Follow-up of Australian aboriginal patients following open-heart surgery in Western Australia.

BACKGROUND: To date there has been only one published report pertaining to the outcomes following open-heart surgery in Australian aboriginal patients. METHODS: The records of 57 consecutive aboriginal patients who underwent open-heart surgery at our institution over a 6-year period were retrospectively reviewed. Attempts were made to contact the patient by telephone, by letter, through their listed next of kin, general practitioners, community nurse or through the aboriginal liaison officer. RESULTS: There were 38 males and 19 females. Forty-five patients had coronary artery bypass grafting

The influence of coronary artery disease on quality of life after mechanical valve replacement.

BACKGROUND AND AIM OF THE STUDY: Coronary artery disease (CAD) is known to impact negatively on long-term survival following valve replacement (VR). However, its influence on quality of life (QOL) remains undefined in patients with mechanical VR. METHODS: A total of 318 consecutive patients undergoing VR with the St. Jude Medical (SJM) mechanical valve were matched for age and gender with 318 patients who had VR (SJM valve) and coronary artery bypass grafting (VR+CABG). The VR group comprised 197 men and 121 women; the VR+CABG group also comprised 197 men and 121 women. The mean age of all pa

Biological vs. mechanical aortic root replacement.

OBJECTIVES: Although age and co-existing coronary disease are major determining factors when deciding valve choice (mechanical vs. biological) in simple aortic valve replacement, no studies have documented selection criterion for biological (BIO) vs. mechanical (MECH) aortic root prosthesis. METHODS: Two hundred and twenty-one consecutive patients underwent elective aortic root replacement with either BIO (homograft, n=111, Freestyle, n=25) or MECH composite grafts (n=85). Median age in BIO was 53 years and in MECH 54 years (P=NS). Groups were similar in gender, NYHA class and ejection fracti

Influence of PMEA-coated bypass circuits on perioperative inflammatory response.

BACKGROUND: Poly(2-methoxyethylacrylate) (PMEA) is a new coating material, and several experimental studies have revealed excellent biocompatibility of PMEA-coated cardiopulmonary bypass circuits. The clinical utility of the PMEA-coated circuits was compared with that of uncoated circuits, focusing on perioperative inflammatory response. METHODS: Twenty-two patients were randomized to PMEA-coated (group P; Capiox RX25; n = 11) or uncoated (group U; Capiox SX10; n = 11) circuit group, and underwent coronary artery bypass grafting and/or valve operations. The following markers, as well as clini

Surgical methods to reverse left ventricular remodeling in congestive heart failure.

In this review article, we describe the most common surgical procedures currently used to reverse or arrest remodeling of the left ventricle in patients with congestive heart failure (CHF). The selection of the appropriate operation in a patient is a complex decision-making process, rigorously based on pathophysiologic considerations. In this population, all factors affecting the surgical risk should be carefully evaluated preoperatively, and surgery should be recommended when definite benefits in survival and quality of life can be reasonably predicted. Quite often, patients with CHF require

Reoperation after fresh homograft replacement: 23 years' experience with 655 patients.

OBJECTIVE: Through a retrospective study on the use of fresh homografts in 655 aortic valve replacement patients over a period of 23 years, we aimed to assess the reasons for eventual reoperation and causes of valve dysfunction. METHODS: Between January 1980 and December 2002, 655 patients received fresh homografts. All homografts were antibiotic sterilized and stored at 4 degrees C. During this time, 139 patients (116 male and 23 female) with a mean age of 46.7 years (range 18-72) required reoperation. RESULTS: The 30-day hospital overall mortality was 2.87%. The mean durability for all hom

The use of a rigid instrument prior to stentless valve implantation facilitates correct alignment of the prosthesis and native aortic valve apparatus.

BACKGROUND AND AIM OF THE STUDY: When correctly seated in the aortic position, stentless valves are associated with a relatively low transvalvular gradient. This leads to regression of left ventricular hypertrophy and long-term survival. The alleged complexity of stentless valve implantation has led to a preference for stented/mechanical prostheses. Here, the use of a rigid instrument to prevent technical complications after stentless valve implantation was investigated. METHODS: Aortic valve replacement was performed in 75 patients (44 males, 31 females; mean age 72 years; range: 33-89 years

The clamshell approach for the treatment of extensive thoracic aortic disease.

