Hemihypoglossal-facial neurorrhaphy after mastoid dissection of the facial nerve: results in 24 patients and comparison with the classic technique.

OBJECTIVE: Hypoglossal-facial neurorrhaphy has been widely used for reanimation of paralyzed facial muscles after irreversible proximal injury of the facial nerve. However, complete section of the hypoglossal nerve occasionally results in hemiglossal dysfunction and interferes with swallowing and speech. To reduce this morbidity, a modified technique with partial section of the hypoglossal nerve after mastoid dissection of the facial nerve (HFM) has been used. We report our experience with the HFM technique, retrospectively comparing the outcome with results of the classic hypoglossal-facial n

Facial nerve reconstruction using a split hypoglossal nerve with preservation of tongue function.

A prospective study conducted on 13 patients suffering from complete facial nerve injury (for 4 months up to 2 years) aimed to show that using the split hypoglossal nerve allows for reconstruction of the facial nerve with preservation of tongue function. The hypoglossal nerve was split longitudinally. For each half, a split of the hypoglossal nerve's response was measured intraoperatively by recording the compound muscle action potential of the tongue muscle. The half that showed the least response was selected for anastomosis. The facial nerve was transected at the stylomastoid foramen, and i

Right-sided vagus nerve stimulation in humans: an effective therapy?

Vagus nerve stimulation (VNS) is an additive treatment option for refractory epilepsy. The electrode is placed on the cervical trunk of the left vagus nerve. In patients who are not suitable for left-sided vagus nerve stimulation (L-VNS) right-sided vagus nerve stimulation (R-VNS) may be as effective. In animal models epilepsy is sufficiently suppressed by R-VNS. In a 16 years old boy suffering from medically refractory psychomotoric seizures with secondary generalisation, L-VNS reduced the frequency of generalized seizures. A deep wound infection required the removal of the system eight weeks

Influence of the tooth section technique in alveolar nerve damage after surgery of impacted lower third molars.

This study is a randomized control split-mouth non-blinded prospective trial, the aim of which was to evaluate clinically the frequency and type of injury to the inferior alveolar nerve (IAN) following mandibular third molar surgery using or not using the tooth section technique. The sample consisted of 50 lower third molars in 25 patients, in the control group the tooth section technique was not used, and it was used in the experimental group. The outcomes have shown a frequency of 8% of IAN injury for both groups (in both genders), and there were no statistically significant associations bet

Traumatic optic neuropathy therapy: an update of clinical and experimental studies.

Serious injury to the optic nerve, including direct and indirect events, induces significant visual loss and even blindness. For the past decade corticosteroids and/or optic canal decompression surgery have been widely embraced therapeutic paradigms for the treatment of traumatic optic neuropathy. There is little clinical evidence, however, to support the effectiveness of these strategies, raising questions about the efficiency of current therapy for improving visual outcomes. Recently, experimental studies have yielded a wealth of information related to the protection and regeneration of reti

Safely navigating around the facial nerve in three dimensions.

Although much literature discusses the anatomy and injury of the facial nerve branches, the fascial plane of the nerves, particularly where they transition from one plane to another, is rarely emphasized and information is often contradictory. This article describes in three-dimensional, accurate surgical terms where the facial nerve branches are located and how they can be protected during dissection above and below the superficial musculoaponeurotic system-platysma plane.

Giant olfactory groove meningioma: ophthalmological and cognitive outcome after bifrontal microsurgical approach.

OBJECT: Olfactory groove meningiomas arise in the midline along the dura of the cribriform plate and may reach a large size before producing symptoms. We conducted a retrospective study of patients with these lesions focused on pre- and post-operative investigations for ophthalmological, personality and cognitive disturbances. METHODS: The authors reviewed 36 patients with giant olfactory groove meningiomas surgically treated via a bifrontal approach. Ophthalmological evaluation included visual acuity, fundoscopy and visual fields while psychological evaluation included a Mini-Mental State Ex

Microvascular decompression in hemifacial spasm resulting from a cerebellopontine angle lipoma: case report.

