Evaluation of the Intraparotid Branching Patterns of the Facial Nerve During Parotidectomy: A Clinical Study Based on the Davis and Alomar Classifications.

BACKGROUND: The branching patterns of the facial nerve (FN) vary among individuals, which may affect the risk of surgical complications. AIM: The aim of this study is to examine the relationship between FN branching patterns and clinical variables including facial paralysis in patients undergoing parotidectomy in our clinic and to reveal the region-specific anatomical distribution of the FN. METHODS: Data from 39 patients who underwent parotidectomy were analyzed. In all cases, the main trunk of the FN was located at the exit of the stylomastoid foramen and dissected. Intraoperative nerve mo

Multimodal Novel Recurrent Laryngeal Nerve Monitoring Techniques in Anterior Cervical Spine Surgery: Initial Experience and Proof of Concept.

Injury to the recurrent laryngeal nerve (RLN) during anterior cervical spine surgery is an uncommon yet impactful sequela. This study evaluates the feasibility and intraoperative behavior of laryngeal adductor reflex (LAR) and motor-evoked potentials with vocalis recordings (MEP-VC) during anterior cervical spine surgery. LAR and MEP-VC were employed for 13 patients undergoing anterior cervical spine surgery. LAR alerts were issued when clinically correlated reductions in amplitude, typically ≥20% in previously stable, non-habituating recordings, and/or temporal dispersion of the LAR waveform

Spatially resolved single-cell analysis of transcriptomic changes linked with neuropathic pain in human neuromas.

Injuries to the trigeminal nerve, responsible for sensory innervation to the face, may occur during routine dental procedures, resulting in the formation of a neuroma accompanied by loss of sensation and/or symptoms of pain. To gain insight into the molecular mechanisms underpinning the sensory changes, single nuclei RNA sequencing and spatial transcriptomics were used to profile the transcriptional landscape at single cell resolution of human trigeminal nerves and neuromas. Cellular and transcriptional changes were identified that correlated with the presence of pain, including an expansion o

Non-iatrogenic spinal accessory nerve palsy due to lymph node compression.

Injury to the spinal accessory nerve (SAN) is most commonly of iatrogenic or traumatic origin. The incidence of SAN palsy resulting specifically from lymph node compression is difficult to ascertain, as such cases are typically reported in association with malignancies or following surgical procedures. We report the case of an adolescent who presented with clinical features consistent with SAN palsy, including spontaneous shoulder asymmetry, in the absence of prior trauma or surgical intervention to the cervical region. Surgical exploration revealed compression of the SAN by enlarged lymph nod

Distribution and subacute modulation of endocannabinoid metabolizing enzymes in the trigeminal complex and midbrain in a pre-clinical model of post-traumatic headache.

BACKGROUND: Post-traumatic headache (PTH) is a debilitating neurological sequela of mild traumatic brain injury (mTBI) characterized by secondary cephalic pain. The endocannabinoid system (ECS) is a critical modulator of nociception, yet the specific spatiotemporal changes in its metabolic machinery within cephalic pain circuits following mTBI are poorly understood. METHODS: Using in-situ hybridization (ISH), we first characterized the levels of gene expression of major endocannabinoid (eCB) synthesizing enzymes (Napepld, Gde1, Dagla, Daglb), hydrolyzing enzymes (Faah, Mgll) and cannabinoid re

Altered sensory neuron activity in a mouse model of post-burn pain and itch.

Burn injury induces pain and is frequently accompanied by persistent itch during wound healing. However, the underlying neural mechanisms remain poorly understood. Here, we developed a mouse model of post-burn pain and itch and examined changes in primary sensory neuron activity using in vivo calcium imaging of trigeminal ganglion (TG) neurons. To induce scald burn injury, anesthetized mice were exposed to boiling water on the cheek skin. Following injury, mice exhibited both spontaneous pain-related behaviors (wiping) and itch-related behaviors (scratching). Pain-related behaviors peaked on d

Anterior loop of the mental nerve: a cone-beam computed tomography analysis of its prevalence and surgical implications in implant dentistry.

