fight against the spread of tuberculosis. Diagnostic tools exist for both MTB itself and its drug-resistant strains, most of which are technically complex, resource-intensive, and time-consuming. This paper presents the development of a colorimetric test system, requiring no expensive equipment, for the rapid and accurate determination of the SNPs associated with dr

Maleylated-BSA-Coated Gold Nanopopcorns for Pulmonary Delivery of Quercetin.

A non-motile bacterium, Mycobacterium tuberculosis (Mtb), causes tuberculosis (TB), an infection of the lower respiratory tract. Currently, rifampicin, isoniazid, ethambutol, and pyrazinamide are used as first-line treatment for drug-susceptible TB for two months, followed by four months of rifampicin and isoniazid. This treatment plan is linked to serious hepatic, dermatological, and hematopoietic adverse effects. The exploration of natural substances like quercetin for treating TB was driven by the significant side effects of the standard therapy protocol. Protein-coated gold nanopopcorns we

Carbonic Anhydrases in Mycobacteria: Linking Metabolic Adaptation to Phenotypic Drug Tolerance.

Tuberculosis (TB) treatment is increasingly challenged by both antimicrobial resistance (AMR) and phenotypic drug tolerance, the latter representing a reversible physiological state that contributes to persistence and prolonged therapy. While AMR is driven by heritable genetic mutations, drug tolerance arises from reversible physiological adaptations that enable bacterial survival during prolonged antibiotic exposure. Increasing evidence suggests that metabolic remodeling plays a central role in establishing these drug-tolerant states. Carbonic anhydrases (CAs) are zinc-containing metalloenzym

Neurological Manifestations in People Living with Human Immunodeficiency Virus (PLHIV) in India: A Scoping Review and the Need for a National HIV-Neurology Special Interest Group.

BACKGROUND AND OBJECTIVES: Despite a low national prevalence of 0.20%, India's absolute burden of 2.544 million people living with human immunodeficiency virus (HIV) presents clinicians with varied neurological complications. While combination antiretroviral therapy (ART) has extended survival, the neuro-HIV profile in India, characterized by the predominance of HIV-1 clade C, remains distinct from that reported in Western cohorts. This scoping review aimed to identify patterns of neurological manifestations in India across the pre-ART, ART, and "treat-all" eras and to support the establishmen

Barriers to biomedical care: Qualitative insights into health-seeking behaviors and systemic challenges among people affected by leprosy in Tanzanian communities.

BACKGROUND: Delayed diagnosis in leprosy reflects both how people interpret symptoms and seek care, and how effectively health systems recognize and manage the disease. Qualitative evidence from Tanzania remains limited on how the interaction between patients and health care workers affect provision of care to people affected by leprosy in a timely manner. The main aim of this study was to explore health-seeking behaviors and systemic challenges affecting access to biomedical care among people affected by leprosy in selected Tanzanian communities. METHODS: Ethics Statement: Ethical approval f

A Proposed Consensus Definition of Severe COPD: a modified Delphi Study.

RATIONALE: Advanced interventions are available for patients with more severe COPD despite guideline-recommended therapies, yet this population is heterogeneously defined and inconsistently recognized, and a standardized work-up is lacking, which could lead to mismanagement. OBJECTIVES: To formulate a new, expert-based consensus definition of severe COPD and a minimal set of diagnostics and characteristics to be assessed in the work-up of these patients. METHODS: A modified Delphi study with two online rounds was conducted between June 2024 and January 2025 with 55 international COPD experts

Case Report: Analysis of antimicrobial therapy and pharmaceutical care of mixed Aspergillus and Mycobacterium tuberculosis infection following renal transplantation: a case study.

Invasive infection is a significant complication following renal transplantation. Due to long-term immunosuppressive therapy, renal transplant recipients are at high risk for fungal infections and tuberculosis, which are also major causes of mortality in this population. Triazole drugs, as first-line agents for invasive fungal infections, can cause fluctuations in the blood levels of immunosuppressants due to drug-drug interactions. Furthermore, the concomitant use of antituberculosis drugs further complicates the balance between maintaining immunosuppression and anti-infective therapy, often

MSMEI_4050 modulates resistance to the antitubercular drug rifampicin in Mycolicibacterium smegmatis.

OBJECTIVES: Drug-resistant tuberculosis increases the risk of first-line drugs failure, and one strategy involves the identification of new molecular mechanisms of drug resistance. Mycobacterium tuberculosis lipases play a role in the synthesis of bacterial cell wall lipids. In addition, different studies have shown that lipases are associated with drug resistance. The NLH-H protein (Rv1399c) from M. tuberculosis has been characterized, and its overexpression decreases susceptibility to tetrahydrolipstatin. The MSMEI_4050 gene of Mycolicibacterium smegmatis encoded a protein which has more tha