OBJECTIVE: Hemifacial spasm caused by a cerebellopontine angle lipoma is extremely rare. We describe a patient with left-sided hemifacial spasm caused by vascular compression of the facial and vestibulocochlear cranial nerves by the anteroinferior cerebellar artery embedded within a cerebellopontine angle lipoma. CLINICAL PRESENTATION: A 77-year-old man presented with a 10-year history of left-sided facial spasms that progressively worsened over time and significantly interfered with his ability to read, drive, and interact in social situations. Neurological examination showed obvious left he

Modifications in infrared intraoperative neuromonitoring camera and supports.

At my request, modifications were made to the visual oculography (VOG) camera described in the March 2008 article in this journal: "Use of an Infrared Camera to Improve the Outcome of Facial Nerve Monitoring," Am J Electroneurodiagnostic Technol 48.:38-47. The camera now has a wider and a more distant range and a clamp onto a rod. The company (Micromedical Technologies, Inc.) has named the camera the KlereView Camera in honor of my father, Fred Klerekoper.

New safety margins for chin bone harvesting based on the course of the mandibular incisive canal in CT.

OBJECTIVES: Altered pulp sensitivity of anterior lower teeth is a frequent finding following chin bone harvesting. Persistent loss of tooth sensitivity has been reported in up to 20% of the patients. The aim of this study was to evaluate current recommendations for the location of the harvest zone with respect to the course of the mandibular incisive canal (MIC), the intrabony continuation of the mandibular canal mesial to the mental foramen. MATERIAL AND METHODS: On computed tomographic (CT) scans of 50 dentate mandibles, the MIC was located and its distance to the root apices, to the labial

Investigation of infraorbital nerve injury following zygomaticomaxillary complex fractures.

The aim of this study was to investigate the severity of infraorbital nerve injury following zygomaticomaxillary complex fractures and to estimate the treatment methods facilitating its functional recovery. A total of 478 patients with unilateral zygomaticomaxillary complex fractures were treated. Infraorbital nerve sensory disturbances were diagnosed in 64.4% of the patients. Injury of the infraorbital nerve was expressed as asymmetry index, which was calculated as a ratio between the affected side and the intact side electric pain detection thresholds at the innervation zone skin before trea

The recurrent laryngeal nerve in thyroid surgery.

The recurrent laryngeal nerve (RLN) is an essential nerve for phonation and psychosocial interaction. Throughout the annals oflaryngological history, vocal cord paralysis has been a well recognised disorder. A rich heritage of scientific investigation and research has considered the complexity of vocal cord function, laryngoscopic presentation and surgical rehabilitation. Identification and preservation of the RLN is the major concern in thyroid surgery. Controversy exists regarding the correct pre- and postoperative management in vocal cord evaluation. We fully review this topic and challenge

What is the learning curve for intraoperative neuromonitoring in thyroid surgery?

BACKGROUND: The study describes the initial experience and learning curve of intraoperative neuromonitoring (IONM) during thyroidectomy. We describe the prevalence and patterns of IONM technical problems. METHODS: Prospective series of 152 consecutive thyroid operations (304 nerves at risk) were analyzed. Standard technique consists of monitoring vagal and RLNs before, during and after resection. Personal gain of experience was defined by the preceding number of thyroid operations. To establish the number of thyroidectomies required before achieving an effective and safe IONM technique, all o

The evaluation of surgical factors related to recovery period of upper lip hypoaesthesia after Le Fort I osteotomy.

PURPOSE: It is unclear whether surgical factors can affect the upper lip sensitivity. The aim of this study was to assess the factors that can affect the recovery period of hypoaesthesia of the upper lip after Le Fort I osteotomy, using trigeminal somatosensory evoked potential (TSEP) objectively. PATIENTS AND METHODS: Twenty-nine patients with mandibular prognathism underwent Le Fort I osteotomy with and without artificial pterygoid plate fracture. Trigeminal nerve hypoaesthesia at the region of the upper lip was assessed bilaterally by the TSEP method. The distance between the infraorbital
