Two case histories are presented of patients having adverse reactions to urographic contrast media, one severe and the other fatal. The role of the family physician is stressed in communicating with consultants and the radiological department of the hospital to which patients are referred for contrast media studies.The patient's previous history of allergies, particularly sea food allergy, and any previous reaction to urographic contrast media are of great importance. This information should be given to the radiologist at the time of referral.

Of those exposed to Chinese restaurant food, our studies indicate that 25% report adverse reactions (Chinese restaurant syndrome (CRS)), presumably to the mono-sodium glutamate (MSG) content. The possible significance of the symptoms is discussed in the light of the known neuroexcitatory activity of MSG. It is suggested that CRS may result from a "benign" inborn "error" of metabolism that is deserving of further study, particularly in individuals with certain other metabolic abnormalities or who are on certain types of drug therapy.

Fifty-four infants with the malabsorption syndrome and cow's milk intolerence seen during 1962-1971 were investigated. All had diarrhoea and failed to thrive. Most had vomiting and about 20% had atopic eczema and recurrent respiratory infections. Laboratory investigations revealed malabsorption, raised serum IgA, and precipitins to cow's milk. Among the cereal ingestants, rice, rye, wheat, corn and oats, provocative skin tests of patients differed according to age. Sensitivity to rice and oats was low for children and that to rice in particular was high for adults, suggesting a differential immunological response to rice allergens. Compared to patient grass pollen reactions, response to ingestants was noticeably lower. Cross-reactivity among ingestants was also lower, particularly between oats and the other cereals.

Possible interrelationships between maternal drug ingestion and infections during the first trimester and the occurrence of congenital malformations have been studied. Of 1 142 pregnancies, 3,4% ended in malformations clinically detected within 1 week of birth. For the group as a whole there was no evidence of an increased risk ratio in those mothers who ingested medicines. Possible reasons for this are discussed.
