Computational modeling of thrombosis after transcatheter aortic valve implantation: A systematic review of hemodynamic and mechanical risk metrics.

BACKGROUND AND OBJECTIVE: Although transcatheter aortic valve implantation (TAVI) generally provides favorable procedural outcomes, thrombotic complications remain an important concern for the long-term performance and durability of transcatheter heart valves. A subset of patients develops hypo-attenuated leaflet thickening (HALT), reduced leaflet mobility, or late thrombotic complications, often in the absence of overt clinical manifestations. This has motivated the use of computational modeling to investigate post-TAVI hemodynamics and valve mechanics and to identify flow- and device-related

Complex infective endocarditis with perivalvular abscess and fistulation: two high-risk cases requiring extensive reconstruction and valve replacement.

BACKGROUND: Infective endocarditis (IE) complicated by aortic root abscess, fistulation and multivalve involvement is rare but carries high mortality. Fistulation from the left ventricular outflow tract (LVOT) to the right atrium (RA) represents an exceptionally uncommon sequela due to extensive perivalvular tissue destruction. Early surgical intervention remains crucial, but postoperative outcomes vary widely depending on host factors, organism control and the feasibility of durable reconstruction. CASE PRESENTATION: We report two cases of severe aortic valve endocarditis with aortic root ab

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Vascular access: Femoral, radial and large-bore alternative access.

Vascular access for coronary, peripheral, and structural interventions has changed significantly over the past two decades. The evolving demand for both large-bore access for valvular interventions and mechanical support devices, and for safer access for coronary interventions, in patients with comorbidities have driven progress in these areas. This review will provide an overview of the techniques of arterial access in the femoral, forearm (radial and ulnar), and alternative (transcarotid, transaxillary, and transcaval) locations based on the latest evidence and experience.

Analysis of the haemodynamic changes caused by surgical and transcatheter aortic valve replacements by means fluid-structure interaction simulations.

Aortic valve replacements, both surgical and transcatheter, are nowadays widely employed treatments. Although clinically effective, these procedures are correlated with potentially severe clinical complications which can be associated with the non-physiological haemodynamics that they establish. In this work, the fluid dynamics changes produced by surgical and transcatheter aortic valve replacements are analysed and compared with an ideal healthy native valve configuration, employing advanced fluid-structure interaction (FSI) simulations. The aim of the study is to investigate how existing tre

ECLS-SHOCK and DanGer Shock: Implications for Optimal Temporary Mechanical Circulatory Support Use for Cardiogenic Shock Due to Acute Myocardial Infarction.

Despite concerted efforts to rapidly identify patients with cardiogenic shock complicating acute myocardial infarction (AMI-CS) and provide timely revascularization, early mortality remains stubbornly high. Although artificially augmenting systemic flow by using temporary mechanical circulatory support (tMCS) devices would be expected to reduce the rate of progression to multiorgan dysfunction and thereby enhance survival, reliable evidence for benefit has remained elusive with lingering questions regarding the appropriate selection of both patients and devices, as well as the timing of device

A Beating Heart is a Happy Heart, Especially in Patients with Left Ventricular Dysfunction Undergoing Coronary Artery Bypass Grafting.

Beating-heart coronary artery bypass grafting (CABG) in patients with left ventricular (LV) dysfunction can provide the best of all words by limiting myocardial injury purported by cardioplegic arrest. Complete revascularization is possible and graft numbers are not different when compared to arrested heart CABG. Furthermore, beating-heart CABG more often reduces the need for intraoperative and postoperative mechanical support reducing the complications and costs associated with these devices.

Long-term mechanical circulatory support in cardiogenic shock secondary to acromegaly. Case report.

Heart failure is a complication that may develop in patients diagnosed with acromegaly. This complication can progress to cardiogenic shock, which in cases like the one described, may be refractory to optimal medical management, necessitating the use of mechanical circulatory support as a bridge to decision. Given the specific morphology of this patient's heart, the likelihood of finding a suitable donor in our environment was very low. Therefore, the decision was made to utilize long-term ventricular assistance, which emerges as a viable option in our context.

