Chest Port Placement in Freestanding Outpatient Vascular Access Centers.

Background Ports have traditionally been inserted in hospitalized inpatients; however, there has been an increasing transition to outpatient placement by interventionalists in hospital imaging suites. To our knowledge, port implantation in nonhospital settings has not been reported in peer-reviewed literature. Here, we report our experience with port placement in freestanding outpatient vascular centers. Methodology The electronic medical record for 47 centers was retrospectively searched to identify port placements between January 1, 2012, and December 31, 2018. Data included indications, pla

Association of Diagnostic Code Priority With Identification of True Cardiac Arrest in Patients Presenting to the Emergency Department.

BACKGROUND: Sudden cardiac arrest (SCA), a leading cause of mortality, is administratively coded as ventricular tachycardia (VT), ventricular fibrillation (VF), or unspecified SCA. Diagnostic codes are widely used in research, yet their accuracy in reflecting true SCA events is uncertain. The objective of this study is to evaluate the relationship between administrative diagnostic code rank (listing order) and the likelihood of true SCA in patients presenting to the emergency department (ED). METHODS: From a database of 22 369 patients with VT/VF/SCA, we randomly selected 380 patients for det

A Comparative Analysis of Opioid Overdose Hospitalizations Pre- and Post-Pandemic: Insights from the National Inpatient Sample Database.

BACKGROUND: The COVID-19 pandemic disrupted healthcare systems, affecting opioid overdose-related hospitalizations. National data examining how demographic and clinical outcomes evolved during this period, particularly by overdose intent, remain limited. METHODS: Using the National Inpatient Sample (2018-2021), we identified adult hospitalizations for opioid overdose using ICD-10 codes and categorized cases as intentional, unintentional, or unspecified. Patients were grouped into pre-COVID (2018-2019) and post-COVID (2020-2021) cohorts. Multivariable logistic and linear regression models adju

Perioperative visual loss after spine surgery: a systematic review of risk factors and report of a reversible case.

BACKGROUND: Perioperative visual loss (POVL) is a rare but devastating complication of spinal surgery, most commonly associated with posterior ischemic optic neuropathy (PION) and central retinal artery occlusion (CRAO). Its pathophysiology remains multifactorial and incompletely understood. METHODS: A systematic review of the English-language literature was conducted using MEDLINE/PubMed and Scopus according to PRISMA guidelines. Studies reporting POVL after spinal surgery were included if they provided sufficient clinical, surgical, intraoperative, or outcome data. An illustrative reversibl

GLP-1 Receptor Agonists and Risk of Age-Related Macular Degeneration in Older Adults With Obesity: A Target Trial Emulation.

AIMS: Age-related macular degeneration (AMD) is a leading cause of irreversible vision loss in older adults. Whilst glucagon-like peptide-1 receptor agonists (GLP-1RAs) are increasingly prescribed for obesity management, their potential impact on AMD risk in individuals without diabetes remains unclear. This study aimed to evaluate the association between GLP-1RA initiation and the risk of incident AMD in obese adults without diabetes. MATERIALS AND METHODS: We conducted a target trial emulation using the TriNetX global health research network (January 2014-June 2025). Eligible participants w

A Scoping Review of Dietary Management for Nondiabetic Gastroparesis: Evidence Limitations and Research Gaps.

BACKGROUND: Dietary management is essential in gastroparesis, but existing guidelines primarily focus on diabetic gastroparesis, highlighting a lack of evidence-based guidelines for patients with nondiabetic gastroparesis, who experience higher malnutrition and mortality rates. Management is complicated by a suggested bidirectional relationship with eating disorders. OBJECTIVE: To investigate evidence on dietary interventions, patterns, and intake in nondiabetic gastroparesis, including effects on symptoms, nutritional outcomes, quality of life (QoL), and their potential role in disordered ea

Inflammatory Bowel Disease and Risk of Periprosthetic Joint Infection After Total Joint Arthroplasty: A Systematic Review of the Current Literature.

BACKGROUND: Periprosthetic joint infection (PJI) is a severe complication after total joint arthroplasty (TJA). Inflammatory bowel disease (IBD) may plausibly increase this risk because of chronic systemic inflammation, intestinal barrier dysfunction, malnutrition, and exposure to immunosuppressive therapies; however, the available orthopedic literature is sparse and heterogeneous. METHODS: This systematic review was revised in accordance with PRISMA 2020 principles and the PRISMA 2020 for Abstracts recommendations. The primary review question was whether patients with IBD undergoing total hi

Psychosocial stress and risk of first-ever stroke: a systematic review and meta-analysis.

