The surgical value framework in hand surgery: a conceptual model for evaluating surgical pathways.

INTRODUCTION: Hand surgery has robust patient-reported outcome measures to assess symptoms, disability and function, but surgical pathways may also differ in how treatment is delivered, experienced, recovered from and resourced. A conceptual model is needed to consider these wider dimensions without assuming that one pathway is superior to another. METHODS: This Further Knowledge article presents the Surgical Value Framework as a practical model applicable to any surgical pathway, including established open procedures, procedure-room care, Wide Awake Local Anaesthesia No Tourniquet (WALANT),

Preoperative Carriage of Respiratory Viruses and Acute Respiratory Distress Syndrome After Cardiac Surgery: Protocol for the VIRUS-ATTAC Prospective Cohort Study.

BACKGROUND: Cardiac surgery patients are at particular risk of postoperative acute respiratory distress syndrome (ARDS), with an estimated incidence of 5% to 10%. Cardiopulmonary bypass (CPB), ischemia-reperfusion injury, and blood product transfusions are well-recognized contributing factors. Asymptomatic carriage of respiratory viruses has been hypothesized to prime the lungs and potentiate ARDS development when combined with the pulmonary insults of cardiac surgery. However, no study has directly assessed the relationship between preoperative asymptomatic viral carriage and postoperative AR

Current approaches and challenges in pediatric neuromuscular blockade monitoring: A scoping review.

BACKGROUND: Neuromuscular blocking agents (NMBAs) are a fundamental component of balanced anesthesia and are widely used in pediatric perioperative and intensive care settings. Despite their benefits, NMBA administration carries the risk of postoperative residual curarization (PORC), which may lead to respiratory complications, particularly in small infants. Developmental differences in physiology, pharmacokinetics, and pharmacodynamics further complicate NMBA management in this population, making accurate neuromuscular blockade monitoring (NMBM) essential to ensure efficacy and safety. Pheochromocytoma Across the Perioperative Course: Current Concepts and Clinical Management.

Pheochromocytoma is an uncommon neuroendocrine tumor with myriad nonspecific clinical presentations. If not recognized and treated, it is associated with the development of target organ damage and complications such as myocarditis, cardiomyopathy, cerebrovascular accidents, and diabetes mellitus, among numerous others. The pheochromocytoma diagnosis is based on biochemical proof of catecholamine excess followed by imaging localization of the tumor. Patients with newly diagnosed pheochromocytoma should be seen by a multidisciplinary perioperative team, including the anesthesiologist, operating

Intraoperative Lateral Hinge Fractures and Early Postoperative Pain Burden After Medial Opening Wedge High Tibial Osteotomy: Implications for Augmented Postoperative Analgesia.

Background Intraoperative lateral hinge fractures (LHFs) are common complications during medial opening wedge high tibial osteotomy (MOWHTO). While their radiological outcomes are well-studied, their impact on immediate postoperative pain remains underexplored. Purpose The aim of this study was to evaluate the relationship between LHFs and early postoperative pain intensity (visual analog scale [VAS] scores) following MOWHTO. Materials and methods We retrospectively reviewed 244 patients undergoing MOWHTO under spinal anesthesia. Patients were divided into a hinge fracture group (n = 95) and a

Critical Rheumatic Mitral Stenosis Unmasked During Pregnancy Complicated by Preeclampsia With Severe Features and Managed With Urgent Balloon Mitral Valvuloplasty.

Pregnancy imposes significant hemodynamic changes that can unmask previously undiagnosed cardiovascular disease and complicate management when concurrent obstetric conditions are present. We present the case of a 32-year-old immigrant woman at 21 weeks' gestation with a limited medical history due to a significant language barrier, who presented with hypertensive crisis and was diagnosed with preeclampsia with severe features. During her evaluation, she developed worsening dyspnea and acute respiratory decompensation. Further cardiac assessment revealed critical rheumatic mitral stenosis, acco

Tracheal intubation in ICU: Past, present, and future.

Tracheal intubation in the intensive care unit is a high-risk procedure, with hypoxemia, cardiovascular collapse, and cardiac arrest remaining frequent despite substantial progress. This narrative review outlines the evolution of ICU intubation from a primarily technical intervention to a physiologically complex procedure requiring systematic anticipation, optimization, and teamwork. Current evidence supports individualized respiratory preparation, including noninvasive ventilation and high-flow nasal oxygen in selected hypoxemic patients, head-up positioning, and cautious peri-induction venti

Successful cutaneous sensory blockade following single-injection and double-injection techniques of ultrasound-guided superficial parasternal intercostal plane block: a randomized clinical trial.

