PURPOSE: The pterygomeningeal (accessory meningeal) artery is an inconstant branch of the middle meningeal or maxillary artery. A variant that penetrates the skull anterior to the foramen ovale is rare, and bilateral coexistence has not been reported. A cadaver with bilateral variants of this type is described.
METHODS: The head of a 79-year-old man was examined during a gross anatomy course. Origin, course, and relations of the variant vessels were recorded on both sides.
RESULTS: On the left, an accessory middle meningeal branch of the middle meningeal artery penetrated the greater wing ap

BACKGROUND: Recent randomized clinical trials have shown middle meningeal artery embolization (MMAE) reduces the need for subsequent burr hole drainage of chronic subdural hematoma (cSDH), both as an adjunct to initial drainage or as a standalone procedure. We compared the 6-month total hospital charges for standalone MMAE (sMMAE) versus nonsurgical medical management in patients with cSDH.
METHODS: We retrospectively reviewed patients who presented with unilateral, >1cm in size, cSDH initially managed without drainage from January 1, 2020, through July 30, 2024. Seventy-two patients were ide

INTRODUCTION: Fistulas forming between cerebrospinal fluid (CSF) and spinal veins is a common cause of spontaneous intracranial hypotension (SIH) with subdural fluid collections(SDFC). CSF-venous fistulas embolization (CVFE) can be performed with a combined middle meningeal artery embolization (MMAE) to close the fistula and treat the SDFC.
OBJECTIVE: Assess the safety and efficacy of combined MMAE and CVFE treatment.
METHODS: We performed a retrospective cohort study of 40 patients who underwent combined MMAE and CVFE or CVFE alone with SIH and SDFC at a single institution. Patients deemed

BACKGROUND Middle meningeal artery embolization (MMAE) treats chronic subdural hematoma (cSDH) by devascularizing the hematoma membrane, but the timing of radiological resolution remains incompletely defined. This prospective 2-center study evaluated computed tomography (CT) outcomes at 1 and 3 months after MMAE in Vietnam. MATERIAL AND METHODS Consecutive patients treated from November 2021 through December 2024 were enrolled. Maximum hematoma thickness and midline shift were measured on baseline imaging and follow-up non-contrast-enhanced CT. Paired measurements were compared using paired t

White epidermoid cysts (WECs) are an uncommon radiologic variant of intracranial epidermoid cysts, accounting for approximately 1.5 to 8% of cases. Unlike classic epidermoids, which are typically T1 hypointense, T2 hyperintense, and diffusion restricting, WECs characteristically demonstrate T1 hyperintensity, variable or low T2 signal, and absence of diffusion restriction. These atypical imaging features frequently lead to diagnostic confusion with dermoid cysts, proteinaceous arachnoid cysts, or neurenteric cysts. WECs arising in the ventral posterior fossa, particularly the prepontine and pr

Spinal cord injury (SCI) induces secondary brain pathology, but the mechanisms linking spinal and brain injury remain unclear. Here, we identify meningeal lymphatic dysfunction as a contributor to secondary brain pathology after SCI. In mice, meningeal lymphatic vessels (mLVs) progressively deteriorated from 14 to 56 days post-injury, accompanied by glymphatic dysfunction. In patients with SCI, diffusion tensor imaging along the perivascular space (DTI-ALPS) revealed impaired glymphatic function, which correlated with motor cortical and hippocampal atrophy. mLVs ablation exacerbated neuroinfla

Ventriculoperitoneal shunt (VPS)-associated bacterial meningitis is a significant neurosurgical complication that can occasionally present atypical symptoms. We report a rare case of Streptococcus pyogenes meningitis in a 40-day-old infant with a history of hydrocephalus managed by VPS placement. The patient presented with vomiting, diarrhea, fever, and -abdominal distension. Microbial culture of the removed VPS valve confirmed the diagnosis, revealing Streptococcus pyogenes. Despite the strain's sensitivity to penicillin G, the patient was treated with cefotaxime and vancomycin due to negativ

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

Newborns are especially susceptible to bacterial meningitis, primarily caused by group B Streptococcus (GBS), due to incomplete maturation of immune and barrier systems. While meningitis is well known to break down the blood-brain barrier, its effect on the meningeal arachnoid barrier remains poorly understood. Using a neonatal bacterial meningitis model, we demonstrate that GBS infection significantly increases arachnoid barrier permeability, disrupts tight junctions, and elevates meningeal production of proinflammatory cytokines. CD206+/LYVE-1+ border-associated macrophages undergo morpholog

