Anaphylaxis Following Intravenous Methylprednisolone Sodium Succinate: A Case Report.

BACKGROUND: Immediate hypersensitivity to systemic corticosteroids is rare but potentially life-threatening, and it may be overlooked when the cardiovascular presentation is atypical. Identifying the responsible determinant, whether the corticosteroid molecule, a solubilizing ester, or a formulation excipient, is often difficult. CASE PRESENTATION: A 45-year-old woman with rheumatoid arthritis developed a generalized pruritic, erythematous, raised eruption, dyspnea, hypoxemia, bradycardia, and reduced consciousness within 5 minutes of starting intravenous methylprednisolone sodium succinate 2

Tryptase Locus Variation Contributes to Normal-Range Basal Serum Tryptase Variability and Is Associated With Anaphylaxis.

BACKGROUND: Increased basal serum tryptase level (BST) is associated with anaphylaxis. In the absence of clonal mast cell disease and hereditary α-tryptasemia (HαT), the contribution of the tryptase locus variation to BST and anaphylaxis remains undefined. OBJECTIVE: To determine the impact of tryptase locus variation on BST and anaphylaxis. METHODS: Individuals with systemic Hymenoptera venom allergic reactions, large local reactions and asymptomatic sensitization underwent a novel ddPCR-based tryptase genotyping to quantify active tryptase alleles. Individuals positive for KIT p.D816V or H

Allergic bronchopulmonary mycosis caused by Schizophyllum commune in a patient with severe food allergy and asthma.

Schizophyllum commune is a basidiomycete. Allergic bronchopulmonary mycosis (ABPM) is mainly caused by Aspergillus species; however, that caused by S. commune is occasionally found, especially in Japan. The patient was a 46-year-old female with a history of severe food allergies, asthma, and anaphylaxis. Atelectasis and bronchiectasis were observed in the left lower lobes of the lungs. The patients showed high levels of IgG and IgE against Aspergillus; however, anti-Asp f 1 IgE, Aspergillus antigen in serum, and Aspergillus species in respiratory specimen cultures were not detected. S. commune

Development of novel human monoclonal antibodies for botulinum neurotoxin type F subtype 1.

Different types of Botulinum neurotoxins (BoNTs) cause human botulism. Among them, type F botulism, caused by genetically diverse BoNT/F subtypes, is rare and may be overlooked despite its clinical importance. Toxin-neutralizing human antibodies, which carry the minimum risks of inducing serum sickness or anaphylaxis, are ideal for the treatment of human botulism. In this study, we generated human monoclonal antibodies (mAb) against BoNT/F using trans-chromosomic monoclonal antibody (TC-mAb) mice. A recombinant toxin fragment consisting of the light chain and the heavy chain N-terminal domain

Vespula spp. venom immunotherapy: basophil excitability and FcεRI/FcγRIIb expression.

BACKGROUND: Basophils are recognized as important effector cells during the sensitization and effector phases of IgE-mediated allergy. Besides histamine release during anaphylaxis, basophils contribute to early immune polarization as an important source of Th2-associated cytokines. Vespula spp. venom immunotherapy (VIT) represents a prime example of clinically highly effective allergen-specific immunotherapy. This study investigated functional and phenotypic changes in circulating basophils during early VIT in a homogenous case series. METHODS: Twenty-four patients eligible for VIT were exami

Esculetin mitigates mast cell-mediated allergic inflammation and anaphylaxis by suppression FcεRI signaling.

OBJECTIVE AND DESIGN: Mast cells are critical mediators in the initiation and propagation of allergic inflammation by secreting histamine and pro-inflammatory cytokines. Current treatments, including corticosteroids and antihistamines, provide symptomatic relief but do not directly target FcεRI-mediated mast cell activation. Esculetin, a natural bioactive compound from Fraxinus rhynchophylla Hance with potent antioxidative and anti-inflammatory effects, offers a promising but underexplored strategy for modulating allergic responses. This study investigates the pharmacological potential of escu

Complex mast cell CCL2-chemokine trafficking patterns during local allergic responses.

BACKGROUND: IgE/allergen-activated murine mast cells (MCs) release potent granule-stored mediators by degranulation and secrete newly synthesized chemokines to locally recruit inflammatory cells. Whereas the fusion and trafficking steps during degranulation have been extensively studied, chemokine vesicular trafficking remains poorly understood. OBJECTIVE: We explored the trafficking of C-C motif chemokine ligand 2 (CCL2) in IgE-activated MCs and in a murine model of passive cutaneous anaphylaxis (PCA). METHODS: Bone marrow-derived and peritoneal cell-derived MCs from wild-type and newly est

Anesthetic procedure outcomes after initial perioperative anaphylaxis: A retrospective multicenter US review.

