The continuous evolution of concepts in compartmental resections for oral cavity cancer: principles, indications, and outcomes.

PURPOSE OF REVIEW: This paper is aimed at synthetizing the conceptual basis of compartmental surgery (CS) for tumours of the anterior (floor of mouth and oral tongue) as well as posterior (base of tongue, superior and inferior gingival-buccal sulcus extending into the masticator space and infratemporal fossa) oral cavity, its indications, and outcomes. RECENT FINDINGS: In spite of numerous bioendoscopic, radiological, and surgical advancements in management of oral cancer, its oncologic surgical control continues to be hampered by a considerable risk of local recurrence rate. Use of a convent

Radiological and Histological Findings of Primary Pleomorphic Rhabdomyosarcoma Arising from the Mandibular Gingiva: A Case Report and Literature Review.

Background: Pleomorphic rhabdomyosarcoma (RMS) is a rare, adult-predominant high-grade sarcoma that usually arises in the deep soft tissues of the extremities. Primary oral pleomorphic RMS is exceptionally rare, and detailed CT and MRI characteristics of oral pleomorphic RMS remain sparsely documented. Case Presentation: A 66-year-old woman presented with a two-month history of lower anterior tooth pain and progressive mandibular swelling, initially misdiagnosed and treated as a dental infection. CT and MRI revealed a 3.8-cm heterogeneously enhancing mass centered in the mandibular gingiva, wi

How to Do a Split Mentalis Muscle Flap for Central Marginal Mandibular Defects.

Tumors of the central mandible requiring marginal mandibulectomy and floor of mouth resection require reconstruction for cosmetic and functional purposes. Numerous reconstructive techniques have been described for small-moderate sized defects of the central mandible/anterior floor of mouth. Robust local flaps have been well described; however second stage procedures are required for pedicle division. The split mentalis muscle flap is a simple, quick and low morbidity option for central mandibular defects and has been demonstrated effectively with the use of a split skin graft. We provide a mod

Bilateral hypoglossal nerve damage due to the use of the harmonic focus scalpel.

We describe the case of a 68-year-old man who presented with an anterior floor of mouth squamous cell carcinoma and underwent bilateral neck dissection. Subsequently, the patient developed permanent bilateral hypoglossal palsies, causing significant dysphagia, dysarthria and breathing difficulties. Hypoglossal nerve damage from surgical causes is uncommon, occurring in only 0.42% of functional neck dissections. However, it is the bilateral and permanent features of this case which make it unique. This injury may have been caused by incorrect use of the Harmonic Focus scalpel. Therefore, we dis

Small cell carcinoma of the floor of mouth: Difficulties of a therapeutic choice.

INTRODUCTION: With only 14 cases and as many different therapeutic strategies reported in the literature, small cell carcinoma of the oral cavity remains a real challenge with diagnostic and therapeutic pitfalls. CLINICAL CASE SUMMARY: A 65-year-old patient with an active smoking habit consulted for a painful oral pelvic mass. The staging confirmed the diagnosis of small cell carcinoma of the anterior floor of mouth, classified as cT3N0M0. By analogy with pulmonary locations, medical management combining chemotherapy specific for small bronchial cell carcinoma (carboplatin-etoposide) and radi

[Neurocutaneous flaps applied on head and neck surgery: Anatomic study of feasibility using superficial cervical plexus branches].

OBJECTIVE: To evaluate the feasibility of neurocutaneous flaps for head and neck surgery using the cervical superficial plexus branches. METHODS: Anatomic study on cervical superficial plexi (focusing on the great auricular and cervical transverse nerves) of five fresh specimens, which were injected with green colored latex. RESULTS: A constancy of anatomic landmarks has been observed with little inter-individual variability. The cervical superficial plexus branches were easily isolated, their length and course not varying much. Each cervical superficial plexus nerve is accompanied by a peri

Medial sural artery perforator flap for intraoral reconstruction following cancer ablation.

In oral cavity reconstruction, the fasciocutaneous flaps of the distal extremities have always been preferred to any other kind of flap because of their thinness and pliability, which makes them adaptable to different areas in the oral cavity. The radial forearm flap is frequently considered the first choice for intraoral reconstruction, but the disadvantages of donor site morbidity include sacrificing a major artery to the hand and leaving a conspicuous donor site scar. The search for another primarily thinned skin flap as an alternative has led to the application of the medial sural artery p

Transition to Medicare Part D: an early snapshot of barriers experienced by younger dual eligibles with disabilities.

BACKGROUND: Cleft lip and palate (CLP) is among the most common congenital anomalies, necessitating complex, multidisciplinary care from infancy through adolescence. Caregivers often turn to online resources, particularly YouTube, for supplemental guidance on presurgical management, feeding techniques, and long-term care. However, the reliability and educational value of such freely available video content remain uncertain. METHODS: A systematic literature search was conducted in PubMed/MEDLINE, Scopus, Cochrane, and relevant grey literature from 2014 to 2026. Eligible studies were cross-sect

Management of malignant sublingual salivary gland tumors.

The majority of tumors of the sublingual gland are malignant, with adenoid cystic carcinoma and mucoepidermoid carcinoma being the most frequent. Many other malignant tumor types have also been reported. The sublingual gland anatomically is not a unit organ and while it is described anatomically as being confined to the anterior floor of the mouth, salivary tissue may be located laterally along the submandibular duct and posterior floor of the mouth. Diagnosis should be suspected when any thickening or raised lesion presents in this area and a biopsy performed to confirm malignancy before plan

Repair of anterior floor of mouth defects by a central or paramedian island tongue flap.

One method for restoring the anterior floor of mouth defects resulting from T(1) and especially T(2) cancer excision involves the creation of a myomucosal island flap from the dorsal median portion of the tongue and transferring it as a transit flap to the floor of the mouth. We are not the first to advocate this method. In this paper we present a previously unreported, slightly modified technique utilizing this tongue flap. The modification consists of a 90 degrees twisting of the flap to achieve a more appropriate adaptation to the defect. In the case of spreading carcinoma from the floor of

Tumor size and pretreatment speech and swallowing in patients with resectable tumors.

The pretreatment relationship of tumor burden to speech and swallowing function in 230 patients with oral or oropharyngeal cancer before surgery was assessed. Reduced articulation, reduced conversational understandability, or self-reported dysphagia were present in at least 34% of patients before treatment. Videofluoroscopy showed at least 9% of patients had reduced swallowing efficiency on liquid, paste, or cookie boluses. By use of regression techniques, the percentages of the oral tongue and of the anterior floor of mouth affected by neoplasm were found to be significantly related to reduce

Central island tongue flap.

Pedicled tongue flaps have proved to be an effective method of repairing defects due to tissue loss in the oral cavity. Their central position, mobility, and excellent blood supply make their use feasible in a variety of sites. This paper describes the use and applications of central island tongue flaps to reconstruct defects of anterior floor of the mouth. This procedure was conducted at the Department of Oral and Maxillofacial Surgery, Medical University Hanover, introducing an improved surgical method and presenting the actual operation performed in our department. This method is considered
