Paradoxical bradycardia from occult venous hemorrhage mimicking vasovagal syncope - A case report.

BACKGROUND: Hemorrhagic shock typically presents with tachycardia; however, profound hypovolemia can paradoxically induce bradycardia via the Bezold-Jarisch reflex. This can lead to misdiagnosis of vasovagal syncope, causing dangerous treatment delays. CASE: A 50-year-old woman developed profound bradycardia and hypotension after vomiting in the post-anesthesia care unit after undergoing hysterectomy. Despite initial management of vasovagal syncope, her condition remained refractory to vasopressors. A critically low hemoglobin level of 5.5 g/dL strongly suggested massive occult hemorrhage. Sh

The PVL threat: A multicenter, single-cohort retrospective study of Panton-Valentine leukocidin staphylococcal infections in critically ill children.

UNLABELLED: Staphylococcus aureus carrying the Panton-Valentine leukocidin (PVL) genes, in both methicillin-susceptible (MSSA) and methicillin-resistant (MRSA) strains, causes severe infection in previously healthy children. We described critically ill children with PVL-positive infection and explored predictors of prolonged intensive care. In this multicenter, single-cohort retrospective study, we included 44 consecutive children aged 0-16 years with PVL-producing S. aureus infection admitted to a neonatal or pediatric intensive care unit (PICU) at 13 Italian centers between 2022 and 2025. Virtual Reality for Pain, Anxiety, and Patient Experience in Awake Urological Procedures: A Systematic Review and Meta-Analysis of Randomised Controlled Trials.

Awake urological procedures, including cystoscopy, transrectal ultrasound-guided prostate biopsy (TRUS-Bx), extracorporeal shock wave lithotripsy, vasectomy, and urodynamic studies, are consistently associated with procedural pain and anxiety, with downstream consequences for surveillance compliance and patient experience. Head-mounted display (HMD) virtual reality (VR) has emerged as a non-pharmacological adjunct, but existing reviews have been narrative or restricted to a single procedure type. This systematic review and meta-analysis evaluated head-mounted display VR across the full breadth

Temporary mechanical circulatory support in cardiogenic shock.

Cardiogenic shock (CS) remains a heterogeneous syndrome associated with persistently high mortality despite advances in temporary mechanical circulatory support. One recent randomized controlled trial suggested a mortality benefit of a percutaneous microaxial flow pump in a cohort of patients with ST-elevation myocardial infarction complicated by CS, whereas routine veno-arterial extracorporeal membrane oxygenation in populations with acute myocardial infarction complicated by CS has not improved survival and is associated with higher complication rates. These findings underscore the importanc

Bioactive adrenomedullin dynamics and microcirculatory changes in a porcine model of hemorrhagic shock.

BACKGROUND: Hemorrhagic shock is a leading cause of preventable trauma death, marked by impaired oxygen delivery and endothelial dysfunction. Bioactive adrenomedullin (bio-ADM) regulates endothelial barrier integrity, but its dynamics during hemorrhagic shock and resuscitation remain unclear. This secondary analysis of a porcine hemorrhagic shock model investigated bio-ADM and its association with macro- and microcirculatory parameters during resuscitation. All animals received crystalloid infusion (10 mL/kg/h) from anesthesia induction onward. Shock was induced by blood withdrawal to maintain

Multiple Neurological Complications and Multidrug-Resistant Bacterial Infections During Prolonged Intensive Care for Undocumented Meningoencephalitis in Côte d'Ivoire: A Case Report.

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

Emergency Transcatheter Aortic Valve Implantation for Decompensated Aortic Stenosis accompanied by Cardiogenic Shock.

Patients with severe aortic stenosis (AS) often present acutely with decompensated heart failure, cardiogenic shock or cardiac arrest requiring immediate intervention. Treatment options for these patients include palliative care, balloon aortic valvuloplasty (BAV), emergency transcatheter aortic valve implantation (TAVI), or surgical aortic valve replacement. TAVI procedure is performed electively in majority of the patients. However, it can be considered a rescue therapy in patients with cardiogenic shock secondary to acutely decompensated valvular AS. Here, we report the successful use of em

A person in their twenties, assigned male at birth and diagnosed with gender incongruence, underwent gender-affirming surgery (GAS) after starting hormone therapy. Orchiectomy, penectomy, labiaplasty and vaginoplasty were planned. The preoperative evaluation revealed no contraindications to general anesthesia. Induction was performed with sevoflurane, propofol, fentanyl, remifentanil, and rocuronium. Cefazolin was administered as a prophylactic antibiotic. After bilateral orchiectomy, the patient developed rapidly worsening urticaria, wheezing, decreased oxygen saturation and hypotension. Anap

An Unusual Presentation of Invasive Group A Streptococcus-A Review of Epidemiology and Challenges in Management.