OBJECTIVE: Management of extensive thoracic aortic disease may present an immense technical challenge. The choice of surgical access and subsequent exposure determines whether a single-stage or a 2-stage approach can be adopted. METHODS: Fifteen patients with extensive thoracic aortic disease underwent resection of the ascending aorta, the aortic arch, and varying segments of the descending aorta. Four patients had concomitant coronary artery bypass grafting and 3 patients had aortic valve reconstruction. All patients were treated with a single-stage approach via a bilateral anterior thoracos

Concomitant CABG-procedures in elderly patients undergoing aortic valve replacement. An additional risk factor?

OBJECTIVE: Preoperative coronary angiography in elderly people referred to the hospital for aortic valve replacement (AVR) often shows additional significant stenoses of the coronary arteries (CAD). The benefit of concomitant coronary artery bypass grafting (CABG) in these patients is still discussed controversially. By some authors, an isolated AVR in elderly patients with additional CAD is even described to have a better outcome. PATIENTS AND METHODS: We analyzed 283 patients (> or =75 years), undergoing AVR with or without concomitant CABG-procedures. We particularly analyzed those patient

Mortality and morbidity after total arch replacement using a branched arch graft with selective antegrade cerebral perfusion.

BACKGROUND: The early outcome after aortic arch surgery has improved. However, some operative survivors have died as a result of postoperative problems soon after discharge. This study determines the factors affecting mortality within 1 year of total arch replacement. METHODS: Between July 1993 and November 2001, 103 patients (mean age 65 +/- 11 years, 26 women, 35 dissections) underwent total arch replacement through a median sternotomy using a branched arch graft with selective cerebral perfusion. Eighteen operations including 14 acute dissections were performed on an emergency basis. Conco

Noncardioplegic surgery for ischemic mitral regurgitation.

Twenty-seven consecutive patients underwent surgery for ischemic mitral regurgitation (MR): papillary muscle rupture (1), papillary muscle dysfunction (11) and annular dilatation (15). The grade of MR was moderate or severe, and the ejection fraction (EF) was less than 30% in 8 patients (mean, 43%). Three cases were reoperation and 3 were emergencies. Under ventricular fibrillation (VF) and intermittent aortic cross-clamping at moderate hypothermia, coronary artery bypass grafting (CABG) was performed first, followed by the mitral procedure through a right-sided left atriotomy (repair 21, repl

Cardiac operations for patients with chronic liver disease.

BACKGROUND: Due to the systemic and hepatic effects of cardiopulmonary bypass (CPB), open-heart surgery for patients with chronic liver disease is associated with high mortality and morbidity. In this retrospective study, we present our results of cardiac surgery on patients with non-cardiac cirrhosis. METHODS: Between March 1996 and April 2000, 10 patients with chronic liver disease had open-heart surgery in our institution. Six patients were male and four were female, with a mean age of 57.1 +/- 6.85 years. Preoperative severity of liver disease was determined according to Child classificat

[Surgery in advanced heart failure].

BACKGROUND: ACE inhibitors and beta blockers have reduced morbidity and mortality from chronic heart failure significantly during the last years. Apart from medical therapy several surgical strategies for the treatment of heart failure are available. PREREQUISITES OF SURGICAL TREATMENT: Surgical treatment should be directed towards the underlying etiology and mechanisms. To decide for a special surgical therapy indications and contraindications must be considered. SURGICAL PROCEDURES: The surgical therapy of heart failure can be performed with or without replacement of the heart. Especially

Surgical anterior ventricular endocardial restoration (SAVER) in the dilated remodeled ventricle after anterior myocardial infarction. RESTORE group. Reconstructive Endoventricular Surgery, returning Torsion Original Radius Elliptical Shape to the LV.

OBJECTIVES: The goal of this study was to evaluate the safety and efficacy of surgical anterior ventricular endocardial restoration (SAVER). The procedure excludes noncontracting segments in the dilated remodeled ventricle after anterior myocardial infarction. BACKGROUND: Anterior infarction leads to change in ventricular shape and volume. In the absence of reperfusion, dyskinesia develops. Reperfusion by thrombolysis or angioplasty leads to akinesia. Both lead to congestive heart failure by dysfunction of the remote muscle. The akinetic heart rarely undergoes surgical repair. METHODS: A new
