Occult intraocular aluminium foreign body causing rhegmatogenous retinal detachment: a case report.

BACKGROUND: Ocular trauma is complex and varied, and some occult intraocular foreign bodies (IOFBs) can lead to uncommon symptoms and signs. We report a case of rhegmatogenous retinal detachment (no obvious wound, no pain, no intraocular infection or other symptoms) caused by an occult intraocular aluminium foreign body, which could have been easily missed. CASE PRESENTATION: A 42-year-old male presented to the outpatient department of our hospital complaining of fluttering black dots and decreased vision in his left eye that began 3 months earlier. He was diagnosed with "floaters" at a commu

Haptic Amputation Under Endoscopic Guidance in Uveitis-Glaucoma-Hyphema Syndrome: A Case Report.

Uveitis-glaucoma-hyphema (UGH) syndrome is a rare ophthalmic postoperative complication in which the intraocular implants or devices like intraocular lenses (IOLs) produce chronic mechanical chaffing to the adjacent uveal tissues and/or trabecular meshwork (TM) resulting in a wide spectrum of clinical ophthalmic manifestations ranging from chronic uveitis to secondary pigment dispersion, iris defects, hyphema, macular oedema, or spiked intraocular pressure (IOP). Spiked IOP is a result of direct damage to the TM, hyphema, pigment dispersion, or recurrent intraocular inflammation. UGH syndrome

Differential Diagnosis of Changes in Intraocular Lenses.

Differentiating between various intraocular lens (IOL) changes can be a challenge. In particular, certain IOL models carry the risk of late postoperative calcification. A major cause of IOL exchange surgery could be avoided if appropriate modifications were made during the IOL manufacturing process. The use of a hydrophilic acrylate carries the risk of IOL calcification, especially when a secondary procedure, such as a pars plana vitrectomy or other procedures using gas or air, is performed. In secondary IOL calcification, there is a wide range of opacification patterns, which are usually loca

Cataract surgery and intraocular lens placement in patients with Fuchs corneal dystrophy: a review of the current literature.

PURPOSE OF REVIEW: Late-onset Fuchs endothelial corneal dystrophy (FECD) is seen in approximately 4% of individuals over the age of 40. With the growing population of adults over the age of 65, ophthalmologists need to be aware of the preoperative, perioperative, and postoperative considerations involved in cataract surgery in Fuchs patients. RECENT FINDINGS: Management of cataract patients with FECD requires preoperative assessment of endothelial cell size, density, and morphology. Considerations for perioperative endothelial cell loss include patients with hyperopia and shallow anterior cha

Foldable Bulk Anti-adhesive Polyacrylic Intraocular Lens Material Design and Fabrication for Posterior Capsule Opacification Prevention.

Posterior capsular opacification (PCO) is a primary complication after phacoemulsification combined with intraocular lens (IOL) implantation, which is attributed to adhesion, proliferation, and migration of residual lens epithelial cells on IOL. Although surface hydrophilic coating is considered to be a powerful way to inhibit PCO incidence after surgery, it requires complex post-production processes, thus limiting their applicability. In comparison, bulk modification is a stable, effective, and facile IOL synthesis method for PCO prevention. Herein, a new anti-adhesive IOL material was design

Measures of visual disturbance in patients receiving extended depth-of-focus or trifocal intraocular lenses.

The degree of visual disturbance associated with a particular model of intraocular lens (IOL) depends on several factors, including IOL optic, material, and mechanics. Characterization of visual disturbance profiles is paramount for informing clinical IOL selection. Although many studies evaluating presbyopia-correcting IOLs include subjective assessment of visual symptoms, the types of patient-reported outcome measures (PROMs) used to capture these outcomes are inconsistent across studies, complicating data contextualization. Furthermore, some tools produce more meaningful results than others

Piggyback miLOOP-assisted phacoemulsification combined with intraocular lens scaffold in hypermature cataracts.

Hypermature cataracts with liquified cortex pose a higher risk for capsular complications due to a distended capsular bag, floppy posterior capsule, and absence of any cushioning epinucleus. Disassembly of a brunescent nucleus is more difficult because of the thickened, leathery posterior plate. Relying on mechanical forces, miLOOP nucleotomy reduces ultrasound time and is assured of bisecting the posterior nuclear plate. However, using this instrument with brunescent nuclei might impart significant stress on the zonular fibers and posterior capsule, particularly if there is no epinuclear shel

[Justification for the use of modular intraocular lens (experimental study)].

UNLABELLED: The rising number of cases requiring intraocular lens (IOL) exchange, including those for refractive reasons, drives the search for methods to minimize the trauma of the procedure. Lens implants with modular optics may lower complication risks. PURPOSE: To test the use of modular IOL on a laboratory animal in an experiment. MATERIAL AND METHODS: A novel modular IOL was implanted into rabbit eyes in disjointed form with subsequent intraocular assembling. Presence of long-term side effects of implantation and possibility of atraumatic optical disk exchange were evaluated 4 months a

Polymeric hydrogel as a vitreous substitute: current research, challenges, and future directions.

