Prognostic utility of immune biomarkers for identifying individuals at risk of unfavourable treatment outcomes in pulmonary tuberculosis.

INTRODUCTION: Tuberculosis remains a major global health challenge, with many patients expe- riencing unfavourable treatment outcomes despite standard therapy. Host immune biomarkers, including cytokines, chemokines, and matrix metalloproteinases (MMPs), have shown promise in predicting disease severity and treatment response, but require validation across independent cohorts. METHODS: We validated previously identified immune biomarkers using baseline plasma samples from a nested case-control cohort within the RePORT India consortium (cases: n=74; controls: n=161). Cytokines (IL-2, IFN-γ, TN

Disseminated Tuberculosis With Ocular Involvement After 12 Years of Anti-TNF-α Therapy.

BACKGROUND: Ocular tuberculosis (TB) is an uncommon yet potentially vision-threatening manifestation of Mycobacterium tuberculosis, particularly difficult to diagnose in immunocompromised individuals. The increasing use of anti-tumor necrosis factor-α (anti-TNF-α) agents has markedly heightened the risk of TB reactivation, often with disseminated or extrapulmonary involvement despite negative baseline screening. CASE PRESENTATION: We describe a 52-year-old woman with rheumatoid arthritis receiving long-term adalimumab and methotrexate who developed disseminated TB with pulmonary, splenic, and

A Whole-blood Transcriptional Signature Associated With Obstructive Post-tuberculosis Lung Disease.

BACKGROUND: Post-tuberculosis (TB) lung disease (PTLD) affects ∼50% of persons with pulmonary TB (PTB). We recently discovered whole blood transcriptional signatures associated with PTLD. We examined whether a minimum gene signature predicts PTLD as a clinically useful biomarker. METHODS: We prospectively enrolled 301 treatment naive adults with newly diagnosed PTB (cohort A). We collected whole blood at 0 and 6-month visits, isolated RNA, and measured a modified MTB host response (HR) signature (mHR) based on expression of DUSP3, GBP5, and TMBIM6. We recorded spirometry at 6 (n = 216) and 12

18F-FDG PET/CT and post-treatment lung function in adolescent tuberculosis.

BACKGROUND: Data on post-tuberculosis outcomes in adolescents are scarce. Although fluorine-18 fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) has been used in adult tuberculosis research, little is known about the association between PET/CT findings and functional outcomes in general. METHODS: Early FDG PET/CT images of 50 adolescents (31 female, median age 17.2 years) with confirmed pulmonary tuberculosis were read visually and quantitative data of disease burden (total lung volume, cavity volume, total hard volume and total glycolytic activity (TGA))

Catalysis and allostery in bacterial ATP phosphoribosyltransferases.

ATP phosphoribosyltransferase (ATPPRT) catalyses the first reaction of L-histidine biosynthesis: the condensation of ATP and 5-phospho-α-d-ribose 1-pyrophosphate to form N1-(5-phospho-β-d-ribosyl)-ATP with the release of inorganic pyrophosphate. L-Histidine allosterically inhibits ATPPRT as a mechanism to regulate this energy-intensive pathway in histidine-synthesising organisms, such as bacteria, archaea and plants. Absent in humans and essential in many disease-causing bacteria, such as Mycobacterium tuberculosis and Acinetobacter baumannii, ATPPRT is a promising drug target for novel antibi

Birth and clinical outcomes of pregnant participants with advanced HIV disease.

INTRODUCTION: Clinical trials for the evaluation of novel diagnostics and treatments for HIV-associated opportunistic infections have historically excluded pregnant persons. Little information is available regarding the clinical outcomes and pregnancy outcomes of pregnant persons with advanced HIV disease while taking opportunistic infection prophylaxis for tuberculosis (TB) and cryptococcosis. In this study, we describe characteristics at enrollment, clinical outcomes, and birth outcomes of pregnant people with advanced HIV in Uganda. METHODS: We prospectively enrolled pregnant adults with a

Objective rest-activity rhythm amplitude and lung function in US adults: A cross-sectional analysis of the 2011-2012 National Health and Nutrition Examination Survey.

