A nomogram for predicting ischaemic muscle sequelae after revascularization in patients with traumatic femoral-popliteal artery injuries: a retrospective cohort study.

PURPOSE: This study aimed to investigate the incidence and associated risk factors of ischaemic muscle sequelae in patients with traumatic femoropopliteal artery injuries following revascularization, as well as to develop a nomogram to predict the risk of ischaemic muscle sequelae. METHODS: Data from patients with acute traumatic femoropopliteal artery injuries between January 2008 and December 2022 were collected. All patients with successful limb salvage were divided into two groups based on the occurrence of ischaemic muscle sequelae: the ischemic muscle sequelae group (IG) and the non-isc

Functional Salvage of Ischemic Myopathy at the Neuromuscular Junction Level: A Mouse Model Study on Prolonged Muscle Ischemia in the Upper Limb.

Ischemic myopathy in the upper limb may develop progressively in cases of peripheral arterial disease or acutely following traumatic vascular injuries. Prolonged ischemia can lead to catastrophic damage to distal muscles, with a significant risk of irreversible motor function loss. It is hypothesized that the neuromuscular junction (NMJ) sustains substantial damage beyond a critical threshold of arterial ischemia. Furthermore, concomitant upstream nerve injuries may exacerbate NMJ degeneration, potentially resulting in permanent dysfunction. This study aims to evaluate the NMJ-level changes in

Traumatic Skeletal Muscle Injury and Recovery.

Skeletal muscle is integral to life and functions to produce movement and locomotion, postural control, respiratory activity, and heat production. Yet, injuries to skeletal muscles occur across the lifespan, and depending on the injury mechanism, nuances in physiology and severity, how and if recovery of muscle function is possible is varied. This chapter presents the current physiologic understanding of various traumatic skeletal muscle injuries with attention to landmark work that has built the foundation for our current understanding. Injuries ranging from those that leave the extracellular

Feasibility of Targeted Light Sedation Strategy in Open Abdomen Management: A Retrospective Study.

AIM: Open abdomen management (OAM) is now used for non-traumatic conditions like gastrointestinal ischemia. The optimal sedation strategy for patients undergoing OAM is unclear, especially for light sedation. We evaluate the feasibility of targeted lighter sedation in patients undergoing OAM. METHODS: We retrospectively studied non-trauma patients undergoing OAM in the ICU. A lighter sedation was implemented for a cohort of patients than the conventional sedation target in an earlier cohort. In our strategy, we set a goal of sedation depth of 0 to -2 on the Richmond Agitation and Sedation Sca

Therapeutic strategies for critical limb ischemia: a focus on endogenous cell behavior modulation.

Critical limb ischemia (CLI) represents the advanced stage of peripheral artery disease, with an extremely high rate of amputation and mortality. Conventional treatments, including surgical revascularization and percutaneous interventions, are constrained by invasiveness and unsuitability for certain patients, highlighting the urgent need for novel therapeutic approaches. Emerging strategies that regulate endogenous cell behavior offer an effective, less traumatic alternative with wider applicability. This review systematically summarizes therapeutic approaches focused on key cell populations-

Spinal Cord Injury and Ageing: The Role of Chronic Neuroinflammation.

As the global population ages, there is an increasing prevalence of spinal cord injury (SCI) among elderly individuals, accompanied by significant challenges in treatment and recovery. Age-related conditions, such as osteoporosis, muscle atrophy, and impaired balance, predispose older adults to falls and traumatic injuries, leading to worse neurological outcomes compared to younger patients. SCI pathophysiology consists of two phases: the initial mechanical injury and the secondary injury that involves a cascade of pathological events, including ischemia, apoptosis, and neuronal cell death. Regulation of N-Glycosylation of CDNF on Its Protein Stability and Function in Hypoxia/Reoxygenation Model of H9C2 Cells.

Myocardial ischemia-reperfusion (I/R) injury is a cause of high post-interventional mortality in patients with acute myocardial infarction (MI). Cerebral dopamine neurotrophic factor (CDNF) is an endoplasmic reticulum (ER) resident protein, and its expression and secretion are induced when tissues and cells are subjected to hypoxia, ischemia, or traumatic injury. As a novel cardiomyokine, CDNF plays a crucial role in the progression of myocardial I/R injury. In our previous study, we reported that the overexpression of CDNF inhibited tunicamycin-induced H9C2 cell apoptosis. Moreover, there is

Ischemic and non-ischemic myocardial injuries at autopsy- an overview for forensic pathologists.

Cardiovascular diseases are major causes of morbidity and death worldwide, and most cardiac deaths are related to ischemic injury of the myocardium (myocardial infarction). As underlined in the current clinical definition and classification of myocardial infarctions, not all myocardial injuries are due to ischemia: irreversible injury, ending in necrosis, can be induced also by various other factors, such as infections, immune disorders, physical and chemical agents, and trauma. This is supported by clinical studies showing that elevated serum levels of cardiac troponins, as a measure of myoca

Effect of mindfulness-based cognitive therapy on rumination and post-traumatic growth in patients with acute cerebral infarction: A randomized controlled trial.

BACKGROUND: The prevention and management of cerebrovascular diseases, particularly cerebral infarction - the most prevalent type of cerebrovascular disease - pose significant challenges, severely affecting patients' physical and mental health. Mindfulness-based cognitive therapy (MBCT) has been shown to be effective in treating various mental and chronic conditions. However, there is limited evidence on its effectiveness for treating rumination in patients with acute cerebral infarction (ACI). This study examines the effects of MBCT on rumination, anxiety and depression symptoms, post-traumat

Diaphragmatic pacing system from the point of view of the patients: 14 years of clinical experience and follow-up.