OBJECTIVE: The balance between surgical resection and morbidity can be a challenge for parasagittal meningiomas (PMs) in terms of superior sagittal sinus (SSS) manipulation. In the era of multimodal approach, the role of microsurgery alone is still controversial, but not well defined. The authors investigate the factors associated with recurrence and morbidity for surgically treated WHO grade I PMs.
METHODS: A hundred and fifteen patients underwent microsurgery for newly diagnosed WHO grade I PMs from 2005 to 2025. The factors associated with recurrence and surgical morbidities were retrospec

BACKGROUND: Tick-borne encephalitis (TBE) is a severe viral infection of the central nervous system (CNS), causing acute meningoencephalitis and often long-term neurocognitive sequelae. The lack of specific treatment for TBE highlights the need to improve understanding of disease mechanisms and identify novel biomarkers. Enteroviral meningitis is typically a milder infection of the CNS and rarely causes encephalitis.
OBJECTIVES: In this hypothesis-generating study, we aimed to characterise temporal changes in the cerebrospinal fluid (CSF) proteome in TBE and identify candidate biomarkers asso

Tuberculous meningitis (TBM) is a severe and often fatal form of extrapulmonary tuberculosis. We describe a case in which the initial cerebrospinal fluid (CSF) analysis showed no specific abnormalities, but a diagnosis was established after repeat CSF evaluation. A 41-year-old female presented with a 10-day history of persistent fever and headache. Initial CSF analysis demonstrated an opening pressure of 150 mmH2O, a white blood cell (WBC) count of 10/μL, a glucose level of 50 mg/dL, and a protein level of 31.4 mg/dL. Despite treatment with an antiviral agent and doxycycline, her symptoms did

Lead (Pb) and chromium (Cr) are prevalent aquatic contaminants capable of inducing oxidative and neurological disturbances; however, the consequences of their combined exposure on brain oxidative homeostasis remain poorly understood. The present study examined the progressive oxidative, molecular, and histopathological effects of chronic Pb and Cr exposure, individually and in combination, in zebrafish (Danio rerio), with particular emphasis on modulation of the Nrf2-Keap1-ARE pathway. Adult male zebrafish were exposed to environmentally relevant concentrations of Pb (2.5, 5, and 10 ppb), Cr (

Nuclear receptor NR1F regulates circadian rhythms in mammals, metamorphosis in insects, and molting in free-living Caenorhabditis elegans; however, its role in parasitic nematode development remains elusive. Angiostrongylus cantonensis, a zoonotic pathogen that causes eosinophilic meningitis in humans, exhibits distinct host adaptation, with mice and most mammals serving as non-permissive hosts and rats as the permissive host. We combined multi-omic analyses, lentivirus-mediated RNAi, LC-MS/MS, and dual-luciferase assays to elucidate AcNR1F functions in development and host adaptation. Acnr1f

Endovascular treatment (EVT) is a useful modality for traumatic carotid‑cavernous fistulas (CCFs); in rare cases, combined antegrade and retrograde transarterial approaches may be applied for embolization of the fistula with concomitant internal carotid artery (ICA) sacrifice and preservation of the ophthalmic artery (OphA). A 62‑year‑old female patient sustained a motor vehicle accident and developed proptosis, chemosis, and congestion in her right eye, while no abnormalities in ocular motility or visual acuity were noted. Computed tomography angiography confirmed the CCF. Under general anest

BACKGROUND: Post-infectious inflammatory response syndrome (PIIRS) is an increasingly recognized complication of cryptococcal meningoencephalitis (CM) in non-HIV infected hosts with significant morbidity and mortality. Data describing long-term outcomes following immunomodulatory therapy are limited.
METHODS: We conducted a prospective observational study of 15 immunocompetent adults with CM-PIIRS treated with a pulse corticosteroid taper (PCT) regimen. Performance status, neurocognitive, ophthalmologic, audiologic, and radiologic assessments were performed at greater than one year following

BACKGROUND: Traumatic brain injury (TBI) triggers microglial activation and disrupts the inflammatory microenvironment in the hippocampus, which are key contributors to neuroinflammation and subsequent secondary brain damage. Nevertheless, the cellular heterogeneity of microglia, their pro-inflammatory activation mechanisms, and microenvironmental regulatory networks have not been fully characterized.
METHODS: By integrating multi-source single-cell transcriptomic datasets and establishing a mouse TBI validation model, we constructed a high-resolution cellular atlas of hippocampal microglia.
