Redo surgical aortic valve replacement: An outdated technique in the era of valve-in-valve procedures?

BACKGROUND: The treatment of degenerative prosthetic aortic valves is increasingly important. However, redo surgical aortic valve replacement (Re-SAVR) carries higher perioperative risks than primary surgical aortic valve replacement. AIM: This study aims to identify predictors of early morbidity and death after Re-SAVR. METHODS: A retrospective analysis of 220 patients scheduled for elective Re-SAVR between 2009 and 2017 was conducted. Patients were divided into isolated (n=87) and combined (n=133) redo procedures. The primary endpoint was in-hospital death, and secondary endpoints were pos

Predicting Major Adverse Cardiovascular Events After Cardiac Surgery Using Combined Clinical, Laboratory, and Echocardiographic Parameters: A Machine Learning Approach.

Background and Objectives: Despite significant advances in surgical techniques and perioperative care, major adverse cardiovascular events (MACE) remain a leading cause of postoperative morbidity and mortality in patients undergoing coronary artery bypass grafting and/or aortic valve replacement. Accurate preoperative risk stratification is essential yet often limited by models that overlook atrial mechanics and underutilized biomarkers. Materials and Methods: This study aimed to develop an interpretable machine learning model for predicting perioperative MACE by integrating clinical, biochemi

Extracorporeal membrane oxygenation-supported one-stop surgery for transcatheter aortic valve replacement and percutaneous coronary intervention in high-risk complex coronary artery and aortic valvular disease: a case report.

Aortic stenosis (AS) is frequently associated with coronary artery disease (CAD), particularly in high-risk patients for whom treatment strategies lack standardized clinical guidelines. We report a case of a patient with severe AS and high-risk CAD, leading to significant heart failure, for whom surgical aortic valve replacement and coronary artery bypass grafting were unsuitable because of very high risk of morbidity and mortality. The patient underwent a one-stop procedure combining extracorporeal membrane oxygenation (ECMO)-assisted transcatheter aortic valve replacement (TAVR) and percutan

Integrative Evaluation of Atrial Function and Electromechanical Coupling as Predictors of Postoperative Atrial Fibrillation.

Background and Objectives: Postoperative atrial fibrillation (POAF) remains one of the most frequent complications after cardiac surgery, increasing the risk of morbidity, prolonged hospitalization, and adverse long-term outcomes. Although several clinical and echocardiographic factors have been associated with POAF, the integrated contribution of atrial conduction delay, biatrial mechanics, and atrioventricular coupling to arrhythmogenesis remains unclear. Materials and Methods: This retrospective study included 131 adult patients undergoing coronary artery bypass grafting and/or aortic valve

"I had a knot in my heart": unusual complication after implantation of a Swan-Ganz catheter.

BACKGROUND: The implantation of a Swan-Ganz catheter for invasive hemodynamic monitoring is an established measure after cardiac surgery. A rare but serious complication is the formation of a knot in the heart, which can be diagnostically challenging. We report on a patient who developed left heart failure postoperatively after quadruple bypass surgery combined with aortic valve replacement and in whom a knot formed inadvertently during monitoring using a Swan-Ganz catheter. CASE PRESENTATION: An 82-year-old female underwent combined quadruple coronary artery bypass grafting and aortic valve

Early intensivist driven analgesic intervention in ICU vs. surgeon driven analgesia in fast-track post-sternotomy patients in private community cardiac surgery center.

OBJECTIVE: We performed retrospective study to determine whether early management of analgesia by intensivists reduced opiate utilization and improved patient outcomes at our institution. METHODS: Consecutive patients undergoing primary sternotomy for coronary artery bypass graft, valve were studied. Those who underwent aortic dissection patients, non-sternotomy (e.g. thoracotomy), mechanical ventilation longer than 24 h, history of drug addiction, re-do sternotomy, or that required take back to surgery for anything during the index hospitalization were excluded from study. Fluoroscopy-free Impella repositioning after coronary artery bypass grafting: case report of a novel echocardiographic-guided technique.

BACKGROUND: Cardiogenic shock (CS) complicates 5-10% of myocardial infarctions and is a major cause of morbidity and mortality. Temporary mechanical circulatory support (MCS), such as the Impella CP device, has become crucial for stabilizing patients with severe left ventricular dysfunction. However, device positioning remains challenging, particularly when complications arise, such as the need for repositioning or replacement. This case report presents an innovative approach to fluoroscopy-free Impella repositioning using transesophageal echocardiography (TEE) guidance, which has not been pre

Repair Versus Replacement in Mitral Valve Papillary Muscle Rupture: A Multicenter Study.