Vitreoretinal surgery is an essential approach to treat proliferative diabetic vitreopathy, retinal detachment, retinal tear, ocular trauma, and macular holes. The removal of the natural vitreous and the replacement with substitutes are critical steps for retina reattachment. Vitreous substitutes including silicone oil (SiO), air, sulfur hexafluoride (SF6), and perfluoropropane (C3F8), have been widely applied in clinical practice. However, these substitutes are reported to cause complications such as emulsification, high intraocular pressure, and lens opacification. Polymeric hydrogels are a

Cataract Following Pars Plana Vitrectomy: A Review.

Introduction: The indications for pars plana vitrectomy (PPV) have increased over the years. The vitreous is no longer considered an inert ocular structure and it is well known that its removal has anatomical and physiological consequences. The vitreous is no longer considered an inert ocular structure. The vitreous plays a key role as an intraocular physiologic oxygen regulator. In order to maintain its transparency, the crystalline lens needs protection from an excessive oxygen exposure. PPV leads to progression of nuclear sclerosis in most eyes. Methods: A systematic review of the literature

[Capsule polishing during cataract surgery: techniques and its role in preventing posterior capsular opacification].

Posterior capsular opacification (PCO) is the most common complication that leads to vision loss after cataract surgery. It is classically divided into fibrotic type and regenerative type according to clinical manifestations or pathological mechanisms. The widely used technique for preventing PCO is to mechanically polish anterior and posterior capsules after an uneventful phacoemulsification surgery. However, the efficacy of polishing anterior capsules on prevention of PCO is debatable. It has been found that polishing anterior capsules has an inhibitory or no effect on fibrotic PCO, but a st

NIR-triggered drug delivery system for chemo-photothermal therapy of posterior capsule opacification.

Posterior capsule opacification (PCO) is the most common complication after cataract surgery and is likely to cause the second loss of vision. Pharmacological PCO prophylaxis has been proved to be effective, yet no clinical option is available due to the lack of a suitable mode of administration. In this work, we propose a unique concept of NIR dual-triggered drug release from black phosphorus (BP)-based implantable intraocular lens (IOL) for controlled drug release and chemo-photothermal combination therapy of PCO. Here, IOL is used as a "reservoir" of doxorubicin-loaded black phosphorus (BP-

The use of soft contact lens for intraocular foreign body removal to avoid inadvertent cataract formation.

Ocular injuries carry a significant morbidity worldwide. Visual outcomes vary depending on the mechanism of injury, the damage on arrival and the surgical technique used. The presence of an intraocular foreign body further complicates matters, due to its constituents, the infectious potential, and/or damage to intraocular structures. Another described problem is the inadvertent cataract formation due to contact with the natural lens during removal of the foreign body. The purpose of this paper is to describe a case with a mechanical foreign body lodging in the posterior chamber without harming

Removing residual cortical material during cataract surgery: Visco polishing and the grindstone effect as a new surgical technique.

PURPOSE: To describe a new technique for removing residual cortical material adjacent to posterior lens capsule during cataract surgery with the help of cohesive ophthalmic viscosurgical devices. METHODS: In this technique, if there are still cortical remnants and lens epithelial cells left after the routine irrigation-aspiration and polishing at the final steps of cataract surgery, the capsule is filled to about one-third with a highly viscous cohesive ophthalmic viscosurgical device. Rinsing is performed from the anterior chamber with the water jet directed tangentially to the ophthalmic vi

[Posterior YAG capsulotomy: selection of the application pattern].

BACKGROUND: The posterior neodymium-yttrium-aluminum-garnet laser capsulotomy is an established and simple method with a low complication rate for the treatment of secondary cataracts; however, the risk of intraocular pressure elevation, pit marks of the intraocular lens (IOL), anterior hyaloid damage, cystoid macular edema and retinal detachment may increase with high pulse number, pulse energy, and total energy. PURPOSE: The optimization potential of the method through a rational choice of the laser pattern and the strategy, taking into account the mechanical properties of the posterior cap

Drug-loaded and Blue-ray Filtered Hydrogel as a Potential Intraocular Lens for Cataract Treatment.

BACKGROUND: Indomethacin (IND) is a class of non-steroidal, anti-inflammatory drugs, which is used to treat various kinds of ocular inflammation, and has been reported to prevent posterior capsule opacification (PCO) by inhibiting the mitosis and collagen synthesis of human lens epithelial cells (LECs). In addition, the specific absorption spectrum of indomethacin shows the effect of absorbing short-wavelength blue-violet light. OBJECTIVE: We prepared an indomethacin-loaded hydrogel as a potential intraocular lens (IOLs) material to prevent endophthalmitis, PCO and filter harmful blue light.
