Endoscopic transorbital surgery: a transformative approach to pediatric orbital trauma with functional and structural complications.

The endoscopic transorbital approach (ETA) has emerged as a promising technique in minimally invasive skull base surgery. However, there is limited data on its safety and efficacy in pediatric patients. We operated on pediatric patients with (Cerebrospinal Fluid) CSF leakage, compression of orbital structures and orbital subperiosteal hematoma due to trauma. Patients were operated to eliminate limitation of eye movements, to prevent the persistence of diplopia, to prevent CSF leakage and to reduce proptosis. This study aim to evaluate the feasibility and outcomes of ETA in the surgical treatme

Special Considerations in the Management of Pediatric Facial Trauma.

The pediatric facial skeleton has different characteristics when compared to the adult face, and fractures require modified treatment approaches. Nonoperative approaches to facial fracture management are preferred given future bony growth and development of the pediatric facial skeleton. Location of growth centers and active sutures are important in surgical decision-making, as facial fractures in children carry long-term functional sequelae. When operative interventions are indicated, key nuances set pediatric facial trauma apart from its adult counterpart. Several subsites of facial fracture

Single subciliary/subtarsal incision approach for decompression of the orbit and complex fractures, using the inferior oblique muscle as a safety guide to the medial wall of the orbit: a 25-year experience in 361 cases.

The transconjunctival approach (TCA) is popular to gain access to the walls of the orbit, despite some unfavourable outcomes, such as entropion and damage to the lacrimal apparatus. Access to the medial wall is also challenging because of the position of the inferior oblique muscle (IOM), which can lead to iatrogenic palsy of the IOM and diplopia after its detachment. As an alternative, it is possible to use a subtarsal (STA) or subciliary approach (SCA) to avoid IOM injury. The present paper reports 361 orbits treated through a single SCA or STA for bone decompression and fixation of complex

A Retrospective Study of Facial Fractures Associated With the Amazon Region at a Trauma Center in Northern Brazil.

BACKGROUND/AIM: Facial trauma is a major public health challenge due to its aesthetic, functional, psychological, and socioeconomic impacts. In Northern Brazil, particularly in the Amazon region, subsistence activities such as plant extraction, animal handling, and river transportation are essential to daily life and may contribute to an increased risk of facial injuries. These environmental and cultural factors are often underrepresented in the literature. This study aimed to investigate the epidemiological profile of facial fractures associated with etiologies specific to the Amazon region.

Investigation of the effect of impacted third molar position and orientation on bad split fractures in sagittal split ramus osteotomy using finite element analysis.

BACKGROUND: This study aims to evaluate the impact of different angular orientations and buccolingual positions of impacted third molars on the incidence of bad splits during sagittal split ramus osteotomy using finite element analysis. MATERIALS AND METHODS: A total of 12 mandibular models were constructed, each representing a unique combination of third molar orientations (vertical, distoangular, horizontal, mesioangular) and positions (buccal, lingual, central). A force of 20 N was applied from the osteotomy line toward the proximal and distal segments to simulate the working principle of

Possible complications of frontal sinus fractures after conservative or surgical treatment: retrospective study.

BACKGROUND: Frontal sinus fractures are rare. Although they can manifest as isolated fractures, they mostly occur as a part of larger panfacial injuries of the facial bones. OBJECTIVE: Considering the relative rarity of this type of fracture, we decided to perform a retrospective analysis of patients with this type of injury at our institution, focusing on post-injury complications. MATERIAL AND METHODS: We retrospectively evaluated 26 patients with frontal sinus fracture (isolated or as a part of a complex injury to the facial skeleton) treated and subsequently followed up at our institutio

3D printing and acute maxillofacial trauma: an overview of the literature.

BACKGROUND: The consequences of maxillofacial injuries are functionals and aesthetics. The initial treatment must lead to consolidation without any sequelae (complete healing with no clinical symptoms). 3D printing is increasingly used in maxillofacial surgery, but data on its use in the initial phase of trauma is scarce. METHOD: We conducted a bibliographic search in PubMed’s electronic database with the following terms: maxillofacial traumatology, maxillofacial fracture, mandible fracture, maxillary fracture, zygoma fracture, LeFort fracture, fracture of the naso-ethmoido-maxillo-fronto-orbi

Which is the fastest and safest approach? A comparison of time and complications of transparotid and anteroparotid retromandibular approach in management of mandibular subcondylar fractures: A prospective randomized controlled trial.

INTRODUCTION: Mandibular condylar fractures are among the most common facial fractures and its incidence has been rising since the past few decades. Although various approaches for management of mandibular condylar fractures have been described in literature there is a gap in the published literature when it comes to comparison of these approaches. There have only been a handful of studies which have compared use of Retromandibular Transparotid against Retromandibular anteroparotid approach in management of mandibular condylar fractures with conflicting results. The aim of this study was to ai

Retrospective and Comprehensive Evaluation of Posttraumatic Enophthalmos after Intraorbital Fat Injection.

OBJECTIVES: Post-traumatic enophthalmos (PE) is characterized by a posteriorly displaced eyeball (enophthalmos), deep superior sulcus (sunken upper eyelid), and bilateral asymmetry. Intraorbital injection of autologous fat has shown potential for correcting enophthalmos, but several limitations remain, such as patient heterogeneity, small sample sizes, non-quantitative assessment methods and a focus solely on enophthalmos. This study aims to assess both enophthalmos and sunken upper eyelids in a homogeneous patient group using quantitative methods. METHODS: Forty-three sighted PE eyes were re

Costal Cartilage Chip Grafting for High-Arched Narrow Palate Deformity Secondary to Palatal Bone Fracture.

Palatal bone fractures are often associated with vertical maxillary fractures, but direct reduction of the palatal bone itself is rarely performed. We report a case in which a high-arched narrow palate deformity following palatal bone fracture associated with multiple facial fractures was successfully corrected by grafting costal cartilage chips to the hard palate. The patient was a 25-year-old man. At the age of 19, he sustained a cervical spinal cord injury at the C4 level and multiple facial fractures-including maxillary fracture with zygomatic and palatal bone fractures, as well as mandibu

Orbital Fat Herniation Through Inferior Orbital Fissure and Persistent Diplopia Caused by Facial Trauma.

Orbital fat herniation is often accompanied by orbital floor fractures. Furthermore, herniation through the inferior orbital fissure (IOF) is rare, and its pathophysiology remains largely unknown. Here, the authors report a case of a patient with IOF herniation and diplopia caused by orbital trauma. The patient was a 58-year-old male who had undergone reduction of a left zygomatic fracture 15 years previously; however, reduction of the lateral orbital wall displacement was not performed at that time. The patient presented with refractory diplopia following a sports-related orbital injury. Alth

Comparison of patient specific implant reconstruction vs conventional titanium mesh reconstruction of orbital fractures using a novel method.

To compare the reconstruction of orbital fractures using patient-specific implants (PSI) and conventional pre-formed titanium mesh; to develop a method of three-dimensional (3D) superimposition and analysis of the reconstructed orbits; and to present the pitfalls in 3D planning of orbital PSI and how to avoid them. This was a retrospective study of patients with orbital fractures who were treated in our institution between the years 2022 and 2023 using PSI or conservative prefabricated titanium mesh. Three different methods for virtual reconstruction of orbital fractures were used and are deta