Patient Characteristics, Management and Long-Term Outcomes of Patients With Cardiogenic Shock at a Large Safety Net Hospital.

Data regarding cardiogenic shock (CS) from safety-net hospitals serving socioeconomically-disadvantaged patients are limited. In addition, little is known regarding long-term outcomes and management of heart failure-related CS (HF-CS), a population potentially especially vulnerable to adverse social determinants of health (SDOH). A single-center retrospective cohort study of patients with Stage C, D, or E CS at a public safety-net hospital between 2017 and 2023 was performed. Management and outcomes were compared between patients with HF-CS and myocardial infarction-CS (AMI-CS). The primary ou

Angulated titanium bases screw channel abutments for single implant restorations: A systematic review and meta-analysis.

PURPOSE: Screw-retained implant crowns are a common solution to rehabilitate single or multiple edentulous sites. Angulated screw channel (ASC) abutments have been developed as a new system of connection to correct the angulation of the implant and avoid a possible undesired emergence of the access hole. The purpose of this systematic review was to examine available literature related to the effects of ASC abutments in rehabilitating single implant crowns. METHODS: A protocol was registered in the PROSPERO database (ID = CRD42022295521). An electronic search of the Cochrane Central Register o

Comparative analysis of the DanGer Shock trial to randomized cardiogenic shock trials and real-world registries.

BACKGROUND: The Danish-German Cardiogenic (DanGer) Shock Trial reported lower mortality with a percutaneous micro-axial flow pump compared to standard care in patients with STEMI-related cardiogenic shock. It remains unclear how the DanGer Shock trial population compares to randomized controlled trials (RCTs) and real-world registries studying temporary mechanical circulatory support (tMCS) for acute myocardial infarction with cardiogenic shock (AMICS). METHODS: A systematic review and meta-analysis of RCTs and registries involving tMCS for AMICS was performed. Patient characteristics and out

The Role of a Cardiogenic Shock Team in Decision Making Surrounding Mechanical Circulatory Support.

Cardiogenic shock (CS) confers high mortality rates and remains a challenge for cardiovascular specialists. The difficulty in treating CS lies in its complexity, phenotypic heterogeneity, and the need for expedient treatment. Emerging evidence suggests that cardiogenic shock teams (CS teams), consisting of multiple specialists working in tandem with set protocols and care pathways to offer standardized team-based care, may reduce mortality and morbidity in patients with CS. A key reason for improved outcomes may be the team's decisions surrounding the use of temporary mechanical support device

Precise monitoring of transpulmonic resistance in bridge-to-transplant patients supported by the Aeson total artificial heart.

The Aeson total artificial heart (TAH) features embedded pressure sensors for flow autoregulation, which can also be used to estimate pressure gradients. In this bridge-to-transplant study, right and left ventricular pressure data and pump output from the TAH were analyzed to estimate transpulmonic resistance (eTPR) every 30 days. Three patients (aged 60, 35, and 54) with preimplant pulmonary vascular resistance of 7.0, 3.2, and 7.1 Wood Units, respectively, were supported by the TAH at pump outputs of 4.5 to 6.5 liter/min. All showed significant eTPR reductions (1.4-1.5 Wood Units) and receiv

ECLS-SHOCK Trial.

ECLS-SHOCK is the largest randomized control trial on Veno-Arterial Extracorporeal Membrane Oxygenation (VA-ECMO) in Acute Myocardial Infarction-related Cardiogenic Shock (AMI-CS). Unfortunately, ECLS-SHOCK failed to demonstrate a reduction of mortality in AMI-CS with VA-ECMO. Interpretation of these findings must account for frequent crossover to VA-ECMO or other temporary mechanical circulatory support (tMCS) devices in the control group, as well as the exceptionally high severity of illness among participants. We detail here point by point what could explain the outcomes and the unanswered