OBJECTIVES: Papillary muscle rupture (PMR) is a rare but potentially fatal mechanical complication after acute myocardial infarction (AMI). Although surgery is considered the gold-standard treatment for post-AMI PMR, the optimal surgical strategy remains unclear. METHODS: Data from post-AMI PMR patients submitted to mitral valve replacement (MVR) or mitral valve repair (MVr) surgery in the period between 2001 and 2019, from 20 international centres, were collected in the CAUTION study database. In-hospital and long-term post-discharge mortality were the endpoints. A multivariable logistic reg

Anterolateral Papillary Muscle Rupture: A Rare Complication of Myocardial Infarction.

BACKGROUND: Myocardial infarctions can result in mechanical complications such as rupture of papillary muscles. Rupture of the anterolateral papillary muscle is rarer because of its dual blood supply. CASE SUMMARY: A 44-year-old man presented with subacute chest pain and was diagnosed with a non-ST-segment elevation myocardial infarction. Coronary angiography revealed multivessel coronary artery disease, for which the patient underwent urgent coronary artery bypass grafting to revascularize. Postoperatively, he developed acute respiratory failure due to pulmonary edema in the setting of decom

Aortic root replacement with a mechanical valve conduit: A single-institutional experience.

BACKGROUND: There is a lack of consensus regarding the best strategy for aortic root replacement with current guidelines supporting the use of mechanical valves in younger patients when a valve repair is not possible. We sought to describe our institutional experience with mechanical aortic root replacement. METHODS: This was a descriptive study using an institutional database of aortic root replacements with a mechanical valve composite graft performed at our center from 2010 to 2022. All patients undergoing aortic root replacements with a mechanical valve were included. Patients undergoing

National Trends in Racial and Ethnic Disparities in Mortality from Mechanical Complications of Cardiac Valves and Grafts (1999-2020).

Background: The volume of cardiac valve and coronary artery revascularization procedures is rising in the United States. This cross-sectional study explores ethnic disparities in mortality in cardiac surgery attributed to mechanical failures of implantable heart valves and coronary artery grafts. Methods: We used the CDC Wide-Ranging Online Data for Epidemiologic Research Multiple Causes of Death database to identify patients whose single cause of death was categorized by complications of cardiovascular prosthetic devices, implants, and grafts (ICD-10 code T82) between 1999 and 2020. Reverse-U Aortotomy (Kırali Incision) for Aortic Valvular Interventions.

BACKGROUND: Surgical incisional approach to the ascending aorta is the main strategic step during valvular and/or subvalvular aortic interventions. Classic aortotomy incisions (transverse or oblique) can be challenging and can cause suboptimal exposure of the aortic root especially for the patients with small aortic annulus or for redo coronary artery bypass patients with patent proximal grafts interposed to the ascending aorta. METHODS: The Kırali incision was used in 91 patients (including 13 reoperations) who underwent an aortic intervention for valvular and subvalvular pathologies. Aortic

Safety and effectiveness of neuromuscular electrical stimulation in cardiac surgery: A systematic review.

BACKGROUND: Lack of mobilization and prolonged stay in the intensive care unit (ICU) are major factors resulting in the development of ICU-acquired muscle weakness (ICUAW). ICUAW is a type of skeletal muscle dysfunction and a common complication of patients after cardiac surgery, and may be a risk factor for prolonged duration of mechanical ventilation, associated with a higher risk of readmission and higher mortality. Early mobilization in the ICU after cardiac surgery has been found to be low with a significant trend to increase over ICU stay and is also associated with a reduced duration of

Prophylactic corticosteroids for cardiopulmonary bypass in adult cardiac surgery.

BACKGROUND: Cardiac surgery triggers a strong inflammatory reaction, which carries significant clinical consequences. Corticosteroids have been suggested as a potential perioperative strategy to reduce inflammation and help prevent postoperative complications. However, the safety and effectiveness of perioperative corticosteroid use in adult cardiac surgery is uncertain. This is an update of the 2011 review with 18 studies added. OBJECTIVES: Primary objective: to estimate the effects of prophylactic corticosteroid use in adults undergoing cardiac surgery with cardiopulmonary bypass on the: -

German Heart Surgery Report 2023: The Annual Updated Registry of the German Society for Thoracic and Cardiovascular Surgery.

Based on a voluntary registry, founded by the German Society for Thoracic and Cardiovascular Surgery (DGTHG) in 1980, a well-defined but limited dataset of all cardiac and vascular surgery procedures performed in 77 German heart surgery departments is reported annually. For the year 2023, a total of 168,841 procedures were submitted to the registry. Of these operations, 100,606 are defined as heart surgery procedures in a classical sense. The unadjusted in-hospital survival rate for the 28,996 isolated coronary artery bypass grafting procedures (relationship on-/off-pump 2.8:1) was 97.6%; 97.7