BACKGROUND: Psychosocial stress has been recognized as a potential risk factor for stroke; however, the strength and consistency of its association with first-ever stroke remain uncertain. This systematic review and meta-analysis aimed to determine the overall association between psychosocial stress and the risk of first stroke in a stroke-free population. METHODS: We conducted a systematic review and meta-analysis in accordance with the PRISMA and MOOSE guidelines. PubMed, Scopus, Web of Science, and Ovid were searched from inception to 1 January 2026 for cohort or case-control studies. Elig

Evaluating alloimmunization risk in patients with sickle cell disease and in prenatal care patients with RHCE variants.

BACKGROUND AND OBJECTIVES: Clinically significant alloantibodies complicate transfusion and prenatal care, especially in individuals with genetic variants affecting high-frequency antigens. Many patients from African descent carry RHCE*ceVS alleles, which alter the expression of c (RH4), e (RH5) and hrB (RH31). However, the risk and clinical impact of alloimmunization remain uncertain. We evaluated the alloimmunization in a cohort of patients with a RHCE*ceVS genotype. MATERIALS AND METHODS: We conducted a retrospective study on 48 patients with a RHCE*ceVS genotype divided into three categor

Complications and outcomes after supracerebellar transtentorial approach for mediobasal temporal lobe intra-axial tumors: A systematic review and meta-analysis.

Mediobasal temporal lobe (MBTL) intra-axial tumors present a surgical challenge due to their location and proximity to eloquent structures. Surgical resection remains the first-line management of these tumors, especially when they are associated with mass effect and intractable symptoms. The supracerebellar transtentorial (SCTT) approach provides direct access to these tumors via a posterior transtentorial route; however, data regarding its complications and outcomes remain largely limited in the literature. The authors searched several databases for studies that reported clinical outcomes and

Epilepsy following acquired brain injury: A systematic review and meta-analysis.

BACKGROUND: Acquired brain injuries (ABIs) are among the most common causes of neurological morbidity worldwide and are increasingly recognized as important factors in the pathogenesis of epilepsy; however, existing literature on the relationship between different ABI types and subsequent epilepsy remains inconsistent and unclear. As such, this review sought to evaluate the association between four major ABI types-traumatic brain injury (TBI), hemorrhagic brain injury (HBI), ischemic brain injury (IBI), and immune-related brain injury (IrBI)-and subsequent risk of epilepsy. METHODS: A compreh

Postoperative Outcomes and Risk Profiles in Dual Liver-Lung Transplantation: A Single-Center Retrospective Analysis.

BACKGROUND: Dual liver-lung transplantation (DLLT) is an uncommon but definitive therapy for carefully selected patients with concurrent end-stage hepatic and pulmonary disease. The combined operative complexity and dual-organ immunosuppressive burden may predispose recipients to early morbidity and graft-threatening complications [1-4]. OBJECTIVE: To characterize early and late postoperative events (including acute cellular rejection (ACR), infectious complications, malignancy, and hospital readmissions) after DLLT and to describe associated clinical patterns. METHODS: We performed a retros

Trends in perioperative systemic chemotherapy for resected colorectal liver metastases: A Surveillance, Epidemiology, and End Results-Medicare analysis.

BACKGROUND: The optimal treatment strategy for patients undergoing curative-intent resection of colorectal liver metastases remains uncertain. While perioperative and adjuvant chemotherapy are widely used, randomized trials have not demonstrated a consistent overall survival benefit, and guidelines continue to endorse approaches ranging from surgery alone to multimodal therapy. Randomized studies evaluating perioperative or adjuvant chemotherapy-along with trials incorporating biologic therapy-have shown improvements in disease control but inconsistent overall survival benefit. We evaluated na

Vitamin D for preventing acute respiratory infections in children up to five years of age.

RATIONALE: Acute respiratory infections (ARIs) are a leading cause of morbidity and mortality in children under five years of age and contribute to healthcare visits and hospitalisations globally. Vitamin D deficiency is common amongst pregnant women and young children. Given that vitamin D supplementation is safe, affordable, and easy to administer, its potential to reduce ARI-related healthcare visits holds substantial public health relevance. This review evaluates the benefit and harm of vitamin D supplementation in preventing ARI-related healthcare visits in young children, aiming to infor

Fistulotomy for superficial or minimal sphincter-involving fistulae in perianal Crohn's disease: do they heal?

BACKGROUND: Fistulotomy is highly effective (> 90%) for cryptoglandular fistula-on-ano, but fistulotomy in perianal Crohn's disease (pCD) is limited due to increased risk of recurrent fistulae, diarrhea, and fecal incontinence. We hypothesized that superficial fistulotomy resulted in wound healing in most patients. METHOD: We conducted a single-center retrospective review of adult patients with pCD who underwent fistulotomy from 1999 to 2022. Baseline characteristics, pCD characteristics, and short- and long-term surgical and functional outcomes were reported. Matched-pair Wilcoxon signed-ran