Invasive Group A Streptococcus (iGAS) infection is a disease state with increasing incidence internationally. This case report outlines a patient with severe septic shock from Group A beta-haemolyticStreptococcus within a provincial intensive care unit (ICU) in New Zealand. Consequently, this case has prompted discussion on the local change in epidemiology for Group A beta-haemolytic Streptococcus (GAS) and a review of the evidence of recognised interventions for iGAS and Streptococcal toxic shock syndrome (STSS).

Intravenous Immunoglobulin Administration in Cardiac Surgery, Heart Transplantation and Infective Endocarditis: A Scoping Review.

Perioperative infections are a common cause of morbidity in cardiac surgery. Intravenous immunoglobulin (IVIG) is increasingly used as a rescue treatment for septic shock. However, data on efficacy and safety in cardiac surgery patients are limited. A scoping review of available literature aimed at identifying studies investigating the use of IVIG in patients undergoing cardiac surgery or with infective endocarditis was performed. Studies reporting data on the use of IVIG in these populations were included, including case reports. Data on study design, population, control group, indications fo

Phenotype-based appropriateness of IGM-enriched polyclonal immunoglobulin use in critical illness: report of an expert consensus.

Sepsis and septic shock result from a dysregulated host response to infection, encompassing both hyperinflammatory and immunosuppressed phenotypes. Although immunomodulatory adjunctive therapies have been extensively investigated over the past decades, none have demonstrated consistent clinical efficacy. In contrast, low circulating immunoglobulin levels are common in septic patients and are associated with greater disease severity and poorer outcomes, providing a biological rationale for the use of polyclonal intravenous immunoglobulins (IVIg) as phenotype-targeted adjunctive therapies. This

Polymyxin B hemoadsorption in endotoxic septic shock: ten questions on the Tigris trial.

The Tigris trial recently reported that polymyxin B hemoadsorption (PMX) achieved a high posterior probability of benefit for mortality in adults with endotoxic septic shock (endotoxemia with multiple organ failure). The trial used a Bayesian framework with a prior derived from a subgroup of an earlier trial (EUPHRATES) and this subgroup was used to design eligibility criteria for Tigris. Ten questions that expose the trial's strengths and limitations are answered in this review. Writing as the trial investigators, our intention is to make the reasoning behind these decisions explicit, so that

Burden of sepsis in obstetric critically ill patients: A qualitative systematic review.

BACKGROUND: Sepsis is a major contributor to maternal morbidity and mortality worldwide and represents one of the leading causes of intensive care unit (ICU) admission among obstetric patients. However, the burden and clinical profile of the most severely affected women remain incompletely defined. This systematic review aimed to estimate the epidemiology, characteristics, and outcomes of sepsis in critically ill obstetric patients. PATIENTS & METHODS: A comprehensive search was conducted for studies published between 2013 and 2025, including studies of women requiring ICU admission for sepsi

Impact of opioid-free general anesthesia with locoregional blocks during breast cancer surgery on systemic inflammatory response: results from a randomized controlled trial.

BACKGROUND: For breast cancer patients with localized disease, surgery remains a cornerstone of treatment. Anesthesia during surgery is essential for pain management and influences patient recovery. Opioid-free general anesthesia with locoregional blocks (OfGA), as an alternative to traditional opioid-based general anesthesia (GA), may improve patient outcomes. METHODS: We set up a randomized trial to compare the impact of different anesthetic approaches on post surgical inflammation in breast cancer patients undergoing quadrantectomy. Patients were randomized to receive GA (n=34) or OfGA (n=

Early multimodal vasopressor strategy in septic shock: results of the TRICYCLE randomized controlled trial.

BACKGROUND: Given multifactorial mechanisms driving vasodilation in septic shock, there is a strong physiological rationale for early initiation of multiple vasopressors (angiotensin II, vasopressin and norepinephrine) with complementary mechanisms of action. Such an approach could more effectively counteract the complex pathophysiological processes characteristic of septic shock. Here we tested the physiological and clinical effects of an early multimodal vasopressor strategy in patients with septic shock. METHODS: We randomly assigned patients with septic shock receiving ≥ 0.15 µg/kg/min of

Hyperbaric Oxygen Therapy in Septic Shock: A First-in-Human Pilot Trial.

INTRODUCTION: Septic shock, a severe, life-threatening infection resulting in organ failure, has seen no successful, novel therapies emerge in the past two decades. In addition to hypoperfusion and inflammation, organ dysfunction in septic shock is caused by microvascular, mitochondrial, and immune dysfunction. Animal studies support the use of hyperbaric oxygen treatment to improve mitochondrial function and reduce the inflammatory cascade, suggesting a potential role for hyperbaric oxygen in treating septic shock. METHODS: A phase I open-label matched clinical trial was conducted to measure