BACKGROUND: While superficial parasternal intercostal plane blocks can improve analgesia after cardiac surgery, the optimal site and the number of injections remain uncertain. This study aimed to compare the efficacy of single versus double injections of superficial parasternal blocks, hypothesizing that double injections would achieve superior cutaneous sensory blockade. METHODS: 70 cardiac patients undergoing median sternotomy were randomly assigned to receive either single or double injections of superficial parasternal blocks bilaterally. Each patient received 40 mL of 0.25% bupivacaine w

Risk profiles of common brachial plexus block sites: results from the net-ra registry.

INTRODUCTION: Regional anesthesia is frequently used for upper limb surgeries and postoperative pain control. Different approaches to brachial plexus blocks are similarly effective but may differ in the frequency and severity of iatrogenesis. We, therefore, examined large-scale registry data to explore the risks of typical complications among different brachial plexus block sites for regional anesthesia. METHODS: 26,947 qualifying adult brachial plexus blocks (2007-2022) from the Network for Safety in Regional Anesthesia and Acute Pain Therapy registry were included in a retrospective cohort

Perioperative dexmedetomidine reduces the risk of postoperative complications in high-risk patients undergoing non-cardiac surgery: A randomized controlled trial.

BACKGROUND: Dexmedetomidine may suppress the surgery-induced inflammatory response, which is considered the underlying mechanism of postoperative complications. This study was designed to investigate whether perioperative dexmedetomidine could decrease the risk of postoperative complications among high-risk patients. METHODS: This multicenter randomized controlled trial is a superiority trial. Central randomization was used. Elderly patients (age ≥60 years) with revised cardiac risk index (RCRI) ≥3 and scheduled for major non-cardiac surgery were enrolled. Patients in dexmedetomidine group re

Ultrasound guidance does not improve local anesthetic distribution in suprazygomatic maxillary nerve blocks in pediatric patients: a clinical, randomized, controlled, observer-blinded, crossover MRI trial.

BACKGROUND: Maxillary nerve blocks are often used as part of a multimodal postoperative pain management regimen for children undergoing cleft palate surgery. It has not been demonstrated yet that ultrasound guidance provides superior local anesthetic distribution and better success rates compared with the landmark-based block technique. The aim of the study was to compare the effect of ultrasound guidance for suprazygomatic maxillary nerve blocks on local anesthetic spread to the pterygopalatine fossa. METHODS: 20 pediatric patients scheduled for cleft palate repair received bilateral suprazy

N-hexyl cyanoacrylate (Magic Glue) for middle meningeal artery embolization: a retrospective multicenter analysis.

BACKGROUND: Chronic subdural hematoma (cSDH) is common, with an incidence of up to 5/100,000, increasing with age. Surgery carries a recurrence risk of around 20%. Three randomized controlled trials demonstrated the benefit of middle meningeal artery embolization (MMAE) using ethylene vinyl alcohol (EVOH)-based embolics like Onyx (Medtronic) or Squid (Balt Group). Other agents, including particles and N-butyl cyanoacrylate (NBCA), are under investigation. N-hexyl cyanoacrylate (Magic Glue, Balt Group) is a newer NBCA-based embolic with slower precipitation, enabling potentially safer and more

Dextran vs. Crystalloid Priming Solution in Cardiac Surgery: A Randomized Trial on Acute Kidney Injury.

BACKGROUND: Acute kidney injury (AKI) is a frequent complication following cardiac surgery involving cardiopulmonary bypass (CPB). This is partly attributable to crystalloid-based priming solutions causing both hemolysis and loss of oncotic pressure with tissue edema. While colloids like albumin and starches have not shown clear benefits, pilot studies using dextran-based priming reported improved oncotic pressure, reduced hemolysis, and lower levels of a renal injury marker, suggesting potential renal protective effects. OBJECTIVE: We hypothesized that a dextran-based priming solution can re

Cancer Patients with Sepsis: Prognostic Insights from a Population-Based Cohort Study in Norway.

BACKGROUND: Patients with cancer who had sepsis experience a higher mortality risk than noncancer patients. However, it remains unclear how this risk varies with cancer type and metastasis status, sex, age, and infecting microbe. METHODS: This nationwide cohort study included all noncancer patients and those with cancer ≥18 years hospitalized with sepsis in Norway from 2008 to 2021. Cancer status, sepsis, and hospital mortality were identified from the Norwegian Patient Registry (International Classification of Diseases 10th revision codes). Multivariable regression analyses estimated absolut