BACKGROUND: High cervical spinal diseases (C1-C4), resulting in paralysis of the respiratory muscles, makes the person dependent on invasive mechanical ventilation (IMV) for life support. Although effective from a ventilatory point of view, IMV is associated with increased morbidity and mortality; ventilated patients have higher rates of respiratory complications, a significant reduction in life expectancy, and above all, a serious psychological repercussion with estrangement from the world. Diaphragmatic pacing system (DPS) is a well-established technique capable of restoring paraphysiologica

Part I: Acute Lower Limb Ischemia-Reperfusion Injury: Pathophysiology and Experimental Interventions.

Background: Ischemia-reperfusion injury (IRI) secondary to acute limb ischemia is a complex clinical phenomenon induced by a variety of pathologies, including arterial embolization, arterial thrombosis, vascular trauma, advanced peripheral vascular disease, traumatic crush injuries, and/or prolonged tourniquet application. The restoration of blood flow and oxygen to the limb after an ischemic period, while necessary, is ultimately the driving force of IRI. The exaggerated inflammation, oxidative stress, and tissue damage caused by IRI often lead to limb dysfunction, limb loss, multi-organ fail

Prognostic value of temporalis muscle thickness as a marker of sarcopenia in intracerebral hemorrhage.

INTRODUCTION: Estimating the prognosis of spontaneous intracerebral hemorrhage (ICH) is of great importance. It has not been conclusively clarified whether sarcopenia is predictive for the functional outcome in ICH. Determining the temporalis muscle thickness (TMT) may be helpful for estimating sarcopenia. An association of TMT with outcome (mRS) has been shown in cerebellar ischemia and traumatic brain injury. METHODS: The present retrospective study of 488 consecutive patients with ICH aimed to investigate the association of sarcopenia as assessed by TMT with mRS. In addition to biometric d

Endothelium glutamate receptors in brain pathology.

The endothelium in brain microcirculation functions not only as a barrier but also as a signal transduction component within a system that regulates multiple vascular processes, including muscle tone, permeability, and structural integrity. The control of local blood flow is vital to ensure adequate oxygen and nutrient supply, efficient removal of catabolic waste, and the maintenance of proper brain cell function. The role of endothelial glutamate receptors in brain pathology is an emerging area of research, particularly important for understanding how these receptors contribute to neurologica

Prolongation of the Time Window From Traumatic Limb Amputation to Replantation From 6 to 33 Hours Using Ex Vivo Limb Perfusion.

INTRODUCTION: Continuous extracorporeal perfusion (ECP), or machine perfusion, holds promise for prolonged skeletal muscle preservation in limb ischemia-reperfusion injury. This study aimed to extend the amputation-to-replantation time window from currently 6 hours to 33 hours using a 24-hour ECP approach. MATERIALS AND METHODS: Six large white pigs underwent surgical forelimb amputation under general anesthesia. After amputation, limbs were kept for 9 hours at room temperature and then perfused by 24-hour ECP with a modified histidine-tryptophan-ketoglutarate (HTK) solution. After ECP, limbs

Microvascular Shunts, Intracranial Pressure, and the Impact of Drag-Reducing Polymers.

In the 50 years of my membership in ISOTT, I, Edwin M Nemoto, have enjoyed the application of many of the technologies developed in our society including microelectrodes for pH, PO2, and near-infrared spectroscopy (NIRS) in the measurement of tissue oxygenation and metabolism. The greatest joy has been the number of great scientists I have had the pleasure of knowing and exchanging scientific ideas with across the United States, Europe, and Asia. This will be the enduring legacy of ISOTT for me personally as we continue beyond our half-century existence.Every organ in our body, including the t

Monoacylglycerol Lipase Inhibition Using ABX-1431 Attenuates Cerebral Ischaemia Early After Traumatic Brain Injury.

An early event in the pathology of traumatic brain injury (TBI) is a reduction in cerebral blood flow (CBF), which exacerbates secondary injury development and inhibits brain recovery. The endogenous cannabinoid system signalling (eCBs) might be critical in TBI recovery due to modulating synaptic activity and exerting neuroprotective and anti-inflammatory effects. In the brain, eCBs predominantly occur at cannabinoid receptor type 1 via the eCB 2-arachidonoylglycerol (2-AG). The aim of this work was to test the efficacy of potentiating 2-AG signalling by monoacylglycerol lipase (MAGL) inhibiti

Netrin1-Enriched Exosomes From Genetically Modified ADSCs as a Novel Treatment for Diabetic Limb Ischemia.

Diabetic limb ischemia (DLI) is a frequent complication of diabetes and the leading cause of non-traumatic amputation. Traditional treatments like stent placement and bypass surgery may not suit all patients. Exosome transplantation has emerged as a promising therapy. Netrin1, a protective cardiovascular factor, has an unclear role in DLI. This study investigates the role of Netrin1 in DLI patients and evaluates the therapeutic potential of exosomes derived from Netrin1-overexpressing adipose-derived stem cells (N-ADSCs). The expression of Netrin1 is significantly decreased in both endothelial

EFFECTS OF LOCAL HYPOTHERMIA ON LIMB VIABILITY IN A SWINE MODEL OF ACUTE LIMB ISCHEMIA DURING PROLONGED DAMAGE-CONTROL RESUSCITATION.

Background : New strategies are needed to mitigate further tissue injury during traumatic limb ischemia in cases requiring damage control resuscitation (DCR). Little is known about the pathophysiology and injury course in acute limb ischemia (ALI) with DCR in polytraumatized casualties. We therefore investigated the effects of therapeutic limb hypothermia in a swine model of ALI and DCR. Methods : Fifteen swine underwent a published 6-h DCR protocol of hemorrhage and then resuscitation. After hemorrhage, animals were randomized to 5°C or 15°C cooling of one hindlimb; the contralateral limb ser
