Impact of healthcare setting on survival in adults with acute lymphoblastic leukemia in Latin America: insights from a large multinational cohort.

BACKGROUND: Outcomes of adults with acute lymphoblastic leukemia (ALL) in Latin America remain poorly characterized, particularly across public and private healthcare systems. In this study, we evaluated overall survival (OS) and relapse rates in a multinational cohort, aiming to identify key factors associated with these outcomes. METHODS: We retrospectively analyzed patients aged ≥15 years with newly diagnosed ALL treated at 45 centers in Brazil, Argentina, and Chile between 2015 and 2023. Overall survival (OS) and event-free survival (EFS) were estimated using Kaplan-Meier methods. Cox mod

Oblique Vertical Component and Dual Anteroposterior Fixation in Sagittal "L"-Shaped High Tibial Osteotomy: Technical Considerations and Biomechanical Reasoning.

AIM: To describe an oblique vertical modification of the sagittal "L"-shaped high tibial osteotomy combined with dual anteroposterior fixation and to present the biomechanical reasoning supporting its potential application. METHODS: A technical note was developed based on the previously described sagittal "L"-shaped high tibial osteotomy (HTO). The proposed modification consists of introducing a slight obliquity (10°-20°) in the vertical osteotomy component posterior to the tibial tubercle while maintaining supplementary parallel anteroposterior fixation. The surgical technique, preoperative

A Novel Ca2+-Dependent O-Methyltransferase Involved in Flavonoid Methylation in Tartary Buckwheat.

Methoxyflavonoids have attracted increasing attention because of their diverse biological activities and improved physicochemical properties. Although methylation is pivotal to methoxyflavonoid biosynthesis, the O-methyltransferases (OMTs) responsible for this process in Tartary buckwheat remain poorly characterized. In this study, the Tartary buckwheat cultivar "Y324" was used to systematically analyze the accumulation patterns and spatial distribution of methoxyflavonoids during achene development. Most methoxyflavonoids accumulated progressively during achene development, and they were spec

Structure-activity relationship (SAR) studies of abaucin derivatives targeting the lipoprotein transport system in Acinetobacter baumannii.

Carbapenem-resistant A. baumannii (CRAB) has been classified by the World Health Organization (WHO) as a critical priority antibiotic-resistant pathogen, making it one of the highest priorities for global antibiotic development. In 2023, abaucin, a narrow-spectrum antibiotic selectively targeting the lipoprotein transport system in Acinetobacter baumannii, was identified through deep learning-based screening. In this study, we designed and synthesized 30 abaucin derivatives to further explore the structure-activity relationship (SAR). SAR analysis revealed that the spirocyclic scaffold, linker

Alpha-fetoprotein change after systemic treatments in advanced hepatocellular carcinoma: a systematic review and trial-level meta-analysis.

Alpha-fetoprotein (AFP) kinetics after systemic therapy for unresectable or metastatic hepatocarcinoma has been evaluated as an early prognostic factor in many studies, but the effect size has varied largely. The current study aims to synthesize the evidence about the relationship between AFP changes and overall survival (OS). A literature search of clinical studies of metastatic hepatocarcinoma patients receiving systemic treatments was conducted in two databases, PubMed and Web of Science. After selecting studies evaluating the relationship of any AFP changes with OS, a meta-analysis was per

Astragali Radix-Curcumae Rhizoma synergistically potentiates 5-fluorouracil antitumor efficacy in colorectal cancer by disrupting DNA repair-cell cycle coordination.

BACKGROUND: Astragali Radix (HQ) and Curcumae Rhizoma (EZ), collectively termed AC, are commonly used as an herb pair in Traditional Chinese Medicine (TCM) for colorectal cancer (CRC). Although AC has been used as an adjuvant to chemotherapy, the molecular basis by which it enhances the efficacy of 5-fluorouracil (5-FU) remains incompletely defined. METHODS: The antitumor effect of AC alone and in combination with 5-FU was evaluated in a HCT116 xenograft mouse model and in CRC cell lines with different genetic backgrounds. Potential systemic toxicity was assessed using serum biochemical marke

Brain Penetration Optimization and Anti-neuroinflammation Evaluation of Potent 6-Methyl-7-azaindol-3-yl-quinazoline DYRK1A Inhibitors for Alzheimer's Disease.

Targeting dual-specificity tyrosine phosphorylation-regulated kinase 1A (DYRK1A) to regulate microglia-mediated neuroinflammation represents a promising therapeutic strategy for Alzheimer's disease (AD). To address the limited brain penetration of our earlier lead ZJCK-6-46, an intramolecular cyclization strategy was employed to mask the HBD ability of the P-gp efflux-prone amino and hydroxyl groups in the (S)-2-amino-2-phenylethanol moiety, facilitating comprehensive optimization of physicochemical properties. This effort yielded a more promising candidate, ZJCK-6-72, which maintained the ove

Regional divergence in female cancer mortality in Brazil signals the unequal pace of the cancer transition.

BACKGROUND: The cancer transition framework describes the shift from infection-related cancers, such as cervical cancer, to cancers more commonly observed in high human development settings, such as breast cancer, as societies develop. Brazil, with marked regional disparities in health and socioeconomic indicators, offers a unique context to analyze how this transition unfolds unevenly across subnational regions. This study aimed to describe and compare levels and temporal trends in age-standardized mortality rates for breast and cervical cancer across Brazilian macroregions from 1980 to 2021,

Mast cell phenotypic remodeling toward a PD-1-positive, Chymase-negative phenotype is associated with metastatic melanoma and poor survival.