This cross-sectional analysis of 4,150 US adults from the 2011-2012 NHANES examined associations between wrist-accelerometry-derived rest-activity rhythm metrics and respiratory outcomes spanning spirometry, exhaled nitric oxide, and self-reported respiratory symptoms and diagnoses. Relative amplitude (RA), interdaily stability, intradaily variability, M10, L5, and cosinor parameters were derived from hourly monitor-independent movement summary (MIMS) values. The co-primary outcomes were FEV1 and FVC z-scores (GLI 2012); secondary outcomes included FEV1/FVC, FeNO, and latent-class-derived resp

Familial hypercholesterolaemia and thymic ageing in a Chinese cohort.

BACKGROUND AND AIMS: Lifelong exposure to elevated low-density lipoprotein cholesterol in familial hypercholesterolaemia (FH) patients not only increases the risk of atherosclerotic cardiovascular disease, but also affects other organs beyond the cardiovascular system. In a large FH cohort, this study investigated the effects of hypercholesterolaemia on the thymic output and explored whether lipid-lowering therapy can reverse these effects. METHODS: According to the Dutch Lipid Clinic Network criteria, 562 heterozygous FH patients (236 treatment-naïve, 326 on lipid-lowering therapy) and 295 a

Community health workers and promotores de salud: a tiered framework for infectious disease prevention and care among California agricultural workers.

Agricultural workers in the United States face a disproportionate burden of infectious diseases, including tuberculosis, HIV, coccidioidomycosis, viral hepatitis, and COVID-19, driven by occupational exposures, crowded housing, poverty, and limited healthcare access. Despite this burden, farmworkers have among the lowest rates of healthcare utilization of any US working population, and traditional clinic-based delivery models are poorly suited to the realities of agricultural labor. Community health workers (CHWs), known as promotores de salud in Latino communities, offer a well-established st

Tuberculous spinal epidural abscess: a cautionary case report of permanent paraplegia due to delayed diagnosis and management.

BACKGROUND: Spinal epidural abscess caused by tuberculosis is a rare but serious complication of spinal tuberculosis. Its atypical presentation predisposes to diagnostic delay and a poor prognosis. This case not only documents the clinical and radiologic evolution of tuberculous spinal epidural abscess but also suggests that delays in diagnosis and treatment may jointly contribute to serious neurological outcomes. CASE PRESENTATION: A 39-year-old man initially underwent acupotomy for recurrent low back pain with recent exacerbation. Subsequent imaging revealed a paraspinal abscess within the

KLRG1+CD28⁻CD57+ Ag85A-specific CD8+ T cells mark a severity-associated terminal-differentiation imprint in pulmonary tuberculosis.

BACKGROUND: Pulmonary tuberculosis (PTB) is characterized by prolonged host-pathogen interaction, chronic granulomatous inflammation, and sustained exposure to Mycobacterium tuberculosis (Mtb) antigens. How Mtb antigen-specific CD8+ T cell differentiation states relate to clinical disease activity and severity remains incompletely defined. This study investigated the clinical relevance of a terminal differentiation-associated phenotype within Ag85A-specific CD8+ T cells across clinically defined states of Mtb infection and disease. METHODS: A total of 124 participants were enrolled, including

An impaired T-cell-mediated mitogen response as a potential biomarker to aid in clinical diagnosis of pulmonary tuberculosis.

PURPOSE: A considerable proportion of suspected pulmonary tuberculosis (pTB) cases are identified and treated on the basis of clinical diagnoses, as it is often not possible to confirm Mycobacterium tuberculosis (Mtb) infection. In the absence of pathogen detection, supplementary indicators, such as immunological biomarkers, would facilitate TB diagnostic decision-making. METHODS: In this prospective case-control study conducted in Ghana, patients with suspected pTB (n = 83) were divided into three subgroups: confirmed cases with high (pTBhigh) or low (pTBlow) Mtb sputum load, and clinically

Tuberculous Cervical Lymphadenitis (Scrofula) Associated with Massive Pleural Effusion in an Immunocompetent Adult: A Rare Presentation of Extrapulmonary Tuberculosis Confirmed by GeneXpert MTB/RIF.

Background: Tuberculous cervical lymphadenitis (scrofula) is the most common form of extrapulmonary tuberculosis, although its coexistence with massive pleural effusion is uncommon and may delay diagnosis because of its nonspecific clinical presentation. We report the case of a 47-year-old man presenting with a large right-sided pleural effusion associated with a suppurative cervical lymphadenitis, in whom rapid molecular testing established the diagnosis of disseminated extrapulmonary tuberculosis. Case Presentation: A 47-year-old male presented with progressive right-sided chest pain, exerti
