Epidemiology, clinical outcomes, and factors associated with mortality among cerebrospinal meningitis cases in the Upper West Region of Ghana: an eight-year surveillance study.

INTRODUCTION: Cerebrospinal meningitis is a clinical syndrome with multiple etiologies, however, in sub-Saharan Africa, infectious and contagious forms remain highly prevalent and carry high risks of mortality and disability. The Upper West Region of Ghana is found within the meningitis belt and frequently records the highest numbers of incident meningitis cases within the country yearly. METHODS: This study analyzed all routinely-collected data on surveillance-defined suspected and confirmed cases of meningitis in the Upper West Region of Ghana between January 2016 and December 2023. The dat

Not All Extrapulmonary Tuberculosis Is Equal: Site-Specific Mortality in a National Indian Cohort.

Among 777,512 Indian adults with extrapulmonary tuberculosis, in-treatment mortality varied over tenfold by site, highest for tuberculous meningitis (adjusted odds ratio 12.48 vs. lymph node TB), miliary (5.17), and pericardial TB (3.86). Associations of HIV, diabetes, alcohol, and tobacco use with mortality varied across sites. Risk-stratified, comorbidity-aware care is warranted.

Skull infarction and chronic subdural exudates in a pediatric patient with sickle cell anemia: illustrative case.

BACKGROUND: Skull infarction is a rare sickle cell disease (SCD) complication. While epidural hematomas from calvarial infarction have been sporadically reported, subdural collections and sterile exudates mimicking intracranial empyema remain poorly characterized. OBSERVATIONS: A 6-year-old boy with nonverbal autism and SCD presented with progressive periorbital swelling, fever, and extremity pain. CT raised concern for orbital cellulitis, subperiosteal abscess, and subdural empyema. MRI revealed a diffuse heterogeneous calvarial marrow signal with multifocal nonenhancement and restricted dif

Multimodality salvage treatment incorporating ivonescimab plus chemotherapy for EGFR-mutated NSCLC with leptomeningeal metastases after dose-escalated third-generation EGFR-TKI failure: a retrospective study.

BACKGROUND: Leptomeningeal metastasis (LM) is among the most devastating modes of progression in EGFR-mutated non-small cell lung cancer (NSCLC). Although dose escalation of third-generation EGFR-TKIs has been an important central nervous system (CNS)-directed salvage strategy, therapeutic options after progression on this intensification approach remain limited. METHODS: This single-center retrospective study included consecutive patients treated between September 2024 and December 2025. Eligible patients were adults (≥18 years) with histologically confirmed lung adenocarcinoma, a documented

Cerebrospinal fluid and dural accumulation of α-synuclein impairs CNS clearance in Parkinson's disease.

Parkinson's disease (PD) is characterized by the build-up of α-synuclein (α-syn) in the brain, where it drives neurodegeneration. Importantly, aggregated α-syn also accumulates in the cerebrospinal fluid (CSF) of PD patients, and while increasingly used for diagnosis, its pathological role remains poorly understood. Here, we demonstrate that α-syn accumulation in central nervous system (CNS) border tissues promotes CNS clearance deficits, as observed in PD patients. We find that aggregated α-syn accumulates not only in the CSF but also in the dura mater of individuals with PD, and, using acute

Lyme Disease and Other Tick-Borne Infections.

PURPOSE OF REVIEW: The neurologic manifestations of selected tick-transmitted infections in North America and Eurasia were reviewed. RECENT FINDINGS: Both Powassan virus in North America, and Tick-Borne Encephalitis (TBE) virus in Eurasia can cause encephalitis, often with significant neurologic morbidity. Lyme disease, Ehrlichia chaffeensis (the cause of human monocytic ehrlichiosis [HME]), and Borrelia miyamotoi infections can cause meningitis (the third primarily in immunocompromised individuals). Patients with Lyme disease may also develop cranial neuropathy, and/or radiculoneuropathy. Ti

Bedside Transcranial Doppler as a Dynamic Warning Tool for Rapidly Evolving Intracranial Hypertension in Pneumococcal Meningitis.

In severe bacterial meningitis, an initially non-critical or technically unreliable lumbar puncture opening pressure should not reassure when hydrocephalus and coma persist. Bedside transcranial Doppler cannot measure intracranial pressure, but bilateral near-absent diastolic flow may provide an urgent warning signal prompting repeat assessment and cerebrospinal fluid diversion.

Tuberculous Meningitis Alters the Proteomic Landscape of Brain-Derived Extracellular Vesicles.

Tuberculous meningitis (TB meningitis), the deadliest form of Mycobacterium tuberculosis infection, causes devastating mortality and severe neurological disability despite standard therapy. Brain injury and microglial activation are major determinants of outcome, yet the molecular mechanisms linking infection, inflammation, and neuronal injury remain poorly understood. Extracellular vesicles (EVs), critical mediators of cell-to-cell communication, have been investigated in pulmonary TB, but their role in TB meningitis remains unexplored. We used a young New Zealand White rabbit model of TB men

The glymphatic system as a mechanistic bridge between migraine and sleep: Evidence from clinical studies and animal models.

