Vascular and Wound-Healing Implications of Single-Stage Versus Multistage Repair of Anorectal Malformations: A Systematic Review and Meta-Analysis.

BACKGROUND: Anorectal malformations (ARMs) are managed using either single-stage or multistage surgical repair. However, concerns persist regarding wound-related complications and long-term functional outcomes, which remain inconsistently reported across surgical approaches. OBJECTIVE: To compare wound-related complications and long-term functional outcomes following single-stage versus multistage repair of ARMs, with emphasis on wound perfusion, dehiscence, and vascular healing dynamics. METHODS: A systematic review and meta-analysis were conducted in accordance with the Preferred Reporting

Predictive Factors of Cranioplasty Infection: Results From a Contemporary Level I Trauma Center Cohort.

Cranioplasty restores skull integrity after decompressive craniectomy but carries a risk of surgical site infection that may necessitate implant removal and prolonged antimicrobial therapy. We performed a retrospective study of adult patients who underwent cranioplasty between 2014 and 2022 at a single level 1 trauma center to identify predictors of postoperative infection. Demographic, clinical, and operative variables were abstracted from the electronic medical record. The primary outcome was surgical site infection. Associations were assessed using univariate testing, multivariable logistic

Sternal Re-Closure for Refractory Dehiscence Using Ultra-High Molecular Weight Polyethylene Tape.

Ultra-high molecular weight polyethylene tape has been introduced as an alternative to stainless-steel wires for primary sternal closure, yet its efficacy in secondary reconstruction has not been reported. We present a 67-year-old male with refractory sternal dehiscence following coronary artery bypass grafting. Despite 2 failed reconstruction attempts using conventional wires and the Robicsek technique, stable fixation was finally achieved by combining ultra-high molecular weight polyethylene tape with mesh plates. This case highlights the potential utility of ultra-high molecular weight poly

Major wound complications after conventional preoperative radiotherapy in STS patients: incidence, predictors, and timing.

In 2002, a major wound complication rate (MWC) of 35% was reported in the Canadian SR-2 trial after preoperative radiotherapy (RT) in soft-tissue sarcomas (STS). Recent literature reports a lower incidence of MWC. This study aims to evaluate the incidence, potential predictors, and duration of MWC over the last two decades. Data was collected from patients treated between October 2003 and October 2024, in curative setting, in a regimen of 25 fractions of 2 Gy, followed by surgery. Data was collected until 120 days post-surgery. In total, 164 patients were analyzed. The median interval between

Effectiveness of closed incisional negative pressure wound therapy in pediatric spinal dysraphism: a single-center retrospective case series.

INTRODUCTION: Postoperative wound complications - including surgical site infection (SSI), cerebrospinal fluid (CSF) leakage, and wound dehiscence - remain a significant challenge following surgery for spinal dysraphism, particularly in neonates and infants. Closed incisional negative pressure wound therapy (ciNPWT) has demonstrated benefit in several surgical subspecialties; however, evidence regarding its routine use in pediatric spinal dysraphism surgery is limited. METHODS: A retrospective review was conducted of all pediatric patients (age < 18 years) who underwent surgery for spinal dys

Nicotine abuse and 30-day perioperative outcomes after fibula free flap reconstruction: An ACS-NSQIP study.

BACKGROUND: Fibula free flap (FFF) reconstruction is a cornerstone of maxillofacial and mandibular reconstruction, particularly for oncologic and traumatic defects. Although smoking is a well-established risk factor for impaired wound healing and microvascular complications, its specific impact on FFF outcomes remains debated. In this multicenter retrospective study, we evaluate the association between smoking and 30-day perioperative outcomes in FFF reconstruction. METHODS: A retrospective cohort study was conducted using data from the American College of Surgeons National Surgical Quality I

Elevated risk of medical complications in sickle cell disease patients after total shoulder arthroplasty.

BACKGROUND: Sickle cell disease (SCD) predisposes patients to vaso-occlusive and inflammatory complications that may be exacerbated by surgical stress. While SCD has been linked to adverse outcomes after lower-extremity arthroplasty, its effect on post-operative complications following total shoulder arthroplasty (TSA) is poorly understood, particularly independent of avascular necrosis. This study evaluates the association between SCD and post-operative outcomes after TSA using a national database. METHODS: A retrospective cohort study was conducted using the PearlDiver database. Patients wi

Early outcomes of volume replacement using paste-type micronized acellular dermal matrix in oncoplastic breast-conserving surgery.

BACKGROUND: Surgical approaches in breast cancer have evolved to balance oncologic safety with cosmetic outcomes. Oncoplastic breast-conserving surgery (BCS) was developed to overcome the cosmetic limitations of conventional BCS and has been widely adopted in breast cancer treatment. Although the concept of oncoplastic BCS has been established for decades, breast and plastic surgeons continue to refine surgical techniques to achieve oncologic outcomes that are not inferior while improving aesthetic results. Therefore, this study aimed to evaluate the early postoperative outcomes and safety of

Investigating the Role of Fish Skin Grafts in Treating Complex Posterior Spinal Wounds.

BACKGROUND: Complex wound management in spinal surgery poses significant challenges, especially in high-risk patients. Wound dehiscence, infection, and posterior hardware complicate healing. Intact fish skin grafts (IFSGs), derived from North Atlantic cod, have shown promise in reinforcing soft tissue due to their structural similarity to the human extracellular matrix, retention of polyunsaturated fatty acids, and antimicrobial properties. This study evaluates IFSG in reducing wound complications and facilitating healing in complex posterior spinal revisions and high-risk patients. METHODS:

Use of Rotterdam Risk Index in the Assessment of Risk Factors in Abdominal Wound Dehiscence.

INTRODUCTION: Abdominal wound dehiscence (AWD) is a serious postoperative complication characterized by partial or complete disruption of a surgical incision, often leading to increased morbidity, prolonged hospitalization, and higher healthcare burden. Its occurrence is multifactorial, involving patient-related, surgical, and postoperative factors. Risk assessment tools such as the Rotterdam Risk Index (RRI) have been used to evaluate the likelihood of AWD; however, as the index includes postoperative variables, it reflects post-hoc risk stratification rather than purely preoperative predicti

Exploring the impacts of docetaxel and paclitaxel based neoadjuvant therapies on early postoperative complications in HER2-positive breast cancer patients.

BACKGROUND: HER2-positive breast cancer accounts for 15-20% of all breast cancer cases. Anti-HER2-based neoadjuvant therapy combined with taxanes, particularly docetaxel and paclitaxel, constitutes the standard treatment approach. Although these regimens are widely used, most studies have focused on systemic adverse effects, with limited data addressing their impact on postoperative surgical complications. This study aimed to evaluate the association between docetaxel- and paclitaxel-based neoadjuvant regimens and early wound complications in HER2-positive breast cancer patients. METHOD: We r