BACKGROUND: Anterior loops of the mental nerve are important anatomical variations in which the inferior alveolar canal extends anteriorly beyond the mental foramen before looping back. These structures pose a risk of neurosensory injury during procedures such as dental implant placement and bone grafting in the interforaminal region. Cone-beam computed tomography (CBCT) provides high-resolution, three-dimensional images that are crucial for accurately identifying and mapping anterior loops, thereby helping prevent surgical complications. This CBCT study was designed to investigate the prevale

Anatomical variations of lamina papyracea, internal carotid artery, and optic canal dehiscence and protrusion: a computed tomography retrospective cohort study.

BACKGROUND: Anatomical variations of the paranasal sinuses, particularly those involving the lamina papyracea (LP), internal carotid artery (ICA), and optic canal (OC), may significantly increase the risk of complications during functional endoscopic sinus surgery (FESS). High-resolution computed tomography (CT) plays a critical role in identifying these variations preoperatively. In this context, the objective of the present study is to determine the prevalence and radiological characteristics of LP dehiscence, ICA protrusion/dehiscence, and optic canal protrusion/dehiscence in a large CT coh

Optimization of decompression angles in facial nerve decompression surgery: A decompression model.

Facial nerve decompression is a surgical procedure performed for severe facial nerve paralysis associated with conditions such as Bell's palsy and Ramsay Hunt syndrome. Classical Western studies by Fisch first established the surgical principles of facial nerve decompression, providing the foundation for subsequent work on decompression extent and outcomes. However, the optimal extent of bony decompression of the facial nerve canal remains unclear, and a 180° removal of the surrounding bone has traditionally been performed based on empirical judgment. Nevertheless, more extensive bone removal

VIsceral sheath targeted approach for left recurrent laryngeal nerve lymphadenectomy during robot-assisted minimally invasive esophagectomy (VISTA approach).

BACKGROUND: Lymphadenectomy along the left recurrent laryngeal nerve (RLN) is essential for curative esophagectomy but carries a high risk of RLN palsy, which impairs postoperative quality of life. Despite advances in robot-assisted minimally invasive esophagectomy (RAMIE), safe and radical dissection around the left RLN remains technically demanding. To achieve both oncological completeness and nerve preservation, we developed a novel Visceral sheath targeted approach (VISTA) based on precise anatomical understanding of the visceral and alar fasciae. METHODS: This retrospective observational

A High-Quality Endoscopic Image Dataset with Annotated Recurrent Laryngeal Nerve for AI-Assisted Thyroid Surgery.

The integration of artificial intelligence (AI) into surgical navigation represents a pivotal advancement in modern operative medicine. In endoscopic thyroidectomy, safeguarding the recurrent laryngeal nerve (RLN) is of critical importance due to its vulnerability to iatrogenic injury, which affects 3-8% of cases and can lead to serious complications such as vocal cord paralysis. However, existing intraoperative nerve monitoring (IONM) technologies are limited by high costs, operator dependence, and discontinuous signal acquisition. To address the lack of large-scale, annotated datasets essent

Iatrogenic Nerve Injuries in Head and Neck Surgeries: A Systematic Review of Mechanisms, Outcomes, and Prevention Strategies.

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Hypoglossal motor output is altered by C4 epidural electrical stimulation via ascending spinal and peripheral feedback pathways.

After cervical spinal cord injury (cSCI), upper airway dysfunction is highly prevalent, including a dramatically increased incidence of obstructive sleep apnea and swallowing dysfunction. While these deficits have often been attributed to secondary sources such as damage from complications of injury management or anatomical changes, there has recently been a greater appreciation for the modulatory role of spinal populations in upper airway behaviors that are disrupted by injury. Here, we illustrate in a rodent model of cSCI that epidural spinal stimulation (ESS) of the phrenic motor nucleus at

A novel cartilage-based reference point for the recurrent laryngeal nerve: implication for preoperative risk stratification in posterior subcapsular thyroid tumors.

BACKGROUND: Protecting the recurrent laryngeal nerve (RLN) is central in thyroidectomy, yet little is known about preoperative localization of the RLN and its role in assessing injury risk. We evaluated whether a novel cartilage-based landmark, the inferior cricoid-tracheal (ICT) point, located at the intersection of the lateral tracheal border and the inferior margin of the cricoid cartilage, approximates the laryngeal entry point (LEP) of the RLN and enables ultrasound-based measurement of tumor-nerve distance.