Migraine is a highly prevalent, disabling neurological disorder often accompanied by sleep disturbances; however, the mechanisms linking sleep disturbance to migraine susceptibility remain unclear. The glymphatic system, a sleep-modulated perivascular clearance network facilitated by astrocytic aquaporin-4 (AQP4) and functionally connected to meningeal lymphatic drainage, is a potential mechanistic link between migraine and sleep disturbances. This review synthesizes evidence from clinical neuroimaging, cerebrospinal fluid and blood biomarker studies, and preclinical animal models to evaluate

Voxel-Based Mapping of Seizure Risk in Meningioma Identifies the Frontoparietal Interface and Tumor Microenvironment as Key Predictors.

BACKGROUND AND OBJECTIVES: Brain invasion (BINV) and peritumoral edema (PTBE) have repeatedly been implicated as major contributors to meningioma-related seizures (MRS). By contrast, the influence of the precise tumor location on seizure risk is still insufficiently characterized. In this study, we systematically investigated clinical and imaging-derived predictors of MRS, integrating them with spatial analyses using voxel-based lesion symptom mapping (VLSM). METHODS: We reviewed clinical variables-including seizure history, histopathological subtype, World Health Organization (WHO) grade, BI

Syndrome of the trephined following burr hole surgery, decompressive craniectomy/cranioplasty, or craniotomy: contemporary vision from a military neurosurgery department.

The syndrome of the trephined (ST, 1939) described headaches, scar discomfort, and anxiety following cranial surgery. The sinking skin flap syndrome (1977) described neurological worsening after decompressive craniectomy (DC) and immediate improvement following cranioplasty (CP). They have been mistakenly confused with the other since 1984. The aim was to describe the ST. Symptoms were screened among 20 chronic subdural hematome (CSDH) patients operated on by neurosurgeon-1, and compared to (1) DC-CP patients operated on by neurosurgeon-1, (2) any craniotomy patients followed-up by neurosurgeo

Deep cervical lymphaticovenous anastomosis for Alzheimer disease: A critical narrative assessment and translational framework.

Deep cervical lymphaticovenous anastomosis (dcLVA) has been proposed to augment cerebral waste clearance in Alzheimer disease. This critical narrative assessment examines the mechanistic premise, available human reports, registered studies, and governance implications. Experimental loss-of-function and gain-of-function studies support a role for meningeal lymphatics in brain-fluid homeostasis, but they do not establish a rate-limiting cervical outflow abnormality in human Alzheimer disease. The clinical reports identified through this narrative search were uncontrolled single-arm investigation

Risk factors for pseudomeningocele development following craniotomy: a single-center experience from Greece.

BACKGROUND: A pseudomeningocele is a subcutaneous collection of cerebrospinal fluid due to a defect in the dura that occurs after surgery or trauma. Postoperative pseudomeningocele can lead to wound breakdown, intracranial hypotension and meningitis, thus increasing the length of hospitalization and morbidity. Although several clinical and operative variables have been proposed as potential risk factors for this complication, relevant prospective, large-scale studies to investigate these associations are missing. The purpose of this study is therefore to explore the potential correlation betwe

[Neurotropic disease associated with yellow fever vaccine: presentation as post-travel febrile syndrome].

The yellow fever vaccine is an essential tool for preventing yellow fever in people who live in or travel to areas at risk. It is highly effective, and serious adverse events -including hypersensitivity reactions, vaccine-associated viscerotropic disease, and vaccine-associated neurotropic disease (YEL-AND)-, are rare in relation to the millions of doses administered since their implementation. The incidence of YEL-AND ranges from 0.17 to 0.8 cases per 100 000 doses administered. The incidence is higher in males and in people over 60 years of age. YEL-AND can be manifested as meningoencephalit

Waning immunity, an age shift, and rising adult burden of varicella after two decades of single-dose vaccination in South Korea.

BACKGROUND: Many countries have not adopted universal varicella vaccination (UVV), citing concerns that it could shift disease burden toward older, higher-risk age groups and be insufficiently cost-effective. Long-term data under sustained single-dose vaccination remain scarce. We assessed the immunological, epidemiological, and healthcare cost consequences of two decades of South Korea's single-dose National Immunization Program, which has maintained >95% coverage since 2005 without the anticipated decline in incidence. METHODS: We analyzed varicella-zoster virus immunoglobulin G titers in 3

Development and prospect of treating Alzheimer's disease using the glymphatic system.

Alzheimer's disease (AD) is a common neurodegenerative disorder with progressive cognitive decline and typical amyloid-β (Aβ) and tau pathologies, whose pathogenesis remains unclear. The glymphatic system, a core cerebral waste clearance pathway, has become a critical target for AD pathogenic research and novel therapy development. This review explores the glymphatic system's structure and function, illustrates its correlation with AD pathogenesis, summarizes targeted therapies, and highlights research limitations and future directions to facilitate AD translational research.