BACKGROUND: Limited information exists regarding outcomes during repeat surgical procedures in patients with perioperative anaphylaxis (POA). Although current guidelines recommend that future procedures be guided by allergy evaluations, little is known about adherence to these recommendations or risk of recurrence. OBJECTIVE: Our aim was to assess the incidence of recurrent POA and evaluate adherence to allergist recommendations during subsequent anesthetic procedures. METHODS: We performed a retrospective review of 100 patients with POA across the Mayo Clinic Health System (2010-2021) to as

A new experimental model of peanut allergy in BALB/c and C57BL/6 mice.

INTRODUCTION: Food allergy protocols are highly variable. While it allows the exploration of multiple scientific questions, it limits the possibility to make general assumptions and point out specific features of allergy to different food allergens. Considering this, our research group has developed a standardized food allergy protocol that has allowed us to study egg, milk and shrimp allergy. METHODOLOGY: In this study, we used the same protocol to develop a model of peanut allergy. BALB/c and C57BL/6 mice were sensitized with a subcutaneous injection of 40mg of peanut protein extract (PPE)

NAPQI adducts in patients with selective hypersensitivity to acetaminophen.

INTRODUCTION: Acetaminophen, a widely used analgesic and antipyretic, can cause adverse reactions ranging from mild urticaria to severe anaphylaxis. While interindividual differences in pharmacokinetics and genetic polymorphisms are known to affect acetaminophen metabolism, the specific mechanisms underlying hypersensitivity reactions (HSRs) remain unclear. METHODS: We evaluated 28 patients with single-NSAID-induced urticaria/angioedema or anaphylaxis, but no other symptoms after acetaminophen intake. All patients demonstrated selective hypersensitivity to acetaminophen while exhibiting confi

Unexpected anaphylaxis to tranexamic acid confirmed by drug provocation testing in orthopedic surgery: A case report.

Tranexamic acid (TXA) is commonly used in orthopedic surgery to reduce perioperative bleeding. Although generally safe, TXA has been associated with rare, but potentially life-threatening anaphylactic reactions. We report a case of severe anaphylaxis following intravenous TXA. A 33-year-old male with no known allergies developed severe bronchospasm, hypotension, ventricular arrhythmias, and lip swelling within min of TXA administration. Initial treatment included oxygen, intratracheal salbutamol, and intravenous pheniramine with delayed epinephrine due to the transient arrhythmias. Serum trypt

Anaphylaxis Following a Mouse Bite in a Patient Sensitized to Mouse, Rat, and Hamster: A Case Report.

We report a case of anaphylaxis in a man in his 30s following a mouse bite while working in an animal research facility. He developed generalized erythema, wheezing, and hypoxemia consistent with grade 2 anaphylaxis, which resolved after treatment with adrenaline, corticosteroids, antihistamines, and bronchodilators. Serum testing revealed specific IgE to mouse, rat, and hamster, despite no direct exposure to the latter two species. The patient had worked as an animal researcher for 15 years, exclusively handling mice. Before this episode, he reported mouse bites occurring every two to three y

HMGB-1 Mediates The Exacerbation of Anaphylactic Shock Under Hyperlipidemic Conditions.

Coronary heart disease (CHD) may worsen anaphylactic shock, but the exact mechanism is unknown. This study aimed to investigate the mechanisms by which coronary heart disease exacerbates anaphylaxis.C57BL/6 (WT) mice and LDLR-/- mice were fed a high-fat diet for 20 weeks to develop atherosclerosis. Anaphylaxis was then induced using ovalbumin (OVA). Compared to WT mice, LDLR-/- mice showed lower body temperature, worse pulmonary edema and higher mortality. Pulmonary endothelial cell CD31 (PECAM-1) expression decreased, but serum HMGB-1 levels increased. In vitro experiments found that ox-LDL e

Peanut Allergy and the Basophil Activation Test in the Military Healthcare System: A Case Series of Monozygotic Twins.

Based on current standards for accession into the U.S. military, food allergy represents a significant barrier to military service. Skin prick and serum specific IgE testing exhibit high sensitivity but low positive predictive value, often leading to overdiagnosis when a confirmatory oral food challenge (OFC) is subsequently avoided because of concern for anaphylaxis in the clinic. Basophil activation testing (BAT) offers a unique advantage by measuring in vitro basophil responses to peanut allergen and can provide superior risk stratification for safe OFCs. We report the first clinical applic