Mast cells (MCs) display phenotypic heterogeneity across tissues and disease contexts, yet their relevance in melanoma remains unclear. Here, we investigated MC phenotypic programs associated with melanoma progression using high-dimensional phenotyping of MCs co-cultured with melanoma spheroids, trajectory inference, and transcriptomic analyzes. Human melanoma tissues showed differences in MC phenotypic composition despite comparable abundance between outcome groups, with metastatic melanomas enriched for Tryptase⁺ Chymase- MCs. Melanoma exposure induced MC phenotypic remodeling, characterized

Operating Room Air Filtration-Sterilization and Surgical Infections: The ORACLE Randomized Trial in 110,538 Surgeries.

BACKGROUND: Surgical infections are the most common nosocomial infections and cause substantial morbidity and mortality. Operating rooms are relatively clean, but far from sterile. An unknown fraction of infections result from wound contamination by air-borne bacteria. We therefore tested the hypothesis that supplemental air purification reduces wound infections and related complications. METHODS: Operating rooms, our clusters, were each randomized in each 4-week blocks to either active or identical-looking sham supplemental air sterilization. All investigators, clinicians, and patients were

Comparing Suspect and Nontargeted Screening Workflows for Perinatal Chemical Exposome Analysis: A European Interlaboratory Study from the PARC-EU Project.

Early-life chemical exposures may contribute to adverse health outcomes later in life. However, conventional chemical analyses typically cover only a limited subset of the chemical exposureome. Suspect and nontargeted screening (SS/NTS) using high-resolution mass spectrometry (HRMS) workflows provides a powerful framework for comprehensive assessments of exposure, with the potential to detect a broad range of environmental contaminants and emerging chemicals. An interlaboratory assay involving 12 laboratories was performed to evaluate the comparability and complementarity of SS/NTS approaches

Phosphatidic Acid Vesicles Convert the Prion Protein into β-Sheet Fibrils with a Thermostable Structure but Temperature-Sensitive Packing.

Biomolecular cofactors, including nucleic acids and lipids, promote conversion of the cellular prion protein (PrPC) into pathogenic assemblies (prion scrapie, PrPSc), yet the mechanistic role of specific membrane lipids in the earliest stages remains unclear. Previous work showed that phosphatidic acid (PA) vesicles interact in vitro with murine recombinant PrP (recPrP) and trigger PrP aggregation without additional cofactors. Here, we dissect the PA-driven conversion pathway by monitoring PrP assembly and aggregate stability using spectroscopy and by comparing full-length recPrP with an N-ter

Quantitative substrate profiling of Zika NS5 methyltransferase activity.

Zika virus (ZIKV) is a mosquito-borne and sexually transmitted flavivirus associated with severe congenital abnormalities and neurological complications. Despite its significant global health impact, there are currently no approved antiviral therapies that specifically target ZIKV. Its replication relies on the multifunctional non-structural protein 5 (NS5), whose N-terminal methyltransferase (MTase) domain catalyzes RNA cap methylation, a critical process required for mRNA stability, evasion of the host innate immune responses, and efficient replication. Flavivirus NS5 MTase substrate specifi

Identification of Novel Enzymes Regulating ADP-Ribosylation of Uracil and Thymine in Nucleic Acids.

ADP-ribosylation (ADPr) is a conserved modification that regulates a wide range of cellular processes. Although proteins are the best-characterized substrates of ADPr, recent studies have demonstrated that nucleic acids can also be modified. However, the enzymes responsible for installing and removing nucleic acid ADPr, as well as its biological functions, remain poorly understood. Here, we identify human PARP15 as an ADP-ribosyltransferase that robustly modifies uracil bases in single-stranded RNA (ssRNA) and confirm that human TARG1 reverses PARP15-catalysed ADPr. We further identify the fir

In silico conservation and HLA-binding profiles of HMPV F and G protein epitopes: implications for universal vaccine development.

BACKGROUND: Human metapneumovirus (HMPV) is a significant cause of respiratory infection, yet no licensed vaccine exists. Variability across viral subgroups, particularly in the attachment (G) protein, can complicate universal design. We assessed the immunogenetic regions in the fusion (F) and G proteins to identify stable vaccine targets. METHODS: We analyzed N = 324 Pittsburgh and N = 655 global HMPV genomes (2017-2022) for genome diversity per viral subgroup. Previously identified epitope sequences were obtained from the Immune Epitope DataBase (IEDB) for F and G proteins and sequence simi

Validated chromatographic method for mcDNA quantification in complex samples.

Minicircle DNA (mcDNA) is a non-viral vector known for its superior therapeutic potential and safety, making it a promising biopharmaceutical platform for gene therapy, DNA vaccination, and molecular biology applications. However, its structural similarity to parental plasmid (PP), the presence of isoforms (e.g., open circular mcDNA and supercoiled mcDNA), and host-derived impurities pose significant challenges for accurate detection and quantification, which are key steps for effective quality control. This work reports, for the first time, the development and validation of an anion-exchange

Unraveling the use of vaccine therapy in HER2-positive breast cancer: a comprehensive review of current state and future perspectives.

BACKGROUND: Despite significant advances in early detection and therapeutic strategies, breast cancer (BC) continues to pose a major public health challenge. The treatment of HER2-positive (HER2 +) BC has evolved substantially over recent years with the advent of monoclonal antibodies (mAbs), small-molecule tyrosine kinase inhibitors (TKIs), and antibody-drug conjugates (ADCs). Clinical progress has been significantly accelerated by a deeper understanding of the immune-regulatory properties of this subtype and its interactions with the tumor microenvironment. Despite the availability of effect
